[Comparative studies of various methods of measuring bile acid secretion using 75Se-labelled homotaurocholic acid (75Se-HCAT)].

The simple determination of whole-body retention of the bile acid analogue 75Se-HCAT provides results which are in agreement with those of the established but more difficult measurement of 24-14C-TCA excretion in the feces. Determining whole-body retention of 75Se-HCAT is not only practicable with a whole-body counter but also, with precision, with the more widely used gamma camera. Therefore, the bile acid retention test may be employed wherever there is a gamma camera available.

[Systolic time intervals in chronic calcifying pancreatitis caused by alcohol abuse].

Systolic time intervals corrected for heart rate were measured non invasively in 12 male patients (32 to 59 years) with chronic calcifying pancreatitis of alcoholic origin and compared with 24 normal subjects without evidence for chronic alcoholism or heart disease. Systolic time intervals (in detail: the time from the beginning of QRS to the first heart sound (QS1), the isovolumic contraction time (IVCT), the total electromechanical systolic interval (QS2c), the pre-ejection period (PEPc), the left ventricular ejection time (LVETc) and the ratio PEPc/LVETc) in patients with chronic calcifying pancreatitis were not different when compared with healthy man. Therefore we conclude, that the amount of alcohol that induced a chronic calcifying pancreatitis was not able to alter systolic time intervals as seen in an alcoholic cardiomyopathy.

[Indication for operation on the vertebral artery (author's transl)].

Fifty-three reconstructive vascular operations on the vertebral artery are reported. In our opinion, the operable causes of the vertebro-basilar insufficiency are worthy of more attention. The operative discomfort of these surgical interventions is of minor importance and may even be expected of patients well advanced in years. The postoperative results decide in favour of a positive attitude towards reconstructive surgery of the vertebral artery. The Doppler ultrasound technique helped to specify more precisely the indication for cerebral angiography. Do we recognize vertebral insufficiency too seldom?

Non-invasive studies of the extra-cranial cerebral arteries: indication for angiography and operation.

During recent years, the risk involved in cerebral angiography has been considerably reduced. However, in our patient group this risk was still almost as high as that involved in operation. Non-invasive examination by means of the ultrasonic Doppler procedure has been improved with a resulting success rate of over 95%. Color coding stored in a computer program with the "Echoflow"-procedure allows for demonstration of the neck vessels giving an indication of the degree of stenosis. By this method, pictures of document quality are provided which can also be obtained by inexperienced examiners and interpreted more easily than curve registrations of the acoustic signals. In comparison with angiography, the Echoflow method was less sensitive than the conventional ultrasonic Doppler procedure especially in cases of low-degree stenosis of the internal carotid artery. Nevertheless, in 2.7% of the cases, a correct Echoflow diagnosis could be confirmed during operation where angiography had not shown the changes correctly. As the Echoflow procedure does not give sufficient information on intracranial pathology, we have so far dropped angiography in emergency cases only when establishing the indication for operation; however, in follow-up examinations this method is used exclusively instead of angiography.

[Excretion of lead in the gastric and duodenal juice of persons with occupational lead poisoning and normal subjects].

Lead was measured in the gastric and duodenal juice of three patients with lead poisoning (blood concentrations of lead: 4.30; 5.16; 5.50 mumol/l). The lead excretion into the gastric juice was 94.6; 29.9 and 18.3 nmol Pb X h-1 for the poisoned persons and 57.9 +/- 38.1 nmol Pb X h-1 for healthy persons (n = 20). Pentagastrin (6 microgram/kg, i.m.) stimulated lead excretion in both groups (normal + 109%, lead poisoned persons: + 180%; + 187%; + 311%). The lead excretion was correlated with an increase of HCl-secretion and volume in healthy persons. The lead excretion into the duodenal juice after secretion (1 unit/kg) amounted to 14.2 +/- 8.8 nmol Ph X h-1 in healthy persons, whereas after secretion plus caerulein 18.8 +/- 7.2 nmol Pb X h-1 were found. In the duodenal juice of the lead poisoned persons 27.5 and 474.9 nmol Ph X h-1 respec. were found after secretion and 58.9 and 491.8 nmol Pb X h-1 respec. after secretion plus caerulein. Lead excretion was correlated with enzyme secretion (trypsin and chymotrypsin).

[Eosinophilia as a diagnostic clue to primary sclerosing cholangitis (author's transl)].

The history of a 38-year-old, asymptomatic man with primary sclerosing cholangitis is reported. The diagnosis has been suggested by eosinophilia (66%, 27 X 10(9)/l leucocytes) and markedly elevated serum alkaline phosphatase levels (1860 U/l) and has been established by endoscopic retrograde cholangiography. Report of a therapeutic approach with D-Penicillamin. Documentation of follow-up by endoscopic retrograde cholangiography.

[Experiences in 130 operations of carotid elongation (author's transl)].

