A cfDNA fragmentomics classifier for noninvasive differentiation of benign and malignant renal masses.

Noninvasive differentiation of malignant and benign renal masses remains a major clinical challenge, particularly for radiologically indeterminate lesions. Here, we developed and validated a plasma cell-free DNA (cfDNA) fragmentomics-based machine learning classifier for renal mass characterization. The model was trained on 331 participants (171 cancer, 160 benign) and independently validated on 144 participants (73 cancer, 71 benign). Three cfDNA fragmentation features, including copy number variation (CNV), fragmentation-based methylation (FRAGMA), and nucleosome footprint (NF), derived from low-pass whole-genome sequencing, were integrated into an ensemble framework. The model achieved strong discriminative performance, with area under the curve (AUC) values of 0.956 in the training cohort and 0.946 in the validation cohort, outperforming individual feature-based models. At a predefined operating threshold corresponding to 90% sensitivity, specificity reached 0.90 and 0.87, respectively. Notably, most cancer samples exhibited low tumor fraction (TF&#x2009;<&#x2009;3%), yet the model maintained robust performance in low-TF samples (AUCs: 0.952 and 0.941, respectively). Performance remained consistent across tumor stage, grade, and histological subtypes. The classifier also demonstrated potential clinical utility in diagnostically challenging settings, including lipid-poor angiomyolipoma and oncocytoma, with 12 of 13 oncocytoma samples correctly classified in an independent cohort. In addition, the model correctly identified 85.3% of benign masses&#x2009;>&#x2009;4&#xa0;cm, for which surgical intervention is more commonly considered, and 84.6% of malignant tumors&#x2009;&#x2264;&#x2009;4&#xa0;cm, for which management can be challenging. Collectively, these findings support cfDNA fragmentomics as a promising noninvasive liquid biopsy approach for renal mass evaluation and clinical decision-making.

Annotation and expression profile analysis of 2073 full-length cDNAs from stress-induced maize (Zea mays L.) seedlings.

Full-length cDNAs are very important for genome annotation and functional analysis of genes. The number of full-length cDNAs from maize (Zea mays L.) remains limited. Here we report the construction of a full-length enriched cDNA library from osmotically stressed maize seedlings by using the modified CAP trapper method. From this library, 2073 full-length cDNAs were collected and further analyzed by sequencing from both the 5'- and 3'-ends. A total of 1728 (83.4%) sequences did not match known maize mRNA and full-length cDNA sequences in the GenBank database and represent new full-length genes. After alignment of the 2073 full-length cDNAs with 448 maize BAC sequences, it was found that 84 full-length cDNAs could be mapped to the BACs. Of these, 43 genes (51.2%) have been correctly annotated from the BAC clones, 37 genes (44.0%) have been annotated with a different exon-intron structure from our cDNA, and four genes (4.76%) had no annotations in the TIGR database. Expression analysis of 2073 full-length maize cDNAs using a cDNA macroarray led to the identification of 79 genes upregulated by stress treatments and 329 downregulated genes. Of the 79 stress-inducible genes, 30 genes contain ABRE, DRE, MYB, MYC core sequences or other abiotic-responsive cis-acting elements in their promoters. These results suggest that these cis-acting elements and the corresponding transcription factors take part in plant responses to osmotic stress either cooperatively or independently. Additionally, the data suggest that an ethylene signaling pathway may be involved in the maize response to drought stress.

Proton acceleration from microdroplet spray by weakly relativistic femtosecond laser pulses.

Angular distribution of protons is measured from ethanol droplet spray irradiated by linearly polarized 150 fs laser pulses at an intensity of 1.1 x 10(16)W/cm2. Fast protons (with energies >16 keV ) with an anisotropic distribution can be observed only in or near the polarization plane of the laser fields, while the slow protons (with energies <or=16 keV ) emit with nearly an isotropic distribution. Two-dimensional particle-in-cell simulations suggest that three groups of protons originate from different acceleration regimes in the laser-droplet interaction. The first group with the highest energies is accelerated backwards by the anisotropic charge-separation field near the front surface (laser-droplet interaction side) due to the resonance absorption; the second group (forward emission) is generated by the target-normal sheath acceleration mechanism; and the third group, with the lowest energies, is accelerated by the hydrodynamic expansion of the droplet plasmas.

