Von Willebrand factor, fibrinogen and other risk factors of thrombosis in patients with a history of cerebrovascular ischemic stroke and their children.

BACKGROUND AND AIM: Von Willebrand factor (vWF) and fibrinogen (Fb) have recently emerged as plausible familial determinants of atherothrombosis. We investigated whether the vWF and Fb levels in patients with ischemic cerebrovascular stroke (ICS) correlate with those in their children. METHODS AND RESULTS: The study group consisted of 28 families (56 parents and 34 children) with one parent who had suffered an ICS at least three months before the study. All of the ICS patients had hyperlipoproteinemia and most arterial hypertension. The control group consisted of 15 families (30 parents and 20 children). The age of the parents and children did not exceed 55 and 16 years. The ICS parents had significantly higher vWF, Fb and protein C (PC) levels than the controls (vWF--fathers: 121.0 +/- 42.5% vs 79.2 +/- 23.4%; vWF--mothers: 110.7 +/- 40.1% vs 82.4 +/- 20.9%; Fb--fathers: 4.12 +/- 0.74 g/L vs 3.01 +/- 0.54 g/L; Fb--mothers: 3.64 +/- 0.84 gL vs 2.98 +/- 0.35 g/L; PC--fathers: 116.0 +/- 12.3% vs 105.6 +/- 13.7%; PC--mothers: 114.4 +/- 15.8% vs 105.0 +/- 12.2%). The children of the ICS parents had significantly higher PC and body mass index (BMI) values than the controls (PC: 102.6 +/- 13.7% vs 92.7 +/- 10.7%; BMI: 20.6 +/- 3.8 vs 17.8 +/- 3.5 Kg/m2), as well as an atherogenic lipid profile, higher blood pressure (BP) and a tendency toward higher vWF levels. Correlations between the ICS parents and their children were found for vWF, factor VIIc, low-density lipoprotein cholesterol, high-density lipoprotein cholesterol and BP, which were closer in the case of fathers. CONCLUSION: Regardless of gender, the parents with a history of ICS had a procoagulant state, with high levels of vWF, Fb and PC. In terms of inheritance, the most adverse risk factor profile was found in the children of ICS fathers.

[Risk factors for atherosclerosis in offspring of parents with primary hypertriglyceridemia].

High triglycerides and low fibrinolysis activity are considered as a significant predictors for atherosclerosis. The aim of our study was: 1-to compare these risk factors levels in children with positive family history of hypertriglyceridemia (HTG) with children from healthy families and 2-to assess the association between triglyceride and fibrinolysis activity in offspring (y) and parental risk factors (xl ... xn) for atherosclerosis. The study population consisted of: I Group 15 children 7-12 years old and their parents from HTG families and II Group 26 control children 5-12 years old from healthy families. Triglyceride (TG), total cholesterol, cholesterol esters, LDL-Ch, HDL-Ch, HDL2Ch, apolipoprotein AI and BII, fibrinogen plasma level were determined. Plasminogen activator inhibitor (PAI-1) and fibrinolysis activity and (ELT) were determined. We found significant differences in clinical examinations (higher blood pressure and ECG disturbances more frequently), HDL-Ch and HDL2-Ch plasma level was lower in children with positive family history than in control children. Multiple regression analysis of parental variables demonstrated, that children's TG and ELT are strongly determined by parental lipids and haemostasis parameters in children with positive family history of HTG.

[Evaluation of platelet function and tissue plasminogen activator activity in ischemic heart disease depending on concurrence with hyperlipoproteinemia and aspirin therapy].

