A randomized pilot study comparing combination therapy plus enfuvirtide versus a treatment interruption followed by combination therapy plus enfuvirtide.

Most individuals with multidrug-resistant HIV who switch to a new therapeutic regimen containing a single fully effective agent experience incomplete viral suppression. We postulated that interruption of antiretroviral therapy prior to the introduction of such a regimen would improve long-term virological outcomes. Thirty, three-class experienced, enfuvirtide-naive individuals with detectable drug-resistant viraemia were randomized to an immediate enfuvirtide/optimized-background treatment regimen or a 16-week treatment interruption followed by enfuvirtide/optimized-background treatment regimen. The median CD4+ T-cell count and viral load at study entry were 39 cells/mm and 4.72 log10 copies RNA/ml, respectively. There was no evidence of any virological or immunological benefit associated with the interruption. In multivariate analysis, only the baseline phenotypic susceptibility score was predictive of treatment response at week 48 (P=0.009). Only 40% of individuals had evidence of a shift in drug-resistance genotype during the interruption. In summary, interrupting therapy prior to initiating salvage therapy in patients with advanced disease did not result in an improved virological response to enfuvirtide. The collective predictive activity of an enfuvirtide-containing regimen was important in predicting treatment response.

Increased rate of visual field loss associated with larger initial visual field threshold values on follow-up of open-angle glaucoma.

PURPOSE: To determine the changes in the visual field over time and the factors that contribute to these changes on follow-up of open-angle glaucoma patients. PATIENTS AND METHODS: From a retrospective review of patients' charts and visual fields, 30 eyes from 30 patients with open-angle glaucoma with a minimum of 3 years of follow-up were chosen. Visual fields were obtained with the Octopus perimeter about every 4 months. Regression analysis was used to obtain the slope for all the threshold values for each eye for the total field and nine different regions over time. Optic disc area was measured from single fundus photographs obtained at the beginning of the period of observation. RESULTS: The open-angle glaucomas were observed for a mean of 6.7 +/- 2.3 years. The number of visual fields per eye was 20.7 +/- 8.0. The mean threshold value of the total field for the first three visual fields was 18.0 +/- 5.2 dB. The visual fields of the 30 eyes with a mean intraocular pressure of 18 +/- 2 mm Hg during follow-up showed a significant decrease in their visual field slope of -0.384 dB/y for the total visual field. On multivariate analysis, the slope or rate of visual field loss was significantly correlated with the average of the first three initial visual field thresholds for the total visual field, temporal, and central regions so that the greater the threshold value the more rapid the rate of the visual field loss. For the total visual field and for the nasal, superior nasal, and temporal regions with lower initial threshold values, the rate of visual field loss was curvilinear with the rate of visual field loss increasing at greater initial mean threshold values. The other regions of the visual field showed a linear increasing rate of visual field loss with larger initial threshold values. Other factors significantly associated with progression of visual field loss were refractive error, race, the slope of the pulse rate, and the mean percent false positives and false negatives for initial to final observations. The mean and the slope of intraocular pressure from initial to final observation were not significant variables. CONCLUSION: In open-angle glaucomas observed for 6.7 +/- 2.3 years, there was a more rapid rate of visual field loss associated with larger initial threshold values and a less rapid rate of visual field loss associated with smaller initial threshold values indicating that the progression of visual field loss is greater in an earlier stage of disease.

Healthy living in Nunavut: an on-line nutrition course for inuit communities in the Canadian arctic.

OBJECTIVES: It is recognized that empowerment of Indigenous Peoples through training and education is a priority. The objective was to design a course that would provide an innovative training approach to targeted workers in remote communities and enhance learning related to the Nunavut Food Guide, traditional food and nutrition, and diabetes prevention. STUDY DESIGN: A steering committee was established at the outset of the project with representation from McGill University and the Government of Nunavut (including nutritionists, community nurses and community health representatives (CHRs), as well as with members of the target audience. Course content and implementation, as well as recruitment of the target audience, were carried out with guidance from the steering committee. METHODS: An 8-week long course was developed for delivery in January - March, 2004. Learning activities included presentation of the course content through stories, online self-assessment quizzes, time-independent online discussions and telephone-based discussions. Invitations were extended to all prenatal nutrition program workers, CHRs, CHR students, home-care workers, Aboriginal Diabetes Initiative workers and public health nurses in Nunavut. RESULTS: Ninety-six health-care workers registered for Healthy Living in Nunavut, with 44 actively participating, 23 with less active participation and 29 who did not participate. CONCLUSIONS: Despite having to overcome numerous technological, linguistic and cultural barriers, approximately 40% of registrants actively participated in the online nutrition course. The internet may be a useful medium for delivery of information to target audiences in the North.

Absence of biologically important Kaposi sarcoma-associated herpesvirus gene products and virus-specific cellular immune responses in multiple myeloma.

Kaposi sarcoma-associated herpesvirus (KSHV) has been associated with several diseases, but the association between KSHV and multiple myeloma (MM) remains controversial. To address this issue, we studied patients with MM for the presence of viral RNA transcripts as well as KSHV-specific cellular immune responses. Highly sensitive reverse transcription-polymerase chain reaction assays for detection of viral transcripts of KSHV open reading frame (ORF) 26, ORF72, and ORF74 did not detect viral gene transcripts in long-term cultures of bone marrow stromal cells from 23 patients with MM. Moreover, sensitive assays for KSHV ORF65-specific and ORF73-specific cytotoxic T-lymphocyte (CTL) activity that readily and routinely detect CTLs specific for ORF65 and ORF73 in patients positive for human immunodeficiency virus and KSHV did not show any specific responses in 16 patients with MM, despite the presence of positive Epstein-Barr virus-specific CTLs in all cases. These data therefore do not show a biologically important association between ongoing KSHV infection and MM.
