A temperature-sensitive mutation in the nodal-related gene cyclops reveals that the floor plate is induced during gastrulation in zebrafish.

The floor plate, a specialized group of cells in the ventral midline of the neural tube of vertebrates, plays crucial roles in patterning the central nervous system. Recent work from zebrafish, chick, chick-quail chimeras and mice to investigate the development of the floor plate have led to several models of floor-plate induction. One model suggests that the floor plate is formed by inductive signalling from the notochord to the overlying neural tube. The induction is thought to be mediated by notochord-derived Sonic hedgehog (Shh), a secreted protein, and requires direct cellular contact between the notochord and the neural tube. Another model proposes a role for the organizer in generating midline precursor cells that produce floor plate cells independent of notochord specification, and proposes that floor plate specification occurs early, during gastrulation. We describe a temperature-sensitive mutation that affects the zebrafish Nodal-related secreted signalling factor, Cyclops, and use it to address the issue of when the floor plate is induced in zebrafish. Zebrafish cyclops regulates the expression of shh in the ventral neural tube. Although null mutations in cyclops result in the lack of the medial floor plate, embryos homozygous for the temperature-sensitive mutation have floor plate cells at the permissive temperature and lack floor plate cells at the restrictive temperature. We use this mutant allele in temperature shift-up and shift-down experiments to answer a central question pertaining to the timing of vertebrate floor plate induction. Abrogation of Cyc/Nodal signalling in the temperature-sensitive mutant embryos at various stages indicates that the floor plate in zebrafish is induced early in development, during gastrulation. In addition, continuous Cyclops signalling is required through gastrulation for a complete ventral neural tube throughout the length of the neuraxis. Finally, by modulation of Nodal signalling levels in mutants and in ectopic overexpression experiments, we show that, similar to the requirements for prechordal plate mesendoderm fates, uninterrupted and high levels of Cyclops signalling are required for induction and specification of a complete ventral neural tube.

Three-dimensional porous poly-DL-lactide/basic fibroblast growth factor composites for bone defect repair: an experimental study.

OBJECTIVE: To investigate the osteoinductive ability of the composites consisting of basic fibroblast growth factor (bFGF) and porous poly-DL-lactide (PDLLA) for the development of a new absorbable osteosynthesis material. METHODS: Highly porous foams of PDLLA with the pore size ranging from 150 to 300 microm were prepared by a solvent-casting, particulate-leaching technique with NaCl as the leachable component. Animal models of radial diaphyseal defects of 1.0 cm with complete removal of the periosteum were induced in 45 rabbits, which were randomly divided into 3 groups to receive the defect repair with PDLLA and PDLLA/bFGF respectively, leaving one group untreated to serve as the control group. The implant specimens were harvested at 2, 4, 8, and 12 weeks respectively after the surgery and X-ray, histological and scanning electron microscopic (SEM) examinations were performed to evaluate the effectiveness of defect repair. At 8 and 12 weeks after implantation, biomechanical test (for three-point bending strength) was employed to study the quality of bone formation. RESULTS: PDLLA/bFGF composite stimulated more bone formation and had higher bending strength than PDLLA (P<0.05), and the bone formation induced by both materials was significantly more than that observed in the control group in every postoperative stage (P<0.05). CONCLUSION: PDLLA possesses good biocompatibility and absorbability, and when prepared into a porous material, it exhibits good osteoconductibility. As a good bFGF carrier, the foam of PDLLA with three- dimensional structure shows good osteoinductive ability with regard to the rapidity, quantity and quality of the bone formation.

[Early diagnosis and treatment of pulmonary embolism after total joint replacement: report of five cases].

