Korean panic disorder severity scale: construct validity by confirmatory factor analysis.

The Panic Disorder Severity Scale (PDSS) is a seven-item instrument used to rate the overall severity of panic disorder. This scale has previously evidenced excellent psychometric properties in a sample of patients with panic disorder, with or without agoraphobia. However, despite several factor-analytic PDSS studies, the number of factors has not been entirely consistent. Our objective in this study, then, was to evaluate the fit of two competing models to data collected with a sample of 176 patients with panic disorder. Patients (N = 176) who fulfilled DSM-IV criteria for panic disorder, with or without agoraphobia, underwent structured diagnostic assessments and PDSS interviews. Of the subjects, 102 were men and 74 were women. The mean age of the men was 41.2 (SD = 10.10) and of women, 41.0 (SD = 10.54). A model with two correlated factors was considered to be more appropriate in the description of the data with the following indices: Tucker-Lewis index = 0.94, comparative fit index = 0.96, root-mean-square error of approximation=0.08, and standardized root-mean-square residual = 0.04. These results were then cross-validated with subgroups of patients with panic disorder with agoraphobia (n = 128), without agoraphobia (n = 48), and with panic disorder after phamacotherapy (n = 62). Our results indicated that the correlated two-factor model developed and validated using Western samples could be appropriately generalized to Korean clinical samples. Furthermore, our results support the construct validity of the Korean version of the PDSS.

Korean Anxiety Sensitivity Index-Revised: its factor structure, reliability, and validity in clinical and nonclinical samples.

The factor structure and convergent and discriminant validity of the Anxiety Sensitivity Index-Revised (ASI-R) were examined in a community sample, a student sample, and patients with panic disorder in Korea. Results from a confirmatory factor analysis (CFA) comparing our data to factor solutions commonly reported as representative of European American populations indicated a poor fit. A subsequent exploratory factor analysis (EFA) indicated that a four-factor solution provided the best fit. Correlations between the ASI-R and anxiety measures were moderately high, providing evidence of convergent validity. Implications for assessment with Koreans are discussed.

Reliability and validity of a short form of the Severe Impairment Battery in Korean Alzheimer's disease patients.

The objective of this study was to evaluate the reliability and validity of a short form of the Severe Impairment Battery (SIB) for a Korean population. Eighty-four Alzheimer's disease patients with Clinical Dementia Rating (CDR) stages 2 (n = 32) or 3 (n = 52) and scores of less than 15 on the Korean version-Mini Mental State Examination (K-MMSE) participated in this study. Seventeen of the patients were men and 67 were women, and the mean age was 75 (SD = 10.4). Cronbach's coefficient alpha of the shortened SIB (SIB-S) was 0.93, and the item-total correlation was significant. Test-retest correlation for the total SIB-S score and subscale scores was significant, with the exception of the 'orienting to name' subscale. Construct validity was confirmed by evaluating the correlation between the SIB-S and the SIB, K-MMSE, CDR, and S-ADL; Spearman correlation coefficients were 0.96, 0.88, -0.67, and -0.63, respectively, which were found to be significant. The difference in the total SIB-S score and its subscale scores between the CDR 2 and CDR 3 groups was significant, except for the 'orienting to name' subscale. More severely demented patients also showed a wide range of the SIB-S score. Finally, the diagnostic accuracy of the SIB-S was high (AUC = 89.5%) in the differentiation of the CDR 2 and 3 patients. The sensitivity and specificity of the SIB-S were 91% and 80%, respectively, when the cut-off score was 32.5. Our results indicate that the SIB-S is a reliable and valid instrument for evaluating patients with severe dementia in the Korean population.

Efficacy and tolerability of OROS methylphenidate in Korean children with attention-deficit/hyperactivity disorder.

This study evaluated the efficacy and tolerability in the initiation treatment of Concerta (OROS methylphenidate HCl) in Korean children with Attention-Deficit/Hyperactivity Disorder (AD/HD). One hundred and nineteen children with AD/HD were entered into a multi-center, open-label, four-week trial. The dosage of Concerta was adjusted by the investigators based on symptoms and safety assessments performed on a weekly basis. The safety of the drug and its efficacy for attention, behavior, and cognitive function were assessed. The primary outcome measures for efficacy were the Parent and Teacher IOWA Conners Rating Scales, Peer Interaction Items, and the Clinical Global Impression Scale. Cognitive tests (Continuous Performance Test, Matched Familiar Figure Test, Verbal Fluency Test, and Trail Making Test) were included as the secondary outcome measures. In most participants, OROS methylphenidate was well tolerated. There were significant improvements in attention, behavior, and function as measured by parents, teachers, and investigators. The benefit of the initiation of OROS methylphenidate in children with AD/HD was shown on the cognitive tests as well. These data provide support for the benefit of the once-daily methylphenidate preparation, Concerta in the treatment of Korean children with AD/HD. Children were initiated safely in this short-term trial, and its effectiveness was evident in the behavioral, as well as neuropsychological measurements.

Psychiatric comorbidity in Korean children and adolescents with attention-deficit hyperactivity disorder: psychopathology according to subtype.

