[Bacterial vaginosis: what risks for the mother and child?].

Bacterial vaginosis are characterized by a polymicrobial proliferation of anaerobic organisms and the disappearance of the commensal lactobacilli, which can cause ascending utero-adnexitis and endanger ongoing pregnancy. The role of anaerobes in utero-adnexitis and tubal abscess was first raised in 1973 par Eschenbach. Several authors have since established correlations between bacterial vaginosis and both pain from the uterine appendages and gynecologic infections, particularly post-abortum. Bacterial vaginosis is a major problem in pregnant women, given its high prevalence (15-20 p. cent) and significant relationship with premature delivery and premature rupture of the membranes (at less than 37 weeks of amenorrhea) in epidemiological case-control and cohort studies taking into account demographic and concomitant obstetric characteristics. In particular, histological lesions indicative of chorioamniotitis have been correlated with prematurity and with the presence of bacteria (usually those involved in vaginosis) in the amniochorial space. Bacterial vaginosis can give rise to ascending infections which reach the amniochorial space and cause uterine contractions or alter the amniochorial membrane, leading to premature rupture of the membranes. These epidemiological data must, however, be confirmed by controlled trials of antibiotics active in bacterial vaginosis and their preventive effect on prematurity and premature rupture of the membranes. Finally, bacterial vaginosis can play a role in endometritis following both natural and caesarean delivery; this has been shown in multivariate analyses with adjustment for maternal age, duration of labor and rupture of the membranes, and isolation of bacteria associated with bacterial vaginosis from the endometrium.

Roxithromycin treatment of mouse chlamydial salpingitis and protective effect on fertility.

We used a mouse model of acute chlamydial salpingitis to evaluate the efficacy of roxithromycin in preventing irreversible inflammatory damage leading to tubal infertility. Female C3H/He mice were genitally inoculated with a human strain of Chlamydia trachomatis and then treated with roxithromycin glutamate subcutaneously. Treatment was initiated either 7 or 10 days postinfection (p.i.) and continued for 7 days at a dosage of 50 or 100 mg/kg of body weight per 24 h. The course of the disease was monitored serologically, bacteriologically, and histologically. At the end of the treatment, the mice were encaged with males and their reproductive capacity was recorded over a 19-week period. The protective effect of roxithromycin was assessed in terms of fertility parameters in comparison with values for noninfected control mice. When treatment was initiated on day 7 p.i. and given in twice-daily 25-mg/kg doses, all the mice remained fertile and the total number of offspring was similar to that of sham-infected mice (17.3 +/- 3.3 versus 17.2 +/- 2.3). When treatment was initiated on day 10 p.i. and given in a single daily dose of 50 or 100 mg/kg, 90 and 70% of the mice, respectively, remained fertile; however, in terms of total offspring, fertility was lower in the group treated with the lower dose (5.6 +/- 1.4 versus 13.0 +/- 3.8). Roxithromycin was found to be effective against C. trachomatis in the mouse genital tract, but fertility was only partially preserved when the time between infection and treatment was prolonged.

An experimental model for salpingitis due to Chlamydia trachomatis and residual tubal infertility in the mouse.

A mouse model of salpingitis and subsequent tubal infertility induced by a human strain of Chlamydia trachomatis has been studied. C3H/He female mice were inoculated into the ovarian bursa. Some of the mice (six infected, five controls) were killed on days 15 and 23 and the remaining animals (10 infected, 10 controls) were mated on day 15. On day 15, the infection was maximal with intratubal inflammation, elevated antichlamydial antibody titre and positive cultures in 12 cases out of 16. After 19 weeks of housing with the male, the proportion of fertilized females was significantly lower in the infected group (20% versus 100% in the control group P less than 0.01). In the killed mice, hydrosalpinx and or tubal occlusion were noted at this time in nine cases out of 10, despite an apparent bacteriological healing.

Bilateral renal tumours with reference to the reoperation of a solitary kidney resected for tumour.

