[Clinical observation on diabetic nephropathy treated with alcohol of Abelmoschus manihot].

Sixty-eight cases of non-insulin dependent diabetes mellitus (NIDDM) complicated with nephropathy were randomly divided into two groups: treated group, 35 cases treated with alcohol extraction of Abelmoschus manihot, Gliclazide and Captopril tablets; control group, 33 cases treated with Gliclazide and Captopril tablets, over a period of 8 weeks. The total effective rate in treated and control group were 83.87% and 31.03%(P < 0.01), urinary micro-albumin were 31.7 mg/L and 76.3 mg/L (P < 0.05), proteinuria were 0.41 g/24h and 0.77 g/24h (P < 0.01), blood beta 2-microglobulin were 3317.8 ng/ml and 3473.1 ng/ml (P < 0.05), urinary beta 2-microglobulin were 367.2 ng/ml and 641.5 ng/ml (P < 0.01), urinary N-acetyl-beta-glucosaminidase (NAG) were 26.3 u/L and 66.7 u/L (P < 0.01), plasma lipid peroxide (LPO) were 6.13 nmol/L and 8.78 nmol/L (P < 0.05), and plasma superoxide anion were 8.36 kcpm and 10.42 kcpm respectively (P < 0.05). It was suggested that Abemoschus manihot alcohol extraction could eliminate oxygen free radicals, alleviate renal tubular-interstitial diseases, improve renal function and reduce proteinuria.

[Relationship between apo-protein in renal disease patients with diagnosis and TCM syndrome differentiation].

UNLABELLED: The relationship between the level of apo-protein in 132 renal disease patients with diagnosis and TCM Syndrome Differentiation was analysed. The serum apo-protein level of 132 patients group and 135 healthy adults group were monitored with radioimmunodiffusion assay. The patients were divided into three groups. Spleen Qi and Kidney Qi Deficiency (SQKQD), both Qi and Yin Deficiency (QYD), and the Liver Yin and Kidney Yin Deficiency (LYKYD) group. All patients were divided into with-Dampness and without-Dampness group according to their clinical symptoms and signs. Comparison was also made between different groups. RESULTS: Renal diseases including nephrotic syndrome, chronic nephritis, chronic renal failure and patients with SQKQD, QYD, LYKYD, the levels of apo-B and apo-B/apo-A-I were higher than that of normal (P < 0.01) in various degree, while the level of apo-A-I, was not changed or lower than the normal. In TCM Syndrome differentiation, apo-B and apo-B/apo-A-I in the SQKQD group were higher than those in QYD and LYKYD group (P < 0.05). Moreover, these two values in cases with Dampness were much higher than those without Dampness (P < 0.01). Therefore, it is important to treat this secondary abnormality of lipid metabolism for reducing the further deterioration of renal diseases. When primary renal disease was treated, elimination and removal the dampness might reduce the further damage of hyperlipidemia in renal diseases.

The facilitatory effect of N-methyl-D-aspartate on sexual receptivity in female rats through GnRH release.

The purpose of this study was to examine whether N-methyl-D-aspartate affects the sexual receptivity of female rats. Monosodium L-glutamate was used as a neurotoxin to induce hypogonadal status. Matured normal and monosodium L-glutamate-treated rats were ovariectomized and implanted subcutaneously with estradiol capsules. One week later, lordosis responsiveness was observed before and 10 min after N-methyl-D-aspartate (40 mg/kg of BW, ip) administration. The results showed that N-methyl-D-aspartate caused a remarkable increase of lordosis quotient in control rats but not in monosodium L-glutamate-treated rats. Moreover, the possible action site of N-methyl-D-aspartate in the enhancement of receptivity was evaluated by the post-castrational LH rise, pituitary LH release in response to GnRH, and N-methyl-D-aspartate-evoked GnRH releasability. The results revealed that: (a) serum levels of LH in monosodium L-glutamate-treated rats were lower (p < 0.01) than those of control rats after ovariectomy; (b) there was no significant difference of pituitary LH release responsiveness to GnRH test between two groups; and (c) N-methyl-D-aspartate-evoked LH release in monosodium L-glutamate-treated rats was similar to that in the control rats. In conclusion, N-methyl-D-aspartate may facilitate the sexual receptivity through stimulating GnRH release. The failure of N-methyl-D-aspartate in enhancing receptivity in monosodium L-glutamate-treated rats is probably due to the cellular damage by monosodium L-glutamate on specific areas responsible for lordosis.