OBJECTIVE: To study the early diagnosis and treatment of pulmonary embolism (PE) after total joint replacement (TJR). METHODS: >From April 1987 to December 2001, we performed 1,336 total knee replacements (TKR(s)) in 926 patients and 1,745 total hip replacements (THR(s)) in 1,566 patients. In this group there were 5 PE patients after operation. Two patients died (all after TKR), and 3 patients salvaged successfully (2 after TKR, 1 after THR). RESULTS: The total incidence of PE after TJR was 0.2% (5/2,492), the incidence of PE after TKR was 0.4% (4/926), and the incidence of PE after THR was 0.06% (1/1,556). In the patients who suffered PE, 2 died. The incidence of PE before 2000 was 0%. CONCLUSIONS: Death after total joint replacement is due to pulmonary embolism (PE), especially massive PE. The incidence of PE after TKR is higher than that after THR. Pulmonary angiography is the gold standard for the diagnosis of PE. With more understanding on PE and more popularity of joint replacement, the diagnosis of PE after TJR is increased. More attention should be paid to PE in the patients with high risk after TJR.

[Role of antisense oligodeoxynucleotide of inducible nitric oxide synthase in the apoptosis and p-p38 MAPK signal transduction in rat neurons after spinal cord injury].

OBJECTIVE: To study the role of antisense oligodeoxynucleotides (ASODN) of inducible nitric oxide synthase (iNOS) on the apoptosis and p-p38 mitogen-activated protein kinase (p-p38 MAPK) signal transduction of the neurons after spinal cord injury (SCI). METHODS: Antisense and non-sense oligodeoxynucleotides of iNOS were designed and synthesized, and injected into the subarachnoid space 12 h before the compressive injury was given to the rat spinal cord. Reverse transcription-PCR and flow-cytometry were used to examine the iNOS mRNA expression and the apoptosis of the neurons within the injured spinal cord. The changes of p-p38 MAPK signal transduction pathway were detected by Western blotting. RESULTS: Obvious increase in iNOS expression and up-regulated p-p38 MAPK were detected after SCI, and neuronal apoptosis was observed in the injured spinal cord. ASODN-iNOS treatment resulted in decreased expression levels of iNOS mRNA and p-p38 MAPK, and the neuronal apoptosis was alleviated. CONCLUSION: ASODN-iNOS inhibits iNOS expression and neuronal apoptosis following SCI which may be related to the p-p38 MAPK signal transduction pathway.

Effect of aminoguanidine on the recovery of rat hindlimb motor function after spinal cord injury.

OBJECTIVE: To study the effects of aminoguanidine (AG), the inhibitor of inducible nitric oxide synthase (NOS), on the recovery of rat hindlimb motor function after spinal cord injury and explore the possible mechanism. METHODS: Thirty rat models of spinal cord compression injury were established according to Nystrom's method. AG therapy was administered for 4 times at 1 h before and 8, 24, 36 h after the injury, and 24 h after the completion of the therapy, spectrophotography was performed to measure the content of NO and activity of NOS in the injured spinal cord, followed by flow cytometry for determining the apoptotic rate 48 h later. The evaluation of the hindlimb motor function recovery was conducted by electrophysiological method and by measuring the behavior scores. RESULTS: AG significantly decreased the NO content (from 2.2714+/-0.4239 micromol/g.pro to 0.8466+/-0.0477 micromol/g.pro, P <0.05) and NOS activity (from 0.3408+/-0.0228 U/mg pro to 0.2702+/-0.0148 U/mg pro, P <0.05) in the injured spinal cord. The apoptotic rats were also reduced (from 7.88% +/-0.79% to 3.10% +/-0.66%, P <0.05). Four weeks after the therapy, the behavior score of the rats improved from 7.1+/-4.5 to 17.3+/-4.7 (P <0.01), and the latency and amplitude of the motor evoked potentials improved from 0 ms to 8.89+/-0.91 ms and from 0 mv to 1.99+/-0.48 mv respectively, showing significant therapeutic effect of AG (P <0.05). CONCLUSION: AG can improve motor functions of injured spinal cord in rats, possibly resulting from decreased apoptotic cells of the neurons in the spinal cord in the early stages of the injury.

[Treatment of patients with juvenile rheumatoid arthritis with combination of leflunomide and methotrexate].

