Paired parallel locking forceps.

We describe an instrument used to prevent contact between an antimetabolite and the conjunctiva-Tenon's wound edge in filtering surgery. Paired parallel locking forceps were used to keep the everted conjunctival-Tenon's wound edge from touching the antimetabolite sponge during limbal-based filtering surgery while enabling manipulation of the antimetabolite sponge. This allows the surgeon to more easily avoid the conjunctival edge, making antimetabolite touch easier and safer.

A YAG laser technique to facilitate removal of posterior chamber intraocular lenses from the capsular bag.

A technique is presented which facilitates repositioning or removal of a decentered capsular bag-fixated posterior chamber intraocular lens. The anterior lens capsule in front of the encapsulated haptic loop is disrupted with the YAG laser. This frees the loop from the capsular bag so a dialing maneuver will not stress the posterior capsule or zonule.

Giant cell arteritis with normal erythrocyte sedimentation rate: a management dilemma.

Two patients with cranial arteritis and normal erythrocyte sedimentation rates of 13 mm/hr. and 26 mm/hr. are presented. One patient became totally blind awaiting temporal artery biopsy. The other patient was a challenge in titrating increased steroid dose with cardiovascular complications against decreased steroid dose with worsening of visual and neurologic problems. Available tests may be negative in active disease. The clinical picture is paramount in decision making, and a high index of suspicion is mandatory. Since a delay of twenty-four hours may be critical, steroids usually should be started as soon as an erythrocyte sedimentation rate has been drawn, without waiting for a biopsy. Once steroid therapy has been started, temporal artery biopsy should be performed conveniently soon, regardless of the erythrocyte sedimentation rate.

Surgical corneal light occluder made of black HEMA.

A safe, simple surgical light occluder made of black pigmented hydrated HEMA is described. It protects the retina from all wavelengths of light and protects the corneal epithelium against abrasion and opacities due to drying during surgery.
