[Patient with haemoptysis, dyspnoea, fever and lung infiltration].

In this study we presented the case of 55 years old man who was admitted to the Dept. of Lung Diseases and Tuberculosis in Zabrze with haemoptysis, dyspnoea, fever and lung infiltration. Initially the neoplastic disease was diagnosed. From the information gathered from the patient's family it was stated that the patient has been taking acenocoumarol for a considerably long time without any professional supervision. In this study we emphasize the importance of motivating and informing patients about the purposefulness of control examinations while using this kind of drugs.

Citric acid cough threshold in patients with gastroesophageal reflux disease rises after laparoscopic fundoplication.

BACKGROUND: It has been reported that antireflux surgery can diminish chronic cough due to gastroesophageal reflux disease (GERD) that is resistant to pharmacologic therapy. The aim of this study was the assessment of citric acid cough threshold (CACT) in patients with chronic cough due to GERD before and 3 months after laparoscopic Toupet fundoplication. METHODS: Thirty subjects (20 women and 10 men; median age, 45.3 years) with chronic cough due to GERD and 15 healthy volunteers underwent cough challenge with doubling concentrations of citric acid. Twenty subjects with GERD, a group of 14 women and 6 men (mean age, 45.5 years), underwent the same protocol 3 months after laparoscopic fundoplication. Daytime and nighttime cough score questionnaires (verbal category descriptive score) were completed in all groups. RESULTS: The geometric mean of CACT was significantly lower in GERD patients (9.62 mg/mL) than in healthy volunteers (50.8 mg/mL, p < 0.001). The results of cough score measurement significantly improved within 2 weeks after laparoscopic surgery. In 13 weeks of postoperative follow-up, cough disappeared or was greatly improved in 14 of the 20 patients (70%); in 3 other patients, cough resolved partially. In three patients. there was no improvement in cough. Cough challenge after surgery revealed a significant increase in mean cough threshold, from 8.28 to 19.03 mg/mL (p < 0.05). CONCLUSIONS: The results suggest that GERD influences CACT, which was significantly lower in GERD patients compared to healthy subjects. A significant correlation was found between subjective and objective measurements of cough in GERD patients. We found laparoscopic fundoplication to be objectively beneficial in GERD-induced chronic cough, as it reduced the CACT.

[Invasive pulmonary aspergillosis as a complication of prolonged antibiotic therapy].

In this study we present the case of 53 year old woman hospitalized in the Department of Lung Diseases and Tuberculosis in Zabrze, Silesian Medical University in Katowice, due to chronic cough and infiltration in the chest radiogram. Prior to the admission, the patient received long-lasting antibiotic ambulatory treatment leveled at laryngobronchitis. After admission the patient underwent bronchofiberoscopy during which a sample for mycological examination of bronchoalveolar lavage was taken. The result of the examination made possible to give the diagnosis--invasive pulmonary aspergillosis. Proper antifungal treatment was used and in consequence considerable improvement in both clinical and radiological state was achieved. Invasive pulmonary aspergillosis is mostly diagnosed in immunocompromised patients. The patient described in this case had none risk factors. Based on that particular case we may state a hypothesis that a prolonged antibiotic therapy is an important risk factor of invasive pulmonary aspergillosis incidence.

[Usefulness of ventilation tests in the diagnosis and evaluation of treatment results in select laryngeal diseases].

The obstruction of upper airways is a common symptom of some laryngological diseases. The patients need exact diagnosis and proper treatment. The ventilation tests seem to be one of the most important tool in process of diagnosis and treatment, however they are more pulmonological than laryngological procedures. The aim of the study was to evaluate the efficacy of ventilation tests and analysis of their interpretation in patients with laryngeal and/or tracheal obstruction. Also, we intended to show a method of spirometric examination in patients with tracheostoma. Based on our common clinical experience (more than 3000 tests in patients with upper airways obstruction in ENT Departments in Zabrze and Szczecin) we presented our observations. In majority, the tests were used in patients after laryngeal surgery due to the cancer or bilateral vocal cord paralysis and in patients with tracheostomy, before decannulation. The attention was paid on usefulness of some coefficients, especially in diagnosis of upper airway obstruction. For better results interpretation the authors introduced the term of mixed obstruction in contrast to fixed and dynamically variable narrowing. Also the FIV1/N FEV1 parameter was purposed. In conclusion the great role of ventilation tests was emphasized, especially in diagnosis of the narrowing on the larynx and trachea level and also in evaluation of the results after surgical procedures on upper airways.