OBJECTIVE: To evaluate the efficacy and safety of the combined therapy with leflunomide and methotrexate in the patients with juvenile rheumatoid arthritis (JRA). METHODS: Forty patients with active polyarthritis JRA were divided into 2 groups. Group 1 (n = 21) received leflunomide tablet (1 mg/(kg x day) on days 1 - 3; then [(0.2 - 0.4) mg/kg per day] plus methotrexate (0.3 mg/kg i.v. every two weeks till clinical remission, then oral tablet 0.2 mg/kg weekly). Group 2 received the same doses of methotrexate in the same way. Permitted concomitant drugs included stable doses of NSAIDs and a low dose of prednisone during the course of treatments. The clinical assessments included the number of tender and swollen joints, tender articular index, swollen articular index, general articular function score, parents and physician's evaluation score, erythrocyte sedimentation rate, serum C-reactive protein and rheumatoid factor. Drug safety was assessed by observing the reaction of mucous membrane, skin, gastrointestinal tract, nervous system, hematologic changes, liver and renal function. Statistical comparison between two groups was performed by using analysis of variance, t test and chi(2) test. RESULTS: Efficacy and safety was assessed at 12th and 26th week. Average improvement rate of leflunomide plus methotrexate group at 12th week and 26th week was respectively 39.6% and 71.9%; while that of control group was 27.5% and 49.5%, i.e., there was significant difference between the two groups (P < 0.01). Average remission rate of leflunomide plus methotrexate group at 12th week and 26th week was respectively 4.76% and 38.10%; while that of control group (methotrexate only) was respectively 0, 0. The clinical improvement in the group treated with leflunomide plus methotrexate was significantly greater than control group (P < 0.01). There was no significant difference (9.5% v 5.3%) in occurrence rate of side effects between the two groups. Side effects included leucocytopenia and raised aminotransferase. They were mostly mild and tolerable. CONCLUSION: The effect of the leflunomide and methotrexate therapy in patients with active JRA was better than methotrexate alone. The combination therapy with leflunomide and methotrexate was safe and well tolerated.

Multi-directional shifts of optokinetic responses to binocular-rivalrous motion stimuli.

Previous dichoptic experiments showed that dissimilar stationary pattern stimuli resulted in the perception of binocular rivalry, whereas oppositely-directly moving grating stimuli resulted in alternating optokinetic nystagmus (OKN) and the perception of binocular motion rivalry. The present study extended these dichoptic motion experiments by introducing obliquely-oriented targets with the aim of probing further the cortical mechanisms underlying binocular processing of motion. Two-dimensional eye movements were recorded along with their subjective perceptual responses. The stimuli consisted of two tilted gratings, one moving diagonally upwards and to the right (UR, 45 degrees ) and the other diagonally upwards and to the left (UL, 135 degrees ), which were presented dichoptically to subjects under two stimulus modes. For the non-exchange mode, the OKN slow phases exhibited three types of directional shifts. Two of these directional shifts tracked the stimuli (i.e. UR or UL), whereas the third moved purely upwards (UP). Since physically there was no upward-moving target, the OKN and perceptual responses appeared to be associated with a perceptual interocular grouping of the two dichoptic stimuli in their reassembled vector-sum direction. The OKN shifts were also found to be highly correlated with the psychophysical responses of motion perception. For the rapid-exchange mode, in which the stimuli were rapidly exchanged between the two eyes, the OKN slow phases exhibited primarily two types of directional shifts, UR and UL, but no UP responses for most subjects. It also appeared that these two coherent motion percepts, UL and UR, were interocularly regrouped from the exchanged stimuli. Moreover, the lack of perceptual grouping to create an UP response in the rapid-exchange mode indicated that temporal integration of at least 200 ms was necessary for the development of a reassembled vector-sum-direction motion percept. The findings under both stimulus modes support the stimulus-feature rivalry hypothesis, in which higher cortical centers mediate interocular perceptual grouping and the associated motor response.

Analysis of menthol in three traditional Chinese medicinal herbs and their compound formulation by GC-MS.

