Childhood dermatomyositis associated with intracranial tumor and liver cysts.

We present a girl with dermatomyositis, liver cysts and choroid plexus papilloma who was treated and followed for 7 years. Muscle histology revealed an inflammatory muscle disease and similar changes were detected in a brain tumor that was surgically removed at onset. Western blot analysis of the muscle revealed severely reduced calpain-3 protein. She was treated with pulse methylprednisolone treatment (800 mg i.v. for 4 days) followed by oral prednisone treatment (16 mg on alternate day) for 14 months, which improved muscle strength. Moreover, the cystic liver formations disappeared during steroid treatment. This is an unusual association of muscular disorder, steroid-responsive liver cysts, intracranial tumor and secondary calpain-3 deficiency. We speculate that this association is not coincidental, but mediated by an autoimmune attack against an antigen that is shared among the target tissues.

The role of adhesion molecules in atopic dermatitis.

Adhesion molecules may play an important role in the homing of T-cell subsets into allergen-exposed skin of atopic individuals. The aim of this pilot study was to examine the expression of adhesion molecules in atopic dermatitis (AD) skin lesions. Biopsies were taken at acute skin lesions from 10 AD patients and 5 healthy controls, and were studied by immunohistocytochemistry for the expression of E-selectin, L-selectin, ICAM-1 and ICAM-3 on cells in the epidermis and dermis. Study results revealed all AD patients to express ICAM-1 (10/10) and ICAM-3 (10/10) in the dermis, and most of them to express E-selectin (9/10) and L-selectin (6/10) in the dermis, without expression of E- and L-selectins in the epidermis. Our results revealed a high expression of adhesion molecules, especially ICAM-1 and ICAM-3, in the skin lesions of AD patients, which may play an important role in the pathogenesis of AD, and these preliminary results may be of clinical relevance for the treatment of AD.

Prominent involvement of activated Th1-subset of T-cells and increased expression of receptor for IFN-gamma on keratinocytes in atopic dermatitis acute skin lesions.

BACKGROUND: Atopic dermatitis (AD) is an allergic skin disease mediated by antigen-specific IgE and an important role has been ascribed to CD4+ cells (Th cells). The objective of the study was to evaluate humoral and cellular immunological factors in the blood and the skin lesions of AD patients, and to analyze the presence of inflammatory cell-surface markers in blood and skin biopsies. METHODS: The parameters for monitoring of 40 AD patients included results of prick test to inhalant allergens and epicutaneous (patch) test to contact allergens; values of total IgE, serum immunoglobulins (IgG, IgA, IgM) and different cell markers in the sera (CD3, CD4, CD8, CD20, CD21, CD23, HLA-DR). We also analyzed the presence of inflammatory cell-surface markers (CD3, CD4, CD8, CD20, CD20, CD1a, CD23, CD29, CD45Ro, IFNgamma+ markers) in the biopsies of skin lesions from 10 AD patients and 5 healthy controls (HCs) by immunohistochemical analysis (method of avidin-biotin immunoperoxidase). RESULTS: Beside increased total serum IgE and positive skin tests, a significantly higher percentage of CD23+ cells with lower percentage of CD21+ cells was revealed in peripheral blood of AD patients in comparison to HCs. A positive epidermal expression of the majority of markers of T cells (CD3+, CD4+, CD8+, CD29+, CD45Ro+, IFNgamma+) and those of Langerhans' cells (LCs) (CD1a, CD23+), without those of B cells (CD20+) were noted in AD patients, but no in the skin of HCs. Furthermore, significant difference was also found between the two groups for increased expression of CD3, CD4, CD8, CD29, CD45Ro, IFNgamma+ markers (markers for IFNgamma receptor) and higher intraepidermal CD23+ LCs and intradermal CD1a+ LCs in AD skin lesions. CONCLUSIONS: The obtained results suggest involvement of various humoral factors with increased production of IgE and cooperation between Th subsets and LCs, with higher production of related cytokines, and disturbed cellular immunity, including epidermal LCs with IgE receptors of high and low affinity in AD. The annotation of activated Th1 cells with increased producing of IFNgamma in acute AD skin lesions is notable, and might lead to IFNgamma binding to keratinocytes and consequently inflammatory skin changes in the disease.

Patient with immunoproliferative disease and lung carcinoma: a case report.

