Comparison of drug eluting stents with bare metal stents in daily practice for bifurcation lesions in Chinese patients.

BACKGROUND: Recently, numerous randomized and controlled trials have demonstrated great advantages of drug eluting stents (DES) with respect to significant reduction of restenosis and recurrence of symptoms and improvement of clinical outcomes after percutaneous coronary intervention (PCI). Little is known about the comparative effects between DES and bare metal stents (BMS) for bifurcation angioplasty in the Chinese population. We compared the inpatient and 7-month follow-up outcomes between DES and BMS for the treatment of bifurcation lesions. METHODS: From April 2004 to October 2005, 291 Chinese patients [85.9% male, mean age (57.8 +/- 10.4) years] underwent DES (387 lesions) and/or BMS (297 lesions) implantation for bifurcation lesions. Clinical and angiographic follow-up was performed at 7 months. RESULTS: Compared with BMS group, patients in DES group had significantly lower rates of restenosis at main branch (9.5% vs 28.7%, P < 0.001) or side branch (14.5% vs 37.0%, P < 0.001) and major adverse cardiac events (MACE) (14.0% vs 26.3%, P = 0.000). The occurrence rate of late in-stent thrombosis did not differ between the two groups in both main (0.8% vs 0, P = 0.224) and side branches (1.4% vs 0, P = 0.198). Target lesion revascularization (TLR) was less frequent in DES group for main branch (8.3% vs 21.3%, P < 0.001) and for side branch (7.6% vs 23.5%, P < 0.001). Multivariate regression analysis revealed that total stent length (OR = 1.029, P = 0.01), postprocedural in-stent minimum lumen diameter (OR = 0.476, P = 0.03) and stent type (OR = 3.988, P = 0.0001) were independent predictors of TLR for main branch. Prior history of coronary intervention (OR = 2.424, P = 0.041), angulated lesion (OR = 2.337, P = 0.033), postdilation (OR = 0.267, P = 0.035) and stent type (DES vs BMS, OR = 5.459, P = 0.000) were independent predictors of TLR for side branch. CONCLUSION: The implantation of DES may be associated with greater reduction of restenosis and TLR than BMS in bifurcations angioplasty.

A comparison of angiographic and clinical outcomes after sirolimus-eluting versus paclitaxel-eluting stents for the treatment of in-stent restenosis.

BACKGROUND: In-stent restenosis (ISR) remains a challenge for interventional cardiologists. Some data suggest that drug-eluting stents (DES) represent a promising new option for the treatment of patients with ISR. Currently, 2 DES platforms are available [sirolimus-eluting stents (SES) and paclitaxel-eluting stents (PES)], but the superiority of either approach for treating ISR has not been convincingly demonstrated. The aim of the present study was to retrospectively compare angiographic and clinical outcomes after treatment of ISR with SES or PES in a series of consecutive patients with ISR. METHODS: A total of 745 consecutive patients were treated with bare metal stents from April 12, 2004 to December 31, 2004 in our center. Of these, clinically driven target lesion revascularization (TLR) was performed in 54 ISR from 54 patients at 7 months. Of the 54 patients with ISR, 36 received SES and 18 received PES. Follow-up included angiography and assessment of clinical outcome, both performed 7 months after DES implantation. RESULTS: There were no significant differences in baseline clinical data (including medication usage and lesion characteristics) between the two groups. Except for overlapping of multiple stents, procedural parameters were also similar in both groups. Seven-month angiographic follow-up showed that the binary restenosis rate was higher in patients treated with PES than that in patients treated with SES (in-stent binary restenosis: 27.8% vs 5.6%, P < 0.023; In-segment binary restenosis: 44.4% vs 13.9%, P < 0.014). Major adverse cardiac events (MACE) occurring during hospitalization or during the follow-up period including thrombosis and TLR was similar in both groups (22.2% vs 8.3%, P > 0.05). CONCLUSIONS: Results from this small sample size, retrospective, single-center study showed that SES might be superior to PES in treating ISR because of lower 7-month restenosis rates (both in-stent and in-segment binary restenosis) with no increased incidence of MACE.