[Microbiological and molecular analysis of mycobacterium tuberculosis complex strains isolated from patients with pulmonary tuberculosis].

The aim of the study was to perform microbiological and molecular analysis of Mycobacterium tuberculosis complex strains isolated from patients with pulmonary tuberculosis originating from two different regions of Silesia voivodship in 2000 year. One of 135 strains subjected to the study lacked IS6110 insertion sequence. Its belonging to Mycobacterium tuberculosis complex was confirmed by positive test for specific gyrB gene sequence. Strain resistance analysis revealed that in previously diagnosed patients resistance to streptomycin (16,2%) was more frequent than other. In contrast to never treated patients group where the prevalence of resistance to isoniazid was the most frequent (4,5%). In both group of patients resistance to rifampin or ethambutol was not detected. The multidrug resistance strains (4,6%) were detected only in the previously diagnosed group. Combined analysis of microbiological, epidemiological and molecular data are essential for creation of functional registry of patients infected with resistant Mycobacteria, giving the possibility of surveillance of drug resistant tuberculosis in populations.

[Analysis of alpha-1-antitrypsin phenotypes and genotypes in patients with early-onset pulmonary emphysema].

An increased risk for the development of emphysema occurs in all carriers of deficient variants of Alpha-1-Antitrypsin (A1AT). A DNA polymorphism in the 3' promoter region of A1AT gene (locus Pi), which is important for a modulation of gene expression in a response to inflammation, seems to be an additional genetic risk factor for emphysema. In the study, we present two relatively young patients with panlobular lung emphysema associated with a normal level of A1AT in serum. To contribute to our knowledge about a molecular basis of the pulmonary changes, the evaluation of A1AT phenotype with the use of an isoelectrofocusing of serum proteins and the analysis of DNA polymorphism in the 3' region of A1AT gene (Taq I RFLP) have been performed. The obtained results indicate on a necessity to include procedures of A1AT phenotyping and locus Pi genotyping in both the population screening of patients and the detailed clinical diagnostics of young patients with emphysema and/or COPD.

[Change of patients profile hospitalized because of pulmonary sarcoidosis in Department of Pneumonology in Zabrze (Poland) in 1976-80 and in 1996-2000].

The comparision of two groups of patients with pulmonary sarcoidosis hospitalized in Department of Pneumonology Silesian Medical School of Medicine in Zabrze in 1976-1980 (n=120) and 20 years later i.e. in 1986-2000 (n=190) was done. We have noticed, that in comparision with period of 1976-80 the mean age of patients hospitalized in 1986-2000 has significantly increased (41.6 vs. 37.0 p < 0.05) and percentage of females has significantly diminished (50.0% vs. 63.3% p < 0.05). No change in age structure of patients in both groups was observed. Sarcoidosis was observed most frequently (about in 34% of patients) among subjects aged 30-39 years. In group hospitalized in 1996-2000 percent of patients aged above 60 has rised (6.8% vs. 2.5%) and percent of patients without clinical complaints has significantly decreased (30.5% vs. 50.0% p < 0.05), also percent of subjects in II radiological stage has significantly diminished (32.1% vs. 52.5% p < 0.05). We conclude that during 20 years the profile of hospitalized patients with pulmonary sarcoidosis has changed.

[Mucoepidermoid carcinoma of the lung--case reports].

Two patients, a young woman and a young man, with mucoepidermoid carcinoma of the lung, who were diagnosed at the Department of Phthisiopneumonology in Zabrze Silesian Medical University in Katowice are described. The principal method of diagnostics in the presented cases were bronchofiberoscopy followed by the histopathological examination of the lesions. There are no cases of massive bleeding as the side effect of this procedures described. This type of carcinoma of the lung is mostly found in young patients, usually without any risk factors of lung cancer, what confirms the rule, that all pulmonological symptoms of unknown origin should be verified by bronchoscopy, because other methods of diagnostics, including TC of chest-like in our patient, may be false-negative. The method of choice in the treatment of mucoepidermoid carcinoma is surgery. One should avoid a through bronchoscope resection, as it may be only partial. Metastases, mostly found in the local lymph nodes, are rarely described.

