[Normal findings with duplex sonography of liver blood vessels--experience in the examination of 100 patients with healthy livers].

100 patients of our kidney transplantation program and uneventful as regards their liver history were indexed by the pulsatile flow (PFI) of the hepatic artery and the damp curve (DI) of the hepatic veins. The average age of the patients was 36 years (20-50y). The pulsatile flow index (PFI) was calculated as 0.4 to 0.7. The damp index (DI) as 0.3 to 0.54. In 10 of these volunteers the measurement was performed 3 times per day for 5 days: there was no difference to be found in comparison to the values of the total group. The presented values of the PFI and DI give the normal range and will be the basis for further investigations in patients after liver transplantation or suffering from liver diseases.

Activation of a truncated PR-1 promoter by endogenous enhancers in transgenic plants.

PR-1 genes are induced by various environmental stimuli such as pathogen attack or exposure of the plants to certain chemicals. To examine the regulation of these genes, the 5' flanking regions of the PR-la gene and of two PR-1 pseudogenes were joined by a transcriptional fusion to the Escherichia coli beta-glucuronidase (GUS) gene. These constructs were stably integrated into the tobacco genome and independent primary transformants were monitored for the expression of the reporter gene. Unexpectedly, out of 55 transformants analysed, four plants exhibited considerable GUS activities without any inductive treatment of the plants. Expression of the endogenous PR-1 genes, however, could not be detected in these plants. Primer extension analyses revealed correct initiation of the PR1/GUS hybrid transcripts from the PR-1a TATA box. When the plants were analysed at the cellular level, clear differences regarding the tissue specificity of expression of the reporter gene were observed. These results strongly suggest that the PR1/GUS hybrid promoter expression cassettes may be activated when integrated in the vicinity of heterologous enhancer elements dispersed in the tobacco genome. In order to support this hypothesis, domain B of the enhancer of the 35S RNA promoter from cauliflower mosaic virus (CaMV) was fused to various PR1/GUS hybrid genes upstream as well as downstream from the RNA start site. These constructs were stably introduced into the tobacco genome. In any primary transformant analysed, strong GUS activities were observed with the PR1/GUS hybrid RNAs originating from the normal transcription start site of the PR-1a gene. The tissue specificity of gene expression was identical to that described previously for the CaMV 35S domain B enhancer element. Thus, modulations of the transcriptional activity of the PR-1 promoter can be achieved by heterologous enhancers in transgenic plants and may be encountered upon random integration of PR-1 promoter constructs into the tobacco genome.

High-dose intravenous immune globulin therapy for hyperbilirubinemia caused by Rh hemolytic disease.

We conducted a multicenter controlled trial to test the hypothesis that high-dose intravenous immune globulin (HDivIG) therapy can modulate bilirubin production and reduce the frequency of exchange transfusions in newborn infants with Rh hemolytic disease. Thirty-four patients with Rh incompatibility proved by positive direct antiglobulin test (Coombs test) results were randomly assigned to receive conventional treatment including phototherapy, with or without additional HDivIG therapy at 500 mg/kg given for a 2-hour period as soon as the diagnosis was established. Exchange transfusions were performed if serum bilirubin concentrations exceeded the modified curves of Polácek by more than 2 mg/dl. Two patients were excluded because of protocol violations. The results in 32 infants were analyzed. In the HDivIG group, 2 (12.5%) of 16 children required exchange transfusions, whereas it became necessary in 11 (69%) of 16 children in the control group (p less than 0.005). Bilirubin levels in the HDivIG group were lower despite reduced frequency of exchange transfusions. No side effects of HDivIG treatment were observed. We conclude that HDivIG therapy by a yet unknown mechanism reduces serum bilirubin levels and the need for blood exchange transfusions in children with Rh hemolytic disease.

Treatment of pharmacological priapism with phenylephrine.

Pharmacological prolonged erections or priapism due to anesthesia were treated in 36 patients with intracorporeal injections of phenylephrine. Detumescence was obtained rapidly in every case and no marked side effect was reported. Considering the possible toxicity of other adrenergic agents, phenylephrine is strongly recommended in the treatment of priapism.

A new approach towards defibrillation electrodes: highly conductive isotropic carbon fibers.

A new carbon fiber material was studied for its potential benefit in defibrillation electrodes. Miniaturization of the fiber production can result in small strands with extremely large surface areas. Two carbon fiber patches and a single carbon fiber bundle were used for the purposes of this study, and the results were compared to those obtained with conventional titanium/mesh patch electrodes. Tests performed in a saline filled tank revealed considerably lower resistances through the carbon material when compared to standard mesh electrodes. There was also a higher peak current flow with lower voltage. The carbon fibers exhibited lower impedance for defibrillation and this may be related to low polarization known to occur with carbon materials. This new carbon material may prove to be useful and further research is required.

[Intrapericardial teratoma in childhood].

The case of an intrapericardial tumour in a female newborn is reported. It was a differentiated teratoma. Diagnosis was based on the clinical symptoms, x-ray film, echocardiography, and histology. The tumour was operated on complete resection. Apart from a pneumothorax, no complication occurred during or after the operation.

Child life services and persons with mental retardation.

We admitted an adult with mental retardation and an eating disorder to a pediatric inpatient unit. Child life services were a core feature in the assessment and treatment of the patient. The role of child life services in the care of persons with mental retardation should be expanded.

[Spontaneous labor in the 38th week of pregnancy under intermittent hemodialysis treatment of the mother. Alloimmune thrombocytopenia of the newborn infant].

Uncomplicated pregnancies and spontaneous deliveries of term infants are rare if mothers are treated by haemodialysis because of terminal renal insufficiency. Pregnancies under these circumstances mostly end in abortion or stillbirth; live births are mostly delivered by Caesarean section. Independent of the mother's basic disease the infant of this case was born at 38 weeks spontaneously with a neonatal alloimmune thrombocytopenia, which is analogous to Rh-haemolytic disease of the newborn.

[Echovirus 11 outbreak among premature newborn infants in a neonatal intensive care unit].

During a period of 3 weeks, 17 premature neonates 4 of them weighing less than 1000 g at birth became infected with echovirus 11 in a neonatal intensive-care unit. Beside 3 inapparent infections, severe 'septicaemic' illness was observed among 5 premature neonates combined with meningitis and apnoeic attacks. 4 neonates presented meningitis as the predominant clinical feature and 3 showed gastrointestinal symptoms without neurological involvement. 2 children experienced a febrile illness with apnoea and bradycardia. All babies recovered. Echovirus 11 could be isolated in 51 of 74 (68.9%) specimens, mainly from stool samples. The source of this epidemic outbreak has remained unclear. Virus isolations and serological examinations of antibodies of the IgM class showed that one mother and her eldest child and 3 staff members of the unit had been infected during this epidemic outbreak. The agent was identified as an antigenic variant of echovirus 11 resembling other echoviruses of this type which were isolated in other children hospitals in Bremen during this outbreak.