A report on 130 operations of carotid elongation is presented. Indications for reconstruction are based on angiography, neurologic symptoms, and recently, on ultrasonography (Doppler's). Several surgical methods have been performed: 25 resections of the internal carotid artery, 5 resections of the common carotid artery, 3 resections of loop aneurysms. The main operative techniques were resection and neoimplantation, the latter with patch-plastic surgery. Usually, a shunt was used. Mortality was 0%, lethality, 2%. During a follow-up period of 6 weeks to 2 years, 86% of the patients were found to be free of complaints, and 14% were without change of the neurologic symptoms. The good neurologic results are convincing.

[A mathematical model for the determination of loss of venous capacitance using impedance plethysmography].

The results of impedance plethysmographic examination were evaluated by a mathematical curve analysis. The magnitude of the exponential coefficient of the saturation and discharge function provides a highly accurate measure of the loss of functionality. The percentage change in the flow rate may be obtained by substitution of the exponential coefficients in a model formula.

[Vein surgery, functional investigation, surveying equipment (author's transl)].

The different physical screening tests (ultrasound-Doppler technique, plethysmography, phlebodynamometry) are easily documented and provide a less dangerous, less expensive, and more comprehensive statement about the venous hemodynamics of the lower extremity. The number of diagnostic errors can be minimized, and the indication for operation is reliable.

[Drug resistance of phage types of Salmonella typhimurium (author's transl)].

Antimicrobial susceptibility testing (disk diffusion test) and phage typing (method according to Anderson, performed by Prof. Brandis, Bonn) of 703 strains of Salmonella typhimurium from humans, isolated in Essen and surrounding during 1972 to 1975, was performed to dertermine whether characteristic patterns of drug resistance were associated with a single phage type or not. The most frequently isolated phage types are phage type 17 (12.2%), 12 (10.1%), 49 (8.1%), 15a (5.7%), 2 (3,3%) and the untypable phage type (46.7%). Resistance to tetracycline (45%) was most common, followed by resistance to sulfanilamide (19%), ampicillin (9%), and chloramphenicol (7%). Resistance to other antimicrobial agents (trimethoprim/sulfamethoxazol, gentamicin, colistin) was quite rare. Antibiograms of different phage types were found to be different, not only as far as the ratio of sensitive to resistant strains is concerned, but also for the ratio single-resistant to multiple-resistant strains. These differences were found to be statistically significant (chi-square-test, table 2). Comparison of antimicrobial resistance to tetracycline showed, that the portion of resistant strains was about 70% for the untypable phage types, 40% for phage type 17 and 7% for phage type 12. Among isolates of the phage type 49 multiple resistance was most common. The combination of resistance determinants is specified in figure 3. A possible interrelation between resistance pattern and the presence of R-plasmids in isolated strains is discussed.

[Sex specific action of insulin on the glucose metabolism of lymphatic tissue of rats and humans (author's transl)].

Glucose metabolism of lymphoid cells isolated from thymus and spleen of Wistar rats, weighing 140-150 g, was found to be sensitive to insulin. The influence of insulin on glucose uptake by isolated cells from both lymphoid tissues displayed a significant sex specificity. Insulin at 0.1-10 nmol/1 stimulated glucose uptake of cells from male rats, whereas in cells from females, inhibition of glucose uptake was observed. However, lactate production was enhanced in lymphoid cells of both sexes. Cortisol (100 nmol/1) displayed a significant anti-insulin action on glucose uptake and lactate release by thymocytes of male rats. In contrast, in cells from females, cortisol and insulin both exhibited an inhibitory action on glucose uptake, whereas the hormones were found to antagonise lactate production. The sex specific stimualtory influence of insulin on glucose uptake by thymocytes of male rats was reversed and the hormone became inhibitory when animals were castrated. The action of insulin on glucose uptake was also shown to be age-dependent. In experiments with rats weighing either about 10 g or 40 g, sex-specific effects on glucose influx were found, that were similar to those of rats weighing 140-150 g. However, stimulation of glucose uptake was found with thymocytes from rats of both sexes weighing about 80 g. Comparable results were obtained with isolated thymocytes from immature humans (2 months-10 years old). Incubation of thymocytes from males with insulin (10 nmol/1) stimulated glucose uptake and lactate release, whereas insulin caused an inhibition of glucose uptake and an enhancement of lactate production in thymocytes from female. Age dependence and sex specificity of insulin action on glucose metabolism in lymphoid cells and tissues may explain the contradictory results reported by other authors.

Identification and partial characterization of specific oestrogen-binding components in human kidney.

In normal human kidney from adult males cytoplasmic components which bound 17beta-estradiol specifically and with high affinity were demonstrated by dextran-coated charcoal assay, sucrose gradient centrifugation and agar gel electrophoresis. The dissociation constant of the oestradiol-binder complex amounted to 2.2 +/- 0.1 x 10(-9) mol/l. The binding capacity was limited to 34.0 +/- 9.7 fmol/mg of cytosol protein. Sedimentation in sucrose gradient revealed the bulk of these components to be in the 4S region. The binding entities could be clearly separated from sex hormone-binding globulin by agar gel electrophoresis. The ligand specificity for binding to these components indicated a requirement for oestrogens. The fact that an excess of aldosterone had no competitive effect on oestradiol binding suggests that the oestrogen-binding sites are independent of mineralocorticoid receptors. It is concluded that the specific binding components in human kidney have the properties of oestrogen receptors.
