Percutaneous treatment of coronary subclavian steal syndrome.

Coronary subclavian steal syndrome arises when a stenosis of the subclavian artery results in reduced antegrade or retrograde flow in an internal mammary artery with resultant coronary ischemia. This occurs in patients who have previously undergone surgical coronary revascularization utilizing an internal mammary artery graft. This syndrome can be successfully treated percutaneously with excellent immediate and long-term results.

Effects of selective alpha1- and alpha2-adrenergic blockade on coronary flow reserve after coronary stenting.

BACKGROUND: Coronary flow reserve (CFR) is not normalized shortly after coronary stenting. We hypothesized that alpha-adrenergic coronary vasoconstriction acts to limit CFR. METHODS AND RESULTS: We assessed flow velocity by Doppler wires and cross-sectional area by angiography in 46 patients undergoing coronary culprit lesion stenting (81+/-4% stenosis). Hyperemia was induced by adenosine (24 micro g IC or 140 micro g/kg per minute IV) before and after stenting. Finally, either the alpha(1)-antagonist urapidil (10 mg IC) or the alpha(2)-antagonist yohimbine (3 mg IC) was randomly combined with adenosine. In 8 subjects with angiographically normal coronary arteries, CFR was increased from 3.21+/-0.30 to 3.74+/-0.43 by yohimbine and to 4.58+/-0.65 by urapidil, respectively (P=0.0001). Patients were divided according to the cutoff of CFR > or =3.0 (n=18) or <2.5 (n=28). Revascularization per se did not change CFR. However, 15 minutes after stenting, CFR decreased to 2.05+/-0.55 from CFR 3.64+/-0.58, whereas in patients with CFR 2.39+/-0.51, it remained unchanged. Yohimbine improved CFR to 3.26+/-0.42 and to 3.41+/-0.58 in patients with >3.0 and <2.05+/-0.55 baseline CFR, respectively. Urapidil improved CFR to 3.52+/-0.30 and 3.98+/-1.07, respectively. CONCLUSIONS: Urapidil and yohimbine attenuated the CFR impairment occurring after revascularization by increasing both the epicardial vasodilator effect of adenosine and the blood flow velocity, thus suggesting that the adrenergic system plays an important role in limiting the capacity of the coronary circulation to dilate.

Percutaneous intervention for atherosclerotic disease in saphenous vein grafts.

The long term benefit of coronary artery bypass surgery (CABG) is limited by development of atherosclerotic disease in the bypass conduits. Percutaneous revascularisation is frequently the preferred method of treated symptomatic saphenous vein graft (SVG) atherosclerotic disease. The immediate and long term results of percutaneous intervention for SVGs is reviewed. Therapeutic considerations as well as novel technical advances are overviewed

Triple vessel stenting for triple vessel coronary disease.

BACKGROUND: Randomized clinical trials have established the feasibility of implanting multiple coronary stents and, in selected patients, have measured survival after multiple stenting to be comparable to coronary artery bypass graft surgery (CABG). However, the proportion of candidates for this therapy is unclear. this prospective study was performed to measure the percentage of patients eligible for elective multiple stenting of triple vessel coronary artery disease and to examine their short- and intermediate-term outcomes. METHODS: Between January 1996 and January 1999, percutaneous coronary interventions (PCI) were performed in 6,810 consecutive patients, of whom 115 (1.7%) had triple vessel coronary artery disease treated with > or =3 stents. The mean age of these patients was 62.9 years (range, 35-85 years), 83% were men and 13 patients (11%) were diabetic. The treatment was performed in a single procedure in 80% of patients and 362 vessels were dilated, including the left anterior descending artery in 85%, the right coronary artery in 84%, the left circumflex in 49%, the left diagonal artery in 27%, the posterior descending artery in 8% and the posterior lateral artery in 4% of patients. RESULTS: A total of 483 stents were implanted, representing an average of 4.2 +/- 1.4 stents per patients (range, 3-10 stents). The total length of stents averaged 66.9 +/- 4.9 mm. In-hospital adverse events included 2 deaths (1.7%), 2 emergency CABG (1.7%) and 5 myocardial infarctions (4.3%). The mean hospitalization duration was 2.7 +/- 2.2 days. During a follow-up ranging between 1 and 36 months (mean, 16.7 months), 2 patients died (1.7%), 2 patients underwent CABG (1.7%) and 22 patients (19.1%) underwent further coronary artery dilatations. The 3-year event-free survival was 71.3%. In univariate analysis, patients who underwent additional target vessel revascularization were younger and were more likely to have stented segments > 80 mm in the index procedure. No other clinical, angiographic or procedural characteristic was predictive of adverse events. CONCLUSIONS: In this consecutive series of patients undergoing elective PCI, candidates for stenting of > or =3 arteries were few. Most eligible patients underwent uncomplicated and immediately successful procedures. The long-term outcome was comparable to controlled studies of CABG, except for a higher rate of repeat revascularization procedures due to restenosis.

[Heart failure with preserved left ventricular function: clinical, echocardiographic, and clinical course features. Prognostic factors].

Heart failure is clinically associated with inadequate myocardial contraction, a significant reduction of left ventricular systolic function and ejection fraction and a cardiac enlargement. Some studies have reported that patients with symptomatic heart failure may have an impaired left ventricular filling with a normal or preserved left ventricular systolic function and an ejection fraction > 45%. These patients have a "diastolic heart failure" often neglected or misdiagnosed. The aims of our study is to describe clinical, echocardiographic and hemodynamic characteristics of 64 patients hospitalized for symptomatic heart failure, to determine possible variables with prognosis relevance, and for evaluating the severity of this diastolic left ventricular dysfunction. All patients were assessed by physical and radiographic examination, 12 leads electrocardiogram, and usual laboratory tests. The internal diameter of left atrium and left ventricular end diastolic and tele-systolic diameter were measured following the recommendations of the American Society of Echocardiography, Ejection fraction was determined following Simpson's method. Left ventricular filling patterns were evaluated by pulsed Doppler mitral or venous pulmonary flow. The following parameters were assessed: maximum velocity of E and A waves, E/A ratio, E wave deceleration time and isovolumic relaxation time. The patients were studied following Appleton's classification. 45 patients were submitted to left heart catheterization and coronary angiography. All subjects were routinely followed by cardiologic examinations and the mean follow up is 18 +/- 4, 5 months. 29 women (45.3%) and 35 men with a mean age of 72.5 +/- 3.2 years were included in this study. Left ventricular ejection fraction was in mean 48.5 +/- 4.2%. 65% of patients had ischemic cardiomyopathy with severe coronary stenosis > 50%, often associated with hypertension. 52% of patients had hypertensive heart disease and 38% were diabetics. 34 patients were re-hospitalized for recurrent heart failure despite medical treatment with diuretics, ACE inhibitors (90% of patients), beta-blockers, (37%) or nitrates (36%). 24 patients have been treated by coronary angioplasty. In hospital mortality was 6.2% and during the follow up at 18 months the mortality reaches 18.7%. The factors of poor prognosis are age > 75 years, left ventricular restrictive pattern at doppler diastolic trans mitral flow evaluation, (p < 0.001), history of myocardial infarction, and renal insufficiency defined by creatinemia > 150 micromoles (p = 0.002). In conclusion heart failure with preserved left systolic ventricular function is frequent in women with hypertensive heart disease. The prognosis at mean term is better that prognosis of patients with systolic dysfunction but despite medical treatment there is a high morbidity with numerous re hospitalizations. Restrictive left ventricular filling pattern is significantly related to the occurrence of events and mortality.
