[Determination of creatinine in the urine as a reference value of substances excreted by the kidney].

The creatinine excretion is often used as reference value of renally excreted substances. It is usual to perform the creatinine determination according to the Jaffe method. However, since this reaction is influenced not only by the creatinine content but also by other substances it was for orientation tested to which extent adequate incorrect determinations may appear. For this purpose the creatinine content was determined in 28 casual urine specimens in the analysis automation (A) and manually (M) according to the Jaffe method as prescribed in the German Pharmacopoeia 7th ed. D. L. As comparison the enzymatic determination (E) was performed. The determinations according to Jaffe showed too high values. The mean relative percental deviation for the determination in the analysis automation was for (Formula: see text) (variation 6.7 to 94.3%). There were firm correlations between the values of the three determination methods. Since the creatinine determination after Jaffe is the cheapest and most practicable method, in clinical routine work in creatinine-related data of concentration of various renally excreted substances one should realize that in the individual case deviations of the concentration are possible on average up to 50%.

[Dietary iodine deficiency in East Germany].

The human organism depends on exogenic supply of iodine. The alimentary supply with iodine is not sufficient in the GDR. The deficiency leads to an endemic struma. The causes are among others as follows: small geochemical offer of iodine with low iodine content of vegetable and animal products and of drinking water as well as moderate consume of fish. The most favourable possibility as to cost for the abolition of the iodine deficiency is the use of a stable iodized kitchen salt (potassium iodate instead of potassium iodide). The gradual introduction of such a salt is started. Also after the improvement of the iodine supply representatives of human medicine, veterinary medicine and agriculture should in interdisciplinary cooperation devote themselves to the problems of a controlled optimum supply of iodine and the fight against endemic struma.

[Preliminary studies on iodine administration during parenteral nutrition].

Iodine deficiency is the main cause of endemic goiter. To estimate the iodine supply during parenteral nutrition, the iodine content of some important solutions used for infusions and routine parenteral nutrition has been assessed. It varied from 21 micrograms I/l to 68 micrograms I/l and is considered to be sufficient to supply the daily iodine requirement.

[The radioiodine test for the evaluation of iodine deficiency in the district of Karl-Marx-Stadt and in East Germany].

The values of the per cent 24 h radioiodine uptake in the GDR are above 60--70% and speak for a low alimentary intake of iodine or renal excretion of iodine below 40 micrograms J/d. Like the struma prevalences they show a tendency increasing from north to south and characterize the whole European situation of iodine deficiency including its decrease from west to east. The mean values of radioiodine uptake of 71.7 +/- 13.2% (n = 110) in euthyroids of the district of Karl-Marx-Stadt correspond to the iodine deficiency as it occurs approximately in the districts of Erfurt, Dresden, Munich or Freiburg/B. The alimentary iodine intake of 38.4 +/- 17.2 micrograms J/d and the renal iodine excretion of 29.9 +/- 16.1 micrograms J/d, calculated from the 24 h radioiodine accumulation values of 40 euthyroid persons by means of a mathematical model developed by Oddie and co-workers were low. The latter only slightly differed (P less than 0.05) from its chemically estimated excretion of iodine in the urine: 23.1 +/- 16.9 micrograms J/g creatinine (n = 73). Between the calculated and chemically estimated excretion of iodine there was a relatively strict correlation of r = 0.68 (n = 26; P less than 0.001). The introduction of an iodine prophylaxis is regarded as an urgent necessity. Later on a new estimation of the regional "normal values" is necessary for the per cent radioiodine uptake.

[Infusion therapy and parenteral feeding in diabetes mellitus].

Infusion therapy and parenteral nutrition demand the consideration of diabetes-specific peculiarities in coma diabeticum, in the pre-, intra- and postoperative phase and in the parenteral nutrition which is to be performed for a long time. Adequate points of the centre of the coma therapy are: substitution of fluid, insulin and electrolytes. There exist partly still diverging opinions on the way of the practical approach. The nowadays usual methods are demonstrated as guide lines. The care of the diabetic in the pre-, intra- and postoperative phase is described and the author enters the particular problems of the parenteral nutrition in diabetes mellitus from the pragmatic points of view.

[Incidence of gestational diabetes and changes in insulin secretion of pregnant women (author's transl)].