A single center investigation of bare-metal or drug-eluting stent restenosis from 1633 consecutive Chinese Han ethnic patients.

BACKGROUND: Stents are widely used in China but the clinical impression is somehow that restenosis is less common because of the lower prevalence of coronary artery disease (CAD) and associated risk factors in Chinese populations. However, no large-sample published studies are available on angiographic stent restenosis including those of bare-metal stent (BMS) or drug-eluting stent (DES) in Chinese Han ethnic population. METHODS: A total of 1633 consecutive patients with CAD who had undergone coronary stenting, quantitative coronary angiography (QCA) were retrospectively studied. At the time of stent implantation and at 7 months post-stenting 675 patients had a follow-up angiography. Statistical analysis was made with the chi-square test for categorical variables, unpaired t test for continuous variables, univariate or multivariate regression for baseline and angiographic characteristics and the Kaplan-Meier method for rate of target lesion revascularization (TLR). RESULTS: Stent restenosis was defined as > or = 50% diameter stenosis in the dilated segment. A total of 675 patients with 1074 lesions were subjected to angiographic follow-up for 7 months on average. Of these lesions, 448 were implanted with BMS whereas 626 lesions with DES. At 7 months, bare-metal in-stent restenosis occurred in 148 lesions (33.0%), and bare metal in-segment restenosis in 155 lesions (34.6%) in contrast to drug-eluting in-stent restenosis in 48 lesions (7.7%) and drug-eluting in-segment restenosis in 73 lesions (11.7%) (P < 0.001 compared with BMS respectively). Late loss in both in-stent and in segment was higher in BMS than in DES groups [(1.00 +/- 0.69) vs (0.28 +/- 0.52); (0.78 +/- 0.71) vs (0.21 +/- 0.52), P < 0.001 respectively]. Angulated lesion, lesion length, pre-procedural minimal luminal diameter (MLD), and BMS were independent predictors for TLR, (P < 0.01 respectively), whereas current smoker, ostial lesion, and stent overlapping, post-procedure in-stent MLD, lesion length, and stent types were independent predictors for in-segment restenosis (P < 0.01 respectively). Standard coronary risk factors such as hypertension, hyperlipidemia, diabetes, and history of CAD were not associated with a higher rate of restenosis caused by BMS or DES implantation in our Chinese Han ethnic population. CONCLUSIONS: Coronary stenting including BMS or DES implantation in Chinese Han ethnic patients is associated with a restenosis rate comparable to that demonstrated in previous studies from the western countries, and predictors of stent restenosis are somehow different from those in the western population.

[Effect of co-expressing M-bcr/abl and m-bcr/abl fusion gene transcripts on the clinical features of patients with chronic myelogenous leukemia at diagnosis].

OBJECTIVE: To study the effect of co-expressing M-bcr/abl and m-bcr/abl fusion gene transcripts on the clinical features of the patients with chronic myelogenous leukemia (CML) at diagnosis. METHODS: m-bcr/abl fusion gene transcripts were detected by nested PCR. RESULTS: The percentage of the CML patients who co-expressed M-bcr/abl and m-bcr/abl was 51.4% at diagnosis. Comparison of the patients who co-expressed and those who expressed M-bcr/abl alone showed there was no difference in their hemoglobin concentrations, WBC counts, neutrophilic alkaline phosphatase (NAP) stains, Ph chromosomes and hepatosplenomegaly; however, significant difference in platelet count was seen. In the patients with b3a2 who co-expressed m-bcr/abl, an increased platelet count was noted, compared with that of patients with M-bcr/abl alone. But in patients with b2a2, the two groups showed no difference in all clinical features. Two patients had priapism, their m-bcr/abl being negative. CONCLUSIONS: CML patients who co-expressed b3a2 and m-bcr/abl showed a tendency to have an increased platelet count at diagnosis. Co-expressing m-bcr/abl has no the effect on the clinical features of CML patients with b2a2.