[Relation between dampness-heat syndrome of glomerulonephritis and sialic acid and N-acetyl-beta-D-glucosaminidase (NAG)].

Plasmic and urinary sialic acid and urinary N-acetyl-beta-D-glucosaminidase (NAG) of 87 glomerulonephritic patients with and without Dampness-Heat Syndrome were measured, and the influence of clearing up Dampness-Heat therapy on above-mentioned parameters was investigated. The results showed that Psa, Usa and UNAG of Dampness-Heat Syndrome were significantly higher than those of non-Dampness-Heat Syndrome (P < 0.05-0.01). The further analysis indicated that the patients with acute onset of chronic nephritis manifested as Dampness-Heat, showed marked positive correlation between Usa and UNAG as well as between UNAG and proteinuria respectively (r = 0.75 and 0.722, P < 0.001). With the treatment of Abelmoschus manihot which could remove the Dampness-Heat, the amount of proteinuria, Usa and UNAG were all significantly decreased (P < 0.05-0.001). It suggested that Usa and UNAG might be as diagnostic and curative parameters of Dampness-Heat of glomerulonephritis.

Studies on co-localization of 7B2 and pancreatic hormones in normal and tumoural islet cells.

The distribution of protein 7B2, a protein with structural characteristics of GTP-binding proteins, has been studied in normal pancreatic islets and in a series of 70 pancreatic endocrine tumours with emphasis on the co-localization of 7B2 and the different pancreatic hormones. Although all cell types of normal islets were found to store 7B2, variations from intense expression to absence of reaction were seen within each cell type. In particular, B cells showed intense immunostaining for 7B2 in small compact islets and weak or no staining in larger islets with lobular arrangement. Pancreatic polypeptide (PP) cells expressed 7B2 intensely in the PP-rich area of ventral embryological origin, but were mostly non-reactive in the PP-poor area. The A cells, located along intralobular blood vessels, were more frequently immunoreactive for 7B2 than those at the periphery of the islets. Immuno-electron microscopy revealed a preferential localization of 7B2 in secretory granules of islet cells, with more intense localization in the peripheral halo of alpha granules. Benign islet cell tumours more frequently expressed 7B2 than their malignant counterparts. Although often expressed in a lower number of tumour cells than the tumour-specific hormone, 7B2 was usually co-localized with the latter. In contrast, no relationship was found with the localization of proinsulin. It is concluded that 7B2 is a non-permanent component of the cell granule compartment, probably involved in events related to exocytosis and without relationship to intracellular prohormone processing.

[Pathogenic factor (Dampness-heat) of glomerulopathy].

Mesangial proliferative glomerulonephritis (MsPGN) induced by chronic serum sickness in rabbits coincide with the human chronic progressive glomerulonephritis resulted from repeated infection, which is similar to pathologic changes of Dampness-Heat Syndrome. The experimental model of MsPGN was treated by Abelmoschus manihot. which could remove the Dampness-Heat. The amount of proteinuria in treating and control group were 62.68mg/24hr and 121.94mg/24hr respectively (P < 0.05), the number of cells in glomeruli were 61.54 and 80.39 respectively (P < 0.01), and diameters of glomeruli were 102.43 microns and 121.13 microns respectively (P < 0.01). It suggested that the drug could alleviate circulating immune complex (CIC) mediated renal injury.

Gastric carcinoids and their precursor lesions. A histologic and immunohistochemical study of 23 cases.

