Pigmented nail disorders.

Nail pigmentation is defined by the presence of melanin in the nail plate. It most frequently has the appearance of a longitudinal pigmented band, called longitudinal mel-anonychia (LM). LM is an important aspect of ungual pathology because it can be the first sign of nail apparatus melanoma. The aim of this article is to help distinguish those types of melanonychia that are worrisome and should lead to biopsy from those that are reasonably reassuring and need only be followed clinically. Histology, differential diagnosis, and biopsy techniques also are considered.

Side effects of nail cosmetics.

Cosmetics applied on the nail encompass three types: (1) coatings that harden upon evaporation; (2) coatings that polymerize; and (3) stick-on nail dressings (synthetic covers). The adverse reactions induced by the two first types present with both local reactions and distant contact dermatitis. Whereas nail enamel applications result especially in ectopic contact dermatitis, polymerizing coatings and synthetic covers represent the main culprit for sometimes severe, local reactions. Whatever the nature of the nail cosmetics, they may produce some adverse reactions. They include: (1) local reactions to cosmetics applied on the nail; (2) distant reactions resulting from the use of nail cosmetics; (3) systemic side effects of nail cosmetics; and (4) infection risks from nail cosmetics.

Artificial nails and psoriasis.

When dealing with artificial nails, it is often mentioned that psoriasis is a contraindication because artificial nails may worsen nail psoriasis by Köbner phenomenon.(1) However, although not thoroughly discussed, other interactions between psoriasis and artificial nails exist. On the basis of clinical cases, the following interactions are described: worsening of psoriasis, the fact that psoriatic patients wearing artificial nails are more exposed to infections, and allergic risks. The clinical similarities that may exist between nail psoriasis and nail alterations caused by allergic reactions to artificial nails are also emphasized.

Superficial acral fibromyxoma: clinical and pathological features.

Superficial acral fibromyxoma (SAFM) is a recently recognized myxoid tumor that usually occurs on the fingers and toes of middle-aged adults. We report on the typical case of a 50-year-old woman with a SAFM in the right big toenail that had been slowly growing for more than 10 years. To our knowledge, this case is the first reported case for which clinical pictures are available. Histologically, the lesion was non-encapsulated and was composed of stellate and spindle cells, arranged in a myxoid matrix. No atypia or mitotic figures were found. Neoplastic cells showed positive staining for CD34 and negative staining for epithelial membrane antigen (EMA), actin, desmin, keratins, S100 protein, CD99, and HMB45. Differential diagnosis encompasses benign and malignant myxoid and spindle cells tumors such as myxoid neurofibroma, sclerosing perineurioma, superficial angiomyxoma, and several low-grade myxoid sarcomas.

Neuroblastoma-like schwannoma: a case report and review of the literature.

We report a case of schwannoma (neurilemmoma) predominantly composed of small cells arranged in rosettes around central collagenous cores and discuss the differential diagnosis of this unusual variant. Schwannoma with giant rosettes, previously designated as neuroblastoma-like schwannoma by Goldblum et al., must be differentiated from neuroblastoma, peripheral neuroectodermal tumors, and malignant change in a schwannoma.

Thalidomide in refractory vulvar ulcerations associated with Crohn's disease.

We report a case of recurrent vulvar ulcerations developed on vegetations associated with Crohn's disease. Lesions responded to low-dose thalidomide treatment. A biopsy revealed a neutrophilic infiltrate with rare giant cells. Vulvar manifestations were consistent with metastatic Crohn's disease or with a reactive neutrophilic dermatitis. Thalidomide reduces the activity of tumor necrosis factor (TNF) alpha by accelerating the degradation of its mRNA. Nowadays, it has become a promising drug in a broad variety of disorders in which TNF-alpha seems to play a pivotal role, including inflammatory bowel diseases. This is the first case report presenting the efficacy of thalidomide in the treatment of vulvar involvement in Crohn's disease.

POEMS syndrome revealed by multiple glomeruloid angiomas.

POEMS syndrome (polyneuropathy, organomegaly, endocrinopathy, monoclonal gammopathy, skin disorders) is a rare multisystemic disease associated with plasma cell dyscrasia. A 68-year-old woman with chronic renal insufficiency and arterial hypertension included in her medical history was admitted to the hospital with confusion, somnolence and asthenia. She presented ascites, hepatosplenomegaly, leg oedema, distal dysesthesias, leuconychia and multiple nodular purple red angiomas on the trunk, upper limbs and fingers. Hypothyroidism was revealed in the laboratory investigations and monoclonal IgG peak in immunoelectrophoresis. Electromyography showed both demyelinisating and axonal degenerative neuropathy. The diagnosis of POEMS syndrome was based on the dermatopathological examination of a cutaneous angioma; histology revealed features of glomeruloid angioma, a specific marker of this syndrome.