[Assessment of thoracic sympathectomy by transpleural posterolateral and thoracoscopic approach in surgical treatment of Raynaud's syndrome].

Operative treatment of patients with Raynaud's Syndrome is rarely used, and is recommended by most authors only in the case of disthrophic changes or so advanced symptoms, which disturb normal social and daily life. Recently, thoracoscopic sympathectomy is applied more frequently. The aim of this study was to assess the usefulness of thoracoscopic sympathectomy by the presentation of early and long-term outcome of 43 thoracic and eight lumbar sympathectomies performed in 41 patients with Raynaud's Syndrome. Transpleural posterio-lateral thoracotomy was performed 29 times in 27 patients, whereas thoracoscopic sympatectomy 14 times in 8 patients. Surgical techniques, early and long-term results were discussed. Based on early and long-term outcome, there were no significant differences between applied surgical techniques. Moreover, thoracoscopic sympathectomy was safe in the aspect of a short duration as well as good therapeutic and cosmetic effect of the procedure. It should be applied instead of others, so far used methods, with the exception of patients with massive pleural adhesions, which need thoracotomy.

PCR-RFLP analysis of a point mutation in codons 315 and 463 of the katG gene of Mycobacterium tuberculosis isolated from patients in Silesia, Poland.

Resistance to antituberculous agents is an important cause of ineffectiveness of antimicrobial therapy. The resistance of M. tuberculosis to antituberculous agents is a result of mutations in genes participating in those agent's action. The antituberculous drug--isoniazid can be activated by Mycobacterium tuberculosis either through a hydroperoxidase I/II or a superoxide-dependent oxyferrous pathway. The present study analyzed the frequency of the mutations occurring in codons 315 and 463 in katG gene of Mycobacterium tuberculosis strains, isolated from patients with pulmonary tuberculosis from Silesia, Poland. In this study 23 isoniazid-resistant Mycobacterium tuberculosis strains were analyzed. For RFLP analysis, a 620 bp amplified fragment of katG gene was digested with restriction endonuclease MspI. Among 24 isoniazid-resistant strains, isolated from patients between 2000-2001, point mutations were found in 30% of analyzed isoniazid-resistant strains in codons 315 or 463 (7 strains). In contrast, no mutations in codons 315 and/or 463 katG gene were found in 16 strains (70%). Obtained results suggests that point mutations S315T (AGC-->ACC) and R463L in katG gene are infrequent in the analyzed population.

[Quantitative analysis of sigma genes expression in Mycobacterium tuberculosis cultures exposed to rifampicin and isoniazid].

The aim of the study was quantitative analysis of five genes encoding Mycobacterium tuberculosis sigma factors sigA, sigE, sigF, sigH, and sigI as well as the 85B reference gene known as the mycobacterial viability marker, in cultures exposed to rifampicin and isoniazid. The mRN levels were assessed using QRT-PCR technique, in the automated system of real time quantification with the ABI PRISM 7700 Sequence Detector System (TaqMan). The number of each analyzed gene transcript copies was expressed as a number of mRNA per 1 eg of isolated total RNA. In cultures exposed to the tested chemicals the number of 85B mRNA copies declined as compared to the controls (without tested chemicals). There was no detectable expression of sigA and sigI in the control cultures. Both, rifampicin and isoniazid induced expression of sigA and sigI genes. The sigE gene expression increased during exposure to isoniazid and decreased under rifampicin exposure conditions. The sigF mRNA was detected neither in the control culture, nor in cultures exposed to rifampicin or isoniazid. Both tested chemicals caused decrease of sigH expression.

[Chronic obstructive pulmonary disease--the range of the problem, clinical stages].

The prevalence of chronic obstructive pulmonary disease (COPD) increases worldwide, as well as in Poland. About 10% of adult population is suffering from COPD. COPD occurs mainly in smokers who are older than forty years. Diagnosis of COPD in case of many patients is not properly established. It refers mainly the patients in a mild or a moderate clinical stage. One-fourth of patients with COPD is suffering from a severe or an extremely severe clinical stage of COPD. When untreated, COPD is a fast progressing disorder, leading to respiratory disability and premature death. Therapy of COPD is related to the clinical stage of disease.
