[The effect of levothyroxine on the proliferative activity of the thyroid follicular cells].

INTRODUCTION: Pharmacological treatment of non-toxic nodular goitre with levothyroxine has caused a discussion about its effectiveness and safety. The aim of the study was to examine the influence of levothyroxine on the proliferative activity of the thyroid follicular cells. MATERIAL AND METHODS: Cytological material was obtained from 32 euthyroid females, aged 34-69 years, by USG-guided Fine-Needle Aspiration Biopsy (FNAB), before and after 6 months treatment with levothyroxine. The patients were divided into 2 age groups: up to 45 years (14 patients) and above 45 years (18 females). Proliferative activity of the follicular cells was assessed by the proliferative index (PI), representing the percentage of follicular cells with positive reaction with the antibody Ki-67. Microscopic evaluation was carried out using morphometric computer system MicroImage InCD UDF (Olympus). RESULTS: After 6-months of treatment IP values decreased significantly in both groups studied (8.59 +/- 3.07 vs 6.51 +/- +/- 1.91; p < 0.001 in females aged < 45 years and 7.72 +/- 1.83 vs 5.09 +/- 1.51; p < 0.001 in the older group). CONCLUSION: Our results indicate that levothyroxine has an influence on the proliferative activity of the thyroid follicular cells, particularly in post-menopausal women.

[The apoptosis of hepatic tissues in hepatitis C].

OBJECTIVE: The purpose of the study was the evaluation of p53 and bcl2 protein expression on intrahepatic hepatocytes in chronic hepatitis C before and after the treatment with interferon alpha 2b (IFNa2b) combined with ribavirin (RBV). METHODS: using immunohistochemical method, the percentage of cells with p53 and bcI2 protein expression was assessed in hepatic tissues of 30 patients suffering from chronic hepatitis C. The examinations were performed in hepatic bioptates before IFNa2b treatment (9MU/week/48 weeks) combined with RBV (1-1.2g/24hrs/48 weeks) and after the therapy. The parameters were analyzed in the responders group (R) and non-responders (NR). RESULTS: The highest increase in p53 protein expression was observed in tissues in which interferon alpha decreased inflammatory activity and fibrosis. The elimination of HCV infection did not have any influence on p53 protein expression. Diminishing of p53 positive hepatocyte percentage was revealed in tissues where inflammatory activity and fibrosis did not change before and after the treatment or where histological damage was intensified. IFNa2b+RBV therapy diminished bcl2 positive cell percentage, specifically in patients with eliminated HCV. CONCLUSIONS: Tissue p53 protein expression increase and bcl-2 decrease can be a prognostic marker as far as a positive effect of IFNa2b+RBV treatment and HCV elimination are concerned. It can occur either due to the elevation of growth factors inducing regeneration or HCV-infected hepatocyte apoptosis. It was shown that IFNa2b+RBV therapy decreases bcl2 protein expression, which can make hepatocyte apoptosis and HCV elimination possible.

The influence of the vascularisation of the follicular thyroid nodules on the proliferative activity of the follicular cells.

The enlargement of the thyroid is, in general, benign in origin and due to nodular goitre. Follicular cellular proliferation of thyroid nodules has been increasingly observed recently. With fine needle aspiration biopsy (FNAB) this is classified as a follicular tumour. These lesions present various patterns of vascularisation in ultrasound examination. The aim of the study was to establish the relation between follicular nodule vascularisation and the proliferative activity of various types of follicular cell. According to the manner of proliferation, patients were divided into groups as follows: (I). patients with hyperplastic nodules (46 cases), (II). patients with follicular adenoma (42 cases), and (III). patients with follicular cancer (9 cases). In each case B-mode sonography, Power Doppler, sonographically guided FNAB (S-FNAB), morphological examination and morphometry were performed. The proliferative activity was detected with immunohistochemical methods (PCNA, Ki 67 and MPM2) to determine the so-called "proliferative index". The study revealed increased proliferative activity in tumours of malignant origin and increased vascularisation in coexistence with increased proliferation of the follicular cells. As assessed by Power Doppler, an increased flow pattern in the centre of the nodules correlates with increased proliferative activity. The results suggest that Power Doppler examination could be helpful in selecting nodules for FNAB, especially in multinodular goitre.

