[Efficacy of a bismuth combination preparation. Efficacy in the treatment of chronic active gastritis and non-ulcerous dyspepsia].

In an open, randomized controlled study, the effect of a combined bismuth preparation, bismuth nitrate and bismuth aluminate on the elimination of Helicobacter pylori, inflammatory activity in the gastric mucosa in chronic type B gastritis, and on the patient's symptoms was investigated. Included in the study were 36 patients with non-ulcerous dyspepsia and chronic gastritis. Twelve patients (Group A) received 4 x 1 tablet a day (800 mg total daily dose), 12 patients (Group B) 2 x 2 tablets a day (800 mg total daily dose), 12 patients (Group C) 2 x 1 tablet (400 mg total daily dose) for a period of 4 weeks. Elimination of Helicobacter pylori was observed in 73% of the patients in Group A, in 87% in Group B, but in only 16% in Group C. Inflammatory activity, measured in terms of polymorphonuclear cell infiltration, regressed noticeably in Group A und B (p less than 0.01), but not in Group C. In all patient groups symptoms regressed significantly (p less than 0.01). Bismuth and methemoglobin concentrations in the serum were within the normal range in all patients on conclusion of treatment.

Loss of alkaline phosphatase activity in duodenal mucosa: a marker for precursors of gastric metaplasia?

Biopsy specimens from duodenal mucosa in 34 patients with upper gastrointestinal symptoms and endoscopically abnormal mucosa (including duodenitis, active duodenal ulcer, and healed duodenal ulcer) and in 9 patients with histologically normal mucosa were examined histologically for gastric metaplasia and endogenous alkaline phosphatase (AP) activity. Using haematoxylin and eosin (H&E) and periodic acid-Schiff (PAS), we found gastric metaplasia in 91.2 per cent (31 out of 34) of patients with altered duodenal mucosa and in 33.3 per cent (3 out of 9) of patients with histologically normal, non-inflamed duodenal mucosa (P less than 0.001). To characterize gastric metaplasia further, histochemical methods for AP activity were applied to duodenal mucosa specimens. No AP activity was detected in complete metaplastic cells, but focal or diffuse loss of AP activity was frequently shown in otherwise normal appearing enterocytes next to metaplastic cell groups. Focal loss of AP activity was also detected in seven out of nine healthy controls (= 77.8 per cent) which appeared normal when stained with H&E. Our results suggest that the loss of AP activity in enterocytes may be an early marker of developing gastric metaplasia or at least a morphological manifestation of cell damage.

[Bismuth aluminate in gastroenterology. Therapeutic effects in chronic erosive Campylobacter pylori-associated gastritis].

14 patients with epigastric pain and chronic active gastritis as well as endoscopically proven erosions underwent an open therapeutic trial with bismuth aluminate (3 x 300 mg/day) for three weeks. Prior to and after treatment five biopsy specimens were taken from the antral mucosa 2 and 4 cm proximal to the pylorus for histological scoring of inflammatory activity and C. pylori detection, for urease quick test (CUT) and culture. Before treatment C. pylori was detected histologically and by CUT in all patients, whereas in culture, C. pylori was not detected in one case. After three weeks of treatment, 9 patients (64%) were C. pylori-negative in culture, and 8 patients (57%) had negative results both on histology and CUT. In 86% (12 patients) chronic erosions were no longer detectable and histological evidence of inflammation was reduced (p less than 0.01). All patients reported symptomatic relief. Complete elimination of C. pylori was observed in just over half the patients. These results confirm the effectiveness of bismuth aluminate in the short-term treatment of chronic active gastritis.

Thyroid inclusion in the lung. Metastasis of an occult papillary carcinoma or ectopia?

The incidental autopsy finding of a thyroid inclusion in the lung of a 26-year-old man, accompanied by an occult papillary carcinoma (OPC) in the thyroid gland, raises the question about the possibility of true thyroid ectopia in the lung vs. metastasis of an OPC. Based on the embryonic development of the thyroid gland and the known thyroid ectopias, as well as on statistical figures on OPCs and their metastases, both possibilities are discussed. The histological pattern of the thyroid inclusion and the lack of regional nodal metastases argue in favour of ectopia, even though a definitive conclusion cannot be drawn.

