Linking people with AIDS in rural communities: the telephone group.

People with acquired immune deficiency syndrome (AIDS) living in rural areas have difficulty obtaining needed information and support. Bringing support groups into the homes of rural residents through telephone conference call technology is an innovative practice strategy to address this problem. The authors describe the development, implementation, and evaluation of a model program consisting of six telephone group sessions. The goals of the psychoeducational group were to increase information and social support, reduce feelings of isolation, and enhance individual coping with living with AIDS.

Improving pediatricians' compliance-enhancing practices. A randomized trial.

Previous evaluations of continuing medical education (CME) have yielded conflicting results regarding its effects on physician knowledge, performance, and subsequent patient outcomes. Poor adherence by mothers to prescribed pediatric regimens is a separate, but well-documented, problem. In the present study we assessed the ability of CME to: (1) increase the knowledge of pediatricians about compliance-enhancing strategies; (2) increase the performance of these practices by pediatricians; and (3) improve mothers' compliance with antibiotic regimens for their children's otitis media. Ninety pediatricians were randomly assigned to either a control group or one of two CME interventions: tutorial plus printed materials or mailed printed materials only. Following the interventions, data on compliance and on reported behaviors of pediatricians were gathered from a random sample of mothers (N = 771) whose children were being treated for otitis media. Findings indicated that CME increased physician knowledge and compliance-enhancing practices and resulted in improvement in mothers' adherence to therapy.

Responding to AIDS: rural community strategies.

Small towns and rural communities often react with fear and misunderstanding toward persons with AIDS. The author discusses factors that influence how rural communities respond to AIDS and proposes strategies for providing education and care and for influencing policy.

Social networks and social support: living with chronic renal disease.

Individuals with chronic renal disease who receive dialysis treatment are continually faced with major adjustments. These may include dealing with changes in work and economic status, social roles, activity levels, self-image, health status, and normal routines, as well as learning to live with uncertainty and loss. The individual's social network plays a key role as the individual experiences and moves through various stages of adjustment. Networks with certain characteristics (e.g. provision of affective support, reciprocal ties) may be more effective than others lacking these characteristics in meeting the individual's changing needs during the process of adjusting to chronic renal disease. This paper examines this relationship between the characteristics of an individual's social network and adjustment to chronic renal illness. The discussion focuses on the impact of chronic renal disease on the individual, the composition and characteristics of the social network, and on the relationships between network members. How the social network affects a person's adjustment to stages of adaptation to chronic renal disease is also addressed. Finally, suggestions are presented for how health care professionals can intervene at the individual, network, and organizational level to strengthen and enlarge social networks in order to enhance social support.