A histologic and immunohistochemical study was carried out in 23 unselected nonantral gastric carcinoids and their precursor lesions classified according to Solcia et al. None of the patients showed Zollinger-Ellison syndrome. Two variants of carcinoids showing distinctive pathologic and pathogenetic characteristics were identified on the basis of presence or absence of associated chronic atrophic gastritis type A (A-CAG). Chronic atrophic gastritis type A was found in 19 cases showing either single or multiple neoplasms, tumor extension limited to the mucosa or submucosa, consistent endocrine cell precursor changes in extratumoral mucosa, and consistent hypergastrinemia and/or G cell hyperplasia. Associated precursor lesions were only hyperplastic in all but two cases with single carcinoids whereas they were also dysplastic in all but one case with multiple carcinoids. The four tumors arising in nonatrophic mucosa were all single, more aggressive, and not associated with extratumoral endocrine cell proliferations or with signs of gastrin hypersecretion. Tumor cells were diffusely immunoreactive for chromogranin A and synaptophysin but usually negative for chromogranin B or HISL-19. Scattered serotonin cells were found in ten carcinoids. They were more frequent in infiltrating than in intramucosal tumors as were the less represented pancreatic polypeptide cells whereas the reverse was found for alpha-subunit-containing cells. These results are of relevance for tumor pathogenesis and may provide the rationale for a less aggressive therapeutic approach in the patients.

Double-edged limit of total fertility rates.

In this paper we present the accurate estimation of total fertility rates, prove the theorem of upper and lower bounds of double-edged limit and provide the formulas of upper and lower bounds of total fertility rates. Based on these estimations and statistical data, we calculate the upper and lower bounds of the double-edged limit for 46 countries including China. Furthermore, we describe the positive growth rate, negative growth rate and zero growth rate of population development. These results are of great importance for us to control population not only in theory but also in practice.

Immunohistochemical localization of factor X-like antigen in pancreatic islets and their tumours.

The authors have investigated by immunohistochemistry the distribution of factor X-like antigen in normal pancreatic islets and in a series of 46 pancreatic endocrine tumours. It was found that both glucagon-producing (A) cells and pancreatic polypeptide-producing (PP) cells are immunoreactive for the antigen. Benign glucagonomas and PP-omas presented the highest concentrations of immunoreactive material whose intracellular distribution was consistent with localization within cell secretory granules. Some benign insulinomas also presented factor X immunostaining in spite of the absence of the antigen in normal insulin-producing B cells. Although malignant tumours usually exhibited very low or no immunostaining, two of three malignant glucagonomas showed scattered, intensely immunoreactive cells. The factor X-like antigen identified in this study was found to differ from chromogranin A and B. The possible implications of the present findings for coagulative disorders associated with glucagonomas or diabetes are discussed.

Tissue iron content in riboflavin and pyridoxine deficient rats.

The effect of riboflavin and (or) pyridoxine deficiency and repletion on tissue iron content was studied in rats. The iron content in liver, spleen, and kidney and plasma iron concentration of riboflavin deficient (RD) rats was lower, but hematocrit was not. In pyridoxine deficient (PD) rats versus control rats, the iron content in liver was significantly higher but not in spleen and kidney. In PD rats hematocrit was lower but plasma iron concentration was not. Although combined riboflavin and pyridoxine deficient (CD) rats had lower iron content in liver and spleen compared with control rats, these values were intermediate between those of RD rats and PD rats. After RD and PD rats were repleted, the iron content in liver, spleen, and kidney returned to that of control rats, and the hematological indices were improved significantly. These results suggest that riboflavin and pyridoxine deficiency may impair the absorption and utilization of iron and may result in altered tissue iron content.

Influence of pinealectomy on sexual behavior of normal and monosodium L-glutamate-treated rats.

