Two rare cases of fenestrated anterior cerebral artery associated with aneurysms are described. The importance of identifying this unusual anatomical anomaly and its implication during surgery is discussed.
Nerve root retraction is a component of lumbar disc surgery. The authors describe a transdural approach for lumbar microdiscectomy through the nerve root, instead of retracting the root. This technique can be safely used in rare cases where root manipulation is considered to be difficult and dangerous.
Graft extrusion following anterior cervical discectomy is a recognized complication. We report a case of acute laryngeal obstruction secondary to synthetic graft extrusion to highlight potential dangers of interbody spacing devices and to stress the importance of investigation of patients who have symptoms attributable to early graft extrusion.
Fifteen consecutive patients with dural loss were treated by the 'inlay' technique of duraplasty with satisfactory results. The technique is described in detail. Its advantages over conventional methods of duraplasty are emphasized.
Gliomas are incurable cerebral tumours. Surgery with adjuvant therapy is aimed at achieving a maximum symptom-free period. We report a case of spontaneous regression of a cerebral oligodendroglioma following infarction in the territory of its feeding artery. Prospects of interventional technique as a part of multimodality treatment of gliomas are discussed.
Foreign bodies in the hand can cause a variable degree of acute or delayed injuries to the important structures. We report a rare case of a progressive ulnar neuropathy caused by delayed migration of a foreign body in the forearm. Ultrasonographic localisation and surgical removal of the foreign body resulted in gradual recovery of neurological function.

Balloon occlusion is the accepted treatment for direct posttraumatic caroticocavernous fistula. We present a case of bilateral traumatic fistulae associated with a pseudoaneurysm. Resolution of both fistulae occurred following treatment of one of them by balloon occlusion of the internal carotid artery. This case highlights the importance of considering a more conservative approach to bilateral fistulae or those associated with a pseudoaneurysm. We review other treatment options.
