[Laparoscopic aid in upper urinary reconstructive operation].

OBJECTIVE: To evaluate the efficacy and feasibility of laparoscopic aid in upper urinary reconstructive operation. METHODS: Fifty-eight patients with ureteropelvic junction obstruction, 5 patients with upper ureter polypous, 2 patients with upper ureter stenosis, and 13 patients with upper ureter lithiasis underwent upper urinary reconstructive operation with laparoscopic aid described as follows:an incision 1 cm long was made, a 10 mm trocar and a 30 degrees laparoscope were wt in, the part with lesion was isolated and resected, and then pyeloplasty or end-to-end anastomosis of ureter was performed. RESULTS: The mean operative time was 33 minutes (25-45 minutes). The mean blood loss was 20 ml (15-25 ml). Complications such as urinary leakage and infection were observed. The double J stent was removed at 1 month after the operation. Follow-up for 3 to 15 months in 20 cases showed alleviation of hydronephrosis. CONCLUSION: An effective and safe method with less wound and operative time, combination of laparoscopic aid and open surgery in upper urinary reconstructive operation helps avoid difficult laparoscopic operation, does not increase trauma of the abdominal wall, and is worth promoting clinically.

[Interventional treatment for occlusive lesions in supra-arch vessels with brain ischemic symptoms].

OBJECTIVE: To evaluate the therapeutic effect of percutaneous transluminal angioplasty and stenting for occlusive lesions in supra-arch vessels with brain ischemic symptoms. METHODS: A total of 56 occlusive lesions of the innominate, subclavian, and internal carotid arteries were treated with angioplasty and intravascular stent placement in 52 patients. The degree of stenosis was more than 60% in all patients and the leading symptom was brain ischemia. In 11 patients, the occluded subclavian artery could not be crossed via the femoral route, therefore the brachial route was used and the occlusion was crossed in a retrograde fashion. RESULTS: Technical success was achieved in 54 of 56 procedures (96.4%). Residual stenosis was less than 30% in all cases. The clinical symptoms improved after procedure in all patients. Complications occurred in 3 of 52 (5.8%) procedures, and the 30-day mortality rate was 0. There were no death and severe stroke in the follow-up period (means 24 months). Restenosis was observed in 2 patients after percutaneous transluminal angioplasty treatment. CONCLUSION: Percutaneous transluminal angioplasty with or without stenting is a safe and valid choice for treatment of occlusive lesions of supra-arch vessels in patients with cerebral ischemic symptoms.

[Access to tuberculosis care among community patients with chronic cough in Yangzhong County, Jiangsu Province].

OBJECTIVE: To describe the access to tuberculosis care among patients with long-term chronic cough (potential TB cases), and to study the impact of socio-economic and demographic factors on the access. METHODS: Randomized cluster sampling was used to select 20 villages as the study sites from 94 villages in a county where modern TB control program with DOTS has not been established--Yangzhong in Jiangsu Province, China. 171 patients with chronic cough, identified from all residents (n=33549) aged >14 years were interviewed using a structured questionnaire. Fluoroscopy and sputum smear were used for diagnosis. RESULTS: The median delay of patients (n=56) who did not seek any health care (quartile: 51 days-93 days) was 67 days, longer than the delay to first health care provider (10 days, quartile: 4 days-31 days) and patient's delay (15 days, quartile: 5-31 days) of those who had sought health care (P = 0.000). Uninsured patients were 42.2% less likely to access health facility and general hospitals than insured patients; meanwhile, they had a longer delay to first health care provider and general hospital. Men were less likely to seek health care service in primary health care provider. Farmers had a longer delay to first health care provider, so did the patients whose houses were far from health facilities. CONCLUSION: Potential TB patients with long-term cough in the non-program area of China have poor access to tuberculosis care,which could affect the detection of TB patients in population. Medical insurance is one of the most important factors to influence patients' access to TB care.

[Clinical observation on acupuncture combined with medication for intractable angina pectoris].

OBJECTIVE: To compare clinical effects of acupuncture combined with medication and simple medication on intractable angina pectoris (IAP). METHODS: Seventy cases of IAP were randomly divided into two groups, a control group and a treatment group, 35 cases in each group. The control group were treated with nitric acid ester, beta-receptor blocking agent, calcium antagonist, aspirin and hypo-molecular heparin, and the treatment group were treated with acupuncture at Danzhong (CV 17) and Neiguan (PC 6) and others, once daily for 10 consecutive days on the basis of the above treatment. RESULTS: The total effective rates for symptoms and electrocardiogram were 88.6% and 62.9% in the treatment group and 60.0% and 31.4% in the control group, respectively, with a significant difference between the two groups; the incidence of acute heart attack and sudden death of 5.7% in the treatment group was lower than 20.0% in the control group, with no serious adverse effect. CONCLUSION: Acupuncture combined with medication is safe and effective for intractable angina pectoris and it can improve short-term prognosis.