Subcutaneous injections of monosodium L-glutamate (MSG) were administered, as a neurotoxin, at a dose of 4 mg/g body weight to rat pups on days 1 and 3 postnatally. Pinealectomy was performed at 6 weeks of age, and animals were allowed to mature. The intact MSG-treated rats showed a decrease of lordosis quotient in females and a reduction of ejaculation frequency and an increase in mount frequency in males. Pinealectomy caused no significant improvement on sexual behavior of the saline-treated normal rats, but it caused a marked improvement of the sexual receptivity in MSG-treated female rats and reinstated ejaculation in MSG-treated males. The circulatory sex steroid levels, estrogen in females and androgen in males, were not significantly different between the intact and pinealectomized MSG-treated groups in the respective sexes, whereas the progesterone level in females was significantly elevated in the pinealectomized MSG-treated group compared with the intact MSG-treated group. These findings suggest that the pineal gland plays an inhibitory role on the sexual behavior of MSG-treated rats and the improvement of sexual behavior following pinealectomy may be dependent on certain mechanisms other than the actions of circulatory androgens or estrogens.

Identification of the endocrine cells detected by the monoclonal antibody HISL-19 in human tissues.

The human endocrine cells reacting with the monoclonal antibody HISL-19 were identified with hormone antisera of proven specificity using a double immunostaining procedure. The epitope for HISL-19 was found in all types of pituitary cells except ACTH cells, in thyroid C cells, in all types of adrenal medullary and pancreatic islet cells and in somatostatin and pancreatic polypeptide cells of the gastrointestinal mucosa. No staining was found in parathyroid cells and in most gastrointestinal endocrine cells. Either paranuclear focal accumulation or diffuse cytoplasmic distribution of immunoreactive material were found. The spectrum of HISL-19 immunoreactive cells was found to be only in part complementary to that of cells immunoreactive for chromogranin A. Thus, it is concluded that the monoclonal antibody HISL-19 is a useful addition to other immunohistochemical markers for the detection of cells showing neuroendocrine features.

Deciduomal estrogen and progesterone receptors in unilateral pregnant hamsters.

Deciduomal reactions in response to artificial stimuli were induced on the tube-ligated uterine horns of unilateral pregnant hamsters. Serum progesterone levels were determined, and the changes of estrogen and progesterone receptors (ER and PR, respectively) were simultaneously studied in the deciduoma. After stimulation on day 4 of pregnancy (D4), the serum progesterone level rose steadily to about 16 ng/ml on D12 and remained at plateau until D14. A sharp increase followed by a rapid fall in progesterone was observed on D15 and D16. The weight of the deciduoma-bearing horn increased more than 5 times to a maximal level on D10. Both deciduomal and myometrial weights were maintained and remained elevated on D16. Histological observation revealed that the maintenance of decidual cells was heterogenous. Cells at antimesometrial site (AMS) started to regress on D10, whereas cells at mesometrial site (MS) remained recognizable on D16. Cytosol ER was detected during the early stage (D6) of decidualization. The cytosol ER in the deciduoma continuously decreased and became undetectable on D10. Nuclear ER was not detected during the time of study. The nuclear and cytosol PR increased and reached a peak level on D8 and D10, respectively. The concentration of PR declined sharply afterward. The PR levels in deciduomal tissues at MS and AMS were separately studied. PR in AMS decreased gradually after D8 to basal level on D14. PR at MS still remained relatively constant on D12 followed by a precipitous decrease. These data show a prolongation of the deciduomal life span and the heterogeneity of deciduomal maintenance and regression in unilateral pregnant hamsters. This strongly suggests a close relationship between the loss of PR and the regression of the deciduoma.

[Mucin histochemical changes in various colo-rectal polyps in relation to carcinoma of the large bowel].

210 various colo-rectal polyps including 46 inflammatory polyps, 21 juvenile polyps, 9 hyperplastic polyps, 65 tubular adenomas, 51 familial polyps, 11 villous adenomas, 7 adenomatous polyps with focal cancer, and 14 carcinoma of the large bowel were investigated by HE,HID-AB,PAT-KOH-PAS staining in order to study the mucin changes of these lesions. N-acetylated and C7,C9 O-acetylated sialomucin were mainly obtained in those adenomas with moderate and severe dysplasia (55-64.3%) and the proportion was even higher in cases of villous adenomas, familial polyps, adenomas with focal cancer and advanced carcinoma. These mucins might be assumed as a criteria in representing malignant transformation.

Comparative effects of diverse vertebrate gonadotropins on androgen formation in vitro from testes of roosters and mice.

