[Analysis of 41 cases of basal cell carcinoma in maxillofacial area].

OBJECTIVE: To explore the incidence, location and relationship between surgical margin and metastasis of basal cell carcinoma (BCC) in maxillofacial area. METHOD: Retrospective study was undertaken of 41 cases of BCCs in the maxillofacial region diagnosed in histopathology during the period of 1996 to 2003. RESULT: From 1996 to 2003, there were 41 cases of primary BBC in maxillofacial region, of whom 17 were males and 24 were females (male-to-female ratio, 1:1.41). The age of BCCs patients was between 55-74 years old, the average age of female patients was 68.1 years old, and that of the male was 58.4 years old. The lesions of BCCs were found in 13 cases in the nose (31.7%), 8 cases in scalp skin (19.5%), 7 cases in periorbital skin (17.1%), 7 cases in perioral region (17.1%), 4 cases in cheek skin (9.8%) and 2 cases in auricle skin (4.89%). In patients who received operation, the difference of the positive rates between 0.3 cm and 0.5 cm of surgical margin of the T1 stage tumor was significant (P<0.05). Of all the 41 patients, 4 patients were found to suffer regional lymph nodes metastasis or local bone destruction at the first visit (9.8%), 3 patients (7.3%) had a recurrent BCC. CONCLUSION: The morbidity in elder female is higher than that of male at the same age. For tumors smaller than 2 cm in diameter, surgical margin of 0.5 cm is enough.

Bone loss induced by ovariectomy in rats is prevented by gene transfer of parathyroid hormone or an Arg-Gly-Asp-containing peptide.

Osteoporosis is a major and growing healthcare concern as the population ages. The genes of both a Arg-Gly-Asp (RGD)-containing peptide and parathyroid hormone (PTH) were used to reduce bone loss induced by ovariectomy (OVX) in rats. Plasmids with either RGD or PTH gene were delivered into the quadriceps of OVX rats. The expression of the genes was detected by RT-PCR and radioimmunoassay. Analysis of bone mineral density, bone mechanical testing and bone mineral content indicated an improvement in bone properties in both RGD-transferred and PTH-transferred rats compared to OVX rats. Gene transfer of either RGD or PTH is therefore a possible approach to prevent bone loss in OVX rats thus providing a potential method to prevent osteoporosis in clinical situations.

The expression of functional human parathyroid hormone in a gene therapy model for osteoporosis.

The pro-peptide sequence of human parathyroid hormone (hPTH) is essential for the efficient translocation of the nascent polypeptide and the precise cleavage at residue +1 from the cleavage site. If residue +1 is not a serine, the bioactivity of hPTH decreases dramatically. In order to express the functional hPTH properly in a gene therapy model, we constructed three sets of eukaryotic expression vectors: one combining hPTH cDNA with its pre-sequence or its prepro-sequence; one combining hPTH cDNA with the insulin pre-sequence or with the insulin pre-sequence and the hPTH pro-sequence; one combining pro-insulin cDNA with the hPTH pre-sequence or prepro-sequence. Radioimmunoassay and reverse transcription/polymerase chain reaction showed that transgenes were expressed in the animal model. The hPTH expressed by the vectors with the pro-sequence of PTH improved bone mechanical properties and bone mineral content, whereas the expressed insulin decreased blood glucose. Our results demonstrated that the pro-sequence of hPTH was required for skeletal muscle cells to secrete functional hPTH into the circulation of our gene therapy model. However, the function of expressed insulin was not affected by the fusion of the hPTH pro-sequence. We concluded that pre-sequence, pro-sequence and mature hPTH acted as an integrated unit in skeletal muscle to guarantee the proper processing and release of functional hPTH into the circulation.

Transfer of a gene containing the Arg-Gly-Asp peptide prolongs the bleeding time of mice.

Peptides containing Arg-Gly-Asp (RGD) have been used to decrease thrombosis by competitive inhibition of the integrin glycoprotein, alphaIIb/beta3a, in platelets. However, they have a short half-life in vivo. A naked plasmid, pCMV-RGD, was transferred into the skeletal muscle of mice and RGD gene expression was observed by RT-PCR. The bleeding time between control mice and RGD-transferred mice was prolonged from the 10th day to the 80th day after gene transfer while the blood glucose and serum insulin-like proteins remained at normal levels. These results provided a convenient and effective approach to relieve patients from thrombi in a single step over a relatively long period.

[Relationship between high frequency hearing threshold and tinnitus].

OBJECTIVE: To investigate change of high frequency (UHF) threshold in patients suffering from tinnitus but with normal pure threshold. METHOD: High frequency audiometry was carried out on 48 tinnitus patients with normal pure shreshold and 30 young students with normal pure shreshold but without tinnitus as a control group. Then the patients were divided into three groups according to the age: < or = 25 years old, 26-35 years old and 36-45 years old. The hearing loss were statistically analyzed between the three groups and the control. RESULT: The results showed very significant differences of HF threshold between ipsilateral ear (tinnitus ear) in three groups and that in the control group, P < 0.01; very significant differences of HF threshold between ipsilateral ear (tinnitus ear) and that of contralateral ear (no tinnitus ear) in each group, P < 0.01; no significant differences of HF threshold between ipsilateral ear (tinnitus ear) in the age of < or = 25 years old and between 26-35 years old, P > 0.05. CONCLUSION: HF audiometry should be carried out on patients who are suffering from tinnitus but with normal hearing sensitivity, because their HF threshold may be abnormal. The change of HF threshold may be related to aging. The start age may be at about 30 years old.

Prevention of osteoporosis in ovariectomized mice by electroporational gene transfer of parathyroid hormone.

Non-viral vector, pCMV-Pth, with full-length human parathyroid hormone (prepro-hPTH) cDNA was constructed and delivered into the ovariectomized mouse quadriceps to explore the effects of electroporational gene transfer of PTH on the bone. The expression of hPTH in the transfected mice was detected by RT-PCR and radioimmunoassay. Mechanical testing and the analysis of bone mineral content demonstrated the improvement of bone properties. These results suggest that the electroporational gene taransfer of parathyroid hormone might be a promising method to prevent the bone loss in the postmenopausal women.

[Radiofrequency ablation for treatment of nasal obstruction secondary to inferior turbinate hypertrophy].

OBJECTIVE: To assess the efficacy of bipolar radiofrequency ablation on inferior turbinate in patients with nasal obstruction caused by inferior turbinate hypertrophy. METHOD: Eighteen patients enrolled in the study and bipolar radiofrequency ablation was delivered to inferior turbinate. The preoperative and postoperative nasal obstruction were investigated by visual analogue scale (VAS). RESULT: All patients were improved in 3 months. Mean obstruction(VAS) improved from 70.5% +/- 10.5% to 53.3% +/- 11.6% (right side), and from 67.7% +/- 9.4% to 45.0% +/- 9.2% (left side). CONCLUSION: The use of radiofrequency for submucosal tissue ablation in the hypertrophied inferior turbinate is an effective modality for reducing symptoms of nasal obstruction.