[Analysis of Toll-like receptor 4 and myeloid differentiation protein-2 interaction with fluorescence resonance energy transfer].

OBJECTIVE: To study the interaction between Toll-like receptor (TLR) 4 and myeloid differentiation protein-2 (MD-2) in living cells using fluorescence resonance energy transfer (FRET) technology. METHODS: The coding sequences of TLR4 and MD-2 (without the signal peptide sequence) were amplified by PCR and cloned into enhanced cyan fluorescence protein (CFP) and enhanced yellow fluorescence protein (YFP) expression vectors carrying TLR4 signal peptides (pECFP-C1-SP and pEYFP-C1-SP). HEK293 cells were transfected respectively or together with the reconstructed plasmids verified by enzyme digestion and sequence analysis, and the expression and sublocalization of these fluorescence proteins in the cells were observed using fluorescence microscope. FRET in the cells coexpressing CFP-TLR4 and YFP-MD-2 was detected using routine and acceptor photobleaching method. RESULTS: The reconstructed plasmids were expressed in HEK293 cells. The cyan or yellow fluorescence was located in the cytoplasm, mainly around the nucleus in the cells transfected with pECFP/TLR4 or pEYFP/MD-2, and both the cyan and yellow fluorescence located mainly in the membrane and occasional in the cytoplasm of cells cotransfected with pECFP/TLR4 and pEYFP/MD-2. Routine or acceptor photobleaching detected FRET phenomena in cells coexpressing CFP-TLR4 and YFP-MD-2, suggesting direct interaction between TLR4 and MD-2. CONCLUSION: This study provides direct evidence of the interaction between TLR4 and MD-2 in living cells.

[Detection and typing of dengue virus using polymerase chain reaction and microwell plate hybridization].

OBJECTIVE: To establish a specific, sensitive and practicable method for detection and typing of dengue virus. METHODS: Based on the genomic sequence analysis of dengue virus types 1-4, 4 pairs of primers were designed. The specific capture probes of dengue virus types 1-4 were amplified using RT-PCR, cloned and sequenced before using them for precoating the microwell plate. The samples were amplified using biotin-labeled forward primer and reverse primer, and microwell plate hybridization was carried out for detection and typing of dengue virus types 1-4. RESULTS: The absorbance of the positive samples were higher than 0.5, while the average absorbance of the negative samples was lower than 0.1, with the S/N higher than 10. CONCLUSION: The method of PCR-ELISA we established for early detection and typing of all 4 dengue viruses seretypes.

[Exogenous PTEN enhances apoptosis in pancreas cancer cell line ASPC-1 induced by hypoxia].