[Protective and curative effects of prophylactic administration of pulmonary surfactant on neonatal respiratory distress syndrome].

OBJECTIVE: To evaluate the protective and curative effects of prophylactic administration of pulmonary surfactant (PS) on neonatal respiratory distress syndrome (NRDS). METHODS: One hundred neonates aged 0.5 h after birth, with the gestational age < 32 w, birth weight < 1500 g, and number of gastric, stable microbubble < or = 7/mm(2) by gastric stable microbubble test (SMT), but without clinical or radiological manifestations of RDS at the admission, were randomly divided into 2 equal groups: prophylactic group (PG), receiving curosurf, a product of PS, immediately after admission; and non-prophylactic group (N-PG), receiving curosurf only after development of RDS. RESULTS: One hour after the administration of PS, the PaO(2), a/APO(2), pH, and PaO(2) of the PG group were 86.2 mm Hg +/- 8.1 mm Hg, 0.30 +/- 0.04, 7.38 +/- 0.06, and 178 +/- 37, all significantly higher than those of the non-PG group (all P < 0.01), and the PaCO(2) of the PG group was 37.3 mm Hg +/- 9.8 mm Hg, significantly lower than that of the non-PG group (53.6 mm Hg +/- 11.1 mm Hg, P < 0.01). In comparison with the level before the administration of PS (0.75 +/- 0.06), the level of FiO(2) of the 47 pediatric patients receiving mechanical ventilation decreased after the administration of curosurf time-dependently, e.g., was 0.50 +/- 0.09, 0.34 +/- 0.06, and 0.25 +/- 0.07 8, 48, and 96 hours after the administration. In comparison with the level before the administration of curosurf 9.0 +/- 1.0 cm H2O, the level of mean airway pressure (MAP) decreased time-dependently after the administration, e.g., were 7.5 +/- 0.8 and 6.0 +/- 0.3 48 and 96 hours after the administration (all P < 0.01). Compared with that before the administration of curosurf (3.02 +/- 0.2), the X-ray chest score decreased time-dependently after the administration of curosurf, e.g., were 1.89 +/- 0.34, 1.82 +/- 0.33, and 1.17 +/- 0.42 6, 12, and 72 hours after the administration (all P < 0.01). The RDS rate of the PG group was 30%, significantly lower than that of the non-PG group (P < 0.01). The severe case rate of the PG group was 20%, significantly lower than that of the N-PG group (53%, P = 0.01). The mortally of the PG group was 0, significantly lower than that of the non-PG group (P < 0.05). The total times of supplemental oxygen administration, assisted ventilation and hospitalization of the 47 patients with RDS in the PG group were significantly shortened compared with the RDS patients in the N-PG group [(3.6 +/- 1.7) d vs. (5.9 +/- 3.6) d, P < 0.05; (8.6 +/- 5.5 d vs. (14.1 +/- 6.2) d, P < 0.01; and (20.5 +/- 10.0) d vs. (32.8 +/- 17.8) d, P < 0.05). CONCLUSION: Prophylactic administration of PS to the preterm neonates with high risk of RDS effectively decreases the incidence of RDS, development of severe cases and mortality, shorten the disease course, the duration of supplemental oxygen administration and assisted ventilation, thus decreasing the potential morbidity associated with long-term oxygen supplement and assisted ventilation.

Inhibitory effects of cardiotonic pills on platelet function in dogs fed a high-fat diet.

