[Anticardiolipin syndrome with only skin involvement].

In anticardiolipin syndrome (ACS) a typical antibody constellation is associated with thrombotic and hematologic disorders. Furthermore, recurrent abortion, cerebral ischemia and different skin disorders occur. We report the case of a 29-year-old female suffering for the first time from painful, necrotic deep ulcers on the upper and lower legs and livedo racemosa on the arms as a rare example of a merely cutaneous manifestation of ACS with no demonstrable underlying disease. After systemic treatment with high-dose methylprednisolone, azathioprin and hydrocolloid dressings, healing of the scar tissue occurred. Simultaneously, a maintenance dose of acetylsalicylic acid (100 mg/day) was administered. So far, neither cutaneous relapse nor other signs of ACS have occurred.

[Behavior and attitude regarding skin cancer prevention and early detection in relation to current knowledge--studies of visitors to an industrial fair].

During the course of the 42nd International Trade Fair 1990 in Munich a total of 1245 visitors were interrogated with regard to their education in the early recognition of skin cancer, and their behaviour in respect of exposure to sunlight. The same persons were also subjected to melanoma screening, both with regard to special assessment of pigment lesions and inspection of the entire skin. The questionnaire showed that women were better informed than men. Approximately 87% of the questioned women had heard of the problems of melanoma, whereas only about 55% of the men belonged to the informed group. The knowledge of cancer education proved to be above average in the age group between 30 and 60 years. Persons with ample knowledge on melanoma education demonstrated more motivation to take part in cancer prevention examinations and also in examinations relating to other organ systems. Thus, the necessity of cancer screening examinations was substantially more often a matter of course among women than among men. The occurrence of skin cancer on the own body as well as its occurrence in members of one's own family does not guarantee essential knowledge on melanoma education, although in the informed group the percentage of persons directly or indirectly affected by melanoma, was higher. With regard to the behaviour towards sunlight, young people dealt more cautiously and consciously with the sun than those aged 45 and over who had exposed themselves to sunlight without exercising any caution, especially in the past.(ABSTRACT TRUNCATED AT 250 WORDS)

Frequency, spectrum and occupational relevance of type IV allergies to rubber chemicals.

3851 consecutive patients patch tested between January 1985 and March 1990 have been analysed for rubber allergies. The incidence of rubber allergy was 3.8% (n = 145). In 80/145 patients (55%), the source of rubber sensitization was occupational, 67 of whom (84%) had acquired allergy from wearing rubber gloves at work. Most of them (36%) were employed in the health services. The most commonly positive rubber-mix in this group was thiuram-mix (72%) followed by carba-mix (25%). 13/80 patients (16%) had occupational rubber allergy from industrial rubber products other than gloves. Patch tests revealed thiuram-mix (62%) as the most commonly positive rubber-mix but, in contrast to the group with glove-induced rubber allergy, black-rubber-mix came second (38%). In 47/145 patients (32%), the source of rubber sensitization was non-occupational; in 18/145 (13%) the origin remained unknown.

Cell cycle kinetics of human anagen scalp hair bulbs in thyroid disorders determined by DNA flow cytometry.

In both, 6 hyperthyroid and 6 hypothyroid patients as well as 10 healthy volunteers, cell cycle kinetics of dissected anagen scalp hair bulbs were determined by means of DNA flow cytometry (DNA-FCM). Compared with the healthy control group in patients with thyroid disorders striking differences of cell kinetic data were evaluated. In hyperthyroidism a significant increase (30%) and in hypothyroidism a significant decrease (15%) of S and G2+M phase cell percentages was found. The proliferation index (S+G2+M %) calculated revealed similar results. A correlation between the height of S phase percentages and plasma T3 levels was recognizable but could not be proven statistically. By means of DNA-FCM the study demonstrates for the first time the influence of thyroid hormones on in vivo cell cycle kinetics of human scalp hair bulbs.

In vitro effects of testosterone, dihydrotestosterone and estradiol on cell growth of human hair bulb papilla cells and hair root sheath fibroblasts.

The influence of testosterone, dihydrotestosterone (each 10 ng/ml up to 300 ng/ml) and estradiol (0.2 ng/ml up to 10 ng/ml) on the growth behaviour (cell count, [3H]thymidine uptake, cell doubling time) of subcultured human hair bulb papilla cells and hair root sheath fibroblasts was studied. Papilla cells and root sheath fibroblasts were isolated by microdissection from the same anagen hair follicles obtained from biopsies of androgen-sensitive scalp regions in 6 healthy male subjects. Dihydrotestosterone and testosterone concentrations above 30 ng/ml significantly reduced the growth of both cell types; lower doses had no effect. Estradiol had no distinct influence on the growth curves of either cell type up to 10 ng/ml, whereas higher concentrations significantly increased the growth of both cell types as shown by [3H]thymidine uptake. Papilla cells reacted more sensitively than root sheath fibroblast to dihydrotestosterone and testosterone, as shown by the growth curves, [3H]thymidine uptake, and cell doubling time.

