A comparison of percutaneous and perconjunctival routes of administration of peri-ocular anaesthesia for day case cataract surgery.

This study of 100 day case patients having cataract surgery under local anaesthesia compared patient acceptance of two peri-ocular techniques; a transcutaneous method and a transconjunctival method. A significant difference was found in relation to the pain felt on injection such that all peri-ocular blocks at our unit are now done via the perconjunctival route.

Human orf. A diagnostic consideration?

We saw four unrelated cases of human orf infection over a 3-month period. Each patient had a clear-cut history of contact with sheep and developed a characteristic painful pustular lesion on the hand. There were no significant systemic symptoms. Examination of an aspirate by electron microscopy confirmed the diagnosis. The lesions resolved spontaneously within 6 weeks. Human orf infection occurs in North America, and although seldom reported, it should be considered in the diagnosis of cutaneous lesions in patients who have exposure to animals associated with it.

Morphological observations on patients with presumed trichiasis.

The clinical findings in 116 patients with a provisional diagnosis of trichiasis are presented. In 69% a small degree of entropion producing lash-globe contact was detected. The term lid border entropion is proposed for this condition, which is recognised clinically by conjunctivalisation of the meibomian gland orifices and anterior placement of the mucocutaneous junction. These features were clearly demonstrated by electron microscopy of a biopsy specimen from one patient. An analysis of other causes of trichiasis is discussed and an attempt is made to clarify the terminology currently in use.

Lidocaine removal during resin hemoperfusion for phenobarbital intoxication.

A deeply comatose patient with severe phenobarbital intoxication was hemoperfused with an Amerlite XAD-4 column. She was receiving lidocaine for ventricular ectopy. The rate of lidocaine infusion had to be increased to control ventricular arrhythmias during hemoperfusion. Plasma clearance for lidocaine by the column was 122 ml/min at initiation and 75 ml/min at 2 h. These findings suggest that an increment in lidocaine infusion may be needed during resin hemoperfusion in patients receiving this drug because of its removal by the column.

Comparison of subclavian vein with femoral vein catheterization for hemodialysis.

Subclavian vein catheterization (SVC) is not widely accepted for vascular access because it is considered more invasive than femoral vein catheterization (FVC). The use of FVC and SVC for acute vascular access was compared for 9 mo each. Complications were minimal with both, but access related hospitalizations were lower and the number of out-patient dialyses performed were higher with SVC than FVC. Complications with SVC might have been minimized by using an 18-gauge metallic needle for insertion, avoiding interdialytic infusions and using SVC for less than 4 wk.

Increment in dialysate sodium with sodium chloride or bicarbonate addition.

Hemodialysis was performed in 12 patients for 2 weeks each utilizing acetate dialysate containing 134 mEq/L sodium and dialysate containing 143 mEq/L sodium, achieved by the addition of sodium chloride or sodium bicarbonate to the acetate dialysate. Intradialytic morbidity was lower, dialysis hypoxemia less marked, and predialysis blood pH higher with the bicarbonate-than with the chloride-added dialysate. The long-term use of sodium bicarbonate-added dialysate in three patients was safe. Dialysate pH adjustment was not required. These findings suggest that the addition of sodium bicarbonate (50-75 g) to acetate dialysate may be preferred to sodium chloride for increasing dialysate sodium in selected patients.

Atrial lead malposition in a dual chamber (DDD,M) pacemaker.

A model 7000 pacemaker in the DVI mode emitted two stimuli, the first stimulating the right ventricle and the second 110 ms later. The atrial lead was found in the right ventricular outflow tract. The QRS complex produced by the atrial lead fell after the blanking period, and "committed" the ventricular stimulus 110 ms after the "atrial." Intracardiac electrography during the original implant would have enabled proper atrial lead position.

Double-blind trial of glycerol therapy in early stroke.

The effects of intravenous glycerol and intravenous dextrose were compared using a double-blind trial in twenty-seven patients with acute stroke. Administration continued for up to 6 days. A standard scoring system was used for neurological evaluation. There was no difference in mortality or in improvement in neurological score between the two groups.

On-line interactive evaluation in course and clinical instruction.

Student evaluations incorporated into feedback for instruction modification are an important aspect of any educational program. It is particularly important for healthcare programs in which teaching processes are continually dynamic. An interactive evaluation was designed and implemented on-line for students. Confidentiality was assured and privacy protected through a unique coding system. Our experience with the interactive, automated system proved particularly successful with respondents and faculty expectations have improved since its integration into course and clinical objectives.

The risk of ocular involvement in non-accidental injury.

A consecutive group of 22 children admitted to our hospital with a diagnosis of non-accidental injury, underwent detailed ophthalmic examination within 24 hours of their admission. Five of these children had obvious periocular soft tissue injury, but only two had serious ocular involvement. The indications for ophthalmic examination in cases of suspected non-accidental injury are discussed in the light of these findings.
