Suicide attempts in bipolar patients.

BACKGROUND: Between 25% to 50% of patients with bipolar disorder make suicide attempts during their lives, but there are some controversies about factors related to suicide attempts in this group of patients. The aim of this study is to investigate the association between suicide attempts and the predictive factors previously described in the literature. METHOD: The sample included all 169 patients with DSM-III-R bipolar I disorder identified in a delimited area (northern Spain). Sociodemographic, clinical, and family history variables measured by Research Diagnostic Criteria-Family History were analyzed. Significant variables were introduced in a logistic regression analysis to control for the effects of other variables. RESULTS: There were 56 patients (33%) who had one or more suicide attempts. Early age at onset, history of hospital admission during depressive episodes, drug abuse, and family history were significantly associated with suicide in the univariate analyses (p < .05). A much higher proportion of patients with onset at or before 25 years of age than patients with onset after 25 years of age attempted suicide (23% vs. 10%). The age at onset was no longer significant after controlling for the other 3 variables included in the logistic regression analysis. CONCLUSION: Suicide attempts are highly prevalent in bipolar patients and are related to drug abuse, family history of affective disorders, and severe depressive episodes. This study suggests that the risk of suicide in patients with an early age at onset could reflect other variables such as drug abuse, a history of hospital admissions for depression, or family history.

[The first description of infection by Actinomyces radingae].

BACKGROUND: To communicate at clinical value of an isolation of Actinomyces radingae from an ulcerated lesion the external supra-malleolus region. MATERIAL AND METHODS: We proceed to study the clinical and bacteriological value of an isolated aerobic gram positive rod in pure culture obtained from an ulcerated lesion ocated at the external supra-malleolus region of a 92 years old woman in whom there was later evidence of an epidermoid carcinoma. Surgical and antibiotic treatment were needed. The biochemistry characterization of the isolated was carried out according to the schemes of Weaver-Hollis and Funke et al, as well as the characterization of the fat acids of their cellular wall by gas-liquid chromatography. RESULTS: The isolated was identified as Actinomyces radingae. The patient follow-up was satisfactory after the treatment. CONCLUSION: The first infection description by Actinomyces radingae is reported wherein its pathological value is evident as it is isolated in pure culture.

[Malignant melanoma of the nasal cavities. Our experience from two cases].

Malignant Melanoma of the nasal cavity is a rare occurrence. Its clinical manifestations at beginning are non-specific and its macroscopical appearance is variable due to the several pigmentation tonalities which render sometimes difficult the diagnosis. The confirmed diagnosis result from the histopathologic study of the tissue. The treatment is surgery, when the cases are operable, and radiotherapy, chemotherapy and immunotherapy either as adjuvant or as palliative management in inoperable cases.

Why do some psychiatric patients somatize?

A study was made of a series of 139 outpatients referred by the medical and surgical services of a general hospital for evaluation by the psychiatry unit. In accordance with established criteria, this population was divided into somatizers (56) and non-somatizers (75), and the socio-demographic and clinical characteristics of both groups were comparatively analyzed. The results show that the group of somatizers was younger, had more histrionic personality traits and more stress factors related with alterations in interpersonal relationships or death or disease of relatives. It is emphasized that somatization is poorly known by psychiatrists--whose diagnostic criteria practically omit these aspects--and by other physicians, in spite of its importance and frequency.

[Inter-observer reliability of negative schizophrenic symptoms].

The interrater reliability of negative symptoms was measured in 30 DSM-III-R schizophrenic patients. The negative symptoms were assessed using the Scale of Emotional Blunting (SEB) of Abrams y Taylor, the Scale for the Assessment of Negative Symptoms (SANS) of Andreasen and the negative symptoms of Landmark's Manual for the Assessment of Schizophrenia (MAE). The interrater reliability of negative symptoms measured using the SEB and SANS scales was good or excellent, while the reliability of the MAE was moderate. The results point out that the negative schizophrenic symptoms can be reliably measured when operative criteria are employed.

[Neuropsychological findings in a case of simulation].

The case of a malingerer is reported with special emphasis on the neuropsychological findings. The diagnosis of malingering is a challenge to medicine, and neuropsychology can come to be an efficient instrument, once greater experience is achieved, and the results can be systematized and generalized. Nevertheless, it must be kept in mind that the manifestations are specially subject to sociocultural, geographic and temporal factors.