There are a number of reports on collision occurrence of non-hematologic cancers and Non-Hodgkin's lymphoma (NHL) and multiple myeloma. In this report we present a case of patient with immunoproliferative disease, extramedullary plasmocytoma and NHL-lymphoplasmocytoid lymphoma (LPL) and squamous cell carcinoma of the lung. After diagnosis of extramedullary plasmocytoma cytostatic therapy was commenced and the patient was well. Five years after patient was clinically worse and diagnostic evaluation this time revealed lymphoplasmocytoid cells in bone marrow. Five months later malignant morphologically undifferentiated cells were found in bone marrow which were by immunocytochemistry established as CD38 positive. After the patient's death, disseminated NHL-LPL and squamous cell carcinoma of lung was confirmed. In the report, we compared clinical course and diagnostic findings of our patient with literature data. We have also discussed the possible relationship of multiple B-cell lymphoid tumors and squamous cell carcinoma concluding that multidiscplinary diagnostic tools are essential not only for carcinoma diagnosis and follow-up, but also for further understanding of carcinogenesis.

Naphthalanotherapy reduces angiogenetic factor in psoriatic lesion.

Immunohistochemical analysis (cell immunophenotyping before and after 3 weeks of treatment with naphthalan oil) was performed on biopsy specimens from 10 patients with psoriasis vulgaris. To determine the angiogenetic factor in psoriatic lesions, immunohistochemical staining on 3-microm paraffin block sections was performed by using monoclonal anti-factor VIII antibody. There was a significant difference in the mean number of new blood vessels before and after the therapy (15.1 vs. 6.7). It seems that naphthalan therapy reduces number of new blood vessels and has effect on neovascularization in patients with psoriasis.

Cytokeratin 10 and Ki-67 nuclear marker expression in keratoacanthoma and squamous cell carcinoma.

The most frequent consideration in the clinical and histologic differential diagnosis of keratoacanthoma is squamous cell carcinoma. In the present study, cytokeratin 10 expression and proliferation rate as measured by Ki-67 expression were compared between 50 clinically and histologically diagnosed keratoacanthomas and 50 squamous cell carcinomas. Tissue sections from the skin were immunohistochemically stained with anti-cytokeratin 10 and anti-Ki-67 monoclonal antibodies. The distribution of cytokeratin 10 expression and proliferative cell count were analyzed. Study results showed higher cytokeratin 10 expression in keratoacanthomas than in squamous cell carcinomas and different distribution of staining in the two entities. The analysis of cytokeratin 10 expression showed a much wider range of values and statistically higher median (p<0.001) in keratoacanthomas than in squamous cell carcinomas. Additionally, the proliferation index of keratinocytes as measured by Ki-67 expression was significantly higher in squamous cell carcinomas than in keratoacanthomas (p<0.01). These results may prove helpful in histologic differentiation of these disorders.

Effect of naphthalan on epidermal proliferation activity and CD3, CD4, and CD8 lymphocyte count.

Immunohistochemistry analysis by cell immunophenotyping before and after a 3 - week treatment with naphthalan oil was performed on biopsy specimens from 10 patients with psoriasis vulgaris. For immunohistochemistry staining of 3- micro m paraffin block sections, anti-CD3, anti-CD4, anti-CD8, and Ki-67 antibodies were used. Peroxidase reaction for T cell (CD3, CD4, and CD8) quantification was done in the epidermis and dermis of each skin sample as total positive cell count per mm sample. Positive Ki-67 (proliferation index) was determined as percentage of positive cells per 100 cells in the basal layer of the epidermis. Results showed naphthalan treatment to decrease the mean CD3 lymphocyte count by 83% and 59%, CD4 lymphocyte count by 81% and 73%, and CD8 lymphocyte count by 60% and 49% in the epidermis and dermis, respectively. The mean proliferation index also decreased with naphthalan therapy.

The presence of surface CD30 on T cells in atopic dermatitis.