A morphometric study of nucleolar organiser regions in cervical intraepithelial neoplasia.

The study sought a correlation between the number of AgNOR granules and the degree of cervical intraepithelial neoplasia (CIN). Thirty-five sections (5 normal, 10 CIN1, 10 CIN2 and 10 CIN3) were subjected to retrospective analysis. The percentage of cells with 1, 2, 3, 4 and more AgNORs was calculated and the number of granules per 100 cells was counted. The number of cells containing single granules decreases. However, the number increases with CIN level when the cells contain 4 and more AgNORs. The number of granules per 100 cells also increases with the degree of CIN. It can be thus concluded that the number of cells with 4 and more AgNOR granules can serve as a CIN differentiation exponent.

Can transforming growth factor-beta1 and retinoids modify the activity of estradiol and antiestrogens in MCF-7 breast cancer cells?.

Retinoic acid and transforming growth factor-beta (TGF-beta) affect differentiation, proliferation and carcinogenesis of epithelial cells. The effect of both compounds on the proliferation of cells of the hormone sensitive human breast cancer cell line (ER+) MCF-7 was assessed in the presence of estradiol and tamoxifen. The assay was based on [3H]thymidine incorporation and the proliferative activity of PCNA- and Ki 67-positive cells. The apoptotic index and expression of the Bcl-2 and p53 antigens in MCF-7 cells were also determined. Exogenous TGF-beta1 added to the cell culture showed antiproliferative activity within the concentration range of 0.003-30 ng/ml. Irrespective of TGF-beta1 concentrations, a marked reduction in the stimulatory action of estradiol (10(-9) and 10(-8) M) was observed whereas in combination with tamoxifen (10(-7) and 10(-6) M) only 30 ng/ml TGF-beta1 caused a statistically significant reduction to approximately 30% of the proliferative cells. In further experiments we examined the effect of exposure of breast cancer cells to retinoids in combination with TGF-beta1. The incorporation of [3H]thymidine into MCF-7 cells was inhibited to 52 +/- 19% (control =100%) by 3 ng/ml TGF-beta1, and this dose was used throughout. It was found that addition of TGF-beta1 and isotretinoin to the culture did not decrease proliferation, while TGF-beta1 and tretinoin at low concentrations (3 x 10(-8) and 3 x 10(-7) M) reduced the percentage of proliferating cells by approximately 30% (67+/-8% and 67+/-5%, P<0.05 compared to values in the tretinoin group). Both retinoids also led to a statistically significant decrease in the stimulatory effect of 10(-9) M estradiol, attenuated by TGF-beta1. In addition, the retinoids in combination with TGF-beta1 and tamoxifen (10(-6) M) caused a further reduction in the percentage of proliferating cells. Immunocytochemical analysis showed that all the examined compounds gave a statistically significant reduction in the percentage of cells with a positive reaction to PCNA and Ki 67 antigen. TGF-beta1, isotretinoin and tretinoin added to the culture resulted in the lowest percentage of PCNA positive cells. However, the lowest fraction of Ki 67 positive cells was observed after addition of isotretinoin. The obtained results also confirm the fact that the well-known regulatory proteins Bcl-2 and p53 play an important role in the regulation of apoptosis in the MCF-7 cell line, with lowered Bcl-2 expression accompanying easier apoptotic induction. The majority of the examined compounds act via the p53 pathway although some bypass this important proapoptotic factor.

Assessment of proliferative activity of thyroid Hürthle cell tumors using PCNA, Ki-67 and AgNOR methods.