It is well-known that more than 50% of attention-deficit hyperactivity disorder (ADHD) cases also have comorbid psychiatric disorders. We evaluated the comorbid psychopathology of Korean children and adolescents with ADHD using a standardized diagnostic instrument. The Korean Kiddie-Schedule for Affective Disorders and Schizophrenia-Present and Lifetime Version (K-SADS-PL-K) was administered and completed in 105 patients who had been referred to the outpatient and inpatient clinics at the Samsung Medical Center from March 2004 to May 2005. All of the cases were diagnosed as ADHD according to DSM-IV criteria. We analyzed their clinical characteristics and psychiatric comorbidities, and assessed the correlation of any comorbidity with gender, age and ADHD subtype. Among our 105 participants, 70 (66.7%) subjects were diagnosed with combined-type ADHD, 22 (21.0%) were the predominantly inattentive type, only 1 (1.0%) was determined to have the predominantly hyperactive-impulsive type of ADHD, and 12 (11.4%) were classified as not otherwise specified (NOS) ADHD. Eighty (76.2%) subjects had at least one comorbid disorder such as oppositional defiant disorder (n = 53, 50.5%), anxiety disorders (n = 35, 33.3%) and affective disorders (n = 15, 14.3%). Our patients ranged in age from five to 16 years. Among the factors including gender, age, and ADHD subtype, ADHD subtype was the only one significant to comorbidity in our study. The results of this study suggest that psychiatric comorbidity in Korean children with ADHD is similar to the results of previous studies in western countries. Out of all the ADHD subtypes, the combined-type group had a significantly higher ratio of comorbid disorders and psychopathologies.

Stress in caregivers of demented people in Korea--a modification of Pearlin and colleagues' stress model.

OBJECTIVES: We investigated the influence of demented people's function and caregiver burden on caregivers' psychological distress. Based on a modified model of Pearlin and colleagues (1990), we assessed the stress of demented people's caregivers in Korea. METHODS: A total of 122 demented people and their caregivers participated in this study. We evaluated dementia severity by the Clinical Dementia Rating (CDR), and caregivers reported their objective and subjective stressors. Objective stressors are the demented people's functional deterioration and behavioral problems, and subjective stressors are caregivers' reaction to objective stressors and the subjective burden in caregiving. The overall psychological distress of caregivers was evaluated by the Beck Depression Inventory (BDI) and the Spielberger Trait Anxiety Inventory-State (STAI-S). RESULTS: There were significant differences in both objective and subjective stressors according to the severity of dementia, but there was no significant difference in caregivers' psychological distress. The goodness-of-fit evaluation model showed the coping and social support variables affected the psychological distress as mediator variables (df = 109, chi (2) = 209.75, NFI = 0.949, RFI = 0.929, IFI = 0.975, TLI = 0.964, RMSEA = 0.087). CONCLUSION: The model suggests that psychological distress of caregivers is related to diverse variables, such as objective and subjective stressors, coping, and social support. This study provides verification of the modified model of Pearlin and coworkers in Koreans, and lays a foundation for practical use and further study.

Validation study of tripartite model of anxiety and depression in children and adolescents: clinical sample in Korea.

Although the currently available literature has provided some empirical support for a tripartite model of child and adolescent anxiety and depression, one of the limitations of these studies was that they have been conducted in America, primarily with Caucasians. In order to make this model more applicable to diverse ethnic and cultural groups, this study used a tripartite model for child and adolescent anxiety and depression in Korea, using confirmatory factor analysis with logically selected items from the Revised Children's Manifest Anxiety Scale (RCMAS), as well as the Children's Depression Inventory (CDI). The results indicated that the model fit of a three-factor model was superior to one- and two-factor models. In addition, the findings of discriminant analysis demonstrated that the correct classification rate with three factors of the tripartite model was superior to the classification rate achievable using CDI and RCMAS. In a departure from Clark and Watson's hypothesis, however, the correlations of three factors were significantly higher than had been expected. The results are discussed on the basis of cultural background.

Regional cerebral glucose metabolic abnormality in Prader-Willi syndrome: A 18F-FDG PET study under sedation.

UNLABELLED: Prader-Willi syndrome (PWS) is a genetic disorder caused by the nonexpression of paternal genes in the PWS region of chromosome 15q11-13 and is the most common cause of human syndromic obesity. METHODS: We investigated regional brain metabolic impairment in children with PWS by 18F-FDG PET. Sixteen children with PWS (9 males, 7 females; mean age +/- SD, 4.2 +/- 1.1 y) and 7 healthy children (4 males, 3 females; mean age +/- SD, 4.0 +/- 1.7 y) underwent brain 18F-FDG PET in the resting state. The images of PWS children were compared using statistical parametric mapping analysis with those of healthy children in a voxelwise manner. RESULTS: Group comparison showed that children with PWS had decreased glucose metabolism in the right superior temporal gyrus and left cerebellar vermis, regions that are associated with taste perception/food reward and cognitive and emotional function, respectively. Metabolism was increased in the right orbitofrontal, bilateral middle frontal, right inferior frontal, left superior frontal, and bilateral anterior cingulate gyri, right temporal pole, and left uncus, regions that are involved in cognitive functions related to eating or obsessive-compulsive behavior. Interestingly, no significant metabolic abnormality was found in the hypothalamus, the brain region believed to be most involved in energy intake and expenditure. CONCLUSION: This study describes the neural substrate underlying the abnormal eating behavior and psychobehavioral problems of PWS.

