Mechanical failure in intramedullary interlocking nails.

PURPOSE: To study clinical and mechanical factors that predispose to failure of interlocking nails. METHODS: Between October 1996 and December 2002, 286 femoral fractures, 211 tibial fractures, and 47 humeral fractures were repaired using variously designed interlocking nails. Fracture pattern, level and site, nail size and type, weight bearing after nailing, and union status were reviewed after a mean follow-up of 22 months. RESULTS: Nail failure occurred in 27 fracture repairs (17 femoral, 9 tibial, and one humeral; 13 from our institution and 14 referred from elsewhere). In 55% of failed repairs, the fracture was distal. A high rate of tibial nail failure was noted. CONCLUSION: Distal fractures and stress concentration at the distal screws predispose to interlocking nail failure and can be prevented by protected weight bearing combined with the use of longer and larger nails. Routine supplementary cancellous bone grafting is unnecessary during renailing surgery when adequate reaming and a larger nail are used.

Bilateral macrodactyly of the hands and feet with post-axial involvement - a case report.

Macrodactyly is a rare congenital anomaly characterized by large digits. The usual involvement of macrodactyly is on the pre-axial side of the limb. There has been only one case report to date with involvement of both hands and feet in a child with post-axial upper limb involvement. We report an adult male who has macrodactyly of both hands and feet with post-axial involvement of both hands and pre and post- axial involvement of both feet.

Reconstruction of the distal radius by fibula following excision of giant cell tumor.

Thirty giant cell tumors of the distal radius were excised and reconstructed using a nonvascular fibular osteoarticular autograft. Four different surgical techniques for stabilization were used. There were ten recurrences, which could not be correlated with Campanacci's radiological or Jaffe's histological grading. Twenty cases without recurrence were followed up over 1.5-25.5 (average 8.5) years. Average time for incorporation of the graft was 5.2 months. The surgical technique using a radiofibular plate and K wire through the wrist had a low nonunion rate, no graft related complications, good range of movement, and good hand functions.

Glomus tumour of the hand.

We report a series of 18 patients with pain in the thumb or finger tips who were diagnosed as having glomus tumour. Fourteen patients were found to have histopathologically proven glomus tumours, one had a neuroma and three had haemangioma. The duration of symptoms before treatment averaged 1.9 years. Cold-sensitivity and Hildreth's tests have sensitivities of 100% and 77.4% respectively and specificity of 100%. Love's pin test had a sensitivity of 100%. Love's and Hildreth's tests showed 78% accuracy, whereas the cold-sensitivity test was 100% accurate. We used a double-tourniquet technique during dissection and excision of the tumour. None of our patients experienced recurrence of symptoms during the post-operative follow up.