[Bismuth subsalicylate treatment in chronic Campylobacter pylori-associated erosive gastritis].

In a prospective open study 21 patients with upper abdominal complaints and chronic active gastritis as well as endoscopically proven erosions were treated with bismuth subsalicylate (4 X 30 ml/day, corresponding to 4 X 314 mg Bi3+) for three weeks. In 20 patients (95%) Campylobacter pylori (CP) was found in at least two of three examinations (culture, CLO quick-test, special histology). After the treatment 17 of 21 patients (81%) were CP negative, and the clinical symptoms had gradually disappeared. The histologically demonstrated degree of activity had regressed significantly after the three-week treatment; in 90% of patients the inflammatory process had completely abated. These results demonstrate that bismuth subsalicylate is highly effective in the acute treatment of CP-associated chronic erosive gastritis.

[Short and long-term survival in fatal street traffic accidents].

Only persons who die within 30 days after an accident are primarily considered accident fatalities. This arbitrary deadline has considerable legal consequences, especially for health insurance companies. Advances in the immediate and optimal treatment for persons injured in traffic accidents at the site of the accident, as well as improvement in surgical techniques, have increased the number of fatalities, with the injured surviving for more than 30 days after the accident. An analysis of reports showed that about 5%-16% of deaths by accident survived the fatal accident for more than 30 days, so the arbitrary deadline of 30 days clearly needs to be revised.

Chronic erosive gastritis--a therapeutic approach with bismuth.

37 patients with epigastric pain and chronic erosive gastritis underwent an open controlled therapeutic trial with bismuthsubsalicylate (BS). Group A (21 patients) was treated with BS, liquid, 4 X 314 mg for three weeks, group B (16 patients) with BS tablets, 3 X 300 mg for two weeks. A significant reduction of symptoms (p less than 0.001) and endoscopically assessed chronic erosions (p less than 0.001) was achieved in both groups. Campylobacter pylori was detected in 89% of the patients before treatment, but was absent in 78% of the patients after treatment. The histological grading of antral mucosa showed a significant reduction (p less than 0.001) of polymorphonuclear cell (PML) infiltration after two and three weeks treatment respectively. While in group A PML cells had disappeared from gastric mucosa in all but two patients, in group B 50% of the patients had some degree of PML cell infiltration left in the antral mucosa. This study confirms the beneficial effect of BS in the treatment of C. pylori associated active chronic gastritis and reemphasizes the pathogenetic role of C. pylori in this disease.

[Immunohistochemistry in pancreatic cancer with new monoclonal antibodies].

Immunhistochemistry using hybridoma-derived monoclonal antibodies (MAb's) directed against tumor associated antigens offers new approaches to morphological diagnosis of tumors, e.g. for classification and typing of tumor subtypes with different prognosis. Before in-vivo application of monoclonal antibodies in man for diagnostic or therapeutic purposes the tissue distribution of the antibody should be determined immunhistochemically for assessment of antibody sensitivity and specificity. The binding capacity of the newly developed MAb BW 494/32 to pancreatic cancer tissues was therefore tested by immunhistochemistry, other benign and malignant intestinal and extraintestinal tissues served as controls. The MAb BW 494/32 showed positive immunoreactivity in all cases of ductal pancreatic adenocarcinomas grading I to II (sensitivity: 100%). In grading III carcinomas the target antigen was only accessible in half of the tested specimens. Specificity was limited by a mostly weak immunoreactivity with normal pancreas tissue in 36%, with acute inflamed tissue in 100%, with inflamed tissue in chronic pancreatitis in 71%. Extrapancreatic specificity was restricted by positive antibody binding to singular goblet cells of gastrointestinal epithelium. The well established antibody CA 19-9 was used for comparison. CA 19-9 showed about the same sensitivity of 88% (BW 494/32: 92%) for ductal pancreatic adenocarcinomas grading I to III. Specificity of CA 19-9, however, was comparable more restricted, as CA 19-9 showed a significant cross-reactivity with other gastrointestinal adenocarcinomas (5 out of 6 colorectal cancers, 3/6 gastric carcinomas, 1/3 carcinomas of the common bile duct), with some extraintestinal cancers (breast 60%, lungs 50%) and with other non-cancerous tissues of intestinal origin. According to the results of this study it is worthwhile to investigate the antibody BW 494/32 for possible applications in experimental immunodiagnosis and -therapy of pancreatic cancer.