[Paraneoplastic Bazex acrokeratosis--disease course in palliative therapy of cancer of the base of the tongue].

A 62-year old quarryman developed distinct, erythematosquamous and keratotic lesions associated with subungual hyperkeratosis and nail detachment on all fingertips within 9 months. Subsequently, scaly erythema of the nose and of the auricles appeared. Clinically paraneoplastic acrokeratosis Bazex (APB) was diagnosed. Thorough endoscopic examination revealed an asymptomatic squamous cell carcinoma at the base of the tongue, which had already metastasized to the regional lymph nodes. Impressive regression of most of the relevant skin symptoms occurred during combined neck dissection and radiotherapy. A survey of recent reports on APB in the literature is given.

[Proliferation behavior of the anagen hair bulb in androgenetic alopecia before and after local administration of 17 alpha- propylmesterolone--DNA flow cytophotometry studies].

10 male test persons with androgenetic alopecia (Hamilton stage IV) were topically treated with an alcoholic 17 alpha-propylmesterolone solution (PM, 3%) for 3 months. Before and after treatment, we took deep biopsies from the fronto-temporal scalp region. Microsurgically, we isolated anagen hair bulbs, connective tissue with epidermis, and fat from the biopsy samples in order to determine cell cycle kinetics by means of DNA-flow cytometry. By comparison, cell cycle kinetics of the pre- and post-treatment samples did not show any significant changes with regard to connective tissue and fat, whereas in anagen hair bulbs, we found a significant increase of both S-phase cells (%) and the proliferation index (%). These results imply that PM, if topically applied, enhances the proliferative activity of hair matrix cells.

Metabolism of glutamine in lymphocytes.

Pathways of glutamine metabolism in resting and proliferating rat thymocytes and established human T- and B-lymphoblastoid cell lines were evaluated by in vitro incubations of freshly prepared or cultured cells for one to two hours with [U14C]glutamine. Complete recovery of glutamine carbons utilized in products allowed quantification of the pathways of glutamine metabolism under the experimental conditions. Partial oxidation of glutamine via 2-oxoglutarate in a truncated citric acid cycle to CO2 and oxaloacetate, which then was converted to aspartate, accounted for 76% and 69%, respectively, of the glutamine metabolized beyond the stage of glutamate by resting and proliferating thymocytes. Similar results were obtained with the lymphoblastoid T- and B-cell lines. Complete oxidation to CO2 in the citric acid cycle via 2-oxoglutarate dehydrogenase and isocitrate dehydrogenase accounted for only 25% and 7%, respectively. In proliferating cells a substantial amount of glutamine carbons was also recovered in pyruvate, alanine, and especially lactate. The main route of glutamine and glutamate entrance into the citric acid cycle via 2-oxoglutarate in lymphocytes appears to be transamination by aspartate aminotransferase rather than oxidative deamination by glutamate dehydrogenase. In the presence of glucose as a second substrate, glutamine utilization and aspartate formation markedly decreased, but complete oxidation of glutamine carbons to CO2 increased to 37% and 23%, respectively, in resting and proliferating cells. The dipeptide, glycyl-L-glutamine, which is more stable than free glutamine, can substitute for glutamine in thymocyte cultures at higher concentrations.

Delayed seroconversion to HIV in a hemophiliac.

In a 14-year-old boy with severe hemophilia B, HIV seroconversion was observed about 2 years after substitution therapy had been changed to heat-treated PPSB concentrates. This shows that HIV-antibodies may appear in hemophiliacs after time periods longer than generally assumed.

[Interpretation of varicella serology in pregnancy].

Chickenpox during pregnancy may result in a severe, in some cases fatal, generalized infection of the newborn. Pregnant women with unknown Varicella-Zoster-virus (VZV) immunity, who have had contact with an individual infected by chickenpox, should be tested immediately for specific antibodies to VZV. The interpretation of test results may be difficult under certain circumstances. This problem is discussed using individual case studies.

[Serologic AIDS diagnosis with polypeptides obtained by genetic technics of the human immunodeficiency virus (HIV-1)].

Serologic testing for human immunodeficiency virus type 1 (HIV-1) is currently based on enzyme linked immunosorbent assay (ELISA) as screening method. Positive ELISA-results have to be confirmed by at least one second procedure such as Western blotting or immunofluorescence. To obtain new diagnostic reagents for confirmatory testing, we expressed viral antigens in procaryotic systems. Peptides representing epitopes of structural core (gag)- and envelope (env)-proteins of HIV were produced in E. coli as stable immunogenic beta-galactosidase fusion proteins. Recombinant proteins were taken for immunoblot-assays. The results of Western blotting with those fusion proteins were in general comparable with conventional ELISA, immunofluorescence, immunoblot with cell-culture derived virus and commercially available ELISA tests based on recombinant proteins. Immunoblots using recombinant transmembrane protein (gp41) derived polypeptide were more sensitive than the conventional procedure with purified virion proteins. Western blotting with recombinant fusionproteins provide reliable and inexpensive serodiagnostics without handling of infectious cell cultures.
