Association between inflammation and insulin resistance in U.S. nondiabetic adults: results from the Third National Health and Nutrition Examination Survey.

Clinical and epidemiological data indicate that inflammation may be associated with insulin resistance. We examined the association between inflammatory markers, such as ferritin, uric acid, white cell counts, fibrinogen, and C-reactive protein, and insulin resistance among 5,959 adults, aged > or =20 years and without diabetes (fasting glucose <126 mg/dl and not taking diabetes medication), who participated in the Third National Health and Nutrition Examination Survey. Insulin resistance was calculated using the homeostasis model assessment (HOMA). Levels of ferritin, uric acid, white cell counts, fibrinogen, and C-reactive protein were significantly higher in individuals with a higher HOMA of insulin resistance (HOMA-IR). After adjustment for age, sex, race, education, physical inactivity, current and former smoking, alcohol intake, use of nonsteroidal anti-inflammatory drugs, systolic blood pressure, BMI, waist circumference, serum total cholesterol, and triglycerides, a 1-SD higher ferritin (126.1 ng/ml), uric acid (1.4 mg/dl), white blood cell count (2.2 x 10(9)/l), and fibrinogen (80.6 mg/dl) was associated with a 0.10 (95% CI 0.03-0.17, P=0.004), 0.16 (0.08-0.24, P <0.001), 0.16 (0.09-0.22, P <0.001), and 0.12 (0.05-0.18, P=0.001) higher HOMA-IR, respectively. Clinically elevated C-reactive protein (> or =1.0 mg/dl) was associated with a 0.63 (0.23-1.04, P=0.003) higher HOMA-IR. These findings indicate that elevated levels of inflammatory markers are positively and independently associated with insulin resistance. Further studies should examine the potential causal effect of inflammation on insulin resistance.

Transfection of mEpo gene to intestinal epithelium in vivo mediated by oral delivery of chitosan-DNA nanoparticles.

AIM: To prepare the chitosan-pmEpo nanoparticles and to study their ability for transcellular and paracellular transport across intestinal epithelia by oral administration. METHODS: ICR mice were fed with recombinant plasmid AAV-tetO-CMV-mEpo (containing mEpo gene) or pCMVbeta(containing LacZ gene), whether it was wrapped by chitosan or no. Its size and shape were observed by transmission electron microscopy. Agarose gel electrophoresis was used to assess the efficiency of encapsulation and stability against nuclease digestion. Before and after oral treatmant, blood samples were collected by retro-orbital puncture, and hematocrits were used to show the physiological effect of mEpo. RESULTS: Chitosan was able to successfully wrap the plasmid and to protect it from DNase degradation. Transmission electron microscopy showed that freshly prepared particles were approximately 70-150 nm in size and fairly spherical. Three days after fed the chitosan-pCMVbeta complex was fed, the mice were killed and most of the stomach and 30% of the small intestine were stained. Hematocrit was not modified in naive and 'naked' mEpo-fed mice, a rapid increase of hematocrit was observed during the first 4 days of treatment in chitosan-mEpo-fed animals, reaching 60.9+/-1.2% (P<0.01), and sustained for a week. The second feed (6 days after the first feed) was still able to promote a second hematocrit increase in chitosan-mEpo-fed animals, reaching 65.9+/-1.4% (P<0.01), while the second hematocrit increase did not appear in the 'naked' mEpo-second-fed mice. CONCLUSION: Oral chitosan-DNA nanoparticles can efficiently deliver genes to enterocytes, and may be used as a useful tool for gene transfer.

In situ expression of thyrotropin receptor in the extraocular muscles of patients with thyroid-associated ophthalmopathy.

OBJECTIVE: To investigate the expression of thyrotropin receptor (TSHR) in the extraocular muscles (EOM) of the eyes of patients with thyroid-associated ophthalmopathy (TAO). METHODS: EOM specimens were obtained from 12 patients with severe TAO and 10 healthy subjects within 1 hour after accidental death and the paraffin-embedded sections were observed by streptavidin-peroxidase (SP) immunohistochemical methods. RESULT: The expression of TSHR was observed in the perimysium, cytomembrane and cytoplasm in 10 of the 12 TAO specimens, accounting for a TSHR expression rate of 83.3% in the EOM of TAO patients. No TSHR protein was found in the healthy subjects. CONCLUSION: High TSHR expression in the EOM of TAO patients indicates that as a common antigen between the orbit and thyroid, TSHR play a key role in the pathogenesis of TAO.