The 50-g oGTT was applied to pregnant women suspected of having contracted diabetes. Carbohydrate tolerances were pathological in 23.6 per cent of all probands and in borderline vicinity in 17.9 per cent. The oral glucose tainting test thus worked according to expectation by revealing a relatiively high frequency of disorders of the carbohydrate tolerance. Yet, impaired insulin secretion was established rarely, with high response having been recorded from only seven per cent and delayed insulin secretion from 4.1 per cent. Low response was not found at all. No correlations were found to exist, by the 50-g oGTT, between disorders of the carbohydrate tolerance and insulin secretion. The groups involved differed but little with regard to their IRI mean value curves. Sugar excretion in urine was found to be increased with significance in response to pathological carbohydrate tolerance in early pregnancy and may be used as a complementary criterion for diagnosis.

[Incidence of gestational diabetes as well as changes in insulin secretion during pregnancy. 1. Studies on pregnant women suspected of diabetes using the glucose infusion test (GIT)].

Based on the glucose infusion test, we find with 17.9 per cent of a group of anamnestically tainted pregnant women gestational diabetes, and with 5.7 per cent of this group a carbohydrate tolerance with disturbed boundaries. With probands having a carbohydrate tolerance with disturbed boundaries we find a significantly more frequent IRI-high-response. This coincidence of a carbohydrate tolerance in the border range and of an IRI-high-response might correspond to the early asymptomatic stage of diabetes. In the following stages, there will take place a depression of the early insulin phase with a pathological carbohydrate tolerance of the pregnant women in the sense of a gestational diabetes. With probands exhibiting a disturbed carbohydrate tolerance, a diminished depression of free fatty acids is found. The total lipid content and cholesterol are not essentially changed. With women suffering from gestational diabetes urinary sugar excretion is significantly higher than with probands showing a normal carbohydrate tolerance.

[Temperature behavior of infusion solutions].

Since rheological qualities (density, viscosity) of solutions depend upon temperature and consecutively influence the conditions of current, i. e. among others the dropping speed of the solutions to be infused, the behaviour of the temperature of the solutions glucose 50, 200, alvesin "new", fructose 400, intralipid 10%, sorbitoli 100, electrolytic infusion 153 DAB 7 and as comparative fluid distilled water was investigated. All solutions showed qualitatively the same behaviour. In an infusion not running after 3 till 4 hours at all levels of the infusion bottle an assimilation of the temperature of refrigerator to room temperature is to be expected. In running infusion after about one hour the infusion solution in the canule reaches room temperature, whereas the temperature of the solution in the infusion bottle is about 10 degrees C lower. With increasing speed of infusion this difference in temperature decreases. A constancy of the rheological qualities of an infusion solution is, therefore, in non-running infusion to be reached after about three hours, in running infusion after about one hour, in case that before the solution had the temperature of the refrigerator (4 degrees C till 10 degrees C). The dropping speed is influenced in such a case only by the changing of the hydrostatic pressure.

[The density--viscosity--temperature relationship of infusion solutions].

Viscosity and density of the infusion solution have an influence on the dropping rapidity when the conventional infusion technique is used and on the elevation volume when infusion pumps are used. They depend on temperature and thus may vary the infusion volume. For selected infusion solutions this behaviour was analysed and described. The changing of these parameters depending on temperature is insignificant and might not be relevant clinically.

[Liver diseases in diabetes mellitus].

In a number of 3,554 clinically manifest diabetics who were admitted for the treatment of metabolism or other diseases from 1967 to 1974 12.1 per cent of hepatopathies were found. In men the incidence was 15.2 per cent, in women 10.7 per cent. Among these the fatty degeneration of the liver (28.8 per cent) and the cirrhosis (17.4 per cent) were most frequent. Referred to the entirety the result was an incidence of cirrhosis of 2.1 per cent. The confirmation of the diagnosis is performed by biopsy and endoscopy in 92 per cent. In 60 per cent of the examined persons the diagnosis was unknown before admission. There was no correlation to the duration of the diabetes. In the number of patients there appeared above all persons older than 50 to 60 years. The following concomitant diseases occurred: hypertension (33 per cent), coronary diseases (32 per cent), pyelonephritis (17 per cent) and adiposity (13 per cent).