OBJECTIVE: To investigate the effect of exogenous phosphatase and tensin homologue deleted on chromosome ten (PTEN) on cell cycle, the expression of vascular endothelial growth factor (VEGF) and epidermal growth factor receptor (EGFR) proteins, and cellular proliferation ability in human pancreas cancer cell line (ASPC-1) exposed to normal oxygen or hypoxia 1% for 24 h were determined. METHODS: ASPC-1 cells were transfected in vitro with an eukaryotic expression plasmid (pEAK8) containing PTEN or not by lipofectin. Positive cell clones were selected, amplified and named ASPC-1-pEAK8-PTEN or ASPC-1-pEAK8 cells. RT-PCR and Western blot were used to determine the target gene expression. PTEN, VEGF and EGFR proteins were assessed by Western blot assay. Cell cycle and the induction of apoptosis were detected by flow cytometry. The tumor growth ability in vivo was assessed in nude mice, and cologenic survival ability was assayed under normal oxygen or hypoxia condition. RESULTS: The expression of PTEN mRNA and protein in ASPC-1-pEAK8-PTEN cells were significantly higher than that in ASPC-1-pEAK8 or ASPC-1 cells. The expression of VEGF protein in ASPC-1-pEAK8-PTEN cells decreased by 23.4%, but EGFR showed no change. The plating efficiency was decreased by 28.0% (F = 4.283, P < 0.05) under normal oxygen condition, compared with those in ASPC-1 cells. The tumor volume in nude mice with ASPC-1-pEAK8-PTEN were significantly different compared to those with ASPC-1 5 weeks after implantation (t = 4.834, P < 0.01). The tumor inhibitory rate was 42.4% in ASPC-1-pEAK8-PTEN group. The expressions of VEGF and EGFR were decreased by 31.4% and 25.0%, respectively. In comparison with ASPC-1 cells, the plating efficiency of ASPC-1-pEAK8-PTEN cells was decreased by 33.2% (F = 9.152, P < 0.01) under hypoxic condition. The cellular apoptosis 8 h after hypoxia and G(2)/M blockage in ASPC-1-pEAK8-PTEN cells were remarkably higher than those in ASPC-1 cells. CONCLUSION: Exogenous PTEN can block ASPC-1 cell cycle at the G(2)/M phase, enhance the cell apoptosis induced by hypoxia, inhibit the expression of VEGF and EGFR proteins under hypoxic condition, and inhibit the proliferation and growth of ASPC-1 cells.

[Characteristic of diagnosis and treatment of spinal cord stab injury].

OBJECTIVE: To investigate the clinical characteristics of the incidence, diagnosis, treatment and prognosis of stab injury of spinal cord. METHODS: Retrospective study of the clinical materials of 42 stab spinal cord injury patients who were treated in our hospital from 1992 to 2004. RESULTS: Among the patients, 38 were male, and 4 were female. 80% of the cases were thoracic spinal cord injury patients. Incomplete injury cases accounted for 71.4% of all the cases. Of all the 30 incomplete injury cases, 14 had the sign of standard Brown-Sequard syndrome, 6 had the sign of non-standard Brown-Sequard syndrome and 10 do not have the sign of Brown-Sequard syndrome. Magnetic resonance image was very useful to the diagnosis and treatment of the disease. Debridement was a routine to do in acute stage. Operations such as scrutinization of the spinal canal, clean-up of the hematoma, extraction of foreign bodies were optional according to different conditions. After treatment, most of the patients got good recovery as far as ASIA scale and ADL scale were concerned. CONCLUSION: According to the results above, it is recommended that enough attention should be given to the unique aspects of the disease to effectively facilitate recovery of such cases.

[Effect of compound shenhua tablet on macrophage migration inhibition factor in renal tissue of 5/6 nephrectomized rats].

OBJECTIVE: To observe the effect of compound shenhua tablet (CST) on the residual kidney expressed macrophage migration inhibition factor (MIF) in rats. METHODS: CST was used to treat 5/6 nephrectomized rats for 12 weeks and the conditions of blood pressure, urinary protein, blood biochemical indices (creatinine, blood urea nitrogen), kidney pathologic change and MIF expression were observed. RESULTS: CST could significantly lower the serum levels of creatinine (P < 0.05), and 24 hrs urinary protein (P < 0.01), reduce the MIF expression and macrophage infiltration in renal glomerulus and tubular mesenchym, and lower the degree of renal glomerular sclerosis and interstitial fibrosis. CONCLUSION: The inhibition on the highly expressed MIF may be an important mechanism of the drug in restraining chronic inflammation in residual kidney, delaying the sclerosis and fibrosis progression and protecting renal function.

[The prevalence and its effective factors of primary angle-closure glaucoma in defined populations of rural and urban in Beijing].