[Analysis of therapeutic effectiveness in 158 childhood acute lymphoblastic leukemia patients treated with ALL-XH-99 protocol].

OBJECTIVE: To analyse the treatment outcome of 158 childhood acute lymphoblastic leukemia (ALL) patients, and explore how to improve the event-free survival (EFS) rate in ALL. METHOD: All of the patients entered the ALL-XH-99 clinical trial. Kaplan-Meier method was used to estimate survival rates and differences were compared with the 2-sided log-rank test, statistics was done by SPSS. RESULTS: Out of the 158 patients, 153 (96.8%) attained complete remission (CR) in a median time of 33 days. The overall EFS rate at 5 years was (72.4 +/- 7.8)% with a median observation duration of 26 months. The EFS rates at 5 years in low-risk (LR), median-risk (MR) and high-risk (HR) groups were (88.9 +/- 5.5)%, (78.5 +/- 8.0)% and (53.4 +/- 10.9)%, respectively (P < 0.05). Relapse occurred in 15 patients (10.0%) in a median time of 12 months, including 13 isolated hematologic relapses, 2 isolated central nervous system (CNS) relapses. Seven patients died of complications, and 13 died of leukemia relapse. CONCLUSION: The early response to therapy was an important independent prognostic factor, high-dose methotrexate (HD-MTX) was effective for preventing haematological and testicular relapse. The ALL-XH-99 protocol decreased the rate of therapy-related death and improved the long-term event-free survival rate.

[Clinical significance and relationship between traditional classification and Lavin-Osband criteria in Langerhans cell histiocytosis].

OBJECTIVE: To analyze the relationship between traditional classification and Lavin-Osband criteria on Langerhans cell histiocytosis (LCH) and to evaluate the importance of the criteria in clinical management and long-term prognosis of Lavin-Osband criteria. METHODS: From 1992 to 2003, 25 cases of LCH were studied. All cases were divided into four types by traditional classification and graded according to Lavin-Osband criteria simultaneously. All patients were analyzed with clinical manifestation, laboratory test, diagnoses, treatment and prognosis. The case numbers distributed in different Lavin-Osband criteria were summed up, and the advantage of the criteria was analyzed. RESULTS: The clinical manifestations according to the traditional classification (four types) overlapped mostly. The age of onset of the disease, the number of involved organs and the function of organs were all considered in the Lavin-Osband criteria, which also contained the characters of the traditional classification and could classify the cases into different severities. So Lavin-Osband criteria could predict the outcome of LCH. According to the traditional classification there were 10 cases with type I LCH, which was all graded to the criteria III and IV (100%), and there were 8 (80%) of criteria IV and none of criteria I and II. The situation was called "low type high criteria". There were 6 cases belong to LCH-II, of them 3 (50%) graded to the criteria III and 1 to the other criteria (17%), respectively. There were 4 cases with LCH-III, of them 3 (75%) graded to the criteria I and II, 1 (25%) to the criteria III and none to the criteria IV. There were 5 cases with LCH-IV, of them 4 (80%) graded to the criteria I and II, 1 (20%) to the criteria III and none to the criteria IV. This situation was called "high type low criteria". CONCLUSION: The Lavin-Osband criteria seemed to be simple, clear and easy to be handled and related to the severity of the disease, which will simplify the diagnoses, help the early-treatment and judge the prognosis.

[A study on micromegakaryocyte in children with idiopathic thrombocytopenic purpura].

