[Hearing loss caused by leisure noise].

Although noise in general can induce hearing loss, environmental noise represents an important risk for children, teenagers and young adults. Epidemiological investigations now support the occurrence of an increasing number of irreversible hearing losses in these groups. Major causes of hearing loss are toys (guns), explosives and electroacoustically amplified music delivered by head sets or heard in discotheques and open air concerts. Clinical indications are discussed.

[Growth retardation and dystrophy in Crohn's disease].

A 14 year old boy with Crohn's disease of the small bowel had a seven year history of growth failure and severe dystrophy. This case demonstrates the significance of growth failure, catch-up growth and intestinal obstruction for children with Crohn's disease.

[Current evaluation of diphtheria].

Between 1973-1983 there were 103 scattered cases of diphtheria in the FRG. The mortality rate was 22%. The question remains open, whether there are juts sporadic cases or the beginning of a new epidemic. During the past 200 years diphtheria occurred in waves with a peak every 30 years. Diagnostically we should be on the alert, and start special treatment already with the clinical diagnosis. For a start hyperimmunoglobulin should neutralize still circulating equine toxins which are no longer caught once they are within cells e.g. heart muscle. Only after this, antibiotics should be considered as an addition to kill toxin-producing bacteria. Antibiotics alone do not prevent the sometimes lethal action of toxins. For the bacteriologic proof of the diagnosis not only the coryne bacteria have to show but the toxin production as well. The transformation of non-toxin-producing into toxin-producing coryne-bacteria following an infection with bacteriophages has been observed recently in children during an epidemic.

[The "definitely" prenatally developed neuroblastoma. A review].

This is a report on 175 "definitely" congenital neuroblastomas, 92 of which were localised in the adrenals. Preferred metastasising takes place into the liver (type: Pepper) and the skin (type: Smith), whereas metastases in the skeleton were only rarely involved (type: Hutchinson). The peculiarities in metastasing and the markedly more favourable prognosis make it possible to differentiate between the "definitely" prenatally developed neuroblastomas (clinical or pathological anatomic diagnosis within the first 14 days of life) with regard to their biological qualities, and the neuroblastomas occurring in older infants and children.

Calcified cerebral necrosis following ALL therapy.

Calcified cerebral necrosis was an unusual finding at the autopsy of a 13-year-old girl who died after prolonged therapy for ALL. The patient had shown symptoms of progressive cerebral damage subsequent to a second cycle of prophylactic high-dose cytostatic therapy combined with cranial irradiation. Pathoanatomic examination revealed extensive florid recurrency of meningosis and leukemic encephalosis with scalloped calcified necroses measuring up to 5 cm, in the medullar layer of brain and cerebellum. Located predominantly near the ventricular area, encapsulated necroses showed many fibrous vessels with thickened walls and stenosed or obstructed lumina. The cerebral cortex remained largely unaffected by tissue destruction. Besides methotrexate toxicity and the enhancing effect of irradiation the vascular involvement was interpreted as a particularly important factor. Formal pathogenesis is attributed to combined chemo- and radiotherapy in parallel to leukemic infiltration of vascular walls and partial obstruction of lumina by tumor emboli. Wall damage, severe fibrosis, and consecutive nutritional defects result in the destruction of cerebral tissue. The preferential occurrence of necroses in cortex-adjacent medullar layers is explained by the relatively poor blood supply of this border zone between meningeal and intracerebral tissue, no safe conclusion can be drawn pathoanatomically with regard to the actually fatal factor, whether it is the leukemic infiltration of vascular walls, the effect of cytostatic agents, or that of irradiation. The proposed multifactorial pathogenesis of cerebral calcification may be supported by computed tomography (CT) intra vitam.

[Neonatal hypoglycemia (author's transl)].

The paper describes the symptomatology, pathophysiology, diagnostics and therapy of neonatal hypoglycemia. Early diagnosis is required to prevent brain damage due to hypoglycemia. In addition to conventional enzymatic blood glucose determination a new test strip sensitive for lower concentrations enables the diagnosis exactly.

[Determination of low blood glucose levels in newborns with the reflectance photometer (author's transl)].

The paper reports the examination of a new test strip Reflotest-Hypoglycemie, which was developed specifically for the hypoglycemic range. In 2 series of 110 respectively 120 samples of capillary blood taken from premature and full term infants the blood glucose concentrations were determined using Reflotest-Hypoglycemie and the hexokineasured with the test strip agree well with the results of hexokinase method in a range less than 50 mg/dl. Bilirubin and hematocrit values do not affect the glucose test. Glutathion and ascorbic acid interfere with the test strip method but this error is neglectable because of physiological low concentrations of these substances.

[Magnesium concentration in erythrocytes and plasma in childhood (author's transl)].

In 243 children of various ages the magnesium concentration was determined simultaneously in erythrocytes and plasma. The analysis began in the mother immediately after delivery, in the umbilical vein and in the newborn infant, and ended at school age (6-14 years). The magnesium concentration in plasma as well as in the erythrocytes showed a constant tendency to increase. Statistically significant differences occurred between the age group 1-7 days on the one hand and 2-6 weeks on the other, and also between school children and adult controls (n=20). The adult controls also differed statistically from mothers following delivery.

[The clinical importance of magnesium during neonatal life (author's transl)].

By atomic absorption spectrophotometry (ASS) the magnesium levels in the serum of 50 mothers, their mature healthy infants and the umbilical vein and the umbilical artery were determined. The differences between maternal and neonatal levels and between umbilical vein and umbilical arteries are statistically significant. The results were as follows: Antecubital venous blood of the mother 1.41 +/- 0.38 mval/1, umbilical vein serum 1.59 +/- 0.35 mval/1, umbilical artery 1.65 +/- 0.36 mval/1, neonate 1.68 +/- 0.45 mva1/1. The magnesium concentration in the erythrocytes of neonates was determined for the first time by AAS analysis. A marked variation of the normal was found. It is possible that the determined low magnesium concentrations in the erythrocytes of neonates were due to latent magnesium deprivation in these infants. The mean values for the erythrocytes of the mother (4.38 +/- 1.16 mva1/1) and the erythrocytes from the umbilical vein (3.99 +/- 1.50 mva1/1) differed significantly, whereas the erythrocytes from the umbilical vein and from the peripheral venous blood of the neonate showed no statistically significant difference. Hypomagnesiumia and hypermagnesiumia are described in brief.