A GC-MS method was developed for identification and determination of menthol in three traditional Chinese medicinal herbs and their compound formulation, a granule for treating colds. Volatile oil was simultaneously distilled and extracted into ethyl ether in a unique glass extractor. The separation was performed on an HP-5 MS column. The standard addition method was used for quantitative determination of menthol content in herbal materials and in the granule. A component in the samples was chosen as the internal standard. The contents were calculated with the ratio of peak area percentage. Menthol was identified as the main component in Mentha haplocalyx Briq., and also existed in spikes of Schizonepeta tenuifolia Briq., Folium perilla frutescens (L.) Britt. and granules. The quantitative calibration range was 0.21-10.5 mg/mL. Good precision was demonstrated by an RSD < 4.0%. The mean content of menthol in Mentha haplocalyx Briq., spikes of Schizonepeta tenuifolia Briq., Folium perilla frutescens (L.) Britt. and granules was 121, 0.234, 1.03 and 1.84 mg/kg respectively.

Neostigmine-induced alterations in fast phase of optokinetic responses in myasthenic ocular palsies.

Closed-loop and open-loop optokinetic nystagmus (OKN) eye movements were recorded with an electromagnetic scleral search coil in 16 patients with myasthenic ocular palsies, 8 patients with nonmyasthenic ocular palsies, and 4 normal controls. We calculated the summation of fast-phase amplitude per second (SFPAps), mean of fast-phase amplitude (MFPA) and mean of fast-phase peak velocity (MFPV) of OKN before and after administration of neostigmine. Under open-loop conditions, the myasthenic patients group demonstrated distinct increases in three parameters: SFPAps (P < 0.000001), MFPA (P < 0.001) and MFPV (P < 0.001) following neostigmine injection, and the increase in SFPAps was greater than those of MFPA and MFPV (P < 0.02), while the other two groups showed no significant increases. Under closed-loop condition, only five of the myasthenic patients showed significant changes. These results suggest that the change in SFPAps during open-loop OKN before and after neostigmine administration was a specific measurement that differentiated patients with myasthenic ocular palsies from the other groups.

Sound radiation from a water-filled pipe, radiation into light fluid.

This paper is concerned with the sound radiation from a water-filled exhaust pipe. The pipe opening and a plate attached to it form a vibrating surface for this radiation. Fluid-structural coupling between the pipe and enclosed fluid is included in the system modeling, but light fluid assumption is used for sound radiation into the space above the vibrating surface. In this paper, a numerical study on the n = 0 mode in the pipe shows that the wave types associated with this mode have different characteristics in two regions of the nondimensional frequency omega. In the first region of 0<omega<0.5 and 1<omega<1.3, the fluid and structural waves are weakly coupled, and the fluid and structural response of the pipe is governed by the resonance of each wave type. The fluid and structural waves are strongly coupled in the second region (0.5<omega<1). Simple correlation between the pipe response and uncoupled mode distributions does not exist. Significant contribution of multiple wave-types to the model energy is evident. The ultimate goal of our system modeling is to illustrate the contribution of all wave types (structural-borne and water-borne waves of the system) to sound radiation from the pipe opening and the attached plate. This paper also demonstrates the effect of these waves and their coupling in the water-filled pipe and across the plate and pipe boundary on the radiated sound pressure. Experimental results obtained in a semi-anechoic room are used to partially validate the theoretical and numerical predictions.

Short sound decay of ancient Chinese music bells.

Ancient Chinese music bells, which have oval-like cross sections, possess the unique feature of a short sound-decay time that is especially suitable for music with fast tempo. Although recorded in the literature nearly 1000 years ago, this feature has never been truly understood. Recent damping measurement of oval pipes with different axial ratios suggested a challenging task for acousticians as the increase of the modal damping with the pipe's axial ratio is significant only when the pipe response is measured in air but not in vacuum. In this paper, the decrease of sound-decay time of oval pipes with the increased cross-section ovality is investigated. This is the first time that such short sound-decay feature is explained in terms of the rich sound radiation characteristics of the symmetric modes of music bells and the oval pipes. Finite element and boundary element methods are employed to explain how axial ratio significantly affects the normal velocity distribution, radiation efficiency, and directivity of acoustic modes.

