[Surgery of the carotid artery: a retrospective study of 35 consecutive cases].

OBJECTIVE: To retrospectively analyze a single institution's experience with surgery of the carotid artery performed as part of an oncological procedure and emergency hemostasis, with the aim to determine the preoperative methods for evaluation of cerebral circulation, selection of surgical procedures and perioperative complications. METHODS: From January 1999 to December 2004, a total of 35 patients underwent carotid artery surgery including repair of carotid artery in 7 cases, ligation or resection of carotid artery in 17 cases, and reconstruction of carotid artery in 11 cases. All the patients were evaluated for blood flow in the circle of Willis with DSA, DSA plus TBO plus SPECT, and TCD and followed up from 4 months to 4 years. The perioperative complications and surgical outcomes were recorded and analyzed. RESULTS: There were 16 carotid body tumors, 1 malignant carotid body tumor, 17 malignant tumors involving the carotid artery and 1 traumatic arterio-venous fistula. Twenty-seven patients underwent carotid occlusion test, 10 were positive and 17 were negative (tolerable). Of 7 cases with repair of the carotid artery, 1 patient died of uncontrolled bleeding due to rupture of the anastomosis, and the remaining was uneventful. Of 17 cases with ligation or resection of the carotid artery, 4 developed neurologic deficit 2 weeks postoperatively. Three patients with malignant tumors died 1 month, 4 months, and 4 months postoperatively, respectively. One patient with carotid artery body tumor complained of inarticulate speech 4 months after operation; another patient complained of incapability and limited movement of the ipsilateral upper limbs; 5 patients complained of dizziness 4, 6, 12, 24 and 48 months postoperatively. Of 11 patients who underwent carotid reconstruction, no major cerebral complications were noted after operation. One patient died of recurrence, 1 patient with carotid body tumor complained inarticulate speech and incapability of the contralateral limbs, the remaining was uneventful. Color Doppler showed patent vascular graft 1 year postoperatively. CONCLUSIONS: TBO plus SPECT is a reliable method for evaluation of the circle of Willis currently. The short term and long term complications of ligation of carotid artery are high, therefore, resection and revascularization of the carotid artery is advocated for carotid artery tumors when possible.

[The establishment of conditional MT transgenic mice model].

OBJECTIVE: To construct the doxycycline-inducible MT transgenic mice model, and provide a basis for the study of hemangioma as well as MT molecular function in vivo. METHODS: Tetracycline-controlled expression systems were employed to this study. A conditional transgenic vector combining the two transcriptional units on a single plasmid was constructed, and the MT gene was subcloned into this vector. To minimize any potential interference, the two elements were spaced with a 1.2 kb cHS4 insulator. To shield the transgene from the affection of chromosomal position effect and improve its expression efficiency, another cHS4 insulator was inserted into the upstream of transgene cassette. After transient transfection of cells in vitro, and analyzing the relative quantification of MT transcripts (target) in mRNA samples by semi-quantitative RT-PCR method, the pronuclear microinjection technique was used to introduce the purified transgene into the chromosomes of fertilized mice eggs, in order to obtain transgenic positive animals. The MT expression in positive mouse was induced through adding deoxycycline in drinking water. Phenotype analysis was done by pathology, and MT expression was confirmed by RT-PCR. RESULTS: The conditional transgenic vector was constructed successfully, and the expression of MT in vitro was regulated by doxycycline. Five transgenic positive mice were obtained through pronuclear microinjection. After MT induction, one transgenic mice developed hemangiomas, and the expression of MT was confirmed by RT-PCR method. The others were active and in breeding. CONCLUSION: Conditional MT transgenic animal model was constructed successfully, and may provide platform for the experimental research of hemangioma as well as the MT molecular function in vivo.

[Try to improve journal quality by improving standards and editing process].

The quality of medical journals depends on several factors involving 3 groups of people with their independent but relevant roles: the authors, the reviewers and the editors. Peer review and editing is the key factor to improve the quality of medical publications and journals. Shanghai Journal of Stomatology (SJS) has been regarded as a leading journal for publishing high-quality work in the field of stomatology in China. In October 2003, it was accepted by the National Library of Medicine, USA, to be indexed and included in Index Medicus and MEDLINE. To further improve the journal's overall quality, the Editorial Agency led by Professor Zheng Jia-wei has made great efforts to formulate its essential requirements in paper style, bilingual abstract writing and statistical analysis for the manuscripts submitted for possible publication. Strict independent peer review system has been adopted to assess the quality of the manuscripts received since it was founded in 1992. The reviewer is required to address detailed aspects of the paper under review and to resend his or her opinion on the paper. The editorial management is a crucial part of the publishing process. The editors begin action with the receipt of the manuscript, direct the various steps of evaluation, correction and re-submission, until a decision is made to accept or reject the paper at the regular meeting of Decision Making Group on Manuscripts of SJS led by the Editor-in-Chief. Once a paper is accepted and carefully revised, the editors will make necessary text and layout editing. Due consideration is given to the statistical, bilingual and ethical aspects as well as to the overall uniformity of the terminology, nomenclatures and style throughout the volume as a whole in the promotion of standards. The journal has not been cited by Science Citation Index (SCI) till now, further steps should be taken to make this journal better known throughout the World, to improve the quality of the publications and reduce the delay between the initial receipt and the final publication of manuscripts.

