[Endoscopic polypectomy in the colon (author's transl)].

A report is given on the endoscopic removal of 81 polyps of the colon by means of high frequency diathermy. This procedure proved to be without risk to the patient and has largely replaced surgical removal by laparotomy as the method of choice today. Total removal of the polyp with subsequent histological examination is naturally imperative to avoid unnecessary subsequent radical surgical procedures. Since most colonic carcinomas arise in the base of a polyp, polypectomy represents a true prophylactic measure and, in some cases, a therapeutic one.

[Cytodiagnosis of the stomach: a comparative survey of cytological and histological investigations (author's transl)].

The consideration was the center of our comparative survey: early diagnosis of gastric cancer by combination of gastroscopy, gastrobiopsy and gastrocytology. We are filled with dismay that this simple method is not yet used in every case of wellfounded suspicion of gastric cancer. In our own material cytological findings of cancer could be proved in 88% and verified histologically. 2% of cytological diagnosis were false negative because the brush samples were not taken from the right location by the gastroscopists. Simultaneously with the evaluation of the dignity, a diagnosis of the different forms of gastritis was made. A vast conformity with pathohistological results could be achieved. In stenosing processes of the stomach and also in diagnosis of lesions in the cardia, the use of a cell brush would be of great advantage toward gastrobiopsy.

[T1 carcinoma of the stomach in endoscopy and microscopy (author's transl)].

After intensification of our gastroscopic examinations in 15 cases of early stage T1-carcinoma of the stomach was found by biopsy and proven in the resection specimen. Further, 144 advanced carcinomata of type T2 and T3 were detected. The incidence was 0,42% of all gastroscopies and 9,3% of all assessed carcinomata. The discovery of 284 high risk patients is of special importance and it is expected that intensive follow-up examinations of this group will enable the early detection and surgical cure of gastric carcinoma. We are not agreeing with pessimistic authors believing that the early diagnosis of the wide spread gastric carcinoma may not be improved. The main problem seems to us the too great time delay until diagnosis. As it is valid in general for the early detection of cancer it has to be an important task for the practizing physician to recognize the very first gastric symptoms in order to admit the patient as early as possible to gastroenteroscopy.

[New amino acid derivatives with oncolytic action].

A total of 27 new derivatives have been obtained by substitution on seven amino acids and betaines, respectively. The present experimental study examines the oncolytic and oncostatic action of four new amino acid compounds. Under experimental conditions in vitro the following remarkable properties of the chemical preparations can be ascertained on Ehrlich's tumor cells of the white mouse: 1. The oncolysis detected by micromorphology only occurs after an incubation of 4-6 hours, the transplantability of the tumor cells is lost much earlier. 2. The anaerobic glycolysis is in no case limited, the cell respiration is stimulated under the influence of amino acid compounds. 3. The Crabtree effect on Ehrlich ascites tumor cells is inhibited by the test substances. The irreversible damage caused by the new amino acid compounds to the Ehrlich tumor cells and observed in vitro only occurs with large excess of the substances. The mechanism of action may be due to a substrate-like excess inhibition, a disturbance of the amino acid sequence within the cell proteins being discussed. Therapeutic inhibition tests on experimental animal tumors have shown that an effect may only be expected after the application of high doses and prolonged administration. The extremely low general toxicity and the lack of any disturbance of the hematopoietic system permit a long-time application of the amino acid compounds.

[Initial clinical experiences with the administration of new amino acid derivatives in the treatment of malignant tumors].

A report is given on the first clinical experience made with the application of new amino acid compounds in the therapy of malignant tumors. The established good compatibility of the substances allows long-termed treatments with great variability of individual dosage. The regressions of tumors observed and the frequent improvement of the general status, even in advanced stages of tumors, should give rise to a broad investigation of the available tumor inhibitors.