A study was carried out to compare the androgen formation activity of gonadotropins from diverse vertebrate species by rooster and mouse testes in vitro. The dispersed testicular interstitial cells from 6- to 7-wk-old mice or testicular slices from 3- to 4-mo-old roosters were incubated with varying doses of luteinizing hormones (LHs)/gonadotropins (GTHs) in Medium 199 containing isobutyl-methyl-xanthine and N-2-hydroxyethylpiperazine-N-2-ethanesulfonic acid buffer (pH 7.4) at 34 degrees C (mice) or 37 degrees C (roosters) for 4 hr under continuous aeration of 95% O2-5% CO2 in a Dubnoff incubator shaken at 100 cycles/min. Androgen in the medium was measured by radioimmunoassay. The results revealed that dose-related androgen formations were obtained both in rooster and mouse systems in response to stimulations of all LHs/GTHs tested. The mouse system was more responsive to mammalian LHs and placental GTHs, less responsive to LHs from chickens, frogs, and turtles, and extremely unresponsive to piscine GTHs. In contrast, the rooster system was highly responsive to LHs from both mammals and chickens in androgen formation; it was also responsive to LHs from turtles and frogs as well as to piscine GTHs, although with relatively lower sensitivity. The rooster testis system is thus suitable for in vitro bioassay of LHs/GTHs from virtually all vertebrate classes, whereas the mouse testis system is more suitable for bioassay of mammalian LHs and placental GTHs. The differential androgen formation potencies of the diverse vertebrate GTHs in testis systems between roosters and mice indicate that a divergence exists in the testicular receptors between the two animal species.(ABSTRACT TRUNCATED AT 250 WORDS)

Metastasis of hepatocellular carcinoma to the proximal jejunum manifested by occult gastrointestinal bleeding.

A 31-yr-old man, who had received left hepatic lobectomy for a ruptured hepatocellular carcinoma 17 months previously, began to have elevation of serum alpha-fetoprotein and occult gastrointestinal bleeding. Abdominal computed tomography, angiography, ultrasonography, upper gastrointestinal panendoscopy, colon-fiberscopy and endoscopic retrograde cholangiopancreaticography failed to disclose any recurrent intrahepatic tumor or bleeder in the gastrointestinal tract. However, a barium meal study revealed an intussusception with a suspicious nodular lesion in the proximal jejunum as the leading edge, which, after laparotomy, was proven to be a metastatic hepatocellular carcinoma and the cause of the gastrointestinal bleeding. The serosal side of this lesion was free from cancer involvement and there was no peritoneal implantation or lymph node involvement. The metastasis is probably hematogenous. The extremely unusual location involved by hepatocellular carcinoma in this patient signifies the ubiquitous nature of metastasis in this cancer.

[Functional change of colorectal adenoma and its relation with colorectal adenocarcinoma--application of mucin histochemical and immunohistochemical techniques].

Using HE, AB/PAS, HID/AB, PAT/KOH/PAS stain and CEA monoclonal antibody BA stain techniques, composition of mucin and distribution of CEA in the tumor tissue were detected in 65 cases of colorectal tubular adenoma, 7 adenoma with malignant change and 14 established tubular adenocarcinoma in order to study the functional changes of colorectal adenoma and its relation with the colorectal adenocarcinoma. According to Konishi's criteria, colorectal tubular adenomas were divided into three groups: mild, moderate and severe dysplasia, added with two of ours: adenoma with malignant change and established adenocarcinoma, totalling 5 groups for comparison. The results showed that CEA positive rate increased with the grade of dysplasia in these 5 groups (33%, 57%, 67%, 86% and 100%). Similarly, the percentage of non-acetylated acid and C7 C9 O-acetylated sialomucin also gradually increased with the grade of dysplasia, showing a parallel tendency. Epithelial cells, secreting a heterogeneous mucin, could be taken as a signal of abnormal cellular differentiation. The increase of non-acetylated acid and C7 C9 O-acetylated sialomucin in colorectal adenoma implies a likelihood of malignant change.