We have undertaken an attempt to compare the application efficacy of the proliferative activity markers in differential diagnosis of thyroid Hürthle cell tumors (HCT) using the PCNA and Ki-67 labeling and AgNOR visualisation techniques. The present work is a retrospective analysis of 78 Hürthle cell tumors: 20 Hürthle cell carcinomas (HCC), 32 Hürthle cell adenomas (HCA) and 26 hyperplastic nodules with Hurthle cell metaplasia (HCM). Five microm sections were stained according to AgNOR technique and labeled with antibodies against PCNA and Ki-67. AgNOR dot count in the nucleus and proliferative index (PI - percentage of cells expressing PCNA and Ki-67) in randomly chosen nuclei (100 in case of AgNOR and over 1000 in case of PI) were evaluated in each slide. The mean values of AgNOR dot count, PI-PCNA and PI-Ki-67 in HCC, HCA and HCM were respectively: 5.1, 61.3 and 54.9; 3.4, 42.4 and 38.6 and 2.5, 39.3 and 34.3. Statistically significant difference was found in all the proliferative activity markers between malignant and benign tumors: HCC:HCA (p<0.01) and HCC:HCM (p<0.001). There was no statistically significant difference between HCA and HCM.

[The formation of new blood vessels in coronary artery disease: where we are now].

New blood vessel formation is of great importance to clinicians and researchers because of its participation in many diseases. Scientists are trying to exploit the mechanisms of vasculogenesis, angiogenesis, and arteriogenesis in the treatment of coronary artery disease. These mechanisms are employed to improve myocardium vascularization in patients where both pharmacological therapy and available revascularization procedures (PTCA, CABG) are not effective. In such cases, factors including vascular endothelial growth factor (VEGF), acid fibroblast growth factor (aFGF), basic fibroblast growth factor (bFGF), granulocyte/macrophage colony-stimulating factor (GM-CSF), and granulocyte colony stimulating factor (G-CSF), as well as stem cells, have been applied so far. This article is a review of the current knowledge about new blood vessel formation, especially in ischemic myocardium. We have also analyzed alternative strategies of treating coronary artery disease using the mechanisms of angiogenesis, arteriogenesis, and vasculogenesis.

The effect of doxorubicin and retinoids on proliferation, necrosis and apoptosis in MCF-7 breast cancer cells.

Doxorubicin (Adriamycin) is the most active drug in the treatment of breast cancer. The aim of this study was to investigate the interaction of doxorubicin and retinoids in the inhibition of proliferation of hormone sensitive (ER+) human breast cancer cell line MCF-7 and to find out whether this combination can result in the enhancement of its therapeutic effect. As a comparison we also used estradiol and tamoxifen. We also made an attempt to elucidate the effect of these compounds on the stimulation of the apoptotic pathway in breast cancer cells. Cell proliferation in a 24-hour culture was assessed by [3H] thymidine incorporation into cancer cells and by immunocytochemical analysis of cellular cycle-related PCNA and Ki-67 antigens expression, after the incubation of the cell culture with 10, 20 and 50 nM doxorubicin (DOX), 2 nM estradiol (E2), 10 microM tamoxifen (TAM) and 1 nM, 0.01, 0.1, 1 and 10 microM of all-trans retinoid acid (ATRA). The assessment of cell viability and analysis of apoptotic and necrotic cells were performed after the 72-hour incubation of the culture with the examined substances and following apoptosis induction using acridine orange and ethidine bromide. Of the doxorubicin concentrations used in the study, 20 nM inhibited thymidine incorporation to 84.83 +/- 10.00% (control=100%). In the same culture conditions, 2 nM E2 stimulated cancer cells to 157.09 +/- 8.84%. Concentrations of 10 microM TAM and 10 microM ATRA inhibited the proliferation to 63.16 +/- 7.85% and 52.19 +/- 3.21%, respectively. A statistically significant reduction of these values was observed when 20 nM DOX was added to medium with E2 - 39.24 +/- 7.6%, TAM - 48.34 +/- 2.05% and ATRA - 21.98 +/- 1.69%, respectively; the percentage of PCNA- and Ki-67-positive cells was also reduced. Despite high antiproliferative efficacy of 20 nM DOX and 10 microM ATRA combination, the percentage of apoptotic cells was only 25 +/- 0.81%, being similar to that obtained in the culture with 20 nM DOX. The concentrations of 10, 20 and 50 nM DOX that were used to inhibit the proliferation of MCF-7 cell line were not particulary effective. The inhibitory effect was obtained when 20 nM of DOX and E2, TAM or ATRA were used simultaneously. The use of E2 caused a two-fold decrease in the percentage of proliferating cells. It was also shown that the effectiveness of DOX in combination with ATRA is significantly higher than that of DOX combined with TAM, which might suggest a valuable approach to the treatment of breast cancer.