Insulin resistance and the consequent metabolic disorders are associated with a state of platelet hyperactivity. Oxidative stress is responsible for the persistent platelet activation. We sought to study the inhibitory effect of cardiotonic pills, an oral herbal component, on platelet function in a dog model with insulin resistance induced by high-fat feeding. We fed 18 dogs with a high-fat diet and six dogs with normal chow as control for 6 months. Then, six dogs were fed with a high-fat diet and received additional aspirin (250 mg/day), and another six dogs received additional cardiotonic pills (1,000 mg/day) for 4 months. Time-course changes in metabolic parameters and platelet function were detected. After high-fat feeding for 6 months, 18 dogs developed a series of metabolic disorders including obesity, dyslipidemia, oxidative stress and insulin resistance. In addition, a platelet hyperactivity state, characterized by increased agonist (arachidonic acid, ADP and collagen) induced platelet aggregation, platelet expression of adhesion molecules (P-selectin and GP IIb/IIIa), and platelet intracellular calcium concentration, was indicated. Cardiotonic pills showed a significant antioxidative activity by presenting an increase in plasma superoxide dismutase and decrease in erythrocyte glutathione, as well as a lipid-lowering effect (decrease in both plasma cholesterol and triglyceride). Either aspirin or cardiotonic pills could significantly reverse the platelet hypersensitivity and hyperfunction. Compared with aspirin, cardiotonic pills showed a more exaggerated inhibitory effect on platelet function (a significantly decreased collagen-stimulated platelet aggregation, and expression of adhesion molecules). In conclusion, cardiotonic pills inhibited platelet hyperfunction in dogs with insulin resistance. This inhibitory effect may mainly be explained by antioxidative activity and metabolic control.

Prevalence of orofacial pain and treatment seeking in Hong Kong Chinese.

AIMS: To estimate the prevalence and pattern of self-reported orofacial pain symptoms and treatment-seeking behavior in adult Cantonese-speaking Chinese people in Hong Kong. METHODS: A cross-sectional population survey involving a telephone survey technique was used to identify 1,222 randomly selected Cantonese-speaking people aged at least 18 years. Standard questions were asked on the experience of 8 orofacial pain symptoms in the previous month and on treatment-seeking behavior. RESULTS: Orofacial pain symptoms were reported by 41.6% of respondents when tooth sensitivity was included and by 24.2% when it was excluded. There was no gender- or age-related difference in pain prevalence (P > .010). Tooth sensitivity was the most common symptom (27.7%), followed by toothache (12.5%), and shooting pain in the face was the least common (1.1%). Almost half those with symptoms reported the pain as moderate to severe, and a fifth had frequent pain. Only 20.3% with pain symptoms sought professional treatment, and use of self-prescribed medication was very low (12.4%). CONCLUSION: Orofacial pain symptoms appear to affect more than a quarter of the adult population in Hong Kong, and prevalence estimates were consistent with those in Western countries. A substantial proportion of the pain symptoms were frequent and of moderate to severe intensity, with the potential for significant morbidity. Professional treatment seeking was very low and may be related to specific pain behaviors and effective coping strategies in this ethnic group.

Epinephrine infiltration on nasal field causes significant hemodynamic changes: hypotension episode monitored by impedance-cardiography under general anesthesia.

PURPOSE: Local infiltration of epinephrine-containing local anesthetics is widely used in clinics particularly in the procedure of surgeries on vascularity field to provide good analgesia and hemostasis. A prospective randomized double blind control study was designed to observe hemodynamic changes caused by local infiltration of epinephrine- containing lidocaine solution on nasal field under general anesthesia. METHODS: 90 adult patients undergoing elective functional endoscopic sinus surgery under general anesthesia were randomly allocated into three groups and received 1% lidocaine 4 mL with different dose of epinephrine (group I 20 microg; group II 40 microg; and group III 0 microg) respectively. Mean arterial pressure (MAP), heart rate (HR), cardiac index (CI), systemic vascular resistance index (SVRI), and acceleration index (ACI) were recorded through impedance-cardiography at every 45 seconds in 6 minutes after the beginning of local infiltration. RESULTS: Compared with the intra-group baseline, statistically significant hemodynamic changes particularly decrease in MAP with increase in HR at 1.5 minutes time point (P < 0.01), and decrease in SVRI and increase in CI, ACI at and from 1.5 minutes time point (P > 0.05) were observed in group I and group II, but not in group III. CONCLUSION: Local infiltration of epinephrine-containing lidocaine solution on nasal field causes significant decrease in MAP and SVRI, and increase in HR, CI and ACI.

Independent differentiation of mammotropes and somatotropes in the chicken embryonic pituitary gland. Analysis by cell distribution and attempt to detect somatomammotropes.