OBJECTIVE: Bone marrow megakaryocytes overly proliferate and abnormally develop among patients with idiopathic thrombocytopenic purpura (ITP). Previous studies showed that it resulted from the abnormal immune function of the body. But the changes in megakaryocytes, especially in micromegakaryocytes in this disease are unclear. The present study was designed to explore the growth and development status of megakaryocytes and the significance of changes in micromegakaryocytes in pediatric cases. METHODS: Routine bone marrow smears assay and enzyme labeling for micromegakaryocytes with CD41 monoclonal antibody (McAb) were performed in 46 children with ITP. The level of platelet-associated immunoglobulin (PA-Ig) was measured with ELISA. RESULTS: Among 46 children, 36 had acute ITP (AITP)and 10 chronic ITP (CITP). The number of megakaryocytes increased or was normal in 45 patients, but decreased only in 1 case of CITP. The positive rate of micromegakaryocytes and type I micromegakaryocytes was 98% (45/46) and 35% (16/46), respectively. The positive rate of type I micromegakaryocytes was higher in CITP (50%) cases than that in AITP (31%) cases, but the median of the other three types of micromegakaryocytes in CITP cases (159) was lower than that in the AITP cases (336). There was no relationship between the numbre of all types of megakaryocytes and the level of PA-Ig. CONCLUSION: Majority of patients with ITP showed an increase in micromegakaryocytes, especially in type II, III and IV. The immune disturbance might not be the only reason for ITP. The abnormality of quality of megakaryocytes might be one of the potential causes for thrombocytopenia in some cases of ITP, especially in those of CITP. The appearance and the number of type I micromegakaryocytes might reflect the prognosis of cases of ITP.

[The effects of glycyrrhizin-18alpha on the expression of NF-kappaB in the renal interstitium of rats with obstructive nephropathy].

AIM: To investigate the effects of glycyrrhizin-18alpha on the expression of nuclear factor-kappaB in the renal interstitium of rats with obstructive nephropathy. METHODS: Obstructive nephropathy model of rats was set up by unilateral ureteral ligation. The rats were sacrificed 7,14, 28 and 56 days after operation. And then specimens were taken from the renal cortex. The morphological changes in renal interstitium were observed under light and electron microscopes. The expression of nuclear factor-kappaB and type III collagen was detected by immunohistochemical staining and analyzed by EIG image analysis system. RESULTS: Progressive fibrosis was observed in the renal interstitium of rats with obstructive nephropathy. The semi-quantitative analysis indicated that the expression level of NF-kappaB and type III collagen in the renal interstitium of glycyrrhizin-18alpha treatment and prophylaxis groups was markedly higher than that of mock operation group (P<0.01) and remarkably lower than that of normal saline control group(P<0.01) 7 days after operation. No significant difference in the expression of NF-kappaB and type III collagen was detected between glycyrrhizin-18alpha treatment group and prophylaxis group(P>0.05). CONCLUSION: Glycyrrhizin-18alpha may prevent renal interstitial fibrosis in obstructive nephropathy by down-regulating the expression of NF-kappaB in renal interstitium.

[Extraction and purification of non-receptor type PTKs-Syk].

AIM: To extract and purify Syk protein from Sf21 cells transfected by Syk gene. METHODS: Sf21 cells were transfected with recombinant syk gene. After 48 h of incubation at 28 degrees Celsius, the transfected cells were collected and sonicated with Sonfier sonicator on ice. Filtered cell extract was loaded onto a Reactive Yellow-3 resin column and Toyopearl AF-Heparin-650 M column respectively. The characters of Syk protein in the fractions were identified by SDS-PAGE, Western blotting and IEF. RESULTS: 225 mg of protein containing Syk were obtained from Sf21 cells (2.5 x 10(9))extract. There were two subpopulations in the elution of Reactive Yellow-3 resin column with the same relative molecular mass (Mr) 72 x 10(3). The two subpopulations were then applied on Toyopearl AF-Heparin-650 M column and two pure proteins were obtained. The results of SDS-PAGE, Western blotting, and IEF showed the two proteins having the same relative molecular mass (72 x 10(3)), corresponding to Syk, but with different pI. CONCLUSION: The yield of Syk was 8 mg from 2.5 billion cells and the purity was > 95%. The two purified Syk proteins have the same Mr and different pI. The purified Syk protein can be applied to study Syk's mechanism, produce anti-Syk antibody and invent Syk diagnosis kit, etc.

Effect of ketamine on NF-kappa B activity and TNF-alpha production in endotoxin-treated rats.

