[Quality assurance in carotid artery surgery pre-, intra- and postoperative].

In our clinic, quality control and the need for safety are an integral part of the clinic's daily regime. For every kind of arterial reconstruction, independent of its locality, we have developed clinical standards and guidelines, which make our procedures transparent, comparable, and evaluable. For every assistant in our educational establishment, these standards have become a fixed guideline for adequate therapeutic treatment.

[Problems and results of treatment of vascular injuries--a 20-year analysis].

From 1.1.1971 until 31.12.1990 we observed and provided 331 patients with 344 acute injuries of arteries. Reasons was iatrogenic and traumatic injuries. In 94% of iatrogenic injuries and in 67.7% of traumatic injuries a full restitutio could be achieved. In 11.9% the extremity was saved with a resting functional deficit, in 11.9% an amputation was required, 9.5% hat a letal outcome.

[Phospholipid concentrations and localization of selected phospholipids in erythrocyte membranes of breast tumor patients].

The phospholipid content within the erythrocyte membrane, its composition and topological distribution was determined in 17 untreated patients with histologically verified breast cancer. The qualitative analysis of phospholipids was performed by one- and two-dimensional TLC. Topological changes of the aminophospholipid asymmetry were examined by TNBS-labelling. In comparison with control values, there were no significant differences of the investigated phospholipid parameters. In conclusion, in the early stage of breast cancer, there is no influence of tumour growth on the phospholipid pattern of red blood cells. Thus, diagnosis of breast cancer using erythrocyte phospholipids is impossible.

Lipid metabolism during treatment of endometriosis with the progestin dienogest.

The effects of a new progestational compound, dienogest (17 alpha-cyanomethyl-17 beta-hydroxy-estra-4,9-dien-3-one) on lipid metabolism have been studied in 84 otherwise healthy women with laparoscopically proven endometriosis. The women, aged 17 to 45, years were treated with 2 mg dienogest in tablet form daily for 24 weeks. The progestin was highly effective on endometriotic lesions and symptoms, showing an objective endoscopic and a subjective symptomatic improvement in 80% and 83% respectively. Triglycerides, total cholesterol, HDL-cholesterol, LDL-cholesterol and the LDL-cholesterol/HDL-cholesterol ratio were determined before and after 1, 3 and 6 months use of the progestin. There was a maximum decrease of -5.8% in HDL-cholesterol and of -6.5% in triglycerides after 6 months of therapy vis-à-vis the pretreatment values. The maximum increase in LDL-cholesterol of +5.0% was recorded by the end of the first month of dienogest ingestion. The LDL-cholesterol/HDL-cholesterol ratio rose from 1.6 to 1.8 during the course of therapy. There were no significant changes. The data suggest that 2 mg dienogest has little influence on lipid metabolism and provides also in this respect a suitable approach to the hormonal therapy of endometriosis.

[Behavior of parameters of liver metabolism in intermediate-term use of the gestagen dienogest in the treatment of endometriosis].

The effects of the progestational compound dienogest (17 alpha-cyanomethyl-17 beta-hydroxy-estra-4,9-dien-3-one) on liver metabolism have been studied in 101 otherwise healthy women with endoscopically proven endometriosis. The women aged 17 to 45 years were treated with 2 mg dienogest in tablet form daily for 24 weeks. Aspartate amino transferase (ASAT), alanine amino transferase (ALAT), gamma glutamate transferase (GGT), alkaline phosphatase (AP), lactate dehydrogenase (LDH), and total bilirubin (BILI) were determined in serum before and after 1, 3 and 6 months use of the progestin. During the therapy period no deviations from the normal ranges were found. There was a slight significant decrease of ASAT and ALAT and a slight significant increase of LDH and BILI (p less than 0.05) remaining within the normal laboratory values. Since no undesirable metabolic side-effects have so far been observed with dienogest, it may be considered an effective new alternative treatment for endometriosis.

Effect of streptozotocin on transaminases, creatinine and urea in serum of rats.

This study investigates the diabetes-induced lesions in liver and kidney and in addition the possible side effects of the diabetogenic substance streptozotocin (SR) on these organs in non-diabetic animals. 5-week-old female Wistar rats were injected 65 or 130 mg SR/kg body mass. Some animals of the drug group did not become hyperglycemic; thus it was possible to separate the drug effect from the diabetic influence on liver and kidney. In serum investigations some metabolic changes concerning the activities of the liver enzymes aspartate aminotransferase, alanine aminotransferase, alkaline phosphatase and the concentrations of urea and creatinine up to 30 days after drug application were studied. SR in hyperglycemic animals causes a time and dose dependent rise in all investigated parameters. Also in normoglycemic rats a significant increase in alkaline phosphatase and in creatinine was observed after 10 days. After 21 and 30 days there were no differences compared to untreated control rats, whereas elevated levels were observed in the hyperglycemic rats. Thus our results support the view of a short damaging effect of SR on liver and kidney without inducing a diabetic state; in hyperglycemic rats the damaging effect is more pronounced.

Interactions of basophilic granulocytes and fat metabolism as indicators in thrombophilia.