Intravenous chemotherapy for resected gastric cancer: meta-analysis of randomized controlled trials.

AIM: To assess the safety and efficacy of different intravenous chemotherapeutic regimens in patients with gastric carcinomas who had undergone gastrectomy. METHODS: A meta-analysis of all the relevant randomized controlled trials (RCTs) was performed. Language was restricted to Chinese and English. RCTs were identified from Medline and Embase (1980-2001/4), and Chinese Bio-medicine Database (1990-2001/1). Literature references were checked at the same time. We included randomized and quasi-randomized trials comparing the efficacy of intravenous chemotherapy after gastrectomy with that of surgery alone in patients with confirmed gastric carcinomas who had undergone gastrectomy. Selection criteria were: randomized or quasi-randomized trials with following-up results; Trials could be double-blind, single-blind or not blind; Chemotherapy groups were given intravenous chemotherapy after gastrectomy without neo-adjuvant chemotherapy, intraperitoneal hyperthermic perfusion, radiotherapy or chemoimmunotherapy; Controlled group included those receiving gastrectomy alone. The following data were extracted: the number of survival and death by the end of the follow-up; the different agents and doses of the intravenous chemotherapy; the baseline of the chemotherapy group and the controlled arm; the serious adverse events; the statistical consideration; cost-effectiveness analysis. The statistical analysis was performed by RevMan4.1 software which was provided by the Cochrane Collaboration. A P value of <0.05 was considered statistically significant. Meta-analysis was done with random effects model. Heterogeneity was checked by chi-square test. Sensitivity analysis was performed by excluding the trials in which Jadad-scale was only 1 score. The result was expressed with odds ratio (OR) for the categorical variable. RESULTS: Fourteen trials involving 4543 patients were included. Meta-analysis was done with random effects model. Heterogeneity and sensitivity analysis were performed also. The effect of intravenous chemotherapy after gastrectomy was better than surgery alone (odds ratio 0.56, 95 %CI 0.40-0.79). There was a significant difference between the two groups by u-test (P=0.0008). Sensitivity analysis revealed the same difference (odds ratio 0.81, 95 % CI 0.70-0.94). Of fourteen trials, only three studies were of high quality according to the Jadad-scale (with three score). There was one meta-analysis trial and the others, about ten trials, were of low quality. There was no trial which mentioned sample-size calculation, allocation concealment, intention-to-treat analysis. Most of the trials didn't describe the blind-procedure. There were five trials which detailed the side-effects according to the toxicity grade by WHO standard. The side-effects halting treatment were haematologic and biochemical toxicity, debilitating nausea and vomiting. There were two patients died of chemotherapy toxicity. CONCLUSION: Based on the review, intravenous chemotherapy after gastrectomy may have positive treatment effect on gastric cancer. However, the evidence is not strong because of the general low methodologic quality of the RCTs. Therefore, we can't make the conclusion that intravenous chemotherapy after gastrectomy may have better treatment effect on gastric cancer than that of surgery alone. Rigorously designed, randomised, double-blind, placebo-controlled trials are required.

A new echocardiographic system for assessment of epicardial and intramyocardial coronary flow in a swine model.

OBJECTIVE: To visualize epicardial and intramyocardial coronary blood flow by using a noninvasive echocardiography system. METHODS: In five juvenile swines, coronary flow imaging was used to visualize the different segments of epicardial coronary and intramyocardial arteries. Pulsed-Doppler spectrums were recorded and analyzed. The left anterior descending artery (LAD) and intramyocardial coronary blood flow signals were recorded at baseline and during dipyridamole administration. RESULTS: Epicardial and intramyocardial coronary arteries could be visualized by coronary flow imaging. The systolic component of coronary flow in the right coronary artery (RCA) was greater than in the LAD. The intramyocardial blood flow was characterized by persistent retrograde blood flow velocity during systole. Vasodilation with dipyridamole produced exaggerated differences in the phasic pattern of coronary blood flow in epicardial and intramyocardial vessels. CONCLUSION: Color Doppler coronary flow imaging provides a noninvasive method to study the coronary blood flow.
