Tension pneumocephalus--a rare complication of radiotherapy: a case report.

We present a rare case of tension pneumocephalus due to high-dose radiotherapy used to treat nasopharyngeal carcinoma. A skull base defect causing tension pneumocephalus was identified and was repaired successfully. The case emphasizes the importance of careful consideration before applying irradiation treatment to patients with head and neck malignancy and urges early detection of potentially life-threatening complications.

Overexpression of cyclooxygenase-2 in nasopharyngeal carcinoma and association with lymph node metastasis.

Cyclooxygenase-2 (COX-2) has been shown to be involved in multiple steps of carcinogenesis. In this study, we examined COX-2 expression in nasopharyngeal carcinoma (NPC). COX-2 mRNA analyzed by reverse-transcription polymerase chain reaction (RT-PCR) was detected in 66% (16 of 24) of tumor tissues. Western blot analysis demonstrated that COX-2 protein level was increased in tumor tissues and was correlated with the expression level of mRNA. Immunohistochemical study showed that COX-2 was predominantly detected in cancer cells, and the staining pattern was cytoplasmic. Ten histologically normal nasopharyngeal tissues obtained from nasopharyngeal hyperplasia were also investigated. We found that COX-2 mRNA was detectable in three tissues and the COX-2 protein level was very low. The frequency of COX-2 overexpression was significantly higher in patients of the N1-N3 group than in patients of the N0 group (P=0.006). Taken together, these data suggest that COX-2 is overexpressed and is associated with increased lymphatic invasion in NPC.

Laser myringotomy for otitis media with effusion in nasopharyngeal carcinoma patients.

OBJECTIVE: To evaluate the effectiveness of laser myringotomy in otitis media with effusion (OME) in patients with nasopharyngeal carcinoma (NPC). STUDY DESIGN AND SETTING: Laser myringotomy was performed with a CO 2 laser otoscope in 53 NPC patients (68 ears) and 32 non-NPC patients (37 ears). All patients had a history of OME. RESULTS: In the NPC group, 40% of ears developed persistent perforation, 37% had recurrent OME, and 23% were free of disease. In the non-NPC group, only 3% of ears developed persistent perforation, and 43% were free of disease. Significant differences in perforation and disease-free state were evident between the 2 groups. Tumor stage was found to be a significant determining factor for clinical outcome in NPC group. CONCLUSION: Laser myringotomy for NPC group carries a significantly higher risk of persistent perforation and lower rate of cure. This procedure may be considered for early staging NPC patients with serious OME symptoms.

Tracheostomal fire during an elective tracheostomy.

Some surgeries, a tracheostomy and oral surgeries in particular, have a higher risk of fire. Often it can occur when leaking flammable gas contacts an ignition heat source. Fire during a tracheostomy can sometimes be seen to produce different degrees of insult to the patient. The essential components of a fire, i.e., the fuel source, ignition, and an oxidizer, can be avoided or blocked in order to prevent fires from occurring. Herein, we discuss a fire during a tracheotomy, and ways to avoid its occurrence, and how to stop once it gets started. This case demonstrates 2 important points regarding tracheostomies. First, extreme caution should be exercised when cautery is used in an approximately 100% oxygen gaseous condition, and second, immediate extubation is not absolutely necessary if a fire breaks out during a tracheostomy. The procedures which can be taken when facing this type of emergency are also considered and discussed.

Intranasal tooth: report of three cases.

Intranasal teeth are uncommon, with only a few reported cases in the past few decades. The clinical manifestations of an intranasal tooth are quite variable. Unilateral nasal obstruction is a common complaint, but even though nasal symptoms are present, an intranasal tooth can be an incidental finding during routine examination in patients without nasal discomfort. Although the diagnosis is not difficult to make, a complete workup that included radiological investigations is important before any surgery is attempted. Transnasal endoscopic surgical approaches have been described with no evidence of recurrence or complications in similar cases. Herein, three patients with an intranasal tooth are described, along with possible etiologies, potential complications, differential diagnoses and their treatments.

Presentation of prostate carcinoma with cervical lymphadenopathy: report of three cases.

The most common method of spreading of prostate cancer is via direct invasion to the pelvic organs or vertebral bodies. Supraclavicular lymph node metastasis is uncommon in patients with prostate cancer. Previously researchers have shown that the prognosis after such a presentation is generally poor. We herein present three patients with prostate carcinoma presenting with supraclavicular lymph node enlargement at the initial diagnosis. Abdominal computed tomography showed multiple enlarged lymph nodes in retroperitoneal space. All of our patients had hormone therapy and the prostate specific antigen (PSA) level declined. The mean follow up time was 16 months (range, 6-24 months). None of our patients experienced progression during follow up. Prostate carcinoma should always be considered in the differential diagnosis of elderly men with cervical lymphadenopathy, even in the absence of lower urinary tract symptoms. Evaluation of the PSA level and lymph node biopsy with PSA immunohistochemistry stain can lead to the final diagnosis. In addition, hormone treatment has been shown to be of benefit even in patients in the advanced stages.

Enhancement of chemotherapeutic drug-induced apoptosis by a cyclooxygenase-2 inhibitor in hypopharyngeal carcinoma cells.