Cognitive processing of emotional information in depression, panic, and somatoform disorder.

Emotional Stroop tasks (subliminal/supraliminal exposures), implicit memory tasks (tachistoscopic word identification), and explicit memory tasks (free recall after incidental learning) with 4 word types (physical threat, positive, negative, and neutral words) were administered to patients with major depressive disorder (n=30), panic disorder (n=33), somatoform disorder (n=25), and healthy control participants (n=33). On the Stroop task, panic patients showed subliminal interferences for physical threat and negative words, depressive patients showed supraliminal interferences for negative words, and somatoform patients showed supraliminal interferences for physical threat words. No patient groups demonstrated implicit memory biases. On the explicit memory task, depressive and panic patients showed memory biases for negative words; somatoform patients showed biases for physical threat words.

Roles of cognitive characteristics in tinnitus patients.

To investigate the cognitive characteristics that affect the emotional and functional distress caused by tinnitus and to decide and test the model to explain their relations, 167 patients with tinnitus, who visited Samsung Medical Center, Seoul, Korea between March 2001 and May 2002 were recruited. To examine their features related to tinnitus, the following scales were administered; Tinnitus-related basic questionnaire including dysfunctional beliefs, Tinnitus Handicap Inventory, State-Trait Anxiety Inventory, Anxious Thought and Tendencies, Self-Consciousness Scale, and modified 'catastrophic thought' from Coping Strategies Questionnaire. The results showed that the duration of experiencing tinnitus was 4.7 +/-7.1 yr, those who complained of hearing one sound were the most common (45.5%), and hearing sounds similarly described to whistling were the most common (22.5%). Also, there were significant correlations among tinnitus features, cognitive characteristics, and distresses from tinnitus. As a result of testing the model, Normed fit index, Incremental fit index, Tucker-Lewis index, and Comparative fit index were over .90, indicating that it is a good model, and Root mean square error of approximation showed a reasonable fit. Also, the direct effects of the trait or severity of tinnitus on distress did not appear to be significant, thus it appeared to be affecting indirectly through the cognitive characteristics. This result shows that cognitive interventions can be important for the psychological adaptations of tinnitus patients.

Distributions of ACE and APOE polymorphisms and their relations with dementia status in Korean centenarians.

BACKGROUND: Genetic polymorphisms of angiotensin-converting enzyme (ACE) and apolipoprotein E (APOE) have been reported to be associated with human longevity and dementia in the elderly. However, whether such putative longevity genes exert the same effects on different ethnic groups living in different environments is not well known. METHODS: We investigated the distributions of the ACE and APOE genotypes and their relations with dementia status in Korean centenarians by cross-sectional study. A total of 103 centenarians (13 men and 90 women, mean age 102.4 +/- 2.6 years) were included in this study. The allele frequencies of the genes were compared with those of two control groups: 7232 apparently healthy adults (4100 men and 3132 women) of mean age 48.5 +/- 9.6 years for the ACE genotyping, and 6435 adults (5008 men and 1427 women) of mean age 50.7 +/- 7.9 years for the APOE genotyping. The dementia status of the centenarians was assessed by clinical psychologist using the Clinical Dementia Rating (CDR) score. RESULTS: The frequencies of genotypes and alleles of the ACE and APOE genes of the centenarians were not significantly different from those of the control groups. There was a lack of association between presence of the D allele on the ACE gene and dementia status. However, the frequency of the epsilon4 allele of the APOE gene was significantly higher in centenarians with dementia than in centenarians without definitive dementia (9.1% versus 1.5%, p <.05). CONCLUSIONS: These results suggest that neither the ACE nor the APOE gene is significantly associated with longevity in the Korean population, but that the APOE epsilon4 allele is still related with dementia even at age 100 and older.

Analysis of depression in continuous ambulatory peritoneal dialysis patients.

It is well known that depression and sense of hopelessness worsen the quality of life in end-stage renal disease (ESRD) patients receiving dialysis. However, the characteristics of depression in continuous ambulatory peritoneal dialysis (CAPD) patients have not been analyzed in detail. We performed this study to investigate the severity of depression and the factors affecting depression in CAPD patients. With 96 CAPD patients, we evaluated each patient's depressive mood and hopelessness with CES-D (Center for Epidemiologic Studies Depression) scale and Beck Hopelessness Scale. We also evaluated the degree of stress of each patient with internal individual stress scale. Most CAPD patients experienced severe depression compared with the general population. Their depression was better explained by psychological factors, such as stress and sense of hopelessness, than by demographic or physical factors. On the basis of these findings, we suggest that the treatment of depression in CAPD patients might be possible by modulation of psychological factors.