The effects of ketamine on endotoxin-induced NF-kappaB activation and TNF-alpha expression were studied in vivo. A sepsis model was created by bolus injection of endotoxin (5 mg/kg) into the tail vein of adult Wistar rats. The rats were immediately treated with various doses of ketamine (0.5, 5, or 50 mg/kg) or 0.9% NaCl (10 ml/kg) i.p. At 1, 4, or 6 hr post-treatment, NF-kappaB and TNF-alpha were assayed in the intestine, liver, and lung by electrophoretic mobility shift assay (EMSA) and reverse-transcription polymerase chain reaction (RT-PCR), respectively. Serum TNF-alpha was analyzed by ELISA. Endotoxin enhanced NF-kappaB activity and TNF-alpha expression in the intestine, liver, and lung and it increased TNF-alpha concentration in serum. Ketamine dosages 0.5 mg/kg suppressed the endotoxin-induced NF-kappaB activation and TNF-alpha expression in the intestine. The lowest dose to inhibit NF-kappaB activity and TNF-alpha expression in the lung was 5 mg/kg. Ketamine did not inhibit endotoxin-induced NF-kappaB activity or TNF-alpha expression in the liver; ketamine itself at a dose of 50 mg/kg enhanced NF-kappaB activity and TNF-alpha expression in the liver. Ketamine dosage 0.5 mg/kg inhibited endotoxin-induced TNF-alpha elevation in the serum. In conclusion, ketamine can suppress the induction of NF-kappaB and TNF-alpha by endotoxin in vivo. Subanesthetic dosages of ketamine have an anti-inflammatory effect, but large dosages may be harmful.

[Treatment of acute respiratory distress syndrome using pressure and volume controlled ventilation with lung protective strategy].

OBJECTIVE: To investigate the significance and effect of pressure controlled ventilation (PCV) as well as volume controlled ventilation (VCV) by lung protective strategy on respiratory mechanics, blood gas analysis and hemodynamics in patients with acute respiratory distress syndrome (ARDS). METHODS: Fifty patients with ARDS were randomly divided into PCV and VCV groups with permissive hypercapnia and open lung strategy. Changes in respiratory mechanics, blood gas analysis and hemodynamics were compared between two groups. RESULTS: Peak inspiration pressure (PIP) in PCV group was significantly lower than that in VCV group, while mean pressure of airway (MPaw) was significantly higher than that in VCV after 24 hours mechanical ventilation. After 24 hours mechanical ventilation, there were higher central venous pressure (CVP) and slower heart rate (HR) in two groups, CVP was significantly higher in VCV compared with PCV, and PCV group had slower HR than VCV group, the two groups had no differences in mean blood pressure (MBP) at various intervals. All patients showed no ventilator-induced lung injury. Arterial blood oxygenations were obviously improved in two groups after 24 hours mechanical ventilation, PCV group had better partial pressure of oxygen in artery (PaO2) than VCV group. CONCLUSION: Both PCV and VCV can improve arterial blood oxygenations, prevent ventilator-induced lung injury, and have less disturbance in hemodynamic parameters. PCV with lung protective ventilatory strategy should be early use for patients with ARDS.

[Influence of Shenshuai Capsule on renal tissue apoptosis of rats with chronic renal failure].

OBJECTIVE: To explore the influence of Shenshuai Capsule on renal tissue apoptosis of Wistar rats with chronic renal failure. METHOD: Wistar rats were fed with adenine to establish chronic renal failure model, and then they were divided into three groups separately fed with Shenshuai capsule (high or low dosage) and Niaoduqing. RIA, immunohistochemical method and electron microscopy were used. RESULTS: The parameters of renal function between the treatment group (high dosage) and the Niaoduqing group were significantly different respectively. Shenshuai capsule could inhibit tubular epicyte apoptosis and Fas genetic expression, and improve Bcl-2 genetic expression. CONCLUSION: Shenshuai capsule could inhibit epicyte apoptosis, and adjust Fas genetic expression. Bcl-2 genetic expression indicated that this may be one of the mechanism to postpone the renal failure by Shunshuai capsule.

[Preparation of rabbit anti-STAT5 antibody and its application in breast cancer diagnosis].