Atopic dermatitis (AD) has cellular immunohistochemical features similar to those of allergic contact dermatitis (ACD) and there is plenty of evidence for T-cell activation in this disease. The involvement of CD30+ T cells in acute stages of atopic dermatitis might establish CD30 as a helpful marker in differentiating those two diseases. Tissue sections from the skin of 12 patients with active atopic dermatitis and 13 with allergic contact (nickel-induced) dermatitis were immunohistochemically analyzed for cell-surface antigens, including CD30, CD3, CD4, and CD45RO. The severity of the disease was graded by the SCORAD clinical scoring system. The analysis of CD30+, CD45RO+, CD3+, and CD4+ cells in the dermis and epidermis showed a much wider range of values and statistically higher median (p<0.01) in the inflammatory infiltrate of acute atopic dermatitis compared with that of allergic contact dermatitis. Our results showed an association of CD30 expression with atopic dermatitis, but not allergic contact dermatitis. CD30 expression in AD might be helpful in histologic differentiation of these disorders and further characterization of atopy patch testing. The results suggested a specific regulatory function of CD30+ T cells in acute AD. Abundant CD45R0+ cells were detected in both AD and ACD lesions.

Nm23 gene product expression in invasive breast cancer--immunohistochemical analysis and clinicopathological correlation.

The nm23 gene/protein is a putative metastatic suppressor identified a decade ago in a melanoma cell line. A number of laboratory, clinical and pathological studies have been carried out to define its real biological and biochemical function as a step in a complex metastatic process. In our study we examined the significance of nm23 expression in 164 breast cancer patients, aged 35 to 74 years, in comparison to other parameters such as age, menopausal status, histological grade, tumor size, lymph node status, and hormone receptor status. Overall survival (OS) and disease-free survival (DFS) were analyzed. The median follow-up was 84 months. Significant changes in OS were found for tumor size, nodal involvement and histological grade but there was no convincing correlation with nm23 expression. When patients were stratified according to nm23 expression, it was shown that overall survival in nm23-positive patients was no longer than that in nm23 negative patients. It was also shown that patients who were lymph node negative and older than 50 years had longer OS than nm23-negative patients. A statistical analysis shows that there is a correlation between axillary node status and nm23 expression (p = 0.018) as well as between patients' ages and nm23 expression (p = 0.043). There was no statistically significant correlation between nm23 expression, lymph node status and their combination on DFS.

[Prognostic significance of newly formed blood vessels in the tumors of patients with breast cancer].

In this study 96 specimens of ductal invasive breast carcinoma were analyzed. The following data of each patient were collected: age, tumor size, histological grade, axillary lymph node metastases, estrogen (ER) and progesterone (PR) receptor expression, percentage of cells in S-phase of cell cycle, cell ploidy status, and overall survival. From paraffin blocks 3-microns sections were stained using anti-factor VIII monoclonal antibody for detection of tumor neoangiogenesis. The number of new blood vessels/mm2 in the stained tumor tissue specimens was analyzed using a light microscope with Weibel graticule on the eyepiece. There was no statistically significant correlation of number of blood vessels/mm2 and age, axillary lymph node status, ploidy of tumor cells, size and tumor grade, ER and PR status, but there is a correlation with number of cells in the S-phase of cell cycle (p = 0.037). The cut-off value of tumor blood vessels/mm2 was 170. Univeriate analysis showed that overall survival correlated significantly with axillary lymph node involvement (p < 0.001), ER (p = 0.012) and PR (p = 0.001) status, and number of blood vessels/mm2 of tumor (p = 0.033). In multivariate analysis only axillary lymph node metastases (p = 0.015) and PR status (p = 0.026) were found to be independent and significant prognostic factors. When patients were stratified according to number of blood vessels/mm2 of tumor it was shown that those with the number of blood vessels/mm2 over 170, aged under 50 years (p = 0.011), number of cells in S-phase of cell cycle over 4% (p = 0.050), diploid tumor cells (p = 0.004), and negative PR (p = 0.059) had shorter survival than patients with tumors with less than 170 blood vessels/mm2 of tumor.

The World Health Organization classification of lymphomas.

The Revised European-American Classification of Lymphoid Neoplasms (REAL) classification has been validated by a multi-institutional study, and project data showed that it is both reproducible and clinically relevant. The new World Health Organization (WHO) Classification of Neoplastic Diseases of Hematopoietic and Lymphoid Tissues, as a joint project of the Society of Hematopathology and European Association of Hematopathologists, is an update of the REAL classification, with minor changes based on newly available information. We analyzed the incidence of different histological types of non-Hodgkin s lymphomas diagnosed in Zagreb University Hospital Center, which were reclassified according to the WHO classification. Furthermore, we present a conceptual grouping of lymphomas into four categories (indolent, aggressive, highly aggressive, and localized indolent).

