The origin of impedance audiometry.

Impedance audiometry is performed every day in nearly every ENT department, office and, audiology clinic all over the world. But hardly anyone occupied with these measurements has any idea of the origin of this Danish contribution to otolaryngology.

Radiotherapy in glomus tumours (paragangliomas). A 25 year-review.

57 patients have been reviewed. 39 had radiotherapy (28 glomus jugulare, 7 glomus tympanicum and 4 glomus caroticum/vagale tumours). Two patients have died of their tumour and one patient had increasing nerve involvement during two months after irradiation. It is concluded that the results bear comparison with those of surgery and that extensive surgery in tumours with extratympanal growth is not warranted.

Chronic facial pain following diseases affecting the maxillary antrum. The influence of peripheral impulses from the maxillary air sinus is not significant in their maintenance.

For the purpose of examining the significance of a pain focus in the maxillary air sinus denervation of the mucosa of the sinus by transantral maxillary nerve section was performed. The material comprises 20 patients with pain in the distribution of the infraorbital nerve and previous disease of the relevant sinus. All patients had been treated previously without any benefit from peripheral surgery of the infraorbital nerve. Only 20% had any benefit of the proximal avulsion for more than three months. It is concluded that impulses from the mucosa of the maxillary sinus do not play any part in the occurrence or persistence of facial pain in this highly selected material.

Epstein-Barr virus in nasopharyngeal and salivary gland carcinomas of Greenland Eskimoes.

Biopsy specimens from nasopharyngeal carcinomas (NPC) or salivary-gland carcinomas (SGC) in Greenland Eskimoes were examined for the presence of Epstein-Barr virus (EBV) DNA and sera from the patients were tested for EBV-specific antibody titres. Six out of 7 NPCs and one from an undifferentiated SGG were positive for EBV DNA. The EBV-specific antibody spectra and titres of the patients with NPC or undifferentiated SGG conformed to the results of earlier studies in other high-incidence areas.

Partial laryngectomy following irradiation.

Partial laryngectomy after full course of irradiation was performed in 65 patients as a salvage for residual tumors and local recurrences. The results are reviewed and compared to results obtained by primary operation. It appears that in glottic lesions excellent results by vertical laryngectomy may be obtained without increased complication rate due to irradiation. In supraglottic tumors horizontal laryngectomy is accompanied by increased postoperative morbidity and severely impaired function to such a degree that total laryngectomy is the operation of choice for irradiation failures in supraglottic recurrences.

The frequency of patulous Eustachian tubes in pregnancy.

Two-hundred and seventy pregnant women (540 ears) were examined for patulous Eustachian tubes. The diagnosis was established by means of a Madsen Zo 70 electroacoustic impedance bridge. Nineteen of the patients were found to have patulous Eustachian tubes in 1 or both ears as evidenced by impedance variations synchronous with the respiration. Only 5 of these women had any subjective symptoms. Nine other women had symptoms, but their Eustachian tubes were closed at the time of examination. The results were related to the duration of the gestation, to oestrogen production and weight gain during pregnancy. A significant correlation was found was found between patulous Eustachian tube and elevated serum oestriol values.

[The treatment of glomus jugulare tumours (author's transl)].

Symptoms and signs in 25 patients with glomus jugulare tumours are described. 14 patients with advanced tumours were treated exclusively by irradiation. No case of progression was noted during a follow-up period averaging 7.9 years.

Inner ear malformations with oto-liquorrhea. Tomographic findings in three cases with a mixed hearing impairment.

The paper deals with three boys suffering from a mixed hearing loss. They were all referred to tomography of the temporal bones in order to find some middle ear malformation amenable to surgery. The tomograms revealed identical malformations of the inner ear; the lumen of the labyrinth are found irregularly dilated and the internal acoustic meatus is widened. Its course in the lateral end is abnormal with a downwards turn towards the basal coil of the cochlea. Explorative tympanotomy in one of the patients revealed a severe otoliquorrhea during an attempt to perform a stapedectomy, and this may serve as a warning against operation in patients presenting the mentioned tomographic changes which, once seen, are easily recognized.

Exfoliative cytology in the diagnosis of laryngeal malignancy.

In searching for a screening procedure in the diagnosis of laryngeal malignancy the authors have evaluated a cytological examination, adapted for indirect laryngoscopy, and present the results of 149 consecutive smears from the larynx. Forty-four per cent of the smears were false negative, and it is concluded that exfoliative cytology with this method has no place as a screening procedure in the diagnosis of laryngeal malignancy. The method is inferior to the evaluation of the clinical picture in indirect laryngoscopy, and the selection for direct laryngoscopy and biopsy for histological evaluation should be based upon the clinical evaluation.

[Toluidine blue staining in laryngeal diseases (author's transl)].

The authors present the results of 83 consecutive in vivo stainings with toluidine blue in various laryngeal diseases. Thirty one (86%) of the 36 malignant tumors showed positive staining. (95% confidence limits: 70-95.) It is concluded that malignant tumors will nearly always fix the dye, and that a negative staining test seems to be evidence against cancer. (Predictive value of negative test: 0.89, 95% confidence limits: 76-96.