Our previous study demonstrated that cyclooxygenase-2 (COX-2) was overexpressed in human hypopharyngeal carcinoma. In this study, we tested the effect of a specific COX-2 inhibitor, NS398, on proliferation of hypopharyngeal cancer cells. Our results indicated that NS398 inhibited growth of hypopharyngeal cancer cells and this inhibition is associated with induction of G1 growth arrest. Western blot analysis showed that expression of G1 cyclins or cyclin-dependent kinases (CDKs) was not changed by NS398. On the contrary, NS398 significantly increased expression of CDK inhibitors p21(Waf1) and p27(Kip1). We also found that treatment of NS398 alone could not induce significant apoptosis in hypopharyngeal cancer cells. However, NS398 potently augmented chemotherapeutic drug-induced apoptosis. Caspase activation and DNA fragmentation were clearly detected in cancer cells pretreated with NS398 followed by chemotherapeutic drugs. Collectively, our results suggest that COX-2 inhibitors may suppress proliferation of hypopharyngeal cancer cells via induction of G1 growth arrest and may be useful in combination with chemotherapeutic drugs for the treatment of hypopharyngeal carcinoma.

RECK is a target of Epstein-Barr virus latent membrane protein 1.

Epstein-Barr virus (EBV) latent membrane protein 1 (LMP1) has been suggested to be involved in tumor metastasis. However, the molecular mechanism of LMP1-induced metastasis is largely unknown. In this study, we investigated the effect of LMP1 on the expression of RECK, a metastasis suppressor gene, in an EBV-negative nasopharyngeal carcinoma (NPC) cell line. Our data demonstrated that LMP1 induced downregulation of RECK via transcription repression in TW04 cells. In addition, we found that LMP1 acted via an Sp1 site to inhibit RECK promoter activity. We next studied the signaling pathway that mediated the effect of LMP1 on RECK expression. Our results showed that LMP1 potently stimulated the activity of extracellular signal-regulated kinases (ERKs) and inhibition of ERK activity by PD98059 antagonized LMP1-induced downregulation of RECK. Conversely, the c-Jun N-terminal kinase inhibitor SP600125 and p38(HOG) kinase inhibitor SB203580 had little effect. We also found that the expression of LMP1 increased the invasive ability of TW04 cells. The importance of RECK in LMP1-induced invasiveness was supported by three observations. First, restoration of RECK expression by PD98059 reduced LMP1-induced release of active MMP-9. Second, suppression of PD98059-induced RECK expression by small interference RNA abolished the inhibitory action of PD98059 on LMP1-induced invasiveness. Third, coexpression of RECK with LMP1 in TW04 cells effectively suppressed cell invasiveness induced by LMP1. Taken together, these results suggest that LMP1 inhibits RECK expression via the ERK/Sp1 signaling pathway and this inhibition is a critical step for LMP1-induced tumor metastasis.

Overexpression of cyclo-oxygenase 2 in squamous cell carcinoma of the hypopharynx.

Upregulation of cyclo-oxygenase 2 (COX-2) expression is frequently found in a variety of human cancers. In this study, we examined COX-2 expression in squamous cell carcinoma of the hypopharynx. COX-2 messenger RNA (mRNA) analyzed by reverse-transcription polymerase chain reaction was detected in 87% (20 of 23) of tumor tissues. Expression of COX-2 protein was examined by Western blot analysis. COX-2 protein levels were increased in tumor tissues and correlated with the expression level of mRNA. Immunohistochemical study was performed to detect the subcellular localization of COX-2. Our results showed that COX-2 was predominantly detected in cancer cells, and the staining pattern was cytoplasmic. Several histologically normal adjacent tissues obtained from these patients were also investigated. We found that COX-2 mRNA was detectable in these tissues. However, COX-2 mRNA and protein levels were lower in these tissues than in tumor specimens. In contrast, COX-2 mRNA and protein levels in normal oral mucosa obtained from healthy volunteers were very low or undetectable. The frequency of COX-2 overexpression was significantly higher in the N1-N3 group than in the N0 group. These results suggest that overexpression of COX-2 is linked with increased lymphatic invasion in hypopharyngeal carcinoma. Collectively, these results suggest that overexpression of COX-2 is a frequent phenomenon in hypopharyngeal carcinoma and may play a role in tumorigenesis of this cancer.

Paraganglioma presenting as chronic otitis media with cholesteatoma: pitfalls and strategies.

The coexistence of paraganglioma with cholesteatoma is a very rare clinical disorder. The clinical presentations are nonspecific. There may be radiological characteristics of either cholesteatoma or paraganglioma in the middle ear area, but the diagnosis of the coexistence of the 2 is usually made only postoperatively. Here is such a case that is made more interesting not only because it initially presented with conductive hearing loss but also because the clinical picture mimicked chronic otitis media. The patient underwent postauricular tympanomastoidectomy with extended facial recess approach to remove the tumor. No evidence of recurrence and complications were noted.

Cholesterol granuloma of the maxillary sinus presenting as sinonasal polyp.

Cholesterol granuloma is usually found in chronic middle ear diseases. However, it rarely occurs in the sinonasal regions and only a few case reports can be found in the literature. The etiology of sinonasal cholesterol granuloma is not yet known, and the clinical manifestations are nonspecific. Most patients presented with nasal discharge. Our patient is the only reported case presenting with nasal obstruction and facial pain. Here we present a patient with cholesterol granuloma of the maxillary sinus with a nasal polyp, whose clinical, imaging, and histological characteristics were unique. The tumor was excised via the transnasal endoscopic sinus approach without recurrence after 3 years of follow-up.
