[Anti-sperm antibodies in women and men in infertile marriages].

The authors examined the serum of 76 women and 33 men from infertile marriages for the presence of antispermatozoal antibodies. Seropositivity was revealed in 19 women (25%) and in 12 men (36%). In control groups of healthy men and women seropositivity was substantially less frequent. In women the difference was statistically significant, but not in men. The ELISA method which was applied for examination appears to be a useful tool for screening of immunological factors of infertility, in particular in couples who where infertile for prolonged periods and where no unequivocal cause of infertility was found. Only more detailed comprehensive examination can explain immunologically conditioned cases of infertility and lead to selection of an optimal therapeutic strategy.

[Surgical treatment of obstructive azoospermia].

In 23 infertile men the diagnosis of azoospermia was established. Bilateral biopsy of the testes was made, in four patients primary deficiency of spermiogenesis was revealed, in another four patients the Sertoli cells only syndrome. In a group of 3 men with hypospermiogenesis revision revealed severe hypoplasia of the epididymis which in context with the bioptic finding did not give hope of successful surgery. In the group of four men with normal spermiogenesis revision revealed in one obliteration of the terminal portion of the d. deferens, treated by transurethral discision of the colliculus and in three its partial or total agenesis. In one of the three latter cases bilaterally artificial spermatoceles were implanted which function satisfactorily; in the aspirated contents, however, even after several months only non-motile sperm cells were found. In seven patients epididymovasoanastomosis was performed by the microscopic technique--five times unilateral and twice bilateral. In one patient a condition after bilateral vasectomy was involved which was resolved by vasovasoanastomosis. In three patients after epididymovasoanastomosis azoospermia persists. In another two patients the check-up spermiogram revealed a positive finding, the wife of one of the patients is pregnant at the moment. In one patient the result of the operation cannot be evaluated yet due to a short time interval after operation. One patient did not come for a check-up examination. The patient after vasovasoanastomosis has a positive spermiogram with 38 million sperm cells per ml. In one patient with a discision of the colliculus the spermiogram is negative. The cumulative index of patient anastomoses reported in the literature is approximately 45%, the percentage of pregnancies only 17-18%.

[Catamnesis of a group of 84 castrated sexual offenders].

The authors examined 84 castrated sexual delinquents after a 1-15-year interval following castration. 18% of the subjects were capable of occasional sexual intercourse. 21% of the subjects lived in a stable heterosexual partnership. Almost half the men had occasional erections in the morning. More than one third of the group masturbates occasionally. Only three men committed another sexual offence after castration. These offences did not have an aggressive character. From a sub-group of 32 men punished previously on account of non-sexual offences 15, i.e. almost half of them, committed further non-sexual offences. One quarter of the examined subjects had objections to the results of castration. Most frequently because sex was lacking in their life. The authors did not observe serious physical or mental consequences of castration in the examined men. Castration must be considered even nowadays an important part of the therapeutic arsenal in sexual delinquents. It should be performed in those patients who cannot be influenced by other types of treatment.

Hormonal status and sexual behaviour of 16 men after surgical castration.

Sixteen castrated sexual offenders were investigated blood plasma levels of testosterone [T], dihydrotestosterone [DHT], luteinizing hormone [LH] and follicle-stimulating hormone [FSH]. Levels of T and DHT were significantly decreased in comparison with normal values. Levels of both gonadotropic hormones were significantly elevated. There was a great reduction in the occurrence of orgasm and ejaculation in castrated men. Only six of these sixteen men were able to reach orgasm. Only two of these men stated that they had a small amount of ejaculate by orgasm. Phalloplethysmographic [PPG] examination were provided with electrocapacitance PPG equipment. In only two men investigated was there no specific erotic reactions to visual stimuli. PPG responses in 8 men were small, in 4 average and in 2 only were these reactions high. No changes in sexual orientation were found after castration. In most men after surgical castration sexual arousal is preserved. Low, with low frequency of orgasm and very low sexual interest.

[Results of ambulatory care of 103 patients who committed sex offenses].

103 sexual delinquents were treated in our sexological outpatient clinic. These men were referred for compulsory treatment by courts on account of: indecent exposure (N = 51, i.e. 49.5%), heterosexual aggression (N = 22, i.e. 21.3%), homosexual offence with minors (N = 18, i.e. 17.5%), hetero-sexual paedophilia (N = 8, i.e. 7.8%), fetishist offence (N = 2, i.e. 1.9%), incest with adolescent daughter (N = 1) and zoophilia (N = 1). Sexological treatment consisted of psychotherapeutic, sociotherapeutic and drug treatment. After a three-year follow-up the results were evaluated as favourable in 44 men (i.e. 42.7%). 33 men (i.e. 32.1%) relapsed and committed a further sexual crime. 26 men (i.e. 25.2%) remain in a long-term medical care, as favourable sexual adaptation was not achieved in these cases within three years of treatment. Psychological, psychosexual and social characteristics of patients are analysed in connection with results of sexological treatment.

[Possibilities of treatment of erectile disorders by the administration of vasoactive substances into the penis using iontophoresis].