In different groups of patients and blood donors with increased thrombophilic or arteriosclerotic risk we found some correlations between basophilic granulocyte values and lipid parameters. Basophilic granulocytes are one of the bodies own origin of sulfated mucopolysaccharides which protects endothelial cells and saves high density lipoprotein cholesterol, as we were able to find in blood donors.

The influence of a sequential-type oral contraceptive with ethinylestradiol-sulfonate soft gelatin capsules and norethisterone acetate on carbohydrate metabolism during the first twelve months.

A prospective study was made to investigate the carbohydrate metabolic effects of the sequential-type oral contraceptive in form of a weekly pill with 0.6 mg ethinylestradiol-sulfonate soft gelatin capsules and 10 mg norethisterone acetate. Blood glucose and plasma insulin levels were measured during an intravenous glucose tolerance test in 17 nondiabetic healthy women. Each woman was tested before and after twelve months of drug use. There were no changes neither in average fasting blood glucose nor in glucose assimilation after intake of ethinylestradiol-sulfonate. At twelve months there were significant elevations of basal as well as reactive insulin levels.

[Relations between parameters of lipid metabolism and hemostasis in vascular diseases].

In 98 patients with arterial insufficiency of arteriosclerotic and diabetic genesis correlation analyses were carried out between total cholesterol, HDL cholesterol, triglycerides, creatinine, blood pressure, platelet counts, platelet function and antithrombin III. Besides the known relations between the parameters of lipid metabolism and blood pressure, correlations were found and analysed between antithrombin III, platelet function, renal function and between platelet counts and HDL cholesterol. The importance of the findings for the diagnosis of vascular insufficiency and the relation to normal values are discussed.

[Effect of sequential preparation with ethinyl estradiol sulfonate soft gelatin capsules and norethindrone acetate on carbohydrate metabolism].

An intravenous glucose tolerance test (i.v.GTT) was done before and after 6 months of using a sequential-type oral contraceptive in form of a weekly pill containing 0,6 mg ethinylestradiol-sulfonate (EES) soft gelatin capsules and 10 mg norethisterone acetate (NEA). 17 fertile, eumetabolic women are examined . There was no change neither average fasting blood glucose nor glucose assimilation after intake of ethinylestradiol. Basal as well as reactive insulin values increased by longer period of intake.

[Subacute and subchronic percutaneous tolerance testing (28-day and 90-day test) of epicutaneously applied dis-infectants as demonstrated by the example of peroxyethanoic acid (author's transl)].

As compared to control animals, guinea-pigs to the shaved backs of which a dose of 0.16 ml of a 0.12% solution of PES/100g. of body mass had been applied twice daily for periods of 28 and 90 d, respectively (5 d of application being followed by 2 d without treatment), showed erythema, loss of hair, slower increase in mass, increased heart rate, smaller body mass/kidney mass and body mass/spleen mass ratios, increases in leucocytes, ASAT, ALAT, LDH I and LDH III. In the 28-d test, slight inflammatory symptoms of the liver, kidneys and heart were observed in the experimental animals. These symptoms were more marked in the 90-d test, granulomata in the livers being particularly striking. A pneumonia of moderate to very marked degree is suggestive of the activation of a clinically latent PES infect whereby the inflammatory alterations in the kidneys might be explained by the formation of metastases.

[Kidney changes in experimental obesity].

In a survey renal changes in human obesity as well as in experimental obesity of laboratory animals are discussed. Own results are described about a comparison of biochemical and morphological studies on an experimental model of dietary-induced obesity after long-time feeding of a high-fat diet. Obese rats show normal concentrations of plasma proteins after feeding of a high-fat diet of some weeks. Urea-nitrogen levels are not modified as compared to control animals. Despite the mean levels of creatinine concentration in the dynamic and static phase of obesity are not different, some obese rats show increased creatinine values in the dynamic phase. Our histological studies reveal no pathological abnormalities of renal structures and blood vessels.

[Biochemical parameters of blood plasma, and body and organ weights of Wistar rats with dietarily-induced experimental obesity].

The authors investigated in rats with dietarily-induced obesity certain biochemical parameters of the blood plasma as well as body and organ weights during the dynamic and the static phase of obesity development. They determined total cholesterol, total protein, albumin, creatinine, urea nitrogen and transaminases. After 4-5 weeks, the animals on a high-diet (50% of fat) had body weights which were, on an average, by 90% higher than those of the control animals. This difference persisted during the static phase. In the animals on a high-fat diet, body length was greater. The high-fat diet (which contains a great proportion of sunflower oil) leads to a decrease of the plasma cholesterol level in obese rats. The plasma-protein bodies, creatinine and urea nitrogen values as well as those for transaminases permit, as parameters for function and damage, to draw conclusions as to kidney and liver damages in the animals on high-fat diet. There were no differences in plasma protein between the control and experimental animals. On the contrary, obese rats showed in some cases high creatinine concentrations during the dynamic phase. Differences in urea nitrogen were not observed between the two groups of animals. Increases in alanine aminotransferase were found in the animals on high-fat diet as a manifestation of fatty degeneration of the liver. A synopsis of weight curves, biochemical parameters and histological findings permits the conclusion that, besides of dietarily-induced metabolic alterations, no additional organic lesions occurred during the present animal experiment on dietarily-induced obesity.
