[Ketoprofen-aspirin interaction].

Eighteen patients with chronic inflammatory joint disease were treated for two successive five day sequences, in a double blind crossover study, with 300 mg per day ketoprofen given alone or combined with 1,500 mg aspirin. Bioavailability and pharmacokinetics of total serum ketoprofen studied in eight patients did not appear to be significantly modified by addition of aspirin. Measurements of Ritchie's articular index and Lee's functional index demonstrated that ketoprofen was clinically as effective when given alone as in association with aspirin. Measurement of the sedimentation rate by the sigma ESR technique showed that aspirin produced a marked reduction of the antiinflammatory biological activity of ketoprofen which did not however reach statistical significance. This study of interactions between ketoprofen and aspirin shows that it is useless and perhaps harmful to combine the two medications.

[Clinical and radiological study of the manubrio-sternal articulation. Comparison between 80 control subjects and 88 patients with chronic inflammatory rheumatism].

The manubrio-sternal joint was studied in 168 subjects divided up into two groups : one group of 80 control subjects and one group of 88 patients with chronic inflammatory rheumatism divided up into 3 sub-groups (rheumatoid arthritis 54 ; ankylosing spondylitis 25 ; peripheral psoriatic rheumatism 9). This was a clinical study (search for spontaneous and induced pain) and a radiological study (A.P. and lateral views) with classification in 5 groups : 0 normal appearance, 1 hazy appearance of the joint space, slight narrowing of the joint space, slight osteoporosis and/or subchondral bony condensation ; 2 irregularity of the margins of the joint space, increased osteoporosis and/or condensation, narrowing of the joint space ; 3 increase in the previous signs, erosion, vacuole formation ; 4 partial or total fusion. The results show that only spontaneous pain and the radiological appearances of class II and class III permit one to differentiate in a statistically significant way the control group from the rheumatism group. considering these data one may note a greater frequency of the clinical and radiological signs, together with greater clinical and radiological agreement in ankylosing spondylitis than in rheumatoid arthritis. The interest of a study of the manubrio-sternal joint in the early diagnosis of ankylosing spondylitis is recalled although the involvement of this joint is usually later than the sacro-iliac joint.

[Normocalcemic hyperparathormonemia and articular chondrocalcinosis. Study of 69 patients compared to 57 controls].

The level of parathyroid hormone was measured by heterologue C terminal radio-immunological assay in 69 patients with clinical or radiological manifestations of the type seen in primary articular chondrocalcinosis. They were divided into three groups: P1 with undetermined clinical arthropathies; P2 with sub-chondral and arthosic arthropathies; P3 with radiologically definite chondrocalcinosis. They were compared with 57 control subjects broken up into four groups: T1 with chronic rheumatic arthritis, T2 with low back pain, T3 with primary hyperparathyroidism due to adenoma, and T4 with secondary hyperparathyroidism with renal insufficiency. A form of normocalcemic hyperparathormonaemia was demonstrated in more than one out of two patients in group P1 (15/29). It was seen in three-quarters of the cases in group P2 (12/16). And it was seen in more than a quarter of the cases in group P3 (7/24). This hyperparathormonaemia was statistically significant only in groups P1 and P2 compared to the normals in groups T1 and T2. The results we obtained in this study seem to be in complete concordance with those we obtained earlier in idiopathic hemochromatosis. This hyperparathormonaemia seems to regress with age and is often only discovered when the characteristic articular lesions have appeared. The discovery of normocalcemic hyperparathormonaemia several years before the appearance of the radiological signs of the disease would appear to be an important argument in favour of the diagnosis of early articular chondrocalcinosis. The existence of raised parathyroid hormone in primary articular chondrocalcinosis as well as in idiopathic hemachromatosis is special etiopathogenic interest even if there remain numerous questions concerning its origin and mode of action.

[Chronic alcoholic liver disease. Ionized blood calcium measurements in 24 patients (author's transl)].

Measurements of ionized blood calcium were carried out in 38 subjects: 14 controls and 24 patients with chronic alcoholic liver disease (non-cirrhotic in 7, cirrhotic compensated in 2 and cirrhotic decompensated in 15). Compared with the control group, a significant decrease in ionized calcium was found in patients with decompensated cirrhosis. Through secondary hyperparathyroidism, this hypocalcaemia might contribute to the demineralization of the bones observed in cirrhotic patients.

[Hyperparathyroidism secondary to cirrhosis. Arguments supplied by ionized calcium, urinary cyclic AMP and blood N-terminal parathyroid hormone].

A laboratory study including estimation of 25 OH vitamin D, terminal parathormone (PTH) C and N fractions, urinary cyclic AMP (AMP cU) and ionised calcium, was carried out in 25 patients, 10 cases of alcoholic cirrhosis without decompensation (group 1) and 15 cases of decompensated cirrhosis (group 2) in order to seek evidence in favour of hyperparathyroidism secondary to cirrhosis. The results show: 1) The existence of hypovitaminosis D which seems to be independent of the liver failure. 2) A very definite increase in terminal PTH N in group 2 compared with group 1 (p < 0.01), without any increase in terminal C fraction. 3) An insignificant increase in urinary cyclic AMP in group 2 compared with group 1. 4) A low serum ionised calcium in group 2 compared with 14 controls (p < 0.05). The terminal N PTH was correlated significantly with urinary cyclic AMP and ionised calcium. The evidence is in favour of secondary hyperparathyroidism where the ionised calcium plays a role, but one wonders whether other factors do not intervene, e.g. serum iron, owing to the discovery of a significant link between serum iron and terminal N PTH levels.

[Bone histomorphometry and osteo-articular manifestations of idiopathic hemochromatosis].

An histomorphometric study of the bones was done on 28 patients suffering from an idiopathic hemochromatosis. There was a normocalcinemic hyperparathyroidism in half the cases. It appeared to be closely linked to hypersideremia and to be responsible for the sub-chondral arthropathy. The other lesions demonstrated were an osteoporous one, whose relations with hypogonadism was shown, and osteoidosis variations, which most often decreased rather than increased, and whose causes still seem uncertain.