OBJECTIVE: To investigate the prevalence of primary angle-closure glaucoma (PACG) of urban and rural residents in Beijing, China. A quantitative comparison was made based on the data collected from this epidemiological survey. We also identify some of the most typical risk factors associated with PACG. METHODS: Glaucomatous screening examination was applied to specific age group populations (aged 40 and older) in the defined district of Beijing and its remote rural county, from June to October, 2001. There are 4451 subjects in all, 1980 rural subjects and 2471 urban subjects, 1939 males and 2512 females. The screen and diagnostic methods used in this survey included van Herick methods and gonioscopy examination to estimate the peripheral depth of the anterior chamber, visual acuity, intraocular pressure, refraction status, stereoscopic fundus photography, and threshold-related visual field tests. Subjects regarded as suspected glaucoma and glaucoma patients are reexamined with standard glaucomatous examination. RESULTS: The response rate of rural and urban population is 79.6% (1980/2488), 87.1% (2471/2836), respectively. The prevalence of PACG (in aged 40 years or older population) resulted from this survey was 1.2% (95% CI 0.9% - 1.5%). However, the prevalence was different between urban and rural residents, 1.1% (95% CI 0.8% - 1.4%) vs. 1.6% (95% CI 1.2% - 2.0%). The prevalence of PACG in female was more than that in male, 1.7% (95% CI 1.3% - 2.1%) vs.0.8% (95% CI 0.5% - 1.1%). A drastic increase in prevalence of PACG with age increase was identified in both survey sites, however, this increase in rural subjects (aged 60-69 years group) occurred ten years earlier than those from urban subjects (aged more than 70 years group). Compared to urban residents, rural residents showed higher prevalence of unilateral low vision (39.3% vs. 20.6%) and blindness (28.6% vs. 14.7%). This survey also confirmed that, as people aging, refraction status became hyperopia, the depth of peripheral anterior chamber became narrow. In the different age groups, female and male groups, the changes of refraction status and the depth of peripheral anterior chamber paralleled the prevalence of PACG. CONCLUSIONS: The prevalence of PACG was obviously different in different groups. This could due to several factors including gender, age, change of refraction status and chamber angle as well.

[The renoprotect effect of shenhua recipe on 5/6 renal ablation rats].

OBJECTIVE: To investigate the reno protective effect of Shenhua recipe on the experimental model of 5/6 renal ablation. METHOD: 5/6 renal ablation rats were underlying this experiment. They were administered Shenhua, irbesartan respectively by gavage during 12 weeks. Body weight, systolic blood pressure, proteinuria, Scr, BUN, total protein, albumin, Glycero and cholesterol were measured. Histologic glomenular and tubulointerstitial damage scores were measured at 12 weeks. RESULT: The treated groups showed significantly less histologic glomerular and tubulointerstitial damage scores at 12 weeks. The plasma albumin were higher ( P < 0.05), urine protein excretion rates, serum cholesterol and creatinine were lower than in nontreated group, but arterial blood pressure was not significantly different in the three Shenhua treated groups compared with nontreated group. CONCLUSION: Shenhua can retard the progression of chronic renal injury in the 5/6 renal ablation without changes in systolic blood pressure.

[Effects of in vitro PTEN transfection on proliferation of human pancreatic cancer ASPC-1 cells].

OBJECTIVE: To investigate the effect of phosphatase and ensinhomology deleted on chromosome ten (PTEN) on the human pancreatic cancer cell line without or low PTEN expression. METHODS: We selected the lowest PTEN gene expressive pancreatic cancer cell line among the four pancreatic cancer cell lines (Miapaca I, Miapaca II, JF305 and ASPC-1) through RT-PCR assay method, and transfected plasmid (Peak8) inserting PTEN or not in vitro into it by lipofectin. The effect of PTEN transfection on the lowest PTEN gene expressive pancreatic cancer cell line was carried out by flow cytometry, immunohistochemical staining, cloning survival assay, as well as tumorigenicity in nude mice. RESULTS: The lowest PTEN gene expression was found in ASPC-1 cells. PTEN mRNA expression and cloning plating efficiency in the ASPC-1, A-pE and A-pE-P cells were 20.3%, 15.0%, 56.8% and 33.3%, 31.7%, 24.0% respectively. We found that ASPC-1 cells transfected with PTEN exhibited significantly more protein, less vascular endothelial growth factor protein, and non-changed epidermal growth factor receptor protein comparing with vector-transfected cells. The tumor volumes were 202.7 mm(3) and 142.4 mm(3), pre-and post-transfection with significance (P < 0.01) within 5 weeks. CONCLUSIONS: Our findings suggested that PTEN mRNA exhibited the lowest expression in ASPC-1 cell line, exogenous PTEN dramatically inhibits the growth of ASPC-1 cells transfected with PTEN gene in nude mice.