A case of bilateral renal tumour is reviewed, where the disease was detected by sonography performed for screening. On one side, nephrectomy, while on the other, partial resection of the kidney were performed in two sessions. Due to occlusion and a resultant renal fistula produced by scar tissue following the second operation as well as to progressive azotaemia, reoperation of the solitary kidney resected for tumour became necessary. As a result of the failure of both cytostatic and radiotherapy, the authors believe that, in similar cases, every surgical method possible should be used for prolonging the patient's life. In the available literature no reference has been found on the reoperation of solitary kidney resected for malignant tumour.

Adenocarcinoma↗

Carcinoma of the penis as a metastasis.

A case of carcinoma of the penis is reported where the tumour was metastatic in origin. The patient recovered postoperatively. Based on the histological study of the removed tumour, suspicion of bronchial cancer arose to be the primary tumour on the right side. Subsequently, despite the physician's advice, the patient left the department of pulmonology for home and so no further pulmonological examination and treatment were made. One year postoperatively, local recurrence in the penis could not be noted.

Carcinoma↗

Combined use of transurethral resection and needle biopsy in the diagnostics of carcinoma of the prostate.

On suspicion of prostatic tumour, the authors performed a combination of transurethral resection and needle biopsy to form a safe histological diagnosis as well as to apply an adequate therapy. Transurethral resection combined with needle biopsy is to be performed if patients present with dysuric complaints, and if tumour can be suspected on rectal palpation or based on the clinical picture. This intervention ensures an unequivocal diagnosis and eliminates the urinary obstruction. They also dealt with cases, in which only needle biopsy or transurethral resection could help in forming the correct diagnosis.

Adenoma↗

Parenchymal operations of the lower pole of the kidney with a double collecting system.

The authors present four cases of unilateral kidney with a double collecting system where calculous change has occurred in the lower collecting system. In one of them, marginopolar nephrotomy, while in three, heminephrectomies in the destructed lower segment of the kidney were performed. The remaining upper part functioned perfectly later on. The avoidance of unjustified nephrectomy is stressed.

[Transurethral resection in bladder tumors].

The experience gained in the course of 127 transurethral resections of bladder tumours are described. After discussing the indications and the technique the authors present their own results. The advantages of transurethral resection are emphasized: reduced operational stress, early mobilization, quick removal of the catheter, few postoperative complications, low cost of nursing. Therefore the operation is given preference mainly in the treatment of elderly patients with bladder tumour.

[Surgical treatment of induration in penis plastica].

In the past 5 years the authors have removed the scar tissue in the case of the induration of the penis plastica of 43 patients by their special surgical method. Operation had to be repeated in the case of 5 patients. Twenty-five patients were cured, 15 improved and 3 remained unchanged after operation. The method, if applied with the necessary skill, is a recommendable method for the treatment of the induration of penis plastica. If, after operation the restoration of the function is not perfect, the application of supplementary conservative therapy is justified.

[Urologic diseases in the differential diagnosis of acute abdomen].

The urological diseases which alone or intercurrent with surgical and gynaecologic ailments cause most of the problems in the differential diagnosis of the acute abdomen are reviewed. The necessity of a cooperation between the urologist, surgeon and gynaecologist and the importance of intra-operative diagnosis are stressed.

[Subtotal resection of ureterocele].

After a review of the diagnostic and therapeutic possibilities of ureterocele, the authors' own experience with transvesical subtotal resection is presented. After the resection of the ureterocele, two-third of the patients were free of complaints and symptoms, their pyuria ceased and subsequent intravenous pyelography showed normal conditions.

[Possibilities of vascular surgery in urology and nephrology].

The vascular surgical procedures most frequently applied in urology and nephrology are described. The urological aspects of the various methods are stressed and cases suitable for joint urological and vascular surgery are presented. With respect to the occlusion of the renal artery the difference between the recanalization of this vessel and of the peripheral vessels is pointed out and the importance of preserving the kidney is emphasised. The hope is expressed that by the increasing use of vascular surgical methods a more effective therapeutical possibility will be available to increasing numbers of urological-nephrological patients.