The influence of chemotherapy on bone marrow in patients with chronic lymphocytic leukaemia.

In chronic lymphocytic leukaemia (CLL) bone marrow trephine biopsy (BMT) is not required for diagnosis but can have a significant prognostic value and can be used for the detection of the minimal residual disease (MRD) and for assessment of the effectiveness of the treatment applied. The aim of the study was to evaluate the morphological changes in bone marrow after treatment with purine nucleoside analogues cladribine and fludarabine. Bone marrow trephine biopsy was taken routinely from 15 patients with CLL. Bone marrow trephine biopsy was performed on every patient before as well as after chemotherapy. The number of cell elements of the marrow (the degree of atrophy), the patterns of bone marrow infiltration, the presence of reticulin and collagen fibres and the disturbances in bone marrow stroma were assessed. The infiltration of bone marrow by neoplastic cells was observed in all the patients before administration of chemotherapy. The infiltration was followed by an increase in the number of reticulin fibres. After the treatment a regression of the reticulin fibres was observed with the lessening of the infiltration. After the treatment the levels of marrow infiltrate were decreased. Increased hypoplasia of the bone marrow was observed after the chemotherapy.

The morphological analysis of the haematopoietic microenvironment of the bone marrow at foetuses from preterm pregnancies.

The aim of the study was morphological assessment of the haematopoietic microenvironment of the bone marrow in foetuses from preterm pregnancies. For the morphological assessment the bone marrow was taken from sternum during an autopsy examination. After standard preparation, cellular elements of individual developmental lines were identified with the application of immunohistochemical methods. Evaluation of the bone marrow stroma elements was performed on the basis of configuration of the vascular sinuses, connective tissue elements, mutual topographic relations and integrin occurrence. The assessment of the argentophilic fibres was carried out with impregnation with silver according to the Gomori method. The configuration of the vascular sinuses was defined with morphometric evaluation with MicroImage-Olympus Kit. Identification of the 'young vascular forms' was performed with immunohistochemical methods with the use of endothelial cell antibodies and factor VIII. The presence of integrin was discovered in individual megakaryocytes, endothelial cells of sinus vessels and singular cells forming a stroma. However, no differences were found in the quantity of argentophilic fibres. The study revealed that ontogenetic development of haematopoietic tissue not only consists in alterations connected with the maturing process and cellular differentiation, but in topographic changes as well.

The radiological symptoms of lumbar disc herniation and degenerative changes of the lumbar intervertebral discs.

BACKGROUND: The x-ray examination is one of the fundamental diagnostic modalities in patients with low-back-pain. The aim of study was to establish relation between radiological findings and herniation type and its localization. As well, we looked for relation between radiological findings and progression of disc degeneration. MATERIAL/METHODS: The study enrolled 187 individuals operated due to lumbar disc herniation. In each case the x-ray examination of lumbar spine was performed. Herniation was classified as protrusion, prolapse or disc sequestration. According to localization, herniation was defined as central, lateral or intermediate. Removed discs were histologically evaluated to determine degeneration symptoms. The radiological picture was related to the degeneration of the intervertebral discs. RESULTS: The statistical analysis revealed the only one relation -between traction osteophytes and herniation classified as disc prolapse (p<0.05). We found no relations between other radiological findings (narrow intervertebral space, diminished lordosis, scoliosis) and herniation and its localization within the spinal canal. Also there is no relation between disc degeneration and radiological findings. CONCLUSIONS: Classic x-ray examination presents low value in diagnostics of lumbar disc degeneration and its herniation. There is no relation between radiological picture and intensity of degenerative changes within the lumbar discs.

