Biomedical informatics research and education at the EuroMISE Center.

OBJECTIVES: In the paper we described the partial achievements reached by the EuroMISE Center in research and higher education, supporting better healthcare in future information society. METHODS: Through the research cooperation, the EuroMISE Centre suggests publicly available tools and methods that allow the application of new technologies for various healthcare organizations and citizens, with a special focus on the national environment of the Czech Republic. It provides a platform for a greater level of research cooperation with the usage of the most up-to-date information and communication technologies and a wide spectrum of clinical and genetic data and knowledge. RESULTS: Results of research and higher education of the EuroMISE Centre in the field of Biomedical Informatics are described in outputs of national and European projects. Further results have been published and/or presented at national and international meetings and incorporated in education and healthcare practice. CONCLUSIONS: The long-term effect of postgraduate doctoral education in the field of biomedical informatics consists in the rapid increase of scientists that are needed in the Czech Republic to reach the goals of information society in health care. The multidisciplinary research at the EuroMISE Center moves towards direct usage in the informational support of diagnostics and therapy, mining of the medical databases, and in the customization of healthcare based on the clinical and genetic profile of a patient.

Method of GLIF Model Construction and Implementation.

Knowledge acquired in medicine is possible to represent by medical guidelines. The most important and nowadays mostly used for formalisation of guidelines is the GLIF (Guideline Interchange Format) model. Final model can be coded in XML (eXtensible Markup Language). Some situations can be modelled only very hard or no ways in a practice use. This paper describes a method of GLIF model construction and implementation in XML. The method specializes in risks of whole process and tries to find a solution to problematical model situations. The GLIF model universality is kept for any medical guidelines.

Multi-fetal pregnancy reduction does not influence perinatal results in twin pregnancies.

The objective of this study was to compare perinatal results in multifetal pregnancies where the reduction to twins was performed with non-reduced twin pregnancies. Perinatal results in 99 sets of twins after transabdominal multifetal pregnancy reduction of triple and higher-order multiple pregnancies performed in a single center were compared with a control group consisting of 151 twin pregnancies conceived in the same time period after infertility treatment, which were not a result of reduction. The main outcome measures were length of pregnancies, weight of the newborns, percentage of miscarriages and the mode of the delivery were analyzed. No significant difference could be found at a 5% level of significance regarding the average duration of pregnancy or average weight of the twins. Fisher test on 5% significance level did not ascertain any significant difference in the probability of miscarriage between the group with reduction (5.26%) and the group without reduction (12.84%). At a 5% level of statistical significance, no significant difference in probability of perinatal death of the fetus or delivery of a stillborn fetus was found. The percentage of cesarean sections did not differ significantly in both groups. The analysis of both groups demonstrated that reduction of multifetal pregnancies to twins may not influence perinatal results in comparison to twin pregnancies where reduction was not performed.

Evaluation of data in cardiology by new software tools.

We describe an analysis of cardiology data using new software tools. First we use data of 1417 men aged 40 to 50 years living in the center of Prague gathered in the twenty years lasting longitudinal study. We show some results of application of the programs E.T. (Epidemiology Tools) and CORE (COnstitution and REduction of data). Second we show the way of application of the program HYPERTENSION, that supports decision making in therapy of arterial hypertension.

Medical informatics--an interdisciplinary approach.

Medical informatics is the first science that has incorporated all traditional medical disciplines. That progress is possible due to fast development of new technologies, particularly in informatics, as well as due to common needs of all medical disciplines. Information is crucial component of all investigations, and that is why the main goal should be incorporation of information technologies into medical practice. Due to its complexity, medical informatics is interdisciplinary. That is evident through incorporation of different methods, principles, techniques etc., that are used while carrying out the tasks in the field of medical informatics. Implementation of modern technologies, particularly information technologies in medicine, will enable faster data processing; it will reduce expenditures in all branches of medicine (statistics, documentation or documentary etc.). Thus, medical staff will have much more time to devote themselves to primary tasks.
