[Roaccutan in acne and rosacea].

Isotretinoin (Accutane) is a lately developed synthetic oral retinoid for treatment of severe forms of cystic acne resistant to therapy. Its pharmacological effect principally consists in decreased size of the sebaceous glands, reduced sebum production, as well as alteration of the bacterial micropopulation. At a dosage of 0.5 mg up to 1.0 mg/kg body weight daily, isotretinoin led to significant reduction of the inflammatory skin eruptions and long-lasting remission after discontinuation of the drug. With regard to 18 patients suffering from rosacea, the application of Accutane brought about satisfactory results, as well. Mucocutaneous side-effects were almost compulsory, but did never lead to discontinuation of the treatment. Because of its teratogenity, isotretinoin must not be applied in case of gravidity. Accutane offers new modes of therapy with respect to patients suffering from nodulocystic acne or severe rosacea which did not respond to common forms of treatment.

[Sporadic, endemic and AIDS-related Kaposi's sarcoma. Different clinical course variants of the tumor demonstrated by 3 typical cases].

We report on three patients suffering from Kaposi's sarcoma (KS) of the classical, endemic, and AIDS-related variant. We point out the uniformity of this tumor in terms of histomorphology. We found the characteristic findings of angiomatous structures and dense spindle cell formations. All variants of the tumor show histochemical and immunohistological evidence for a histogenesis probably from endothelial cells of venous capillaries. On the basis of our cases and the literature, we discuss to what extent cellular immunity may determine both the tumor biology and therapeutical considerations.

[Pemphigoid lichen ruber].

In two patients with exanthematical lichen planus large bullae occurred in preexisting papules during a severe rush of disease. The bullae were formed subepidermally but Ig G deposits were not found. So both patients suffered from lichen planus pemphigoides as primarily described, and not from bullous pemphigoid with lichen planus.

[Lymphomatoid granulomatosis with "epithelial islands" (author's transl)].

The lymphomatoid granulomatosis was first definded by Liebow, Carrington and Friedman as a vascular angiodestructive lymphoreticular proliferation process. The clinical symptoms and signs are pruritus, acquired ichthyosis and the following cutaneous alterations: widespread skin nodules and plaques, patchy alopecia and anhidrosis. Our patient revealed the cutaneous pattern mentioned above and a pathological pneumonography, showing diffuse infiltrates in the lower lobes of the lung. Epithelial islands are a characteristic feature of the lymphadenoid infiltrate in our patient. To our knowledge, only the two patients with lymphomatoid granulomatosis described by MacDonald and Sarkany (10, especially case 1) revealed this special pattern.