Thyroid function and incidentalomas in kidney transplant recipients.

BACKGROUND: Ultrasound (US) is currently the best method for detecting thyroid nodules, especially those less than 1 cm in diameter, known as 'incidentaloma'. The ultimate diagnosis of non-palpable masses is done by fine-needle aspiration biopsy (FNAB), especially sonographically controlled (S-FNAB). Because of the increasing prevalence of thyroid nodules in Poland, it is essential to assess the nodules for FNAB and S-FNAB, especially in persons at high risk for thyroid carcinoma (patients after transplant with immunosuppression, persons with endemic goiter). MATERIAL/METHODS: Prospective sonographic, color Doppler and power Doppler tests of the thyroid were performed in 44 kidney allograft recipients. The TSH level and urine iodide level was assessed in each patient. The control group included 35 healthy volunteers. RESULTS: TSH levels were normal. The urine iodide concentration was over 30 mg/100 ml in 5 cases. In the rest of the group and in the controls it was below 30 mg/100 ml. In each case the thyroid was enlarged when compared to volunteers. There were thyroid nodules in 19 cases, single in 6 cases and multiple in 13. Of these nodules, 31 were smaller than 1 cm, 20 nodules were 1-2 cm in diameter, and none was greater than 2 cm. Nodules were classified according to whether or not there was flow within the nodule with eventual perinodular flow. FNAB revealed malignancies in 5 cases (nodules with flow). CONCLUSIONS: Goiter was observed in all patients after thyroid transplant. In five of 44 cases malignancies were detected. In patients after transplant the US picture and the TSH and iodide levels should be checked.

Histomorphometric study of megakaryocytes in bone marrow in selected myeloproliferative and lymphoproliferative diseases.

The aim of the study was an estimation of the histomorphometry of megakaryocytes (MK) in bone marrow in selected myeloproliferative and lymphoproliferative diseases. Bone marrow specimens were obtained by trephine biopsies from 41 patients with polycythaemia rubra vera (PV), idiopathic myelofibrosis (MF), chronic lymphocytic leukaemia (CLL), hairy cell leukaemia (HCL) and diffuse large B-cell lymphoma (L). Morphometric evaluation was performed using a standard program set MicroImage (OLYMPUS). The greatest number of typical nucleated MK, "naked" nuclei, anucleated cytoplasmic fragments and the largest area were found in PV. The circular deviation factor of MK and their nuclei increased in all cases. The greatest number of clusters was observed in PV and HCL. A significant increase in the number of dysplastic and "naked" nuclei of MK was noted in all selected haematological diseases. The presence of neoplastic cells in bone marrow increased the morphological changes in MK. Quantitative and morphometrical significant differentiation of MK in separate microscopic field in the same slides confirms the necessity of performing trephine biopsies in each patient with haematological disorders.

[Vascular proliferation within the herniated lumbar intervertebral discs in surgically treated patients].

Blood vessels are observed in intervertebral discs of children, and after individuals skeletal maturity the intervertebral discs become avascular structures. According to some opinions, new vessels in the annulus fibrosus can appear in adults as a result of regeneration processes. The lumbar intervertebral discs resected during surgery due to low-back-pain and during autopsies (control group) were evaluated histologically. The vascular proliferation was observed in 35% of surgical specimens in all types of herniation (protrusio, prolapse, and free sequesters). The statistical analysis revealed that proliferation take place mainly in young individuals.

A preliminary study of the submandibular gland of the rat after long-term cadmium intoxication.

The aim of the study was to establish to what degree the 24-week exposure of a rat to 5 and 50 mg Cd/dm3 affects the proliferating activity of cells with PCNA and Ki-67 nuclear immunoreactivity in the submandibular gland cells. The control animals received only redistilled water to drink. The group I rats were given 5 mg Cd/dm3, while the group II animals were given 50 mg Cd/dm3. The highest concentration of cadmium was observed in group II, with a concomitant increase in the number of PCNA-positive cells. In group I, cadmium concentration was significantly less compared to group II, and there were fewer PCNA-positive cells. The reaction for Ki-67 in both experimental groups was negative.