AIM: To prepare and purify rabbit antibody against STAT5 and evaluate its application in breast cancer diagnosis. METHODS: The rabbits were immunized with recombinant mouse STAT5 protein for preparing antibody against STAT5. The purity and specificity of the antibody were analyzed by Western blot. The proteins expressed in human breast cancer cell lines and normal breast tissue and breast cancer tissues were analyzed by Western blot and immunohistochemical staining with the anti-STAT5 antibody. RESULTS: The titer of anti-STAT5 antibody in the serum was 1:204 800 (A(450)=1.07) at 6 weeks after being immunized with STAT5. The SDS-PAGE and Western blot analysis showed that there was a obvious protein band with M(r) 90 000-95 000, confirming the specificity of obtained anti-STAT5 antibody. The expression of STAT5 could be detected with Western blot in both human breast cancer cell lines MCF-7 and MDA-MB-231. Immunohistochemical staining indicated that STAT5 was expressed in cytoplasm of normal breast tissues while in cytoplasm and nucleolus of breast cancer tissues. CONCLUSION: The rabbit anti-STAT5 antibody with high titer and specificity has been prepared. The STAT5 is expressed in cytoplasm of normal breast tissue, and in nucleolus of breast cancer tissue. The preparation of anti-STAT5 antibody provides an important reagent for clinical diagnosis of breast cancer.

[Report of 8 cases of bcr-abl gene positive thrombocytosis and review of the literature].

OBJECTIVE: To analyse the features of 8 cases of Bcr(+) thrombocytosis. METHODS: The clinical and hematological features and therapeutic outcomes were studied retrospectively in 8 Bcr(+) thrombocytosis and compared with essential thrombocytosis (ET) and chronic myeloid leukemia-chronic phase thrombocytosis (CML-CP-T). BCR-ABL fusion gene was detected with PCR. RESULTS: (1) Except for the presence of BCR-ABL fusion gene, there was no significant difference in clinical and hematological features and therapeutic outcomes between thrombocytosis with or without BCR-ABL. (2) The Bcr(+) thrombocytosis differed from CML-CP-T in the following aspects: female predominance, milder or no splenomegaly, peripheral leukocytes count < 40 x 10(9)/L, less or no basophilia and fewer immature granulocytes in peripheral blood, bone marrow granulocytic and/or megakaryocytic lineage hyperplasia, normal or increased neutrophil alkaline phosphatase score and less blastic transformation. CONCLUSION: Bcr(+) thrombocytosis may be considered as a new member of chronic myeloproliferative diseases, a variant of essential thrombocythemia.

Scanning electron microscopy of the infundibulum, ampulla, and eggs of mice.

The objective of the study reported here was to use scanning electron microscopy (SEM) to discover and describe the details of three-dimensional profiles and the natural (not surgically disturbed) topography/location of the infundibulum in the mouse. It will help new investigators to more quickly identify the infundibulum for successful transfer of microinjected eggs through a small opening into the oviduct/ampulla of pseudopregnant female mice for producing transgenic mice. Results of the study also illustrate the geographic orientation and natural topographic features of the ovary, infundibulum, ampulla, oviduct, and uterus. The presence of cilia on the surface of the crown foldings in the longitudinal section of the infundibular head stained with 1% toluidine blue provided direct evidence that evagination of the internal cilia of the infundibulum/oviduct results in formation of the infundibular crown. The new observation of the narrowing region of the infundibular head after surgical removal of the crown also suggests that formation of the infundibular crown may have resulted from the "evagination process" of internal cilia of the infundibulum/oviduct surface. The results also provide new evidence that the crown, terminal opening, and appearance of the left and the right infundibula of the same mouse differ.

Pilot study of using GIS to visualize health status distribution: case study of Songjiang District, Shanghai.

The aim of this study was to investigate the geographic variation in mortality indicators in Songjiang District of Shanghai under rapid change with urbanization. Geographic Information System (GIS) was used to visualize the distributions of health status and to test the extent of spatial auto-correlation in the indictors. In 2001, total crude mortality was 687/100,000 for the district; the rates for the townships (N=15) in the district ranged from 444/100,000 to 805/100,000. GIS maps indicated that the mortality was higher in the marginal areas of the district. Significant positive auto-correlations were found in total crude mortality rates, death rate for infectious diseases, and death rates for digestive diseases. The measures that consider intra-region inequality in health needs will be required in the regions under urbanization.