The authors applied to 18 men with erection disorders vasoactive substances by iontophoresis of the penis. In 12 patients partial improvement was achieved, in four marked improvement. Only in two the condition was not influenced. The applied method is non-invasive, practically without negative side-effects. It can extend the range of therapeutic methods in psychogenic and mild organic erection disorders.

Castrated men exhibit bone loss: effect of calcitonin treatment on biochemical indices of bone remodeling.

To test the hypothesis that the reduction in gonadal function can lead to bone mass loss, a group of 12 men who had undergone bilateral orchidectomy at the age of 28.2 +/- 6.8 yr was evaluated. A progressive loss of the lumbar bone density was observed as a function of time after orchidectomy. Both the biochemical indices of bone resorption (urinary hydroxyproline/creatinine ratio and plasma tartrate-resistant acid phosphatase) and bone formation (serum osteocalcin and bone isoenzyme of alkaline phosphatase) were significantly increased in the patients compared with healthy controls. A positive correlation was found between urinary hydroxyproline excretion and percent change in spinal bone mineral density per yr. Because of this increase in the biochemically indicated bone resorption, nine of the patients were studied again after 1-3 yr and were thereafter treated with intranasal calcitonin. Urinary hydroxyproline excretion normalized after 3 months of treatment, and a significant decrease, but not to normal levels, was also observed in the mean values for the other biochemical indices of bone remodeling. Thus, testosterone deficiency, like estrogen deficiency, is associated with accelerated bone loss. The increase in osteoresorption was partially corrected by calcitonin treatment.

[An attempt to define the therapeutic goals in sex deviants].

The comprehensive adaptational therapy of sexual deviations comprises a social, psychological and biological component. The centre of gravidity as regards work with these patients is individual psychotherapy supplemented by group therapy. The main aims of this treatment are: 1. The provision and supplementation of knowledge on normal and deviant sexuality. 2. Modification of the patient's attitude to sexuality. 3. Obtaining insight into the sexual deviation. 4. Adaptation of the patient to his deviation, elaboration of an individual sexual ideology. 5. Inhibition of sexual activity by short-term or long-term biological treatment. The components of treatment are selected on an individual basis.

[A new method of treating male infertility: percutaneous transfemoral embolization of varicoceles with Vilan 500].

The authors describe a new method of treatment of male infertility. In 25 men aged 19-45 years with varicocele, after percutaneous transfemoral catheterization, embolization of the spermatic vein with the Czechoslovak preparation Vilan 500 (hydrolyzed co-polymer of polyvinyl acetate and polyvinyl alcohol in 96% ethyl alcohol) was performed. Six patients were treated on account of marked subjective complaints and 19 because of reduced fertility. Of these in 16 after embolization the spermiogram improved. The authors describe the diagnostic procedure, the technique of embolization, its complications and results. The described method is simple, safe, effective and as compared with other embolization methods it is cheaper.

[Surgical therapy of impotence with implantation of a penile prosthesis. The psychological status of the impotent male].

The results of a psychological examination of 22 male patients which requested a penile prosthesis implantation are reported. The mean age was 47 years and the intelligence score of the patients was higher than the normal value. In a personal self-judgement they described oneself as stable, minimally neurotic and rather introverted personality. The test results were complicated by a high lie score. A great difference between the self-judgement and the ideal was established. The sexual anamnesis of the most patients was not striking out of the erectile dysfunction.

[Forensic expert opinion on sexual delinquents].

The author gives a critical account of the development of views regarding the imputability of sexual delinquents and the possibility of protective therapy in sexual deviations. The attitudes of specialists are changing constantly and an uniform opinion was not achieved so far. The author conceives sexual deviation as a mental disorders which in some instances may reduce the imputability of the person in relation to the sexual delict. Modern therapeutic procedures try to apply in indicate cases a comprehensive therapeutic approach combining biological therapy and psychotherapy.

[Attitudes of women and men toward condoms].

The authors used a ten-item test of attitudes to condoms for enquiries among 200 patients (100 women and 100 men). The attitudes of women were more frequently negative (22.5%) than the attitudes of men (16%). Women also replied less frequently in a positive way to questions of the test than men (42% as compared with 48.5%). Seventy-two respondents (30 women and 42 men) had no experience with condoms, 92 had a single experience (50 women and 42 men) and only 36 respondents (20 women and 16 men) used at least occasionally condoms frequently. Greater experience with condoms increases the positive evaluation of this contraceptive.

Steroid and steroid regulating hormones in human male castrates.

Testosterone, dihydrotestosterone, androstenedione, dehydroepiandrosterone and its sulphate, 11 beta-hydroxyandrostenedione, 17 alpha-hydroxyprogesterone, cortisol, LH, FSH and sex hormone binding globulin (SHBG) were determined in 16 otherwise healthy male castrates, orchidectomized for the forensic reasons. Their hormone levels were compared with those, obtained in normal males of the same age category. There were found in the castrates decreased levels of testosterone, dihydrotestosterone, dehydroepiandrosterone sulphate and androstenedione, unaltered concentrations of cortisol and dehydroepiandrosterone, whereas the levels of 11 beta-hydroxyandrostenedione, 17 alpha-hydroxyprogesterone, both gonadotrophins and SHBG were increased. The results point to the importance of the selective increase of the incretory activity of the adrenal cortex in situations, when the gonadal function is impaired or missing.
