[Arrhythmia during extraction of the third molars under intravenous sedation. A case report of postoperative tetralogy of Fallot].

Extraction of the third molars [formula: see text) was done under local anesthesia with intravenous sedation using diazepam in a 14 year-old girl. She had the history of radical operation for tetralogy of Fallot at 4 years of age. During extraction procedure, this patient showed many multifocal premature ventricular contraction in ECG. These arrhythmias were able to control with intravenous lidocaine. In this case, the preoperative evaluation of her cardiac function was not pertinent possibly due to the judgment by her ability of physical exercise in the past history. We reflect that inadequate preoperative evaluation of cardiac function may result in inadequate management.

[Determination of the desirable epinephrine concentration containing in dental local anesthetics. Comparison between two lidocaine solutions containing 1/80,000 and 1/200,000 epinephrine].

This study is a pilot study to determine the desirable epinephrine concentration containing in dental local anesthetics. Two percent lidocaine solution containing 1/80,000 and 1/200,000 epinephrine were compared in view points of onset and duration of anesthesia and hemodynamic responses after conduction and infiltration anesthesia in oral cavities of six voluntary dental students. Conduction anesthesia to the right inferior alveolar nerve, using 2% lidocaine solution containing 1/80,000 and 1/200,000 epinephrine showed no significant difference in onset and duration of anesthesia, changes in blood pressure and pulse rate. Duration of anesthesia determined by pin prick showed the longest in infiltration anesthesia, using 2% lidocaine solution containing 1/80,000 epinephrine, (103.4 +/- 18.5 min.), and followed by that containing 1/200,000 epinephrine (52.0 +/- 13.0 min.) and that excluding epinephrine (23.0 +/- 5.1 min.). These findings suggested that the desirable epinephrine concentration containing in dental local anesthetics may be 1/200,000, judging from the time of a dental practice.

Copper metabolism in the macular mutant mouse: an animal model of Menkes's kinky-hair disease.

The tissue copper contents were measured in mutant (hemizygous macular male and homozygous macular female), heterozygous macular female and normal mice. The copper content in kidney and small intestine from 7-day-old mutant and heterozygote were extremely high compared to normal mice, whereas the copper content in other tissues (liver, brain, spleen and serum) was low. Copper content in whole body of mutant mice was extremely low at three stages (18 days gestation, 1 day old, and 7 days old) compared to normal mice with the exception of the mutant fetus at 14 days of gestation. Renal copper contents in the mutant fetus at 18 days of gestation and in the 1-day-old mutant were not changed compared to normal mice, whereas that in 7- and 14-day-old mutant mice was significantly elevated. Hepatic copper content of the mutant mice was extremely low at all stages compared to normal mice. Copper therapy was applied to 7-day-old mutant mice. At 1 day after injection, hepatic copper content in the mutants had increased slightly in comparison with the normal control mice, whereas renal copper content was extremely increased. At 7 days after injection, hepatic copper content in the mutants was decreased greatly in comparison with normal control mice, whereas an increase in renal copper content had remained.

[Blood reflux into cartridge in dental anesthesia. Detection of total protein and human hemoglobin in residual anesthetic solution].

The present study was designed to elucidate the blood reflux into cartridge under infiltration anesthesia. When imitation examination was carried out using pontamine blue dye solution in 7 kinds of syringes for the use of cartridge, dye reflux was observed in all of them. Compared 4 kinds of cartridges on the market, dye reflux was observed in all except one. The amounts of protein in residual anesthetic solution of cartridges after infiltration anesthesia were quantitatively measured by dye binding method. Human hemoglobin was also measured by enzyme immunoassay (EIA). The detection rates of protein and human hemoglobin were 26.2% (85/324) and 24.2% (24/99), respectively. From these facts that blood reflux cannot be avoided in the cartridge system, it is necessary to prohibit the reuse of residual anesthetic solution of cartridge as quickly as possible to avert the risk of cross infection of HB and AIDS virus.

[Bone marrow scintigraphy with 111In-chloride--a clinical value for the hematological diseases].

Bone marrow scintigraphy with indium chloride (111In) was performed in fifty-one patients with the hematological diseases. The results of the investigation were that 1. in all patients, as well as in patients with aplastic anemia, no correlation was there between the degree of the indium chloride accumulation and peripheral blood counts, 2. in patients with aplastic anemia and pure red cell aplasia (PRCA) a tendency to reduction in uptake of indium chloride in bone marrow, 3. in patients with these two good correlation between the degree of indium chloride accumulation and histology of the erythroid bone marrow, but in patients with chronic myelocytic leukemia (CML) and atypical leukemia no correlation between the two, so it seemed unlikely that indium chloride should reflect the effective production of erythrocytes, 4. four patients with leukemia were studied with indium chloride bone marrow imaging two times to evaluate their responses to chemotherapy, and peripheral expansion was no change or reduced in two patients with acute myelocytic leukemia (AML) and one patient with acute lymphocytic leukemia (ALL) who obtained complete remission, but on the other hand, it enlarged in one patient with acute myelocytic leukemia who obtained partial remission, and 5. in two patients with chronic myelocytic leukemia it enlarged up to the ankle joints, which was considerably specific.

[An analysis of regional ventilation in pulmonary sarcoidosis by inspired 133Xe washout test].