[The value of frequency doubling perimetry in glaucoma screening of aged 40 or more population].

OBJECTIVE: To investigate the diagnosis value of the frequency doubling perimetry (FDP) screening program in the aged 40 or more natural population-based glaucoma screening. METHODS: In 3 natural villages of Daxing District and 4 northern metropolitan communities of Beijing, 4439 subjects aged 40 or over were screened for glaucoma from May to October in 2001. The screening protocol C-20-1 of the frequency doubling perimetry (FDP) was used in all subjects. For suspect glaucoma subjects, TOP threshold visual field testing with an Octopus perimeter, gonioscopy and stereo fundusgraphy were performed on different days. Detected Glaucoma patients were classified as mild (MD 5 dB or less), moderate (MD > 5 dB and < 10 dB) and advanced (MD 10 dB or more) degrees according to their Octopus test results. FDP parameters including test time, numbers of abnormal location, comtotalon score and score by quadrant were analyzed. Data from one eye of normal subjects and milder eye of optic nerve damage in glaucoma subjects were analyzed. RESULTS: After excluding subjects whose fixation error or false positive error was > 0.33, data from 4330 subjects (97.54%) were analyzed. If abnormality of one location or more was used as a FDP cutoff, and subjects were divided into normal and glaucoma two groups (excluded other eye diseases in non-glaucoma subjects); the FDP had a sensitivity of 50.70% (72/142), a specificity of 90.58% (2260/2340), an area under the ROC curve (AUC) of 0.74, a positive predictive value (PV) of 47.71%, a negative predictive value of 96.95%, a positive likelihood ratio (LR) of 14.83 and a negative LR of 0.51. If excluded the eyes without glaucomatous optic nerve changes and visual field damage, the FDP had a sensitivity of 76.39%, an AUC of 0.87, a positive PV of 40.74%, a negative PV of 99.25%, a positive LR of 22.34 and a negative LR of 0.24. The sensitivity of detecting mild, moderate and advanced glaucoma was 54.17%, 76.00% and 100.00%, respectively. There was strong negative correlation between FDP quadrant score and quadrant mean sensitivity (MS) of Octopus perimetry in glaucoma patients (Spearman correlation, Rs = -0.732, -0.628, -0.639, -0.679, all P = 0.000). CONCLUSIONS: The FDP screening protocol C-20-1 is a very rapid functional test for screening glaucoma in large natural populations aged 40 or more and is proven to be feasible. It has high specificity and good sensitivity for moderate and advanced glaucoma.

[Microbial community composition and phylogenetic analysis of deodor biofilter under different pH conditions].

Molecular biological methods were applied to analyze the genetic diversity of microbe community in biofilter. V3 variable fragments of genes coding for 16S rRNA were amplified by Polymerase Chain Reaction. Then PCR-DGGE combined technique were used to analyze the microbial population composition and phylogeny of deodorant biofilter. The results indicate that there are prominent differences in microbial diversity and abundance between acid and neutral conditions. Owing to the selective pressure of extreme acidity, there is less diversity of microbe compared with that in neutral environment. It is demonstrated the obvious distinction of spacial diversity in different biofilter bed as well. PCR and T-vector cloning sequencing results shows that the sulfur-oxidizing bacteria are the predominant population. The research offers the valid scientific basis for better treatment of odorant gas and the theoretical foundation for application of bio-deodorization.
