[Chronic fatigue syndrome--51 cases in the Jikei University School of Medicine].

Between April 1991 and August 1992, we diagnosed 51 cases of CFS who met definition of CFS designated by CDC, 1988. They are 41 female and 11 male, and 78% are women. At first visit, their ages are ranged from 16 to 64 years old, and approximately 45% is 20 to 30 years old. In periods of illness from onset, 39.2% of the patients are in period of 6 month to 1 year, 19.6% within 2 years, and 15.6% within 3 years, respectively. The sufferer who have symptoms of CFS over 10 years long are in 6 cases. Most of patients have already been examined by many other clinics and hospitals. They have been told as no abnormal medical condition, or often as neurosis, depressive state and autonomous imbalance etc. Interesting things are trigger of CFS. 77.5% of patients have onset of flu-like symptom, including 5 cases of acute infectious mononucleosis. In many female patients, symptoms of CFS begun after hand work in addition to psychological factors. Specific laboratory results are not shown in CBC, urinalysis, biochemical studies and inflammatory marhers. 6 cases hare positive Rheumatoid factor and positive ANF are shown in 16 cases (31.3%). Specific patterns of anti EBV antibodies are not shown. Lymphocyte subsets used by monoclonal antibodies are not specific. At the present, prognosis is good and 56.8% of CFS patients are generally improved. For severe cases, NSAID, Sulpiride, Amitryptiline and minor tranquilizer are used.

[Cardiac herniation after right pneumonectomy].

Cardiac herniation is a rare, but highly lethal complication of intrapericardial pneumonectomy for progressive carcinoma of lung, and occurs in early postoperative period. Our patient with this complication presented sudden onset of severe hypotension and cyanosis after several ventricular premature beats. We suspected cardiac herniation as we observed dilatation of cervical veins and rightward deviation of heart sound. The patient was immediately positioned on his left side and systemic blood pressure increased soon. After verification of the incident by chest X-ray film, emergency operation was performed. Fortunately, vital signs were stable during and after reoperation and the patient could survive.

Plexiform neurofibromatosis of the ileum in an infant.

An infant is reported in whom severe malabsorptive symptoms developed shortly after birth associated with intermittent episodes of intestinal obstruction. Plexiform neurofibromatosis involving the terminal ileum was found at laparotomy at the age of 6 mo. The infant died from gastrointestinal functional impairment at the age of 1 yr.

A simple hemostatic method in suprapubic prostatectomy: extracapsular pulling sutures.

To block the vessels of the prostate, extracapsular pulling sutures were placed from the 4 and 8 o'clock positions on the bladder neck to the perineum prior to enucleation in 25 cases of prostatectomy. The removable pulling sutures successfully reduced blood loss during and after prostatectomy. Besides, the average operating time was markedly shortened. Postoperative courses of the patients were uneventful on the whole and there was no remarkable complication.

Inhibition of granulation tissue growth by histamine.

Granulomas were induced in rats by subcutaneous implantation of formalin-soaked filter-paper disks. Daily subcutaneous injection of histamine at doses of two times 0.05 mg/kg and above inhibited the growth of granulation tissue as measured by a marked decrease in the dry-defatted granuloma weight and of the hydroxyproline and hexosamine content. Histological observations of granulation tissue indicated that histamine inhibited the proliferation of fibroblasts and the formation of capillaries. Inhibitory effects were also observed with the histamine releaser, sinomenine, and the histaminase inhibitor, aminoguanidine. These histamine effects seem not to be mediated by glucocorticoid release, since an effective dose level of histamine produced no change in growth or thymus weight. Prednisolone was less potent than histamine in inhibiting Prednisolone was ineffective at the dose tested. Subcutaneous injection of the H2-receptor antagonist, burimamide, blocked these histamine effects and also of sinomeinine and aminoguanidine. The H1-receptor antagonist, mepyramine, did not block these histamine effects. Burimamide alone enhanced the growth of granuloma. These results indicate that granulation-tissue growth in inflammation is affected by the inhibitory effect of endogenous histamine acting through H2-receptors.
