[Transformation of general measures by patients in heart failure].

BACKGROUND AND OBJECTIVE: Non-pharmacological treatment (NPT) and immunization with influenzal and pneumococcal vaccines are an integral part in the management of heart failure (HF). However, few data are available about the implementation of NPT in real life. The aim of this study was to analyse long-term changes in behavior and the use of self-care strategies by patients with HF. PATIENTS AND METHODS: 91 of 151 patients in HF (77 men, 14 women; mean age 68) at the Göttingen University Hospital were recruited to the study. Implementation of NPT was assessed during the hospital stay and one year after discharge by standardized interviews. Additionally, patients were also asked about their preferred sources of information about HF. RESULTS: Compared with the baseline data, NPT were more frequently named after one year (significant increase for "See the doctor in case of weight gain" [24.3% vs. 57.1%; p<0.001] and "Dietary salt restriction" [41.4% vs. 70.0%; p<0.001]. Patients who had been readmitted reported NPT more frequently than those who had not previously been admitted to hospital (3.6 vs. 3.1, n.s). The majority of patients considered general practitioners to be the most important source for information on HF. CONCLUSIONS: One year after discharge more patients had become aware of NPT. On the whole lifestyle modification is still relatively uncommon. General practitioners are seen as the most important informants on the management of HF and could use this trust to promote greater implementation of NPT by their patients.

[Modular training in practical medicine: electronic evaluation of student education in general practice].

Effective from spring 2004, new regulations for undergraduate medical education in Germany require a two-week practical training in general practice. Similar to other forms of medical education, this practical training should be regularly evaluated by students. With regard to special conditions of the training, we preferred a web based evaluation. Since adequate models were not available, we designed, implemented and tested an electronic way of evaluation. The following aspects turned out to be of special importance: teamwork, time, data protection and cost. Meanwhile, the evaluation is established and still accessible as demo-version for visitors of the home page. This electronic evaluation of medical training in general practice is highly appropriate for a timely evaluation allowing us to obtain a comparison between students' expectations and actual experience as well as a continuous supervision and to provide feedback to the participating practices. This is an important step for quality assurance of medical education in practices inside and outside the university.

[Guidelines for back pain].

Chronic back pain is one of our society's most important health problems, causing long periods of sick leave and early retirements. Recently the German Society of General Practice and Family Medicine (DEGAM) published an evidence-based guideline for low back pain. It has been developed according to the 10-step concept of guideline development of the DEGAM, aiming for early detection of complicated clinical conditions, avoidance of unnecessary diagnostic tests and prevention of chronicity. Complicated, uncomplicated and radicular pain are defined by patient history and a short clinical examination. Imaging and further diagnostic tests are reserved for patients at risk. Basic therapy consists of structured advice. The aim of the guideline is to get patients back to their usual activity supported by effective pain relief. Psychosocial factors which are important for the prognosis should be evaluated already during the first consultation. Manual therapy might help in acute pain. Patients with persistent pain symptoms and long periods of sick leave should be transferred to multiprofessional management including pain treatment, behavioural therapy and physiotherapy. A randomised controlled trial has been set up to study the efficacy of guideline implementation.
