[Percutaneous drainage of the gallbladder in acute cholecystitis].

Percutaneous cholecystostomy has replaced surgical treatment for acute cholecystitis in surgical high-risk and critically ill patients. We wished to assess the procedures performed at Lillehammer County Hospital. We report 32 drainages performed in the last three years. Clinical records and radiological reports were reviewed retrospectively. Technical problems and complications during and after the procedure were recorded. In spite of relatively few procedures per radiologist, all were initially successful. No serious complications related to the procedure were encountered. Dislocation of the catheter occurred in 5 out of 32 drainages. This corresponds well to earlier reports. We conclude that percutaneous cholecystostomy in acute cholecystitis is a safe procedure in this group of high-risk patients at our hospital.

[Transcervical resection of the endometrium. An alternative to hysterectomy in dysfunctional uterine hemorrhage].

Transcervical electroresection of the endometrium was performed in 104 patients with dysfunctional uterine bleeding which did not respond to conservative treatment. Transcervical resection was offered as an alternative to abdominal hysterectomy. Re-resection was performed in 17 women (16%) because the initial section did not have the desired effect. Altogether 122 resections were performed. 79% of the women were satisfied after the initial resection and 93% when the procedure was repeated. During a follow-up period of six to 28 months after treatment, a hysterectomy was performed in 18 (17%) patients, but unacceptable uterine bleeding was the indication in only five of these cases. One major complication occurred, a small bowel perforation which required intestinal resection. Three uterine perforations occurred without any damage to neighbouring organs. Except for transient headache in a few patients, no clinical symptoms of glycine toxicity were seen. It is concluded that transcervical endometrial resection seems to represent an additional option for surgical treatment of dysfunctional uterine bleeding, but the procedure is not without risk. The long-term results of this procedure must be compared with those achieved by traditional abdominal hysterectomy.

Dosing of neuroleptics in elderly demented patients with aggressive and agitated behaviour: a double-blind study with zuclopenthixol.

A double-blind controlled trial was carried out in 73 hospitalized elderly patients to evaluate the effect of different low doses of the neuroleptic, zuclopenthixol, on behavioural disorders associated with dementia. Patients were randomized into four groups and received treatment for 4 weeks with fixed daily doses of 2 mg, 4 mg or 6 mg zuclopenthixol, or with a dose which could be increased from 4 mg to 20 mg daily. The dose could also be reduced if necessary. Fifty-four of the patients remained on the same daily dose throughout the study. The results of symptom assessments showed that there was a significant improvement from baseline scores in all of the dose groups and, with the exception of patients on 2 mg daily, this was evident after only 1 week of treatment. Although improvement was noted in all the symptoms evaluated, the best effect was achieved on aggressive behaviour, restlessness/agitation, sleep disorders, and shouts/torments others. Only a few, relatively mild side-effects were recorded and there was no increase in frequency with increasing doses. There was significant correlation between the serum concentration and the dose of zuclopenthixol used but not in connection to age and clinical efficacy.

[Drinking water for tourists in the Tropics].

The modern Norwegian tourist may need a simple and rapid procedure to ensure that water is potable. Convenient available products for such include chemicals and microfilters. We describe and comment on the chemicals currently available in Norway. We have also evaluated a series of portable filters. The microfilters reduced the number of microbes quite efficiently. Some also had good flow capacity, and can certainly be recommended as useful traveller's equipment.

Zuclopenthixol and melperon in the treatment of elderly patients: a double-blind, controlled, multi-centre study.

A double-blind study was carried out in 53 elderly patients in 6 geriatric nursing homes to assess the effectiveness of the neuroleptics, zuclopenthixol and melperon (flubuperone), in the relief of restlessness, aggressiveness and other such symptoms. The initial daily dose was 4 mg zuclopenthixol or 75 mg melperon, increased if necessary over the treatment period of 4 weeks. Assessments were made on entry and after 1, 2 and 4 weeks of treatment of the overall severity of illness and of individual symptoms. The results showed that there was significant improvement in the condition of patients in both treatment groups and a significant reduction in mean total as well as in the main single symptom scores. These changes were already apparent after 1 week of treatment. Although there was a tendency for faster improvement in the zuclopenthixol group, there were no significant differences between the groups in any of the parameters assessed. Side-effects were few and generally mild and transient.

Dementia and work load evaluation of the elderly.

Dementia and work load were estimated in 4736 elderly home-nursed or institutionalized persons. Mental status was estimated by means of the Clinical Dementia Rating scale (CDR), and work load by means of a traditional work load scale. Approximately one third of elderly over 85 years were living in an institution. 4.6% of the population over 65 years of age were moderately to severely and 1.8% mildly demented. Work load increased with increasing dementia. In homes for the aged 40% and in nursing homes 85% of the residents create a heavy work load. 14.5% of the estimated population are considered misplaced, the majority of whom should be transferred to a higher care level. Dementias cause a major problem on every care level. Services which are lacking for this group and necessary rehabilitation facilities should be provided.

Evaluation of a home apnea monitoring program.

From July 1978 to November 1981, 251 infants were studied for apnea at Children's Orthopedic Hospital and Medical Center. Idiopathic apnea was diagnosed in 45% of the infants referred. Infants who had histories of severe episodes tended to have more severe epidoses on follow-up and required a longer monitoring period. Age at onset of apneic episodes was related directly o length of monitoring period. Prematurity was not found to correlate with age at onset or severity. Developmental problems evident in some infants will require long-term follow-up.
