The incidence of gastric ulcer and duodenal ulcer in north Norway. A prospective epidemiological study.

From a prospective epidemiological study of peptic ulcer disease in the northern part of Norway, incidence rates for gastric and duodenal ulcer are presented. Over a 3-year period 4234 patients were examined radiologically or endoscopically. The population at risk was 72,537 persons, 16 to 93 years old. The yearly incidence rate for gastric ulcer in women was 0.9 per 1000 and for men 1.4 per 1000. For duodenal ulcer the incidence rates were 0.8 and 1.5 per 1000, respectively. The ratio of gastric ulcer to duodenal ulcer is still 1 to 1.1 for both sexes, 1 to 0.9 in women, and 1 to 1.14 in men in the northern part of Norway. Furthermore, the present study indicates a statistically significant positive correlation between increasing age and the occurrence of peptic ulcers for both sexes.

Iohexol and metrizoate in urography in children. Comparison between a non-ionic and an ionic contrast medium.

Urography was performed with the non-ionic contrast medium iohexol in 21 children and with the ionic contrast medium metrizoate in 18 children. The quality of the urograms was good with both contrast media and there was no statistically significant difference between them in this respect. However, the contrast medium attenuation in the collecting system was higher with iohexol than with metrizoate. No renal damage was observed. Only minor adverse reactions were seen with both contrast media and more rarely in the iohexol group, but this difference was not statistically significant.

Huge arteriovenous malformation in the mediastinum. Clinical presentation and surgical management.

A 24-year-old asymptomatic women presented with a large posterior mediastinal mass and a vertebral defect. Cardiac enlargement was found at roentgenographic and echocardiographic investigation. The ECG was abnormal and right heart catheterization showed cardiac output 17.1 l/min with a large left-to-right shunt at the level of the right atrium and the superior caval vein. The mass was seen on angiograms to be fed by numerous vessels originating from the descending aorta between the aortic arch and the coeliac axis. Computed tomography demonstrated a homogeneous, highly vascular structure. Radical excision was successfully accomplished in a two-step procedure aided by partial cardiopulmonary bypass. This seems to be the first report of a macrofistulous arteriovenous malformation in the mediastinum.

Changes in the pattern of peptic ulcer disease in the northern part of Norway between 1946 and 1981.

The unique pattern of peptic ulcer disease in arctic areas has long been of interest. Results from a retrospective study on the occurrences of gastric and duodenal ulcer for the period between 1962 and 1964 in the northern part of Norway are presented. These results are compared with preliminary data from an ongoing investigation of peptic ulcer in the radiological department at the University Hospital of Tromsø and with those presented by others from the same area in the 1940s. It is shown that the relation between gastric ulcer and duodenal ulcer has changed markedly both for women and men. However, there is still a higher incidence rate of gastric ulcer than of duodenal ulcer in women, whereas the ratio of gastric to duodenal ulcer in men is 1:1. These results contrast sharply with those reported elsewhere in Europe and in the United States.

[CT identification of retroperitoneal haematomas following translumbar aortography (author's transl)].

Following translumbar aortography fifteen patients were examined by computed tomography which revealed retroperitoneal hematomas in seven cases. Further follow-up in four of these showed gradual reduction of the hematomas in three, while in one an examination 7 days after aorthography showed progression. During operation, rupture of the aorta occurred, and the patient died on the first postoperative day.

MR imaging of apical hypertrophic cardiomyopathy with left ventricular endomyocardial calcification.

A rare case of apical hypertrophic cardiomyopathy with massive left ventricular endomyocardial calcification was studied with MR. The extent of the hypertrophy was clearly demonstrated, enabling accurate estimations of myocardial mass and volumes. Furthermore, an apical calcification was found and its volume estimated. In this case, MR gave the same information that was available by echocardiography, CT, and cardiac catheterization without the disadvantages of these methods.