[Markers of epidermal proliferation in middle ear cholesteatoma].

Middle ear cholesteatoma is characterized by the presence in the middle ear cavity of a stratified squamous epithelium with keratin deposits which by constant proliferation leads to extensive bone destruction. The goal of this study was to evaluate, by immunohistochemical study, the expression of epithelial markers of proliferation--Ki-67 and PCNA in the matrix of cholesteatoma. The materials used in this study were 16 acquired cholesteatoma tissues collected from patients in the age 6-17 years during surgery. The specimens from the skin of the external ear canal were employed as the control. In the immunohistochemical specimens staining intensity and distribution of Ki67 and PCNA positive cells in various layers of the epithelium were assessed in three stages scale. The results were compared to the clinical parameters such as--type of cholesteatoma (pars flaccida or tensa), presence of ear discharge, degree of ossicular destruction and involvement of attic and mastoid. In the cholesteatoma matrix Ki-67 and PCNA positive cells were present in basal and suprabasal cell layers and also more superior layers, unlike the control skin were only basal cells show positive staining. The number of positive cells and intensity of staining was also greater in the cholesteatoma matrix than in skin of external auditory meatus. No correlation was found between results of immunohistochemical examination and clinical parameters.

Effect of melatonin and all-trans retinoic acid on the proliferation and induction of the apoptotic pathway in the culture of human breast cancer cell line MCF-7.

Melatonin in the in vitro conditions inhibits cell growth and proliferation of estrogen sensitive (ER+) cell line MCF-7 in culture. In the present study, during a 48-hour incubation melatonin at a concentration of 10(-5) M inhibited [3H]thymidine incorporation into cancer cells at the level of 69.52% +/- 10.99. Melatonin had no inhibitory effect on the physiological stimulatory action of estradiol. Tamoxifen added to the medium modulated the melatonin action only when the latter was added 24 hours after tamoxifen (46.45% +/- 4.40, p < 0.05). Tretinoin added to the culture caused a statistically significant reduction in [3H]thymidine incorporation into the cancer cells, compared to the melatonin and tretinoin groups, when treatment with retinoid was synergistic (39.05% +/- 5.44, p < 0.05) or sequential (tretinoin and after 24 h melatonin) (39.96% +/- 1.55, p < 0.05). This was confirmed by immunocytochemical investigations, which showed a statistically significant reduction in the percentage of PCNA- and Ki67-positive cells. Apart from the inhibitory effect on MCF-7 cell proliferation retinoids induce the apoptotic pathway in a dose-dependent manner. Melatonin added to the culture enhances this effect, which may indicate the potential for the use of both substances in the treatment of breast cancer in women.

Degenerated lumbar intervertebral disc. A morphological study.

According to many investigators, lumbar disc herniation is formed by the nucleus pulposus displaced to the vertebral canal through the cracks in the annulus fibrosus as a final result of disc degeneration. Specimens obtained during 187 disc surgeries were studied to identify the morphological nature of various forms of disc herniation, especially protrusion and prolapse. In 29% of cases the herniated part was annulus fibrosus and in 21.5% cases the annulus fibrosus was seriously involved. Simultaneously 308 autopsy specimens were studied. On gross examination the signs of degeneration were advanced in 82, and strongly pronounced in 42. In 15 specimens, aged 40 years or older, fibre bundles were reversed to normal convex orientation producing annulus fibrosus bulge. Microscopic examination showed pronounced myxomatous degeneration within the annulus fibrosus. In these circumstances, we believe that general opinion must be verified, because in some herniations the nuclei pulposus are not involved and herniations are solely prolapses of annulus fibrosus. So, the lumbar intervertebral disc herniation should be classified on the basis of histological examinations as a herniation of nucleus pulposus, herniation of annulus fibrosus or as a mixed type herniation where both disc elements are involved.
