[A Case of Stage Ⅳ Lung Adenocarcinoma with Bilateral Breast Metastases Mimicking Inflammatory Breast Cancer].

Breast metastases from primary lung cancers with inflammatory breast cancer (IBC)-like features are rare. Here, we report a patient with bilateral breast metastases from an advanced lung adenocarcinoma that mimicked IBC. A 67-year-old woman was diagnosed with right middle lobe lung adenocarcinoma with pleural and peritoneal dissemination (cT2aN2M1c, cStage ⅣB) and received systemic therapy. Twenty-nine months later, skin erythema and edema appeared in both breasts, suggesting bilateral IBC. Core needle biopsies of both breasts revealed atypical carcinomas without intraductal or lobular in situ components. Immunohistochemistry showed thyroid transcription factor-1 and Napsin A positivity and GATA3 negativity, supporting the diagnosis of metastatic lung adenocarcinoma of the breasts. Comprehensive genomic profiling of the breast specimen revealed an EGFR exon 19 deletion, and osimertinib achieved disease control within 1 year. This report highlights a diagnostic pitfall in which lung adenocarcinoma metastasis to the breast mimics IBC and underscores the importance of biopsy, an appropriate immunohistochemical panel, and molecular testing to establish a correct diagnosis and guide optimal systemic therapy.

Evaluation of iron bioavailability from bonito dark muscle using anemic rats.

The bioavailability of iron from ferrous sulfate (FeII-S), heme iron prepared from hemoglobin (HIP), and bonito dark muscle (BDM) was assessed in anemic rats using a hemoglobin regeneration efficiency (HRE) method. Freeze-dried BDM (FD), boiled and freeze-dried BDM (B/FD), and boiled and smoke-dried BDM (B/SD) were used as BDM source. Rats were made anemic by feeding on an iron-deficient diet for 28 days. To replete their iron levels, anemic rats were then fed on a diet containing iron at a level of 17 ppm for 14 days. Rats receiving FeII-S gained significantly more weight and had greater food intake and higher HRE compared to the other four groups. The bioavailability of iron from HIP was poor compared with that from FeII-S and BDM. When the HRE of rats fed FeII-S was 100, that of rats fed BDF was approximately 80. These results suggest that BMD is an effective dietary source of iron.

A sample introduction technique for obtaining a multiphoton ionization spectrum using picogram quantities of analyte in supersonic jet spectrometry.

A new sample introduction techniquefor ensuring the safety of a researcher during the measurement of hazardous compounds, such as dioxins, by supersonic jet spectrometry is described. The solution containing the analyte is introduced beneath the nozzle through a narrow capillary with a glass frit at the end, which is formed to vaporize only the solvent and to deposit the analyte. The first laser, that is, the second harmonic emission of a Nd: YAG laser, vaporizes the analyte and entrains it into a supersonic jet. The analyte molecule is ionized by the second laser, that is, an optical parametric oscillator. Ion measurement is achieved by a time-of-flight mass spectrometer. The analyte molecule is localized in the jet, thus permitting the amount of sample to be minimized. The minimum amount of phenol necessary for measurement of a multiphoton ionization spectrum by scanning the wavelength of the ionization laser in the 3-nm range was 180 pg, which is similar to or slightly less than the tolerable daily intake (TDI) of dioxin.

Development of a compact supersonic jet/multiphoton ionization/time-of-flight mass spectrometer for the on-site analysis of dioxin, part I: Evaluation of basic performance.

A new type of compact supersonic jet/resonance-enhanced multiphoton ionization/time-of-flight mass spectrometer is described. The analytical instrument, consisting of a single turbo molecular pump equipped with a rotary pump, was maintained at < 2 x 10(-3) Pa when a 0.3-atm sample was injected into a vacuum at 10-Hz using a 200-micros pulse valve. The diameters of the extraction and ground skimmer electrodes were expanded to 30 mm in order to avoid strong focusing and defocusing of the ion, and the optimum conditions for the system were investigated. The mass spectrometer functioned as expected: (1) no defocusing of the ion beam was observed even when the potential of the einzel lens was adjusted to zero; (2) the direction of the ion beam to an assembly of microchannel plates deviated in the expected manner when the potential of the defection electrode was changed from 0 to 30 V.

Development of a compact supersonic jet/multiphoton ionization/time-of-flight mass spectrometer for the on-site analysis of dioxin, part II: Application to chlorobenzene and dibenzofuran.

A compact supersonic jet/multiphoton ionization/mass spectrometer (SSJ/MPI/MS) equipped with a single pump system was developed for use in the analysis of dioxin. A mass spectrum and multiphoton ionization spectrum were obtained for chlorobenzene and dibenzofuran using an optical parametric oscillator (OPO) for excitation and subsequent ionization. The detection limit for chlorobenzene (S/N = 3) was 0.3 ppb, which corresponds to an absolute amount of 0.1 pg. The rotational temperature was 35 K. This slightly high temperature can be attributed to the long nozzle throat used in the instrument.

A tunable picosecond dye laser for use in dioxin analysis.

A distributed-feedback dye laser with a quenching cavity was designed and constructed for generating a tunable picosecond pulse with a narrow spectral linewidth. This nearly transform-limited pulse was succeedingly amplified by a triple-pass off-axis amplifier. The pulse duration and the spectral linewidth were 60 ps and 9.4 pm, respectively. The amplified pulse was frequency-doubled by second-harmonic generation, producing a 0.5-mJ pulse with no background emission. The potential advantage of this laser in the analysis of dioxin based on supersonic jet/resonance-enhanced multiphoton ionization/mass spectrometry is discussed.

Use of ring-repeller, double-skimmer electrodes for efficient ion focusing in mass spectrometry.

The design and construction of a new type of time-of-flight mass spectrometer is described. The instrument was designed to improve the ionization efficiency and to efficiently detect ions. The system performance was evaluated using the SIMION software, and was compared with the experimental results. The shapes of the repeller, extraction, and ground electrodes had a strong effect on the trajectory of ions in the processes of ion acceleration and focusing. The major difference between the theory and the experiment can be attributed to space-charge effects.
