A multistate outbreak of Salmonella enterica serotype typhimurium infection linked to raw milk consumption--Ohio, 2003.

In December 2002, the Ohio Department of Health was notified of two children with Salmonella infection. Both had a history of drinking raw milk from a combination dairy-restaurant-petting zoo (dairy). The dairy was the only establishment in Ohio licensed to sell raw milk and reported 1.35 million visitors annually. We investigated to determine the extent of the outbreak and identify illness risk factors. A case patient was any person with pulsed-field gel electrophoresis-matched Salmonella enterica serotype Typhimurium from 30 November 2002 to 18 February 2003. Sixty-two met the confirmed case definition. Forty dairy case patient patrons were included in a case-control study; 56 controls were their well meal companions. Consumption of raw milk was found to be associated with illness (odds ratio, 45.1; 95% confidence interval, 8.8 to 311.9). The dairy discontinued selling raw milk. Because 27 other states still allow the sale of raw milk, awareness of the hazards of its consumption should be raised and relevant regulations carefully reviewed.

[Infectious diseases in Poland in 2000].

The decreasing tendency in incidence of infectious diseases observed in Poland in previous years as compared with 2000 has weakened or stopped. Increase in the incidence of selected infectious diseases can be linked with the improvement of surveillance resulting from the better diagnostics and greater attention paid to these diseases (including borreliosis, salmonella, and Haemophilus influenzae meningitis). Between 1999 and 2000, the most intense decrease in the number of mumps, measles, and scarlet fever cases as an effect of the end of epidemics was observed. At the same time increase in the number of pertussis, rubella, chickenpox, and meningitis cases was noticed. In 2000, the first case of human rabies since 1986 has been reported. In 2000, compared with 1999, among all notified deaths percentage of deaths attributed to infectious diseases (0.83%) and infectious diseases death rate (0.79 per 10,000) were slightly higher and were the highest in the last decade. As in 1999 the observed increase was effect of the influenza deaths increase (358 deaths, mortality 0.022%). The main disease causing the largest number of deaths, as in previous years, was tuberculosis (36.5% of total infectious diseases deaths).

[Hepatitis C in Poland in 2000].

In 2000, a total of 2,086 (5.4 per 100,000 population) acute and chronic hepatitis C cases were noted. The differences in incidence between groups were observed: incidence in urban areas (7.2 per 100,000) was 190% those in rural areas (2.5 per 100,000) and among men (6.4/100,000) was 40% those among women (4.5/100,000). Hepatitis C cases aged 40-49 constituted the most predominant group (475 cases, 22.8%). Because of development of diagnosis and reporting presented data can be a subject to the information bias and thus should be taken cautiously.

Raw eggs--lessons learned from an outbreak of Salmonella serotype enteritidis infections associated with meringue pie.

OBJECTIVES: We investigated a salmonellosis outbreak related to a cafeteria to determine its extent and to identify illness risk factors. METHODS: A case was defined as isolation of Salmonella Group D or serotype Enteritidis from the stool of a person who ate at the cafeteria during June 22-July 10, 2003, and developed diarrhea in 3 days or less. Food histories of case patients (n = 11) were compared with those of their well meal companions (n = 16). RESULTS: Consumption of coconut meringue pie was associated with illness (odds ratio = 150.0; 95% confidence interval = 6.4-6901.4). Meringue was made with raw shell eggs and was baked to an internal temperature of 83 degrees F. CONCLUSIONS: Restaurant operators and public health officials should be alert for recipes containing raw shell eggs. Food service operators should use pasteurized egg products in meringue recipes if meringue will not be cooked to the required temperature of 155 degrees F. For clarification purposes, policy makers should consider adding "meringue" to the Food Safety Rule that lists foods in which pasteurized eggs should be substituted for raw shell eggs.

[Epidemiological methods used in studies in the prevalence of Tourette syndrome].

Tourette syndrome (TS) prevalence was studied since the early 80-ies. Its clinical course is characterised by co-occurrence of motor and vocal tics. Results of previous epidemiological studies were surprisingly divergent: the prevalence varied from 0.5 to 115 cases per 10,000 population. The disease previously recognised as extremely rare and severe is now considered as quite common, with often moderate course. Selected methods used in studies of TS prevalence and analysis of their possible impact on study results are presented. The studies were divided into 3 groups: studies of the hospitalised population, large-scale screenings and studies involving school population, basing on characteristic and size of population, methods of selection of subjects, diagnostic and screening methods used. Studies of the hospitalised population involved patients with most severe symptoms, in different age groups, different methods of final diagnosis confirmation were used. TS prevalence varied from 0.5 up to 15 cases per 10,000 population. Procedures used in large-scale screening studies made possible the elimination of potential selection bias. Large populations were studied using transparent and repetitive confirmation of diagnoses. Their validity was additionally checked in parallel validity studies. TS prevalence was in the range 4.3 to 10 cases per 10,000 population. The highest TS prevalence was obtained in studies involving schoolchildren. Data were gathered from multiple sources: from parents, teachers and children, as well as from classroom observation. Diagnoses were made by experienced clinicians. TS prevalence obtained in school population studies was between 36.2 up to 115 per 10,000 population.