[Management of lymphatic malformations in oral and maxillofacial regions: the rationale according to the new classification].

According to the classification of ISSVA, Waner and Suen, the traditionally called lymphangiomas are now referred to as lymphatic malformations, including both macrocystic and microcystic lesions. They are commonly seen vascular anomalies, and most frequently diagnosed at birth and most often occur in the head and neck area. The etiology of lymphatic malformations is still unknown, resulting in a variety of treatment modalities. This paper reviewed the recent literatures with respect to the indications, contraindications, advantages and disadvantages of surgical excision, sclerotherapy and laser ablation of lymphatic malformations of the head and neck, with detailed discussion of the advances in molecular biology and clinical treatment prospects. It is concluded that although lymphatic malformations are benign lesions, they seldom involute spontaneously. Of all vascular malformations, lymphatic malformations are the most difficult to be eradicated. Their infiltrating nature coupled with the difficulty in distinguishing involved important structures of the head and neck from adjacent normal tissues makes complete surgical extirpation even more difficult. The likelihood of postsurgical recurrence and complications is thus higher than other vascular lesions. Although many treatment protocols are available in clinic, indications, contraindications, advantages and disadvantages exist in each modality. The selection of treatment modalities should depend on the patients' status and techniques available. The treatment protocol should be individualized and comprehensive as well as sequential, in order to obtain the best treatment outcomes. In general, treatment will vary according to the depth and the extent of the lesions. Superficial mucosal microcystic lesions and cervicofacial macrocystic lesions are amenable to ablation with sclerotherapy using Bleomycin and OK-432 with the advantages of the absence of a surgical scar. The sclerosing agent OK-432 is effective for macrocystic lymphatic malformations but showed less promise for microcystic lesions, mixed lesions, and lesions outside the head and neck region. In addition, superficial mucosal microcystic lesions are also amenable to CO2 laser therapy. Deeper microcystic lesions are still challenging head and neck surgeons, which are usually extensive and sometimes need to be resected in stages, and even may be impossible to be completely removed. Somnoplasty shows promise for reduction of tongue lymphatic malformations. Surgical excision, staged when necessary, continues to be integral to management in many cases, but should not be overused without consideration of the histologic types and extent. Localized mucosal microcystic lesions and major cervicofacial macrocystic lesions are amenable to primary excision. Care should be exercised in identifying and preserving important cervical and facial structures because anatomical planes are often distorted. Combined sequential approach is recommended for mixed lesions as well as extensive lesions involving both the mucosa and soft tissues.

[Carotid body tumor: a retrospective study of 26 consecutive cases].

OBJECTIVE: To study the clinical features, diagnoses and operative intervention of carotid body tumors. METHODS: The medical records of 26 patients with carotid body paragangliomas between 1993 and 2004 were retrospectively reviewed. RESULTS: Two of twenty-six patients had bilateral tumors. One patient had malignant tumors with cervical lymph nodes metastases. Twenty-one patients were evaluated with a preoperative angiography. No preoperative embolization was performed. Eleven patients underwent ultrasonography, nine had CT scan of the neck, five had magnetic resonance imaging. Twenty-six patients were all operated. Vascular reconstruction was performed in five cases. The ligation of carotid artery was carried out in four cases and another patient had vagus nerve resection. No death and recurrence occurred in all cases. CONCLUSION: Angiography and ultrasonography are valuable in the diagnosis of carotid body tumor. Surgical removal should be made early to avoid the possibility of eventual metastasis and progressive local invasion. The pattern of operation should be chosed according to the relation of tumor and carotid.

[Writing English abstracts of high quality in standard English acceptable for publication].

