[Conception and conversion of medical technology assessment--an international comparison].

The outcome of adjusting health care spending to the overall economic performance of the country has become a major issue of health care policy of the industrialized nations. How can progress in medicine be consistently financed under the restriction of fixed contributions to mandatory sickness funds? Medical Technology Assessment is an interdisciplinary concept that proposes solutions to this issue. This article discusses the concept of Medical Technology Assessment and shows how Medical Technology Assessment should be implemented into the health care system. This article depicts how Medical Technology Assessment is implemented in the United States of America, in the United Kingdom and in the Netherlands. It also discusses which institutions take steps towards Medical Technology Assessment in Germany. The second article examines how to implement Medical Technology Assessment into the German health care system.

[The future of medical technology assessment in Germany--five proposals for its development].

How progress in medicine will be financed in the future is an important issue internationally and in Germany. Medical Technology Assessment is an interdisciplinary concept that proposes solutions to this issue. This article deals with health political measures which should be taken to implement Medical Technology Assessment in the German health care system. Five proposals are developed: Establishing a coordinating and communication unit for Medical Technology Assessment. Intensifying the research into efficient health care. Increasing the use of Medical Technology Assessment in coverage decisions. Increasing consumer participation in coverage decisions. Establishing a competitive order for the use of expensive technology.

Phase-resolved fluorescence lifetime measurements for flow cytometry.

A flow cytometer capable of measuring fluorescence lifetimes by the phase shift method has been built and evaluated. Under optimal conditions, the resolution of the fluorescence lifetime measurement is shown to be under 200 picoseconds. Pulse intensity variations are normalized using limiting amplifiers and electronic filtering. Normalization of signal intensities provides a lifetime measurement that is independent of fluorescence intensity over at least a 50-fold (17 dB) range in fluorescence intensity. The fluorescence lifetimes of unbound dye, fluorescent beads, cells stained with ethidium bromide, propidium iodide, and phycoerythrin-conjugated monoclonal antibodies have been measured. The fluorescence lifetimes measured for these particles are well correlated with lifetime measurements made using a standard fluorimeter. Cells stained with ethidium bromide and propidium iodide at various nucleotide-to-dye ratios are shown to exhibit similar behavior to static cuvette measurements. The fluorescence lifetime parameter is also shown to resolve phycoerthyrin fluorescence from propidium iodide fluorescence.

[Lipid analytic investigations concerning the establishment of the forms of primary hypercholesterolaemia in patients with myocardial infarction].

The influence of genetic factors in the development of the coronary heart disease can be particularly impressively clarified at the instance of the primary hyperlipoproteinaemias. The exact diagnostics of the genetically determined disturbances of lipometabolism in patients with infarction is, however, still connected with difficulties. Determinations of serum cholesterol and triglycerides, lipid-electrophoretic recognitions as well as determinations of LDL- and HDL-cholesterol in the serum cannot always outline with utmost clearness the proportion of primary hyperlipoproteinaemias in the morbidity of infarctions. Before this background the practical value of the radiochemical screening test worked up at the model of lymphocytes is emphasized, which allows a clear diagnosis of primary forms of hypercholesterolaemia according to pathogenetic viewpoints and thus renders possible also family examinations within the primary prevention of the coronary heart disease.

Macula and periphery: their contributions to the visual evoked potentials (VEP) in humans.

An alternating checkerboard pattern subtending a visual angle of 25 min arc is capable of eliciting remarkably high amplitudes of visually evoked potentials (VEP) outside a visual angle of 2.5 degree. These contributions cannot be neglected when evaluating a VEP response obtained with a larger field of stimulation. When the retina is consecutively stimulated by concentric frame-shaped stimuli, the algebraic sum of all single cortical responses augmented the cortical response obtained when stimulating the whole field simultaneously by approximately 30%. This finding may be explained by the inhibiting interactions within the receptive field organisation of the retina.

[A sudden inexplicable deterioration of vision caused by bromcarbamide drugs (author's transl)].

On two female patients it was shown that intoxication by sedatives containing part bromecarbamide may result in various ocular diseases. The two patients complained of a sudden inexplicable deterioration of vision; semi-dilated slow-responding pupils were diagnosed. Only in one case could a sudden vertical nystagmus be observed. Many possible symptoms were shown. In most cases a discontinuation of drug together with therapy lead to a complete withdrawal of symptoms.

[Operative treatment of Duane's syndrome associated with extreme vertical divergence (author's transl)].

In following an operative treatment of Daune's-Syndrome associated with extreme vertical divergence is described. During operation the external rectus muscle is recessed and inserted lower due to anatomical anomalies. This is performed by a little known method described by A. B. Scott. Our results are very satisfactory. Psychical alterations associated with these disorders also could be sucessfully managed.

[Strabismus in cerebral paretic and normal children. Comparison of motoric symptoms (author's transl)].

Investigations of 94 children with cerebral palsies have shown 55% strabismus incidence. Motoric symptoms like varying angles, nystagmus (latens), A-V phenomena, dissociated hypertropia and compulsive head postures are presented, frequently more often than in a number of comparison cases of undamaged squinting children. However, these symptoms have not permanently been found even in severly damaged children. With the exception of compulsive head postures, no symptoms are found in strabismus cases of cerebral paretic children that do not occur in congenital strabismus patients as well. A frequent family heredity strain of strabismus has also been noted. There is a striking proportion of divergence cases (27%). The authors do not believe in a simple damaging etiology. Pleoptic-orthoptic exercises are hardly ever practicable. Early operation may improve the initial situation as far as the neuropediatric therapy is concerned.

[Diagnosis of primary hypercholesteremia--a classification of genetic factors as causes for myocardial infarct].

Cultvated lymphocytes of patients with myocardial infarction with a primary hypercholesterolaemia as well as of unconspicuous control persons take up LDL-cholesterol labelled by 125iodine. Here is evident that lymphocytes of patients with a primary hypercholesterolaemia may incorporate significantly less LDL-cholesterol than healthy control persons. Supplementing the serum lipid determinations and the lipid electrophoresis the described radiochemical investigations at the model of lymphocytes make possible a biochemically exact diagnosis of the primary hypercholesterolaemia and thus an evident genetic analysis of the patients with infarctions.

[The relationship of amplitude of visual evoked potentials to side length of rectangular stimulus pattern and to abruptness of stimulus alternation (author's transl)].

Cortical potentials evoked by viewing checkerboard pattern reversal stimuli as well as horizontal and vertical rectangular pattern reversal stimuli were recorded. The amplitude of the cortical evoked responses decreased with increasing side length of the rectangular pattern and decreasing velocity of the pattern reversal. Psychophysical and electrophysiologic findings are compared with these data and their clinical relevance is discussed.

[Closure of the central artery of the retina in progressive scleroderma (author's transl)].

In the present paper a case of generalized scleroderma with concomitant central arterial occlusion is reported. To our knowledge this coincidence of generalized scleroderma and central arterial occlusion has not been described so far. As fibrinoid necrosis and proliferative changes of the choroidal vessels occur with generalized scleroderma the possibility is discussed that the central arterial occlusion may be an effect of the scleroderma.

A controlled comparison of two therapeutic soft lenses in a clinical model.

Laboratory data of a new, highly hydrated contact lens (Weicon 72) indicated that it might be therapeutically superior to the known Weicon 38. The great variability of therapeutic contact lens cases, however, makes a sound clinical evaluation difficult. Therefore the two lenses were tested in a clinical model using patients with traumatic superficial corneal defects. The results were statistically significant, the Weicon 72 proving to be superior to the Weicon 38.
