[Long-term follow-up with various Dacron bifurcation prostheses. 5-years results of a prospective randomized study].

In the frame of a prospective randomised study various Dacron bifurcation prostheses with or without velours or with or without gelatin impregnation were implanted in one hundred patients. Fifty-four patients could be reexamined clinically and sonographically after an average of 66 months. A total of 23 patients had died during the period of observation. A comparison of the Fontaine stages during reexamination indicated in 72.3% of cases an operation-related improvement as compared to the condition in the preoperative stage. Sonographically, an average dilatation of 17.3% could be proved. A significant difference due to material or impregnation was not observed. After 72 months the limb salvage amounted to 83.0%. A comparison of the various types of prostheses regarding their limb salvage could indicate no significant difference in the course of 72 months. Based on the only slightly differing results within the period of observation, only the more comfortable intraoperative handling leads to giving preference to the use of double velours prostheses in comparison to prostheses without velours. A positive influence of impregnation with gelatin could not be established.

[The anatomic isthmus in the carotid triangle].

In a limited number of surgically treated patients with blunt trauma to the internal carotid artery (ICA), intimal lesions were found at the intersection of the ICA with the hypoglossal nerve (CN XII). Therefore, these clinical findings were investigated in an anatomic study. In two-thirds of these cases, the congenital "anomalous" relationship of ICA and CN XII caused a strangulation phenomenon during extreme turning manoeuvres of the head and neck. On the basis of our clinical intraoperative findings, anatomic studies und reports in the literature, a further mechanism of vascular injury is suggested in this area.

Strangulation of the internal carotid artery by the hypoglossal nerve.

Various aspects of blunt trauma to the internal carotid artery (ICA) can be found in the literature. Lesions of the arterial wall of the ICA at its intersection with the hypoglossal nerve (CN XII) were observed in a limited number of surgically treated cases. The pathophysiology of the whiplash injury and the anatomic proximity of the crossing nerve to the lateral arterial wall suggest a further mechanism of vascular injury in this area.

[Paralytic ileus as an initial manifestation of malignant VIPoma of the pancreas--case report with review of the literature].

A 57-year old patient with a paralytic ileus of unknown origin was admitted to the intensive care unit. Because of the laboratory findings with therapy resistant hypokalemia, hypercalcemia and metabolic acidosis a VIPoma was suspected. Therapy with somatostatin resulted in correction of laboratory abnormalities and in normalization of gastrointestinal motility. Plasma concentrations of VIP and PP were elevated, ultrasonography revealed a pancreatic tumor. Postsurgical examination of the removal tumor tissue confirmed the diagnosis of a malignant VIPoma. Clinical symptoms, laboratory findings with and without somatostatin-therapy and immunhistochemical properties are described.

Efficacy and tolerability of intra-arterial and intravenous prostaglandin E1 infusions in occlusive arterial disease stage III/IV.

The efficacy and tolerability of intra-arterial and intravenous PGE1 infusions in patients with occlusive arterial disease [OAD] were studied in two independent multicentre therapeutic trials. In the first study 218 patients with OAD stage III/IV were given an i.a. infusion of 1/2 to 1 ampoule of Prostavasin (10-20 micrograms PGE1) in 50 ml physiological saline solution, once a day. 211 patients took part in the second study. Patients in stage III received one i.v. infusion of 3 ampoules Prostavasin (60 micrograms PGE1) once daily, and patients in stage IV received one i.v. infusion of 2 ampoules Prostavasin (40 micrograms PGE1) twice daily, in 100-250 ml physiological saline solution. In both studies the treatment period was 4 weeks. Both i.a. and i.v. administration afforded a significant improvement in the clinical symptoms. Ulcers healed completely in 18.6% of patients under i.a. treatment (i.v.: 10.7%) and partially healed in 38.9% (i.v.: 51.1%). 50% of the patients became pain-free in the course of i.a. treatment (i.v.: 47.7%) and, at the end of the study, 61.5% no longer required analgesics (i.v.: 48.6%). Stratification of the patients into diabetics and non-diabetics showed that in the main the therapeutic response in diabetic patients was as good as in non-diabetics, allowing for the fact that the clinical pictures were usually distinctly more severe in the diabetic patients. The adverse drug reactions were, predominantly, familiar symptoms such as redness, swelling and pain in the infused extremity and phlebitis-like redness along the course of the infused vein, gastro-intestinal symptoms and headache.

Intrathecal production of HIV antibodies in suspected AIDS encephalopathy.

Thirty-one serum and CSF samples from 21 HIV-antibody-positive patients with neurological deficits were examined to prove or exclude intrathecal production of HIV antibodies. By dilution, sera were adjusted to the IgG concentration of the corresponding CSF samples. Both samples were then serially diluted in log2 steps down to the detection limit and were tested in an anti-HIV ELISA. From the dilution obtained at the cut-off level, a quotient QHIV was derived as an indicator of intrathecal production of HIV antibodies. Six of a total of eight samples with a QHIV value of greater than or equal to 2 were correlated which the clinical diagnosis of AIDS-related dementia complex (ARDC). However, a QHIV less than 1 did not exclude the development of ARDC, as was shown during follow-up in one case. Different methods are compared for the determination of intrathecal production of IgG and anti-HIV. A quotient QHIV greater than or equal to 2 is suggested to be highly indicative of intrathecal production of anti-HIV as well as of the development of ARDC.

Results after implantation of gelatine-impregnated bifurcated grafts.

In two randomized prospective trials grafts impregnated with gelatine were compared to those not impregnated. In the first trial a double-velours graft (n = 50) was compared with a graft without velours trimming (n = 50) implanted in the aorto-iliac bifurcation. One half of the grafts in both groups were gelatine-impregnated. In the second trial 24 grafts without velours trimming (Cooley II, Meadox), 24 grafts manufactured by a new warp-knitting procedure without velours trimming (Protegraft 2000, B. Braun AG) and 24 identical grafts of B. Braun AG but with gelatine impregnation were evaluated. Gelatine-impregnated or not the grafts without velours showed no satisfying results. An elevated intraoperative bleeding incidence and a low two-years-patency rate of 64% were noted. On the other hand all double-velours grafts, gelatine-impregnated or not, showed a 100% patency rate two years after implantation. In the second trial a slight reduction in intraoperative blood loss was noted when using the gelatine-impregnated grafts.

[Treatment of chronic non-secretory diarrhea in ileostomy with the long-acting somatostatin analog SMS 201-995].

A patient with chronic refractory non-secretory diarrhea following ileostomy was treated with the long acting somatostatin analogue SMS 201-995. Under treatment with somatostatin stool weights were drastically reduced and electrolyte dysbalance was normalized within a few days. This case report shows that somatostatin is also therapeutically effective in non-secretory diarrhea.

[EEG changes in HIV infections].

A standardized examination, including EEG, was performed in 54 cases of HIV infection. The EEGs were analyzed and compared to 30 EEGs of HIV-negative test persons. The visual analyses with respect to basic activity, dysrhythmias, general slowing, foci and alteration of vigilance were performed without any information on HIV infection. Significant differences in the median frequency were found between both groups, HIV-positive patients presenting with a slower basic activity. Moderate or severe dysrhythmias (2 patients) or foci (2 patients) were found exclusively in HIV-positive patients. The findings described above could not be replicated when only HIV-patients without secondary CNS infections were compared to HIV-negative test persons.