Writing an abstract presents a unique challenge to any author. It requires brevity of the written word, conciseness of thought and accuracy of presentation. A structured abstract is the most important starting point for a well written dental article. Shanghai Journal of Stomatology, a Chinese national journal of oral medicine, oral and maxillofacial surgery, prosthodontics, orthodontics, oral pathology, endodontics, oral radiology, etc, has been included and indexed in Chemical Abstracts, Index Medicus and MEDLINE. This peer reviewed refereed journal demands Chinese and English abstracts of high quality for all manuscripts. Each manuscript should provide qualified abstracts for citation and indexing, which are written in standard English using the correct terms, sentences and grammar. Structured abstracts about 500 characters are required for clinical articles, clinical reports, basic scientific articles; while indicative abstracts (summary) are mandatory to short communications and case reports as well as review articles. Generally, the abstracts should be written in the past tense and passive voice (in the third person), with exception of conclusion(s), which can be in the primary tense and active voice. The structured abstracts are consisted of PURPOSE (S), METHODS, CONCLUSION (S), and CONCLUSION (S), which should offer the important information on the number of the subjects, methods of grouping and statistics, database attained, P values, etc. The conclusion (s) must be detailed rather than conceptive. This editorial introduces the ins and outs of writing an English abstracts acceptable for publication.

[Electromyography study on complete bilateral cleft lip after reparation.].

PURPOSE: This study was undertaken to evaluate the motion effect of complete bilateral cleft lips objectively using electromyography. METHODS: 45 patients of complete bilateral cleft lips after operations several years which included 20 patients repaired using orbicularis oris muscle restoring functional method and 25 patients using straight line suture method were examined with electromyography (EMG) in prolabium, right and left lateral lips respectively. The most potential in every area of each patient at the maximal lip contraction position was measured and the values of two different method groups were subject to statistical analysis. RESULTS: The prolabium and lateral lips of both groups had no electric activity at posture position. At maximal contraction position, the mean potential of prolabium of orbicularis oris muscle restoring method group had no difference with the lateral lips. But the mean potential of prolabium of straight line suture method group was lower than that of the lateral lips significantly, also,it was lower than the potential of prolabium in the muscle functional reparation group. CONCLUSIONS: The reparation method of restoring orbicularis oris muscle to complete bilateral cleft lips was better than the straight line suture method in the motion effect. These findings suggest the superiority and necessity of functional muscle reparation for complete bilateral cleft lips.

[Intraosseous neurilemmoma of the jaw: report of 11 consecutive cases].

OBJECTIVE: This study retrospectively analyzed the clinical data of 11 patients with intraosseous neurilemmoma of the jaw, with the purpose of improve its diagnosis and treatment. METHODS: 11 patients with intraosseous neurilemmoma of the jaw treated from January 1993 to February 2002 were reviewed, including the clinical features, pathological findings, immunohistochemical findings, imaging examinations and treatment methods. 8 patients with benign lesions were treated with radical curettage and cautery. 2 patients with mandibular malignant lesions were treated with extended resection and neck dissection. 1 patient with maxillary malignant lesion concomitant with neurofibrostosis was treated with extensive resection and middle split skin graft and neck dissection, followed by chemotherapy and radiotherapy after surgery. RESULTS: The lesions were located in the maxilla in 5 patients (1 was malignant). 6 patients had mandibular lesions (2 were malignant). The X ray findings had some specificity. 10 patients were followed up for 7 to 122 months without recurrence. 1 patient with maxillary malignant lesion died of multiple metastasis 11 months after treatment. CONCLUSIONS: Intraosseous neurilemmoma of the jaw was rarely seen clinically. CT, MRI and fine needle aspiration biopsy were beneficial for early diagnosis. Surgery is still the dominant choice of treatment. The prognosis of the patients was generally good. Adjuvant radiotherapy or chemotherapy had limited value, especially for patients with neurofibrostosis. A novel regime called CYVADICDE was recently invoked much clinical interest.

[Histological evidences of using human external jugular vein to reconstruct carotid artery: a comparative study with greater saphenous vein].

OBJECTIVES: To investigate the histological architecture of human external jugular vein and greater saphenous vein and to determine the feasibility of using autogenous external jugular vein for reconstruction of carotid artery defects. METHODS: Specimens of 3 external jugular veins and greater saphenous vein and 1 segment of common carotid artery resected due to involvement of malignant tumor were available. The specimens were subjected to HE staining and examined under light microscopy. In addition, elastic fiber stain and Smactin stain were conducted to detect the amount and location of the elastic fibers and smooth muscles in the venous specimens. RESULTS: The smooth muscle cells in external jugular vein and greater saphenous vein were different both in amount and orientation of arrangement. There were more smooth muscle cells in the tunica media of the external jugular vein arranged circumferentially, while in greater saphenous vein, the smooth muscle cells were mainly located in the adventitia and arranged longitudinally. There were fewer elastic fibers in the venous walls. CONCLUSION: Histologically, it seemed more reasonable to reconstruct the carotid artery with external jugular vein.