[Histomorphologic studies of the detection of disordered barrier function of the gastric mucosa].

The gastric mucous barrier is characterized anatomically and by physicochemical processes ("double barrier"). Its main components, such as a mucus cover, foveolar epithelium and mucosal interstitium can be investigated by morphological methods. In the present light microscopical study an analysis of mucosal changes after peroral aspirin administration was performed. A significant difference was found between the reaction pattern of antral and corpus mucosa. The distribution of pathologic changes found in stromal microcirculation (ectasia and hyperemia of blood vessels, deformation and rouleaux-formation of red cells, beginning hemolysis) suggests an indirect action of aspirin through vasoactive mediators rather than a direct effect of the drug. The data are compatible with a significant role of a compromised microcirculation in the pathogenesis of epithelial defects.

[Lipomas of the uterus: rare tumors of this site--case report].

Benign tumours of the uterus except leiomyomas are rare. We report on two lipomas of the uterus. It is a remarkable fact that uterus lipomas can mimic sarcomas. One of our patients was also suspected of having a sarcoma because of rapid progression of tumour size and abdominal pain.

In vitro glycoprotein synthesis and secretion by gastric mucosal tissue of patients with gastritis.

Endoscopically obtained gastric antrum mucosal tissue of 50 patients was examined for inflammatory cellular infiltrations and cultured. The incorporation rates of 3H-glucosamine into tissue and the secretion of 3H-labeled mucus were determined. The total glucosamine incorporated into tissue and secretions was found to be increased in mucosa from patients with histologically proven gastritis when compared to normal gastric mucosa. The differing time-dependent patterns of incorporation and secretion curves in patient groups with various forms of gastritis suggest the presence of various stimulating factors dependent on the histological pattern of inflammatory cellular infiltration. The possible modifying role of cellular and humoral factors associated with gastritis in the gastric mucus production is discussed.

[Histological and ultrastructural findings in the healing phase of duodenal ulcer].

To evaluate and further characterize duodenal ulcer healing macroscopical, histological, and ultrastructural criteria were examined in 24 patients with active duodenal ulcer disease under two different treatment schedules. After the endoscopic confirmation of an active duodenal ulcer patients were randomly assigned to treatment with either an antacid (group A) or with an H2 blocker (group B). Tissue specimen were taken during endoscopy from the margin and 1 cm from the ulcer before therapy and from the ulcer scar or the previous ulcerated region after a 4 week treatment. A macroscopic ulcer healing by endoscopic criteria was confirmed in 83% of the patients in group A and in 83% of group B. The prominent histological feature of duodenal ulcer healing was a significant increase of PAS positive epithelial cells. Ultrastructural changes of the duodenal mucosa were still present at the end of the treatment, even in the presence of complete macroscopic healing. The ultrastructural healing process shows constant patterns independent from the type of treatment. The characteristic morphological feature by electron microscopy of the healing duodenal ulcer is the appearance of a metaplastic mucus secreting cell of the antrum-type. The devastated mucous structure during the ulcerative phase regains the normal net-like structure after 4 weeks therapy.

Morphogenesis and possible precursor lesions of invasive carcinoma of the papilla of Vater: epithelial dysplasia and adenoma.

Surgical specimens from 58 invasive carcinomas of the papilla of Vater were studied histomorphologically. Tubular or villous adenomas, adenomatous residues, and microadenomas were found in the vicinity of the carcinomas in 91.4 per cent of the cases; moderate or severe epithelial dysplasia in adenomatous structures or in surface and ductal epithelium was present in 81 per cent of the cases. The results of the present histomorphologic study support the hypothesis that invasive carcinomas arise from pre-existing mucosal lesions, such as adenoma or dysplasia.