It has been reported that mammotropes in a rodent pituitary gland are derived from somatotropes via somatomammotropes (SMTs), cells that produce both growth hormone (GH) and prolactin (Prl). However, no studies have been done on the transdifferentiation of somatotropes in the chicken pituitary gland. In this study, in order to determine the origin of mammotropes, we studied detail property of appearance of chicken somatotropes, mammotropes and pit-1 cells and then evaluated the existence of SMTs in the chicken embryonic pituitary gland. Immunohistochemical analysis revealed that GH-immunopositive (GH-ip) cells appeared on embryonic day (E) 14 and were mainly distributed in the caudal lobe, while Prl-immunopositive (Prl-ip) cells appeared in the cephalic lobe of the pituitary gland on E16. In situ hybridization (ISH) and RT-PCR analysis showed that expression of GH and Prl mRNA starts at E12 in the caudal lobe and at E14 in the cephalic lobe respectively. Pit-1 mRNA was first detected on E5 by RT-PCR, and pit-1 mRNA-expressing cells were found in the cephalic lobe on E8. Then with the ontogeny of the chicken, these cells spread into both lobes. Using a double staining method with ISH and immunohistochemistry, we could not detect the existence of SMTs in the chicken embryonic pituitary gland even in the marginal region of either lobe. These results suggest that chicken somatotropes and mammotropes independently appear in different lobes of pituitary gland and that transdifferentiation from somatotropes to mammotropes is not the central route for differentiation of mammotropes in the embryonic chicken pituitary gland.

[Effects of taurine on early changes of excitatory amino acids in rabbit brain due to deep hypothermic circulatory arrest].

OBJECTIVE: To study the effects of taurine (Tau) on early changes of excitatory amino acids (EAA) in hippocampus rabbit brain due to deep hypothermic circulatory arrest (DHCA). METHODS: Sixteen New Zealand rabbits were randomly allocated into 2 groups: Tau 150 mg/kg group (group I, n = 8), control group (group II, n = 8). Cardiac pulmonary bypass (CPB) was set up after cerebral microdialysis model was established. High performance liquid chromatography was employed to monitor the continuous changes of glutamic acid (Glu), aspartic acid (Asp), Tau and glycine (Gly) in the hippocampus CA1 region at anaesthesia periods, CPB stage, pre 30 minutes in DHCA, post 30 minutes in DHCA, pre 30 minutes in rewarming, post 30 minutes in rewarming. RESULTS: The increase of Glu in group II was (5.1 +/- 1.5) and (4.1 +/- 1.4) times higher than that at anaesthesia periods (t = 3.74, 5.45, P < 0.01), and the increase of Glu in group I was (2.1 +/- 1.5) and (1.1 +/- 0.4) times than that at anaesthesia periods at pre 30 minutes and post 30 minutes rewarming stage respectively. Rising of Gly in group II was 6.7 (3.6, 13.6) times than that at anaesthesia periods (T = 75.00, P < 0.05), the rising of Gly in group I was 4.2 (3.8, 11.5) times at post 30 minutes in DHCA. Rising of Tau in group I was 6.9 (3.0, 14.2) and 10.6 (2.8, 22.5) times than that at anaesthesia periods (T = 75.00, P < 0.05 and T = 90.00, P < 0.05) and rising of Tau in group II was 4.0 (3.0, 5.7) and 3.2 (1.2, 7.6) times than that at anaesthesia periods at post 30 minutes in DHCA and pre 30 minutes rewarming stage respectively. Asp was no evident change at every periods. CONCLUSION: DHCA might increase the excitatory amino acids in the early phases of circulatory restoration. Exterior Tau 150 mg/kg could inhibit release of EAA.

Effects of Compound Dan-shen Root Dropping Pill on hemorheology in high-fat diet induced hyperlipidemia in dogs.

