Rat striatal cation shifts reflecting hypoxic-ischemic damage can be predicted by on-line impedance measurements.

We investigated the earliest time at which irreversible damage takes place after hypoxia-ischemia in the Levine preparation of rats. In 60 rats anesthetized with chloral hydrate and maintained at one of three body temperatures, we unilaterally ligated the left common carotid artery and placed electrodes in the striatum to measure impedance (reflecting the extracellular space) during hypoxia, recovery, and/or cardiac arrest. We measured blood gases and pH at regular intervals during hypoxia in 47 rats and assessed blood-brain barrier function with Evans blue and tissue damage using Na+:K+ ratios. Shortly after hypoxia, impedance normalized in 24 rats without brain damage (normal Na+:K+ ratios, 4 hours of recovery). Sustained elevation of striatal impedance during recovery in six rats was related to an elevated Na+:K+ ratio and a disrupted blood-brain barrier. Damage was not obviously related to blood gases, pH, or the net reduction of the extracellular space during hypoxia. Hypothermia in 17 rats prevented impedance changes, and no striatal damage was found. Thus, irreversible brain damage very likely occurs during or very shortly after hypoxia. Persistent reduction of the extracellular space indicates tissue damage and can be used to monitor potential in vivo therapeutic measures.

Sensitization to methylenedianiline and para-structures.

A toxic-allergic skin reaction of the face, neck and arms due to accidental contact with methylenedianiline (syn: 4-4'-diaminodiphenylmethane) is reported in a cleaner at a chemical plant. Patch tests revealed sensitization to chemicals of the para-group. Earlier exposure to it was unlikely. Previous sensitization to related chemicals and possible cross-sensitization was considered to be the primary cause of the allergy and the clinical manifestations.

An alarm procedure.

Each disaster procedure is initiated by an alarm scheme, alerting all involved personnel, directly or indirectly in this procedure. Certain mathematical principles can be applied to calculate the number of people who can be reached within a fixed period of time, or to calculate the time needed to alert a fixed number of people. With this methodology a formidable amount of people can be reached within a few minutes. The advantages and disadvantages are further described.

Progressive medical care in disaster situations: a critical evaluation of the current situation in the Netherlands.

An overview of the plan for medical response in the event of a disaster in the Netherlands is presented. An idealized evaluation of the response capability, as well as the probable actual ability, is reviewed. The plan involves a response to the disaster site by teams comprising a surgeon, anesthesiologist, and two specially trained nurses from 1 or many of 20 participating hospitals as well as an ambulance response from associated district ambulance posts. An overview of disaster management in general is presented.

The operated stomach: a premalignant condition? A prospective endoscopic follow-up study.

2633 consecutive patients in whom a gastric operation had been performed between 1931 and 1960 were selected for an ongoing prospective follow-up study. In 741 already deceased patients mortality due to gastric cancer was 5.13%. 504 asymptomatic postgastrectomy patients participated in an endoscopic-bioptic screening program. Up to now, 10 stump cancers have been detected. Seven had a radical curative resection, and all are still alive. In five of them the cancer was limited to the mucosa and not visible at endoscopy. Gastric dysplasia was found in 70 patients. In three patients with severe dysplasia carcinoma was detected during follow-up study. In 22 patients with prior mild or moderate dysplasia no progression could be demonstrated. Cancer in postgastrectomy patients is estimated to be at least twice as high as in an age-matched Dutch control population with intact stomach. The risk depends on the postoperative interval and the age at which the initial operation was performed. In general, patients from the age of 50 onwards or more than 10-15 years after initial surgery are at risk.

The definition and classification of disasters.

The universe, we are told, began with a big bang, and ever since, nature has provided a series of unexpected bangs and calamities of one type or another. Since the advent of the industrial revolution, man has been more or less controlling larger and larger amounts of energy. World population has rocketed. The interaction of crowded humanity and escalating energy has added a series of man-made disasters to the continuing natural calamities.

Determinants of total and high density lipoprotein cholesterol in boys from Finland, The Netherlands, Italy, the Philippines and Ghana with special reference to diet.

We have studied the determinants of total and high density lipoprotein (HDL) cholesterol in young boys from five countries characterized by different lifestyles, dietary consumption profiles and mortality rates from coronary heart disease. All measurements including the estimation of dietary intake, physical activity, body mass index and the concentrations of total and HDL cholesterol were carefully standardized. The mean concentrations of total and HDL cholesterol were higher in the European boys (4.1-4.9 mmol/l and 1.45-1.57 mmol/l, respectively) than in the boys from Ghana and the Philippines (3.3-3.8 mmol/l and 0.93-1.10 mmol/l, respectively). A positive correlation was found between the concentration of total cholesterol and the intake of saturated fatty acids in four out of five countries. The concentration of HDL cholesterol was also related to various dietary variables in some of the groups. Using the regression coefficients from a multiple regression analysis on the pooled data, it could be calculated that on average 24 per cent of the inter-country differences in the levels of total cholesterol is explained by differences in the intakes of saturated fatty acids. Differences between the groups of the different countries in the intakes of carbohydrate explained on average 29 per cent of the differences in the concentrations of HDL cholesterol. The results obtained support the hypothesis that higher concentrations of total and HDL cholesterol are associated with western types of diets rich in saturated fatty acids and relatively poor in complex carbohydrates.

Urinary hydroxyproline as a measure in fracture healing.

The determination of any pattern in urinary total OH-proline following bone fractures has been the purpose of this investigation. This was carried out in a group of 11 patients with major fractures, a group of 8 patients with minor fractures and a control group of 10 healthy laboratory technicians. Indeed, some patterns could be detected. In patients with major fractures a considerable elevation of urinary total OH-proline was found which was correlated to the size and number of fractures and the healing tendency of the fracture. Normal to only slightly elevated urinary total OH-proline values during the course of the healing process of major fractures could perhaps indicate delayed union, or even malunion. Since fracture healing is usually assessed by X-ray examination and clinical experience, this study could provide a more accurate measure of the healing process of fractures.

Chemocommunication among domestic cats, mediated by the olfactory and vomeronasal senses. I. Chemocommunication.

Chemocommunication functions are known in many social mammals, such as gerbils, hamsters, and deer. But also mammals known as at least partly solitary do communicate by chemical signals. The existence and use of pheromones in domestic cats was the subject of this study. It turned out that male and female cats spent much time in exploring their own and foreigners' urine and skin gland secretion rubbed off on several objects. Male receivers performed sniffing and flehmen more frequently than females on the same urine, whereas female cats paid more attention to skin gland secretion. Both urine and cheek gland secretion of the female donor appeared to contain pheromones which inform the tomcat about her hormonal phase.