Hodgkin s disease with nephrotic syndrome as a complication of ulcerative colitis: case report.

We describe a case of a 32-year-old patient with ulcerative colitis complicated by Hodgkin s disease who presented with nephrotic syndrome. The patient had suffered from relapsing ulcerative colitis for 6 years before he developed Hodgkin s lymphoma. He was treated for Hodgkin s disease with 9 cycles of combined chemotherapy (COPP/ABV) and achieved the stabile remission of lymphoma, nephrotic syndrome, and ulcerative colitis. To the best of our knowledge, this is the first report on ulcerative colitis associated with Hodgkin s disease and nephrotic syndrome.

Prognostic value of different factors in breast carcinoma.

INTRODUCTION: The aggressive biological behavior of invasive and metastatic cancer is considered to be the most insidious and life-threatening aspect for breast cancer patients. It is mostly the result of changes in many molecular characteristics of tumor cells, including alterations in the mechanisms controlling cell growth and proliferation. AIM: The aim of this retrospective study was to identify predictors of aggressive biological behavior and metastatic potential in breast carcinoma among a number of intrinsic biomarkers of tumor cells such as steroid receptors and oncogene and tumor suppressor gene products. METHODS: Routine formalin-fixed, paraffin-embedded tumor samples were used and sections were stained immunohistochemically with the DAKO Strept ABC method to determine the expression of estrogen receptors (ER), progesterone receptors (PgR), HER-2/neu, bcl-2, Ki-67, p53 and nm23 in 192 consecutive breast carcinoma patients. The results of the quantitative immunohistochemical assays were correlated with clinical and histological data such as patient age, overall survival, tumor size, axillary lymph node status, hystological type, tumor grade, Nottingham prognostic index (NPI) and therapeutic regimens. RESULTS: Univariate analysis revealed that survival was significantly longer for patients with small tumors (P = 0.007), lower tumor grade (P = 0.021), negative axillary lymph nodes (P = 0.002), presence of nm23 protein (P = 0.002), and for patients treated with adjuvant hormonal therapy (P = 0.010). In multivariate analysis the independent factors positively affecting survival were absence of axillary lymph node metastases (P = 0.002), nm23 expression (P = 0.009) and hormonal therapy (P = 0.050). Among patients with positive axillary nodes there was a significantly higher survival rate in patients with nm23 expression compared with nm23-negative patients (P < 0.001). CONCLUSION: Identification of a subset of node-positive breast cancer patients with a more favorable prognosis according to nm23 expression might be clinically useful.

[Comparison of cytological and histological analysis of gastric mucosa biopsy in diagnosing malignant diseases].

The aim of this study was to determine the value of cytological analysis of an imprint of gastric mucosa biopsy compared to the histological evaluation, in diagnosing malignant diseases of gastric mucosa. In this study 70 patients, with changes of gastric mucosa endoscopically suspect of malignancy were included. Biopsy and imprint of gastric mucosa were performed in order to obtain morphological diagnosis. In the investigated group 46 patients (65.7%) were male, 24 patients (34.2%) were female, mean age 60 years (36-83). In 34 patients endoscopy finding suggested malignancy, in 23 endoscopy finding was described as gastric ulcer, in three as duodenal ulcer, in five as polyp, and in five as gastritis. In 42 patients malignancy was proved by pathohistological examination while in imprint smears of gastric mucosa biopsy the diagnosis of malignancy was made in 46 patients. In five patients malignant cells were found on cytological examination of imprint smears, and pathohistological examination did not find malignant tumour. In one patient malignant tumour was found on pathohistological examination, and cytological examination of an imprint smear did not find malignant cells. The results indicate that cytological examination of an imprint smear of gastric mucosa biopsy is satisfactory for proving malignant changes. In case of positive cytological and negative pathohistological analysis, it is recommended to repeat biopsy and imprint cytology in order to obtain morphological diagnosis.

[The presence of CD30 on lymphocytes in the inflammatory infiltrate of acute atopic dermatitis].