Platelet activation, impairment of fibrinolysis and dyslipidemia are important factors in the pathogenesis and progression of ischemic heart disease. Aspirin therapy will reduce platelet activation both by its negative effect on platelet aggregation (SPA) and by inhibition of granule release which liberates such mediators as platelet factor 4 (PF4) and plasminogen activator inhibitor 1 (PAI-1). The present study was performed in 57 patients with ischemic heart disease (IHD), divided into groups depending on coexistent hyperlipoproteinemia (HLP) and aspirin treatment. The control group included 21 healthy individuals, matched for age and sex. Parameters of hemostasis (SPA, PF4, PAI-1) and concentration of lipid fractions (TC, TG, LDL, HDL) were measured in plasma. Increased PF4 levels were found in all groups with IHD, irrespective of hyperlipoproteinemia or aspirin treatment. Enhanced SPA and higher PAI-1 were limited to group IHD-HLP without aspirin. Highest PAI-1 activities were observed after stimulation of platelets in vitro. In conclusion, patients with IHD and hyperlipoproteinemia presented most pronounced platelet activation and impairment of fibrinolysis. Aspirin had a beneficial effect on these changes. Lower activities of PAI-1, in patients treated with aspirin, can be ascribed to its reduced release from platelets. Aspirin did not satisfactorily reduce the level of PF4, although it strongly inhibited SPA.

[Evaluation of lipoprotein (a) and fibrinogen levels in men after premature myocardial infarction and in their sons].

Lipoprotein (a) and fibrinogen admittedly are independent risk factors of coronary heart disease. Since Lp (a) has been shown to inhibit plasminogen binding to endothelial cells and to fibrin monomers, this led to the speculation that high Lp (a) levels in plasma could interfere with fibrinolysis. Therefore the aim of our work was to estimate the lipoprotein (a) and fibrinogen levels and to find correlation between lipoprotein (a) and fibrinogen levels at the men after a myocardial infarction and also at their sons. We examined 33 men of age 31-57 after a myocardial infarction and their 18 sons 4-14 years old. The control group consisted of 40 men of age 18-57 and their 19 sons 5-14 years old. The Lp (a) was determined with electroimmunodiffusion method and fibrinogen by Clauss method. The fibrinogen level in the examined group was found to be statistically significantly higher (338 mg/dl) than in the control group (313 mg/dl). There was no correlation between Lp(a) and fibrinogen concentration. In the group of sons of patients mean fibrinogen and Lp (a) levels were similar to those of the control group. However there was a negative correlation between the fibrinogen and the triglycerides concentration in sons of patients but not in the control group. The results seem to confirm the significance of fibrinogen as a risk factor for the development of the coronary heart disease.

[Effect of dietetic recommendations on the level of some lipid and hemostatic parameters in offspring of parents with risk factors for coronary heart disease].

Atherosclerosis, the precursor of coronary heart disease may originate in childhood. The atherogenic potential of food is related to its cholesterol and saturated-fat content. The study population consisted of children from CHD-parents under the age 12 years with plasma total cholesterol > 170 mg/dl, LDL-Ch > 90 mg/dl and ApoB > 70 mg/dl. All the patients were advised a 6 month diet following National Program of Cholesterol Prevention recommendation. Plasma, TCh, LDL-Ch, ApoB, TG, Ch-esters saturated/unsaturated ratio and platelet factor 4 concentration decreased, LCAT activity and Ch-esters unsaturated increased during study period. We observed interesting correlation between: PF4 and ApoB, PF4 and LDL-Ch and PF4 and HDL-ECh 18:3. Our data show, that proper diet can modify risk factors of CHD in children with family history of CHD.

[Heparin increases platelet aggregation and inhibits fibrinolysis, especially in primary hypertriglyceridemia and after myocardial infarction].

The authors presented the effect of unfractionated heparin on platelet aggregation and fibrinolysis. The studies have been performed in the control group and with primary hypertriglyceridemia as well as in patients after myocardial infarction. Adrenaline and ADP induced platelet aggregation was examined in plasma rich platelets; fibrinolysis was estimated in euglobulin test. These parameters have been determined before and after (10, 20, 60, 240, 360 min.) intravenous injection of 50 units heparin per kg body weight. We have observed, that heparin increased also platelet aggregation and inhibited fibrinolytic activity in persons with primary hypertriglyceridemia particularly and after myocardial infarction. Correlation was found between investigated parameters and products of heparin lipolytic activity, especially with level of free fatty acids.