The purpose of this study was to examine the effects of Compound Dan-shen Root Dropping Pill (CDRDP) (Tasly Group, Tianjing, China) on hemorheology and biorheology of dogs suffering from hyperlipidemia induced by high-fat diet. Eighteen dogs were randomly divided into two groups: the high-fat diet group (H group); the control group (C group), fed with a standard laboratory diet. Six month later, six dogs in the H group were chosen as the drug-taking group (D group), to which CDRDP was administered, fed with the same diet as H group. In the 4th month, blood was taken from the veins of the dogs, and blood triglyceride (TG), total cholesterol (TC), RBC hemorheological indexes as well as malondialdehyde (MDA), glutathione transferase (GSH-ST) and superoxide dismutase (SOD) activities in plasma and erythrocytes were measured. Compared with H group, TC, TG, plasma MDA levels, the whole blood viscosity, RBC osmotic fragility and the value of CHOL (cholesterol)/PL (phospholipid) of the membrane of D group decreased, however, erythrocyte GSH-ST, histopathological changes in liver, deformation index (DI), orientation index (DI)or, small deformation index (DI)d, electrophoresis ratio and microfluidity of the membrane lipid bilayer of RBCs, increased distinctly. CDRDP can improve micro-hemorheological characteristics, therefore has a significant therapy application of hyperlipidemia.

[Study on the preparation of effervescent tablets of EGb and its quality].

OBJECTIVE: To prepare effervescent tablets of EGb and establish quality preparation. METHOD: Taking pH, disintegration time and rigidity as index, the formulation and technique of EGb effervescent tablets were optimized by orthogonal experiment, the products were prepared and its stability were tested. RESULT: The optimum procedure condition was adding 12.5% citric acid, 20% sodium bicarbonate encapsulated by PEG6000, the products were tested by stability, the preparation was stable. CONCLUSION: It is valuable to be further studied as a new preparation of EGb.

[Estimation on the number of drug users in Hongjiang, Hunan: a capture-recapture study].

OBJECTIVE: To estimate the number of drug users in Hongjiang, Hunan and to develop strategy for drug reduction in the future. METHODS: Two capture-recapture methods were used to estimate the numbers of drug addicts. Random stratified sampling survey was used to verify the optimum allocation. The first capture-recapture method (CR1) referred to the number from optimum allocation random stratified sampling survey conducted in the communities and the number from local Public Security Bureau list being the second capture. The second capture-recapture method (CR2) referred to the collection of records in the detoxification unit with an interval of 4 months. The estimated number was calculated under Seber's adjustment formula. Face to face interview was carried out during the optimum allocation random stratified sampling survey process. RESULTS: Of 1388 interviewed in the communities, 24 (1.73%) were identified as drug addicts under the optimum allocation random stratified sampling survey. When the figure 1.73% was applied to the total population (72,709) in Hongjiang, the result yielded an estimation of 1258 drug addicts. The estimated numbers of CR1 and CR2 were 904 and 1069 respectively. However, the number was 1.3 to 1.6 fold higher than the reported number (687) by local Public Security Bureau. CONCLUSION: The capture-recapture method seemed a better method in estimating the number of drug addicts.

[Comparing the characteristics of psychophysical tests between cochlear implant children with large vestibular aqueduct syndrome and normal inner ear].

OBJECTIVE: The purpose of this study is to compare the characteristics of psychophysical tests between cochlear implant children with large vestibular aqueduct syndrome and normal inner ear and to analyse the characteristics of psychophysical and impedance tests for cochlear implant patients with large vestibular aqueduct syndrome. METHOD: One hundred and thirty-one cochlear implant pre-lingual deafness patients participated in this study. They received cochlear implant surgery at the age of 1.5 to 12 years old from Nov, 1998 to Aug, 2003. The patients were divided into two groups. Group A had 111 patients with normal inner ear. Group B had 20 patients with radiographically proven large vestibular aqueduct syndrome. All of the patients were inserted with Nucleus 24M. The time of the first programming session was approximately 4 weeks after surgery. The psychophysical tests included T and C-Level tests. The programming techniques used in the test were suitable for the age of the patients. The R116 and R126 software were used to test the impedance of the electrodes automatically. RESULT: Using the independent-samples t-test, the impedance and T/C-Level of each electrode did not differ significantly between group A and B ( P > 0.05). The impedance and T/C-Level were not significantly different between group A and B at the time of switch-on, 1 week, 2 weeks, 3 weeks, 2 months, 3 months, 6 months, 9 months and 1 year after switch-on (P > 0.05). CONCLUSION: The parameters of mapping were not significantly different between cochlear implant children with large vestibular aqueduct syndrome and normal inner ear. The management and procedure of mapping used in cochlear implant children with normal inner ear can be used in the children with large vestibular aqueduct syndrome.

