[Analysis of articular cartilage lesions in 5114 knee arthroscopies].

The cartilage lesions become the increasing social problem, which occurs often in young people. It influences the normal knee function and may predispose patients for osteoarthritis. The aim of this study was the epidemiologic and etiological analysis of the knee articular cartilage lesions diagnosed in arthroscopy. From 1997 to 2002, arthroscopy of the knee joint was performed in 5114 patients. Chondral lesions were found in 2931 cases--57.3. Non-isolated cartilage lesions accounted for 67.9%. Grade II according to ICRS classification evaluated in arthroscopy was the most frequent grade of the cartilage lesion (47.8%) and grade 4B the least frequent one (0.11%). The patellar articular surface (43.3%) and the medial femoral condyle (39.6%) were the most common locations of the lesions.

[Assessment of radiological results of anterior cervical discectomy with different fusion cages].

The purpouse of this study was to compare results of anterior cervical decompression and fusion for cervical disc disease with 2 different interbody devices: titanium cylinder-design cage and carbon box-design cage. The results of operations of 46 patients (60 instrumented levels) divided in to groups according to implant type were examined to asses the restoration of disc height, clinical outcome, bony fusion and complication rate. In the cylinder-design cage group average postoperative restoration of disc height was 72% but with narrowing rate of 26% in follow up. In the carbon box group postoperative disc height restoration was 51% with narrowing in follow up to 6%. Clinical outcome was comparable in both groups and loss of disc height in cylinder-design cage group had no effect on clinical outcome. There were 2 cases of pseudoarthrosis in cylinder cage group. Overall the study shows better radiological outcome of carbon box-design cages in comparison to cylinder design cages with no influence on clinical effect.

[Total knee arthroplasty].

Total knee arthroplasty (TKA) is considered to be a common surgical intervention in the patients with widespread deformation of the knee joint during the process of arthritis or rheumatoid arthritis. It is also considered to be the method of choice for those patients. TKA is based on removing the destroyed parts of the knee and replacing them with new metal or plastic elements. It is commonly believed that TKA is the process of replacing the old destroyed joint with the new artificial one. Based on own experiences and the literature data the authors presented the most important indications for the TKA, the rules of the prosthesis selection and the operative techniques, contemporary opinions, and current trends in TKA.

[Intra- and postoperative complications in the arthroscopy of the knee joint].

Arthroscopy of the knee joint is one of the invasive surgical procedures that are connected with a relatively low risk of complications. Currently it is considered that in the group of diagnostic arthroscopies and technically simple operative arthroscopies all possible complications (less and more serious) account for 0.8-1.2% and in technically more advanced operations performed by means and under control of arthroscopy--1.5-5%. The purpose of this study was a retrospective analysis of intra- and postoperative complications in 10,770 arthroscopies performed in the years of 1986-2001. In our material a number of 731 (6.98%) various complications was observed. General complications (anaestesiological and cardiovascular) represented 0.15%, intraoperative complications--0.31% and postoperative complications--6.34%.

[Frequency of neurological complications connected with transpedicular spine fixation].

To asses a frequency of neurological complications concerned with transpedicular spine fixation 1127 inserted transpedicular screws were reviewed. Penetration of pedicle was found in 5.5% of inserted screws. Radicular symptoms accompanied 0.8% of screw insertions. Complications were concerned only to medial or subpedicular penetrations. Highest number of potentially hazardous medial wall penetrations was found in lower lumbar spine. Treatment consisted of removal and repositioning of screws but with direct visualisation in cases with self tapping screws systems. The results shows that rate of neurological complications during transpedicular fixation is small and procedure is relatively safe.

[Complication of arthroscopy of the knee].

Arthroscopy is an elementary surgical procedure in the management of the knee. Due to its minimal invasiveness the complications are not frequent. However, because of continuously increasing number of arthroscopies being performed worldwide (circa 1,000,000 per year), the total number of complications is significant. More frequent complications accompany particularly more complex and difficult procedures. The percentage of complications after arthroscopy oscilates about 2%. The most common complications are: intraarticular damage, neurological injury, vascular injury, compartment syndrome, instrument breakage, postoperative pain, haemarthrosis, synovitis, thromboembolism, infection, synovial fistula, complex regional pain syndrome 1, infrapatellar contracture syndrome. Current view on the complications of arthroscopy is presented in this paper basing on contemporary literature.