Xenon-133 ventilation study were used to measure regional ventilation in seven patients with pulmonary sarcoidosis and in four normal subjects. For the purpose of analyzing the washout curves on the whole lungs, two ventilation indexes were calculated, which were TA/H and T1/2. TA/H is that the area under the curve divided by the difference in mean count rate during equilibrium and at 120 s after washout started. T1/2 value represents the time required to 50% of the mean count rate during equilibrium. The average time of TA/H on the whole lung was 13.5 +/- 1.1 s in normal subjects (n = 4), 13.0 +/- 4.3 s in stage I (bilateral hilar lymphadenopathy without lung involvement on chest X-ray) sarcoidosis (n = 3), 21.3 +/- 1.8 s in stages I, II (lung involvement) sarcoidosis (n = 4). The average time of T1/2 on the whole lung was 43.7 +/- 4.7 s in normal subjects, 42.3 +/- 16.0 s in stagel sarcoidosis, 72.5 +/- 28.6 s stages II, III sarcoidosis. The ventilation indexes on the regional lungs showed almost a similar tendency to that on the whole lung. The ventilation indexes in patients with stage II, III sarcoidosis were significantly longer than in normal subjects and in patients with stage I sarcoidosis. It was suggested that 133Xe washout test might detect regional ventilation abnormalities which were not recognized on chest X-ray in pulmonary sarcoidosis.

Radioprotective effects of thiomethylhydantoin derivatives on Escherichia coli and mice.

Protection of Escherichia coli NIHJ and C57BL mice from the effects of 60Co gamma-rays provided by S-alk(en)yl-L-cysteines and their hydantoin derivatives was examined. E. coli (10(6) cells/ml) suspended in a 20 mM aqueous solution of one of the drugs was irradiated with 60 Gy of gamma-rays. Five week-old male mice were exposed to 5.0-9.5 Gy of gamma-rays after a single intraperitoneal injection of 0.75 mmol/kg body weight of each compound. In both E. coli and mice, S-allyl compounds afforded more effective radioprotection than S-propyl compounds. The replacement of the alpha-hydrogen of S-substituted cysteines by methyl groups decreased the radioprotective effect. Hydantoin derivatives were much more radioprotective than the original sulfur-containing amino acids. Especially, DL-5-allylthiomethyl-5-methylhydantoin had a remarkable radioprotective effect in mice. The gamma-radiolysis mechanism of thiomethylhydantoin derivatives was discussed in connection with the radioprotective effect of the drugs.

Studies of left ventricular volume estimation from gated blood pool scan with the use of regression equations.

The authors carried out experimental and clinical studies of left ventricular volume measurement using equilibrium-gated blood pool scan. Using phantoms, we experimentally obtained a regression equation between radioisotope levels and camera counts. From this equation, we deduced a regression equation between camera counts and volumes, which was then used for calculation of left ventricular volumes using count technique. In an experiment using phantom with various volumes, we obtained a favorable result that the average absolute error between the true and measured volumes was as small as 6.8%. In 11 patients, we calculated the left ventricular volume and ejection fraction employing the regression equation and compared the calculated values with the values obtained by cinemascopy. This comparison disclosed an excellent correlation (r = 0.955 for end-diastolic volume, 0.997 for end-systolic volume and 0.853 for ejection fraction). Mean absolute error was 16.2% for end-diastolic volume and 16.0% for end-systolic volume. These results indicate that this technique is clinically applicable.

[Study of quantitative evaluation of liver volume and accumulation rate of radioactive agent with single photon emission computed tomography].

The quantitative evaluation on measurement of liver volume and the hepatic accumulation rate of the radioactive agent were studied with respect to body constitution, background activity and splenic consumption rate, using single photon emission computed tomography (SPECT). The optical cut-off level for liver volume determination was proved to be reasonable as 35%. The background less than 5% had no influence to their clinical estimation with the error of less than 1.6%. The estimation of liver volume was mostly influenced by the splenic activity. When the splenic activity was increased two times of the hepatic activity, the liver volume was determined as 4.2% larger. Calculated counts by SPECT was well correlated with the hepatic activity. This study proved that SPECT was useful for the quantitative evaluation of the liver volume and the hepatic accumulation rate.

[Significance of renal high uptake in bone marrow scintigraphy with 111In-chloride].

The significance of high renal uptake found in patients with hematological disease who underwent bone marrow scintigraphy with 111In-chloride was investigated. In patients with a high renal uptake, the uptake in bone marrow was low, suggesting a reflection of erythropoietic activity. The reversal relationship, however, was not necessarily present. With regard to correlation with iron metabolism, significantly higher uptake of serum Fe and lower UIBC were found among those with high renal uptake, suggesting that the level of free trans ferrin in the serum is largely involved in the high uptake in the kidney.

Effects of in vitro and in vivo supplementation with zinc on superoxide anion production in leukocytes.

The effects of zinc on the rate of production of bactericidal O2- of polymorphonuclear leukocytes (PMN) in response to three different types of stimulating agents (serum-treated zymosan (STZ), Con A, and myristate) were studied. The percentage reduction of O2- production of PMN stimulated by STZ, Con A, and myristate were all reduced in response to Zn, irregardless of whether Zn was added to the reaction mixture immediately before SZT addition or following a prior 20 min. incubation of PMN in the presence of Zn. However, when Zn was introduced intraperitonially into guinea pigs before the collection of PMN from the animal, zinc treatment produced inhibition only in STZ-activated PMN; it produced no effect in O2- production of PMN stimulated by myristate, and it further augmented the O2- production stimulated by Con A.