[A study on the promoting effects of recombinant human epidermal growth factor on skin wound healing in rats].

OBJECTIVE: To observe the effects of recombinant human epidermal growth factor (rhEGF) on skin wound healing. METHODS: The dorsal trauma model of rats was used. A total of 68 dorsal wounds in 34 rats were created and divided into the rhEGF group and the isotonic saline group according to self-control. The process of wound healing was observed and a mean healing time was calculated. Furthermore, the dynamic analysis was performed at different times on wound OHP contents, ratio of collagen I/III and cell DNA cycle. RESULTS: The wound healing was accelerated obviously in all wounds treated with rhEGF. The mean time of wound healing of the two groups was (17.2 +/- 1.3) days and (20.5 +/- 1.6) days respectively (P < 0.01). In the rhEGF group, the new granulation tissue was more and the re-epithelialization was faster than that of the saline group. External rhEGF increased OHP content, reduced collagen I/II ratio and accelerated cell DNA replication. CONCLUSION: External rhEGF can shorten wound healing time, increase granulation tissue and OHP contents, reduce collagen I/III ratio and accelerate cell DNA replication, thus obviously promoting skin wound repair.

Ethanol stimulation of HIV infection of oral epithelial cells.

Oral mucosal cells can be infected by exogenous HIV during receptive oral sex or breast-feeding. The risk of oral mucosal infection depends on the infection efficiency of the HIV strains present in the oral cavity, the viral titers, and the defense mechanisms in the oral cavity environment. It is expected that alcohol can weaken the host defense mechanism against HIV infection in the oral cavity. We modified an HIV strain, NL4-3, by inserting the enhanced green fluorescent protein gene and used this virus to infect oral epithelial cells obtained from patients. Various concentrations of ethanol (0%-4%) were added to the infected cells. HIV-infected cells were detected by fluorescent microscopy or fluorescence-activated cell sorting. We found that ethanol significantly increases HIV infection of primary oral epithelial cells (POEs). POEs pretreated with 4% ethanol for less than 10 minutes demonstrated 3- to 6-fold higher susceptibility to infection by the CXCR-4 HIV strain NL4-3. Our studies also demonstrated that HIV infects POEs through a gp120-independent mechanism. We tested an HIV CCR5 strain, JRCSF, and also found its infection efficiency to be stimulated by alcohol. Our results indicate that in cell culture conditions, the ranges of concentrations of alcohol that are commercially available are able to stimulate the infection efficiency of HIV in POEs.

Isolation and analysis of water stress induced genes in maize seedlings by subtractive PCR and cDNA macroarray.

In order to identify genes induced during the water stress response in maize (Zea mays) seedlings, suppression subtractive hybridization (SSH) was performed using mixed cDNAs prepared from maize seedlings treated with 20% PEG as testers and cDNAs from unstressed maize seedlings as drivers. A forward subtractive cDNA library was constructed, from which 960 recombinant colonies were picked and amplified. Through differential screening of the subtractive cDNA library, 533 clones were identified as water stress induced. After sequencing, 190 unique expressed sequence tags (ESTs) were obtained by clustering and blast analysis, which included transcripts that had previously been reported as responsive to stress as well as some functionally unknown transcripts. The ESTs with significant protein homology were sorted into 13 functional categories. A cDNA marcoarray containing the 190 unique ESTs was used to analyze their expression profiles in maize seedling during both PEG treatment and natural drought. The results indicated that 67 ESTs in leaves and 113 ESTs in roots were significantly up-regulated by PEG-stress. 123 ESTs were found to be up-regulated for at least one time-course point in either maize leaves or roots. Correspondingly, 163 ESTs were significantly up-regulated by drought stress. Results from the hierarchical cluster analysis suggest that the leaves and roots of maize seedlings had different expression profiles after PEG treatment and that there was a lot of overlap between PEG- and drought-stress induced up-regulated transcripts. A set of transcripts has been identified, which have significantly increased expression and probably involved in water stress signaling pathway based on data analysis.
