[Acute occlusions of the intestinal arteries--diagnosis and surgical therapy].

Acute obstruction of arterial blood flow in digestive organs has been recorded from two per cent of all patients with acute abdominal disorders and has accounted for roughly eight per cent of all sudden vascular occlusions. Symptoms are generally occult and are quite often misinterpreted. This together with short warm ischaemia tolerance of the intestinal tract, somewhat between two and three hours, has continued to cause death of nearly 90 per cent of such patients. Bilateral retrospective studies were conducted into 66 patients with acute intestinal arterial occlusions over ten years and have confirmed the importance of early diagnosis and immediate surgical action. Particular emphasis should be laid, in that context, on adequate clinical diagnosis and reconstructive arterial surgery along with removal of avital intestinal sections. High mortality among the patients concerned can be substantially reduced by no-delay, thorough medical action.

Arteriosclerosis↗

[Arterial reconstructions spanning the knee joint].

Knee-joint crossing arterial repair is an alternative solution for patients whose extremities otherwise might have to be amputated. Indications are according to Fontaine for Stages III and IV. Specialised angiographic preparatory examination and knowledge of microsurgical techniques are required. Reference is made to peculiarities of surgical access routes, vascular substitute materials, and specific surgical techniques. The use of this approach to peripheral vascular repair appears to be justified against the background of international achievements and the authors' own results.

[Surgical correction of atypical aortic coarctation with hypertension].

Atypically localised coarctation of the aorta is one of the very rare pathological findings. Hypertension of unknown aetiology, atypical vascular sounds, and symptoms of collateral circulation are indicative for diagnosis. Vascular surgery is justified in any case by the juvenile age of the patients concerned and by poor quoad vitam prognosis. Generally, surgical intervention should be undertaken with patients between eight and 14 years of age. Surgical methods and results are reported.

[Brain protection in the surgical reconstruction of the supra-aortic arteries].

The importance of brain protection in surgical handling of supra-aortic arteries may be derived from the specific nature of cerebral blood flow and from the pathophysiology of damaged cerebral vessels. There is a wide range of special protective methods which have proved to be effective, including strict observance of established rules on surgical indication and tactics, maintenance of circulatory homoeostasis, heparinisation, and use of an intra-luminal shunt. Results obtained from operations on 486 patients for occlusion of supra-aortic arteries are discussed, with particular emphasis being laid on the need for intra-operative EEG monitoring. Reference is also made to measures of brain protection in the postoperative phase.

[Peripheral venous pressure in acute and chronic femoral-pelvic vein occlusion--an experimental study].

The behaviour of the peripheral venous pressure was checked after starvation of the venous main vessel of the pelvic limb in 34 mongrel dogs in long-term course about 40 weeks. The influence of recanalisation processes on the pressure development was excluded by the ligature model. The pressure measurements took place continuously statically and dynamically in horizontal and in oblique posture. They produced a changed pressure characteristic with a chronically increased resting pressure and a decreased drop in pressure with load. The insufficient transport capacity of the spontaneously originated collateral circulation could be pointed out.

[Sexual function disorders after vascular reconstruction in the aortoiliac region].

100 male patients who had undergone aorto-iliac surgery were asked by a questionnaire concerning their sexual life. Stenoses or obstructions of the iliac arteries preoperatively demonstrated in 70% could be shown to be connected with sexual disorders, which increased after operation. Especially in patients of younger age the reperfusion of the internal iliac arteries should be taken into consideration.

[Effect of portasystemic anastomoses in childhood on propranolol pharmacokinetics].

Kinetics of propranolol after oral administration were investigated in 9 children aged 5--11 years with intact liver function who had undergone porto-systemic anastomosis surgery for pylethrombosis and in a 16-year-old boy with a Warren shunt. Bioavailability of the drug had increased by twice the values measured in physiologic liver blood flow; elimination half-life had doubled. These alterations must be taken into consideration in medication, e.g. extreme care must be exercised in the oral administration of "first pass" drugs in porto-systemic shunt patients and it is recommended that the initial dose should be at the level of intravenous administration. The reduced rate of elimination should be taken into account in the case of drugs whose elimination depends on liver blood flow.

[Homologous umbilical vein and vena saphena transplants as possible means of vascular access in chronic hemodialysis].

For a patient in the chronic programme of haemodialysis an uncomplicated accession to the vessels is the prerequisite for an optimum medical and social rehabilitation. Each form of a subcutaneous arteriovenous fistula is to be preferred to an epicutaneous shunt prosthesis. The percutaneous puncture of large vessels is necessary for the rapid connection to the dialyses in acute situations. In long-term treatments often all autologous accessions to the vessels are used up. Homologous substitution material for vessels is increasingly used also in patients ongoing dialysis. Apart from the methods for gaining homologous material is reported on first experiences in the application of homologous grafts of the umbilical vein and of a homologous vena saphena graft in patients undergoing dialysis.

[Transluminal angioplasty in combination with surgical procedures in arteriosclerotic occlusive disease of the lower extremity (author's transl)].

In 16 patients suffering from occlusions of the pelvic and/or femoral arteries a combined treatment was performed: percutaneous transluminal angioplasty (PTA) and surgical reconstructive measures. Indications for this procedure are: 1. Stenoses of the pelvic arteries and occlusion of the ipsilateral superficial femoral artery with PTA for the iliac artery and endarterectomy for the femoral artery. 2. Occlusion of the iliac and the femoral arteries with a decreased run-off: PTA and lumbar sympathectomy. 3. Endarterectomy on one side and PTA contralaterally.

[Differential diagnosis of acute leg swelling (author's transl)].

Reference is made to aspects relating to the aetiology of acute leg swelling following surgical or radiological treatment of uterine and ovarian tumours. Four cases are reported, in that context. The swellings described developed up to ten years after initial therapy. Differential diagnosis and therapeutic approaches are discussed. The need for early cooperation between the gynaecologist, radiologist, angiologist, and vascular surgeon is underlined.

[Surgical treatment of segmental occlusions of the popliteal and the crural arteries with modified vascular prostheses (author's transl)].

Possibilities and indications for reconstruction of the femoro-popliteal and femoro-crural region are dealth with. Autogenous veins still are the favoured graft material for the femoro-popliteal/crural bypass. If an autogenous vein is not available bovine arterial grafts, Gore-Tex-PTFE prostheses or modified human umbilical cord vein are alternative materials.

[Late complications after dacron-grafts (author's transl)].

In a series of 850 reconstructive operations for chronic occlusive arterial disease we found 17 anastomotic aneurysms and 6 infections over a period of 8 to 10 years after the operation. In case of vascular infection the excision of the graft is urgent, "extraanatomic" reconstruction can prevent loss of limb and live.
