The benefit of cochlear implantation in the Japanese elderly.

The purpose of this study was to evaluate the benefit of cochlear implant for Japanese elderly people with profound hearing loss. A questionnaire was administered to the five Japanese elderly people over 65 years old with the Nucleus 22 channel device and their family. The questionnaire was designed to gain insight into the patient's daily use and to evaluate the quality of life. Elderly patients who were implanted with the Nucleus device showed good ability to identify environmental sound and revealed no significant deterioration compared to younger patients. Implanted patients showed some difficulties in understanding speech in the noisy environment and conversation among several persons. Nevertheless, 80% of patients over 65 and their family reported communication benefits in the family and in social activities. In addition, 80% of the patients reported that they felt the better or improved quality of life after the implantation. All patients considered the implant was good for their life and would recommend a cochlear implant to a deaf friend. It is notable that 60% of patients and their family report that the recipients have become more cheerful. This study showed the cochlear implant contributes to the better quality of life in the Japanese elderly.

Dimethylsulfoxide potentiates the suppressive effects of lidocaine on synaptic transmission in bullfrog sympathetic ganglion.

The effects of dimethylsulfoxide used as a solvent on the suppressive effects of lidocaine on synaptic transmission were examined in the bullfrog sympathetic ganglion. Compound action potentials (CAP) in response to orthodromic stimulation of the presynaptic nerve trunk were recorded extracellularly from the bullfrog sympathetic ganglion. Application of lidocaine dissolved in normal Ringer's solution significantly decreased the amplitude of CAP. This suppressive effect of lidocaine was markedly potentiated when lidocaine was dissolved in 0.1% DMSO solution. A similar potentiating effect of DMSO was observed on the suppressive action of lidocaine on acetylcholine (ACh)-induced depolarizing response recorded intracellularly from the sympathetic ganglion cell. On the other hand, 0.1% DMSO solution alone had no effect on synaptic transmission, resting membrane potential, resting membrane resistance or ACh receptor activity. The present findings suggest that DMSO as solvent should be used carefully because of its potentiating effect on drug actions. Since the effect of lidocaine is due to the suppression of nicotinic ACh receptor at the postsynaptic membrane, DMSO may allosterically act on the nicotinic ACh receptor causing an increase in efficacy of lidocaine.

[Characterization of cell lines from metastatic maxillary cancer].

Two cell lines were established one from a primary site (OKK-TKp) and the other from a metastatic lymph node (OKK-TKm), from a maxillary squamous cell carcinoma (in the same patient). These cell lines were characterized with regard to morphology, chromosome numbers, tumorigenicity, growth kinetics, expression of tumor markers, sensitivity to immunological effector cells and anticancer agents. The two cell lines had the same modal chromosome number of 69, and formed tumors in nude mice. The median doubling time of OKK-TKp was 14.6 hrs and that of OKK-TKm was 24.3 hrs. Immunohistochemistry showed transferrin receptor (TfR), Ki-67, and epidermal growth factor receptor (EGFR) in OKK-TKm, but less or none in OKK-TKp. OKK-TKm was more sensitive to lysis by lymphokine-activated killer cells (LAK) on a 4-h 51Cr release assay, but was less sensitive to anticancer agents (CDDP, CBDCA) in modified MTT assay. We are convinced that these results are of value for understanding the mechanism of metastasis.

[Induction of cytotoxic activity from regional lymph node lymphocytes in head and neck malignant tumor].

Cytotoxic activities of rIL-2-stimulated effector cells from peripheral blood mononuclear cells (PBMC-LAK), and those from the regional lymph node cells (LN-LAK) from patients with head and neck malignant tumor were examined by 4-hour 51Cr release assay. Cytotoxicity of LAK cells from involved lymph node (LN (+)-LAK) were significantly lower than those of PBMC-LAK. LAK cells from non-involved lymph node (LN (-)-LAK) had a significantly higher cytotoxicity against Daudi cell than PBMC-LAK. Cytotoxic activities against K562 cell and autologous tumor cells mediated by LN (-)-LAK were not significantly different from those by PBMC-LAK. However, autologous tumor cell lysis by LN (-)-LAK from lymph nodes showing follicular lymphoid hyperplasia (FLH) pattern was higher than that by PBMC-LAK. The effector cells against autologous tumor cells were characterized CD56+ cells and CD8+ cells (CD8+CD11b- cells) by phenotypic analysis and negative selection assay using immunomagnetic isolation technique.

Local administration trials of interleukin-2 for head and neck cancer.

Recombinant interleukin-2 (rIL-2) has been administered locally in 20 patients with head and neck cancer. Two complete responses in lower lip cancer and 1 partial response in lingual cancer have been obtained. Immunohistological study reveals that tumor infiltrating lymphocytes (TILs), including activated T lymphocytes and natural killer cells, are increased after rIL-2 use. Intraarterial chemotherapy, performed subsequently to the local use of rIL-2 results in a dramatic decrease in tumor size within a short time, and a high frequency of CR cases is observed. Local use of rIL-2 is beneficial for treatment of head and neck cancers, and induction immunochemotherapy combining locally used rIL-2 and arterially infused anticancer drugs plays an important role in a multidisciplinary treatment for these cancers.

[Clinical evaluation of local administration of RIL-2 in head and neck cancer].

Recombinant Interleukin-2 (rIL-2) was administered locally to 20 patients with head and neck cancer. 800 thousand units of TGP-3 were injected around the tumor mass every day for about 4 weeks. Complete response (CR) was obtained in two cases with lip cancer and one with lingual cancer showed partial response (PR), and minor response (MR) in oral floor cancer. No recurrence of tumors was observed in the two cases of lip cancer. Chemotherapy was performed with rIL-2 in cases responding only insufficiently to rIL-2. It was interesting that arterial infusion of anticancer drugs subsequent to local administration of rIL-2 resulted in a dramatic decrease in tumor size in a short time. It is suggested that local administration of rIL-2 may play an important role in multimodal treatment for head and neck cancer.

[Immunohistology of tumor tissue in local administration of recombinant interleukin-2 in head and neck cancer].

Biopsied specimens from patients treated by local administration of rIL-2 were examined immunohistologically. Lymphocytes infiltrating into the cancer tissue before and after administration of rIL-2 were observed on paraffin embedded sections by Hematoxilin Eosin (HE) stain in relation to the clinical effect. And their subsets were identified on frozen sections by Avidin Biotin peroxidase Complex (ABC) method using monoclonal antibodies to examine what kinds of effector cells were induced and increased in the tumor site by rIL-2 in vivo. The intensity of increasing of tumor infiltrating lymphocytes (TIL) was correlated with the clinical effect of local administration of rIL-2. Tumor Infiltrating Lymphocytes were mainly T lymphocytes, of which Leu-2a+ cells (suppressor/cytotoxic T cells) and Leu-3a + 3b+ cells (helper/inducer T cells) were almost equally observed. Leu-2a+ cells were considered to be mainly cytotoxic T lymphocytes because few Leu-2a+ cells were stained with anti-Leu-15 antibody. After administration of rIL-2, marked infiltration of T lymphocytes was observed and there was no obvious different response in degree of infiltration between Leu-2a+ cells and Leu 3a + 3b+ cells. Furthermore, IL-2 receptor+ T lymphocytes (activated T lymphocytes) were increased after administration of rIL-2. And natural killer (NK) cells were slightly observed and also increased after administration of rIL-2. These findings suggest local administration of rIL-2 has possibility to induce cytotoxic T lymphocytes, to increase NK cells and to activate these lymphocytes in vivo.