Atopic dermatitis (AD) has cellular immunohistochemical features similar to those of allergic contact dermatitis (ACD). There is plenty of evidence for T-cell activation in this disease such us the presence of T-lymphocytes which carry CD30, CD3, CD4, CD45RO markers. The aim of this study was to show the presence of lymphocyte-surface antigens including CD30, CD3, CD4, CD45RO in patients with acute AD and to compare the presence of the same molecules in patients with acute ACD (nickel-induced) because of the possible morphologic difference of these two entities. The presence of the stated molecules is immunohistochemically evaluated in biopsies of lesional skin in dermis and epidermis. Biopsies were obtained from twelve patients suffering from acute AD and from thirteen patients with ACD. The results show statistically significant higher average of presence and also much higher range of presence of CD30+, CD3+, CD4+, CD45RO+ lympohocytes in dermis and epidermis of patients with AD compared to the average and range in patients with ACD. Statistically much higher average of CD30, CD3, CD4, CD45RO is noticed in those occasions more in dermis than in epidermis in both groups of patients. High number of CD30+ lymphocytes in patients with acute AD does not however correlate with the severity of the disease evaluated according to clinical score for the evaluation of the severity of the disease, so called the SCORAD-index (Severity Scoring of Atopic Dermatitis). Our results showed an association betwen CD30 expression and acute AD but not with acute ACD which could evaluate CD3 as the useful marker in differentiating these two diseases.

[The value of searching for additional prognostic factors in combination with Nottingham Prognostic Index in breast carcinoma patients].

One of prognostic factors known as Nottingham Prognostic Index (NPI), which is combination of known prognostic factors such as tumor size, grade and axillary node status, is recently in usage in some European countries in clinical practice in prediction of breast carcinoma patients' survival. Therefore, the aim of this study was to verify, according to our experience, the prognostic significance of Nottingham Prognostic Index (NPI) in breast carcinoma patients in association with other new prognostic factors. In this study 148 consecutive specimens of breast carcinoma patients were analyzed. The following data for each patient were collected: age, tumor size, histological grade, axillary lymph node status, overall survival, estrogen (ER), progesterone (PR) receptor expression as well as expression of bcl-2, Ki-67, nm23, HER-2/neu, and p53. The Nottingham Prognostic Index (NPI) was calculated from the pathological information and patients were grouped according to the standard NPI index into: good prognostic group (GPG), moderate prognostic group (MPG), and poor prognostic group (PPG). The correlation of prognostic groups according to the NPI with other prognostic and predictive factors such as age, ER, PR, p53, bcl-2, Ki-67, nm23, Ki-67, Cathepsin D and HER-2/neu on overall survival was analyzed. The results of univariate analysis showed statistically significant correlation between patients' age, NPI prognostic groups and stage of disease with patients survival. When other prognostic factors were correlated with NPI prognostic groups there was not additional prognostic discrimination in given prognostic groups. Only marginally statistically significant influence of p53 expression was found on patient survival between MPG and PPG. It seems that other prognostic factors in combination with NPI prognostic groups do not have in our group of patients practical clinical relevance for the management of patients with breast carcinoma.

Association of cyclooxygenase-2 immunoreactivity with tumor recurrence and disease progression in superficial urothelial bladder cancer.

AIMS AND BACKGROUND: The main characteristic of urothelial bladder cancer is a clear predisposition to recurrence and disease progression. The aim of this study was to assess the possible relationship between cyclooxygenase-2 (COX-2) immunoreactivity in superficial urothelial bladder carcinoma and tumor grade, stage, number of recurrences and clinical disease progression. METHODS: In this prospective study 70 consecutive patients who underwent transurethral resection for superficial urothelial bladder cancer were included. Tumor slides were immunohistochemically stained for COX-2, and COX-2 immunoreactivity in tumor and inflammatory stromal cells was categorized as negative or mildly, moderately or strongly positive. Patients were followed up for 2 years, and during this period the possible association of COX-2 immunoreactivity with tumor stage and grade, number of recurrences and progression of disease was evaluated. RESULTS: COX-2 immunoreactivity in tumor cells was found in 57 (81.4%) patients and did not correlate with tumor grade, stage of disease, number of recurrences, and progression of disease. COX-2 immunoreactivity in inflammatory cells was found in 16 of the 57 patients with COX-2 positive tumors, and was significantly related to the number of recurrences, time to appearance of the first recurrence, and disease progression. CONCLUSIONS: COX-2 immunoreactivity in inflammatory stromal cells adjacent to the COX-2-positive tumor might be useful in clinical practice for selection of patients with a high risk of tumor recurrence and disease progression.
